유방암 환자에서 골전이와 유사한 소견을 보였던 SAPHO 증후군: 증례보고 (SAPHO Syndrome in a Patient with Breast Cancer Mimicking Bone Metastasis: A Case Report)

  • 우측 유방암이 확진된 66세 여자 환자에서 얻은 수술 전 유방 자기공명영상에서 우측 유방에 약 1.9 cm크기의 조영증강되는 종괴가 보이고, 임파절 전이는 보이지 않아 임상병기는 cT1N0M0 였다. 동시에 좌측 늑흉쇄 부위에 T1-강조영상에서 저신호강도로 보이는 골경화성 변화가, T2-강조영상에서 약간의 골수 신호증가가 보이고, 골스캔에서 강한 신호를 보여 골전이의 가능성을 완전히 배제할 수 없었다. 그러나 조기유방암에서 골전이가 흔하지 않고, 늑흉쇄 부위는 골전이의 일반적인 위치가 아니기 때문에 SAPHO 증후군과 같은 다른 가능성을 염두해 두었다. SAPHO증후군은 축상 혹은 사지의 골염 (osteitis)과 골비대증 (hyperostosis)이 주로 늑흉쇄부위에 오는 경우 의심해 볼 수 있으며 피부병 (dermatosis)이 동반될 경우 좀더 쉽게 진단내릴 수 있다. 늑흉쇄 부위는 유방 자기공명영상에 포함되기 때문에 이 부위에 이상소견이 보이는 경우 골전이와 함께 SAPHO 증후군의 가능성을 생각해 보아야 한다. SAPHO증후군은 특별한 치료가 필요없는 양성의 경로를 보이는 질환이므로 골전이와의 감별은 중요하며 확진을 위해 조직검사를 시행할 수 있다.

Protein tyrosine phosphatase controls breast cancer invasion through the expression of matrix metalloproteinase-9

  • The expression of matrix metalloproteinases (MMPs) produced by cancer cells has been associated with the high potential of metastasis in several human carcinomas, including breast cancer. Several pieces of evidence demonstrate that protein tyrosine phosphatases (PTP) have functions that promote cell migration and metastasis in breast cancer. We analyzed whether PTP inhibitor might control breast cancer invasion through MMP expression. Herein, we investigate the effect of 4-hydroxy- 3,3-dimethyl-2H benzo[g]indole-2,5(3H)-dione (BVT948), a novel PTP inhibitor, on 12-O-tetradecanoyl phorbol-13-acetate (TPA)-induced MMP-9 expression and cell invasion in MCF-7 cells. The expression of MMP-9 and cell invasion increased after TPA treatment, whereas TPA-induced MMP-9 expression and cell invasion were decreased by BVT948 pretreatment. Also, BVT948 suppressed NF-${\kappa}B$ activation in TPA-treated MCF-7 cells. However, BVT948 didn't block TPA-induced AP-1 activation in MCF-7 cells. Our results suggest that the PTP inhibitor blocks breast cancer invasion via suppression of the expression of MMP-9.

Two Separate Episodes of Intramedullary Spinal Cord Metastasis in a Single Patient with Breast Cancer

  • Intramedullary spinal cord metastases are very rare. Patients with breast cancer as the primary source of intramedullary spinal cord metastases tend to do better than other types of cancer. We report the very unusual case of a woman with breast cancer who had two separate episodes of intramedullary spinal cord metastasis.

  • Curcumin has been known for its anti-proliferative and apoptotic effects on several cancer cells. We examined the inhibitory effects of curcumin on cancer cell adhesion, motility, invasion and matrix metalloproteinase-9 (MMP-9) activity in MDA-MB-231 human breast cancer cells. MDA-MB-231 cells were cultured with 0, 5, 10 or $20{\mu}M$ of curcumin. Curcumin significantly inhibited the adhesion of cancer cells to the fibronectin at $20{\mu}M$ and suppressed the motility and invasion of cancer cells at all concentrations. Also, the MMP-9 activity was inhibited by curcumin, but MMP-9 protein amounts were not affected. Our data indicate that curcumin inhibits motility, invasion and MMP-9 activity of MDA-MB-231 cells. Therefore, curcumin may contribute to the potential beneficial food component to prevent the cancer metastasis in human breast cancer.

