Dental implant is increasingly used to recover the mastication function of tooth. Several types of implant were designed to give an optimal stress distribution in surrounding bony regions. In this study, six types of implant were investigated using finite element method and it was studied i) how the variation of cortical bone thickness affects the stress distribution in surrounding bony regions depending upon implant types, ii) which type gives the best characteristics in the sence of stress distribution and stability. The hybrid-type implant with cylinder and screw gave the optimum properties in view of stability and response to the variation of cortical bone thickness.
Primary fixation is one of the most important factor in establishing adequate osseointegration between implant and bone. To evaluate the initial healing response of bone around implants without primary bone contact, this study was designed to create considerable space between implant and bone in 5 mongrel dogs, about 1-year old. After 3 holes of 6.0mm in diameter were prepared at the femur neck of the dogs, commercially pure titanium thread type implants(STERI-$OSS^{(R)}$), 8mm in length and 3.8mm, 5.0mm and 6.0mm in diameter, were inserted. Implants were supported by only nonresorbable membrane($Teflon^{(R)}$), and the penetration of upper soft tissue into the gap was inhibited by it. The each implant was positioned in the center of the drilled hole. 9 implants with different diameters were inserted in 3 dogs for histologic observation, and 12 were inserted in 2 dogs for mobility test and removal torque test.Fluorescent dyes were injected in order of Doxycycline, Alizarin Red S, and Calcein at intervals of 2 weeks. At 4-, 8-, and 12-week after placement, 3 dogs were sacrificed for histologic observation, and at 8- and 12-week after placement, 2 dogs were sacrificed for mobility test using $Periotest^{(R)}$ (Simens AG, Bensheim, Germany) and torque test using Autograph AGS-1000D $series^{(R)}$(Japan). The result were as follows: 1. The wider the gap between bone and implant was, the less bone maturity was, and the later osseointegration was occurred. Trabecular direction of new bone around implant was changed from parallel to perpendicular to the implant, and the gap was filled with new bone, over time. 2. There was a decreasing tendency over time in the mobility of all implants, but the wider gap between bone and implant was, the smaller decrease of the mobility was. 3. There was a increasing tendency over time in the removal torque gauge of all implants, and the wider gap was, the smaller increase of the removal torque gauge was. The results suggest that osseointegration in case of implant without primary bone contact may be obtained by guided bone regeneration technique with prolonged healing period, but the time of second surgery should be considered carefully.

Silibinin is a polyphenolic flavonoid compound isolated from milk thistle (Silybum marianum), with known hepatoprotective, anticarcinogenic, and antioxidant effects. Herein, we show that silibinin inhibits receptor activator of $NF-{\kappa}B$ ligand (RANKL)-induced osteoclastogenesis from RAW264.7 cells as well as from bone marrow-derived monocyte/macrophage cells in a dose-dependent manner. Silibinin has no effect on the expression of RANKL or the soluble RANKL decoy receptor osteoprotegerin (OPG) in osteoblasts. However, we demonstrate that silibinin can block the activation of $NF-{\kappa}B$, c-Jun N-terminal kinase (JNK), p38 mitogen-activated protein (MAP) kinase, and extracellular signal-regulated kinase (ERK) in osteoclast precursors in response to RANKL. Furthermore, silibinin attenuates the induction of nuclear factor of activated T cells (NFAT) c1 and osteoclast-associated receptor (OSCAR) expression during RANKL-induced osteoclastogenesis. We demonstrate that silibinin can inhibit $TNF-{\alpha}$-induced osteoclastogenesis as well as the expression of NFATc1 and OSCAR. Taken together, our results indicate that silibinin has the potential to inhibit osteoclast formation by attenuating the downstream signaling cascades associated with RANKL and $TNF-{\alpha}$.

