Radiography and Computed Tomography in Four Dogs with Lung Lobe Torsion (흉부 방사선 촬영과 컴퓨터단층촬영을 이용한 폐염전의 진단 4례)

  • This case study describes the imaging features of radiography and computed tomography (CT) in four small breed dogs with lung lobe torsion. The medical records, thoracic radiographs and CT images of four dogs with confirmed lung lobe torsion were retrospectively reviewed. Pleural effusion and increased lobar opacity/density were seen on all CT and thoracic radiography in all four dogs. CT revealed narrowed/collapsed bronchus in all four dogs, while this was only appreciated on one thoracic radiography. The vesicular emphysema pattern was seen on four CT but only on three radiographic examinations. The specific findings of lung lobe torsion were vesicular emphysema and a narrowed/collapsed bronchus. These findings were more easily recognized on CT than with thoracic radiographs.

  • This is a report of a case of superior vena caval syndrome due to idiopathic mediastinal fibrosis, which was surgically treated. The patient was 35-year-old Korean male who progressively complained shortness of breath about for 40 days prior to operation. Phlebogram of SVC showed indentation of SVC at the site of cavoatrial junction. The operation was performed under impression of bronchogenic cancer of right hilum. After thoracotomy, it was found a irregular mass in the mediastinum at the level of cavoatrial junction, which was developed to surrounding with SVC, pericardium, trachea and bronchus and they fixed together to immobile. Bypass graft between SVC and right atrial appendage was performed using a pericardial roll tube This is a report of a case of superior vena caval syndrome due to idiopathic mediastinal fibrosis, which was surgically treated. Bypass graft between SVC and right atrial appendage was performed using a pericardial roll tube [$1.3{\times}5$ cm]. After that SVC was decompressed very well. SVC pressure was markedly reduced from 32 cm $H_2O$ in preoperative to 21 cm $H_2O$in postoperative. Mediastinal fibrosis was confirmed by histopathological examination postoperatively. The postoperative course was uneventful.

Endobronchial Leiomyoma Combined with Pulmonary Sequestration -A case report - (폐격리증을 동반한 기관지 내 평활근종 -1예 보고-)

  • A 42-year-old female was admitted to our hospital complaining of a dyspnea. Chest X-ray showed left atelectasis. A mass was detected in left main bronchus by computed tomography and bronchoscopy. The mass was diagnosed as a endobronchial leiomyoma by biopsy exam. After open thoracotomy and bronchotomy, mass removal was done and middle lobe was ventilated normally. Aberrant arterial supply from descending aorta to left lower lobe of the lung was detected and left lower lobectomy was done. The lower lobe of the left lung was pathologically diagnosed as intralobar pulmonary sequestration. Herein we report a rare coexistent case of endobronchial leiomyoma and intralobar pulmonary sequestration.

Effect of Platycodon grandiflorum A. DC Extract on the Bronthus Diseases Bacteria (도라지[Platycodon grandiflorum A. DC] 추출액에 따른 기관지 질환 세균에 미치는 효과)

  • This study was pe바ormed to observe the effects of Platycodon grandiflorum A. DC (3 years) extracts on the bronchus d diseases bacteria(Mycobacterium sp., K. pneumoniae, F. nucleatum, S. aureus, C. diphtheriae, S. pygogenes and N N. gonorrhoeae) and fungi(A. fumigatus). Platycodon grandiflorum A. DC was extracted by ethanol, water, ethyl ether and p petroleum ether. The extraction rates of Platycodon grandiflorum A. DC were identified as 71.8%, 100%, 15.4% and 14.1% i in each extract solution. Each extract solution was injected into culture media with several concentrations and then the b bacteria cell growth was investigated during 32 hours. As a result, the antimicrobial activities of extracts from ethyl ether and p petroleum ether were excellent. Among several concentrations, the percentage of bacteria cell growth inhibitions were observed to be from 0.06% to 0.14%. The rates of antimicrobial activities were over 70%. The degree of cell growth | inhibition of each bacteria was appeared in the order of ethyl ether > petroleum ether > water > ethanol.