  • 유방암 환자의 흉막 전이는 흔한 소견이다. 흉수가 가장 흔한 증상이고 흉수를 동반하지 않는 흉막 결절이나 흉막 판은 비교적 드물다. 본 환자는 말기 유방암으로 인해 흉수를 동반하지 않는 빠른 속도로 악화되는 거대 흉막 종양이 생겨났고 그로 인해 심한 호흡곤란을 경험했던 환자로 보기 드문 증례로서 보고를 하며 이와함께 말기암환자의 호흡곤란의 치료에 대해 고찰해 보고자 한다.

Suppression of Metastasis of Human Breast Cancer Cells by Chitosan Oligosaccharides

  • The present study investigated the antimetastatic property of chitosan oligosaccharides (COS) by evaluating motility, invasion, and the amount and activity of MMP-9 in MDA-MB-231 human breast carcinoma cells. Treatment of MDA-MB-231 cells with increasing concentrations of COS led to a concentration-dependent decrease in cell migration. COS significantly inhibited the invasion of MDA-MB-231 cells through a Matrigel-coated membrane. The treatment of MDA-MB-231 cells with COS reduced the amounts of secreted MMP-9. The activity and amount of MMP-9 protein in MDA-MB-231 cells were decreased by treatment with COS and occurred in a concentration-dependent manner. Our data indicated that COS can serve as a potential novel therapeutic candidate for the treatment of metastatic breast cancer.

Association of miR-1266 with Recurrence/Metastasis Potential in Estrogen Receptor Positive Breast Cancer Patients

  • The Homeobox B13 (HOXB13):Interleukin 17 Receptor B (IL17BR) index of estrogen receptor (ER)-positive breast cancer (ER (+) BC) patients may be a potential biomarker of recurrence/ metastasis. However, effects of microRNA (miRNA) binding to the 3' untranslated region (3' UTR) of HOXB13 and IL17BR and its function on recurrence/metastasis in ER (+) BC remains elusive. The aims of this study were to determine the expression of miRNAs that bind to 3' UTR of HOXB13 and IL17BR in ER (+) BC patients and asess the effects of these miRNAs on recurrence/metastasis. The expression profiles of HOXB13 and IL17BR were evaluated using RT-PCR in tumors and normal tissue samples from 40 ER (+) BC patients. The expression level of 4 miRNAs, which were predicted to bind the 3' UTR of HOXB13 and IL17BR using TargetScan, microRNA.org and miRDB online databases, were further evaluated with RT-PCR. Our findings demonstrated that high miR-1266 levels might be significant prognostic factor for recurrence/metastasis occurrence (3.05 fold p=0.004) and tamoxifen response (3.90 fold; p=0.2514) in ER (+) BC cases. Although we suggest that modulation of miR-1266 expression may be an important mechanism underlying the chemoresistance of ER (+) BC, advanced studies and validation are required.

황정(黃精)으로부터 유래한 조다당류의 선천면역 활성에 의한 유방암 세포주 전이 억제 효과 (Antimetastatic Effects of Crude Polysaccharide Isolated from Polygonati Rhizoma on 4T1 Breast Cancer Cells by Activation of Innate Immune System)