  • A 5-year-old male Pointer dog weighing 24 kg was referred because of a chronic cough. The dog was routinely vaccinated and treated monthly with medications to prevent heartworm and endoparasites. Blood analyses revealed no abnormalities. Chest radiographs showed interstitial and bronchial patterns in the caudal lung fields. Bronchoscopic imaging identified a collapse of the left bronchus with a mucopurulent discharge, indicating the onset of bronchomalacia. Computed tomography also showed bronchial collapse and a tapering angle in the left caudal bronchus. This case study describes the diagnosis of bronchomalacia in a Pointer dog with a chronic cough, and suggests that bronchoscopic examination is a valuable tool for identifying of the onset of bronchomalacia in dogs presenting with an uncontrolled cough.

  • We report successful application of dual trachcobronchial stcnt to the diffuse tracheal stenosis. An one-month-old boy was transferred to the emergency room due to tachypnea and respiratory difficulty with COB retention. Preoperative computed tomography revealed pulmonary artery sling with diffuse tracheal stenosis. We found that the diameter of the both main bronchus was less than 3mm and the trachea was a complete ring. We divided the left pulmonary artery and implanted it to the main pulmonary artery under cardiopulmonary bypass. After that, tracheoplasty was performed with autologous pericardium. However, after the initial measures, CO2 retention and respiratory difficulty persisted due to the granulation tissue and dynamic obstruction of the airway ensued by the overlying pericardial flap. Therefore, we decided to apply a single tracheal stunt. After the insertion of tracheal stent, residual stenosis of the both main bronchus opening continued to cause respiratory difficulty Finally we applied dual tracheobronchial stent and resolved the airway obstruction.

  • Bronchial foreign bodies commonly occur through aspiration or inhalation. Here we present a very rare case of migration of a non-metallic foreign body into the bronchus 5 years after lobectomy. The foreign body, a Teflon pellet used to control parenchymal bleeding in the lung, was removed by a bronchofiberscope. Although the mechanism of migration remains unknown, our case and a previous Japanese case provide evidence that non-metallic foreign bodies can migrate from the lung parenchyma to the bronchus.

  • Broncholith is a calcified lymph node which partially or completely erodes into the bronchial lumen and broncholithiasis is a relatively rare condition which related to late tissue response to healing granulomatous pulmonary infections, most commonly histoplasmosis or tuberculosis. The prominent symptoms of broncholithiasis are coughing followed by hemoptysis and symptoms related to bronchial obstruction. The complications include bronchoesophageal fistula and aortotracheal fistula. We report one case of broncholithiasis caused by Aspergillus. The case was a 53 year-old house wife whose chief complaints were recurrent fever, chill and malaise. The chest film revealed an avoid hazziness on the right middle lobe and chest cr scan showed consolidation of lateral segment of right middle lobe with calcified small low attenuated lesion in right middle lobe bronchus. Aspergillosis confirmed by pathology after bronchoscopic removal of impacted Aspergillus containing muddy plug from lateral segmental branch of right middle lobe bronchus.

A Case of Bronchiolar Papilloma (세기관지 유두종 1예)

  • A 59-year-old woman was admintted to the hospital because of intermittent fever and right side chest pain. She has a same episode eight months before this entry. Chest CT scan demonstrated ill-defined parenchymal consolidation containing dilated bronchi of right lower lung field, but no endobronchial mass in the bronchial trees. Fiberoptic bronchoscopy seeking the cause of recurrent pneumonia revealed a small, round mass nearly completely obstructing me lumen of basal segmental bronchus of right lower lobe. The diagnosis of bronchiolar papilloma was made from the biopsy specimens of the bronchoscopic examination. The patient was treated with right lower lobectomy because of irreversible secondary changes below the obstructed bronchus. This thoracotomic excision resulted in complete relief of symptoms and the postoperative course was uneventful for 12 months. Here we report a extremely rare umor with a brief review of literatures.

  • Leiomyoma of the bronchus is a very rare benign tumor of the lung. Leiomyoma is usually found in the young and the middle age. The symptom depends on the location of the tumor, it's size, and changes in the lung distal to the lesion. Obstructive symptoms due to leiomyoma could be similar to those of asthma and bronchitis, and therefore delayed diagnosis is common. The treatment of leiomyoma is conservative since there have been no reports of recurrence after limited resection. Recently bronchoscopic tumor resection has been applied to selected cases. We experienced two cases of bronchial leiomyoma initially misdiagnosed as bronchial asthma which were successfully excised by resection, end-to-end anastomosis and bronchoplasty. To prevent destructive changes of lung distal to obstruction and to preserve the pulmonary function, early diagnosis and appropriate treatment are important points of consideration.