  • Objective: This study is aimed to investigate the anti-tumor metastasis by innate immunomodulating effects of crude polysaccharide isolated from Polygonati Rhizoma (CP-PR) on 4T1 breast cancer cells. Methods: CP-PR was isolated from Polygonati Rhizoma. Antimetastatic experiments were conducted in vivo mouse model by using 4T1 breast cancer cells. The cell viability of CP-PR was tested with normal spleen and 4T1 breast cancer cells. To observe the activation of macrophages with/without 4T1 breast cancer cells, production of tumor necrosis factor-${\alpha}$ ($TNF-{\alpha}$), interleukin-6 (IL-6), IL-10 and IL-12 were measured with enzyme-linked immunosorbent assay (ELISA), respectively. In addition, the lysis of YAC-1 cells and the production of granzymes were measured to observe the activation of natural killer (NK) cell. Results: Intravenous administration of CP-PR significantly inhibited metastasis of 4T1 breast cancer cells. In an in vitro cytotoxicity analysis, CP-PR affected the growth of normal spleen and 4T1 breast cancer cells above specific concentration. The production of $TNF-{\alpha}$, IL-6, IL-10 and IL-12 were significantly increased in macrophages with CP-PR. As compared with control, CP-PR showed significantly higher production of $TNF-{\alpha}$, IL-10 and IL-12 in macrophages co-cultured with 4T1 breast cancer cells. The lysis of YAC-1 cells and the production of granzymes were significantly up regulated by CP-PR. Conclusion: CP-PR appears to have considerable activity on the anti-metastasis by activation of innate immune system.

전이성 유방암 세포주에 대한 당목향의 항암효과탐색 (Anti-cancer Effects by Saussurea lappa Clarke in Highly-metastatic MDA-MB-231 Breast Cancer Cells)

  • Saussurea lappa Clarke is a well-known transitional medicine in Asia including Korea, China and Japan. It has been reported that Clarke has diverse effects such as anti-viral, anti-inflammatory, anti-cancer in human gastric cells and human prostate cancer cells. However, the anti-cancer effects and the mechanism of actions of Saussurea lappa Clarke are still unknown in breast cancer. In this study, we observed that Saussurea lappa Clarke inhibits the cell growth in a dose- and time-dependent manner in highly-metastatic MDA-MB-231 breast cancer cells. In order to examine whether Saussurea lappa Clarke suppresses cell growth inducing apoptosis cell death or cell cycle arrest, we analyzed DNA contents and cell cycle distribution using a flow cytometer and western blotting in MDA-MB-231 cells. We suggest that Saussurea lappa Clarke dose not induced apoptosis and induced G2/M phase cell cycle arrest. Moreover, Saussurea lappa Clarke also decreased the expression level of metastasis-angiogenesis releated protein such as VEGF. However, dose not changed the expression level of metastasis related protease MMP-1 in highly-metastatic MDA-MB-231 breast cancer cells. Therefore, Saussurea lappa Clarke might be good and useful chemotherapy agent highly-metastatic breast cancer patients.

유방암 환자의 증상 클러스터 (Symptom Clusters in Patients with Breast Cancer)

  • Purpose: The purpose of this study was to identify symptom clusters in patients with breast cancer and to investigate the associations among them with functional status and quality of life (QOL). Methods: A convenient sample of 303 patients was recruited from an oncology-specialized hospital. Results: Two distinct clusters were identified: A gastrointestinal- fatigue cluster and a pain cluster. Each cluster significantly influenced functional status and QOL. Based on these two clusters, we identified subgroups of symptom clusters using K-means cluster analysis. Three relatively distinct patient subgroups were identified in each cluster: mild, moderate, and severe group. Disease-related factors (i.e., stage, metastasis, type of surgery, current chemotherapy, and anti-hormone therapy) were associated with these subgroups of symptom clusters. There were significant differences in functional status and QOL among the three subgroups. The subgroup of patients who reported high levels of symptom clusters reported poorer functional status and QOL. Conclusion: Clinicians can anticipate that breast cancer patients with advanced stage, metastasis, and who receive mastectomy, and chemotherapy will have more intense gastrointestinal-fatigue or pain symptoms. In order to enhance functional status and QOL for patients with breast cancer, collective management for symptoms in a cluster may be beneficial.