The ultimate goal of periodontal treatment is the regeneration of periodontal tissues which have been lost due to periodontal disease. Recently, many natural medicines have been studied for their potential of anti-bacterial, anti-inflammatory and regenerative effects in periodontal tissues. Safflower seeds have been traditionally used as a drug for treatment of fracture and blood stasis in oriental medicine. The objective of the present study is to examine the biologic effects of safflower seeds extract on bone formation and regeneration of rat calvarial defects. The calvarial defects were made with 8mm trephine bur and extract of safflower seeds were placed directly at these defects. 24 rats were divided into control and experimental groups, and each group was sacrificed at 1 week, 4 weeks and 8 weeks. To study a histopathology related to bone regeneration, Goldner's Masson Trichrome stain and histomorphologic measuring was done at each weeks. In the early phase of bone healing, less inflammatory infiltration and capillary proliferation was found in experimental group compared to control. Dense bony tissues and matured bone structures in defect areas were found in experimental groups. And area of new bone formation was significantly increased at 8 weeks in experimental group. These results indicate that direct local application of safflower seeds extract reduces the early inflammatory response and promotes the regeneration of new bone in calvarial defects of rats.
STATEMENT OF PROBLEM: Loss of the marginal bone to the first thread have been accepted but continuous effort have been made to reduce this bone loss by varying implant design and surface texture. PURPOSE: This animal study has examined the histomorphometric variations between implants with micro-thread, micro-grooved and turned surfaced neck designs. MATERIAL AND METHODS: Four mongrel dogs have been used the premolars removed and left to heal for three months. One of each implant systems with turned neck, micro-thread and micro-grooved were placed according to the manufacturers’protocol and left submerged for 8 and 12 weeks. These were then harvested for histological examination. RESULTS: The histologically all samples were successfully ossointegrated and active bone remodelling adjacent to implants. With the micro-grooved implants 0.40 mm and 0.26 mm of the marginal bone level changes were observed at 8 and 12 weeks respectively. The micro-threaded implants had changes of 0.79 mm and 0.56 mm at 8 and 12 weeks respectably. The turned neck designed implants had marginal bone level changes of 1.61 mm and 1.63 mm in 8 and 12 weeks specimens. A complex soft tissue arrangement could be observed against micro-threaded and micro-grooved implant surfaces. CONCLUSION: Within the limitations of this study, it could be concluded that implants with micro-grooved had the least and the turned neck designed implants had the most changes in the marginal bone level. The textured implant surfaces affect soft tissue responses.
The physical tooth movement by orthodontic force is based upon alveolar bone resorption at compression site and new bone formation at tension site of the alveolar socket. The function of the cyclic AMP is to participate not only in initial action of bone cells by mechanical forces but also in the continuous cellular response leading to bone remodelling. This experiment was performed to clarify the role of cyclic AMP in bone remodelling by mechanical force in the NORMAL group, the DIABETES group and the INSULIN TREATED group. The 72 rats were divided into the NORMAL group, the DIABETES group and the INSULIN TREATED group. The same orthodontic forces were applied to the rats of 3 groups. These rats were treated for periods of time ranging from 1 hour, 1 day, 7 days, 14 days, 21 days and 28 days. The samples of alveolar bones were obtained from compression and tension sites surrounding the tipping teeth from NORMAL, DIABETE and INSULIN TREATED rats. The samples were assayed for cyclic AMP by the cyclic AMP RIA kit. The results were as follows: 1. The cyclic AMP levels of alveolar bone in compression and tension sites showed initial decrease, then increased and .remained elevated by the time consuming. 2. The highest cyclic AMP level showed in the DIABETES group and the lowest level was in the NORMAL group. 3. The cyclic AMP levels in the INSULIN TREATED group was similar with the NORMAL group in control and tension sites, but in the compression sites it was similar with the DIABETES group.

Osteomyelitis is an infection of bone or bone marrow, caused by pyogenic bacteria or mycobacterium. Osteomyelitis can be acute or chronic, inflammatory process of the bone and its structures. Chronic osteomyelitis will result in variable sclerosis and deformity of the affected bone. With an infection of the bone, the subsequent inflammatory response will elevate this overlying periosteum, leading to a loss of the nourishing vasculature, vascular thrombosis, and bone necrosis, resulting occasionally in formation of sequestra. These become areas that are more resistant to systemic antibiotic therapy due to lack of the normal Havesian canals that are blocked by scar tissue. At this aspect, not only systemic antibiotic therapy, but also surgical debridement maybe required to remove the affected bone and prevent disease propagation to adjacent areas. We experienced a patient who diagnosed tongue cancer and underwent wide partial glossectomy few years before, with an ulcerative lesion around right retromolar trigon. We diagnosed cancer recurrence because PET indicated hot uptake on mandible which was nearby previous tongue tumor site. The patient received hemiglossectomy via paramedian mandibulotomy, partial mandibulectomy and fibula osteocutaneous free flap reconstruction. But final diagnosis was mandible osteomyelitis on pathology report. Here, we present the case with a review of the related literatures.
Purpose: The aim of this study was to investigate the combined effects of physical and chemical surface factors on in vivo bone responses by comparing chemically modified hydrophilic sandblasted, large-grit, acid-etched (modSLA) and anodically oxidized hydrophobic implant surfaces. Methods: Five modSLA implants and five anodized implants were inserted into the tibiae of five New Zealand white rabbits (one implant for each tibia). The characteristics of each surface were determined using field emission scanning electron microscopy, energy dispersive spectroscopy, and confocal laser scanning microscopy before the installation. The experimental animals were sacrificed after 1 week of healing and histologic slides were prepared from the implant-tibial bone blocks removed from the animals. Histomorphometric analyses were performed on the light microscopic images, and bone-to-implant contact (BIC) and bone area (BA) ratios were measured. Nonparametric comparison tests were applied to find any significant differences (P<0.05) between the modSLA and anodized surfaces. Results: The roughness of the anodized surface was $1.22{\pm}0.17{\mu}m$ in Sa, which was within the optimal range of $1.0-2.0{\mu}m$ for a bone response. The modSLA surface was significantly rougher at $2.53{\pm}0.07{\mu}m$ in Sa. However, the modSLA implant had significantly higher BIC than the anodized implant (P=0.02). Furthermore, BA ratios did not significantly differ between the two implants, although the anodized implant had a higher mean value of BA (P>0.05). Conclusions: Within the limitations of this study, the hydrophilicity of the modSLA surface may have a stronger effect on in vivo bone healing than optimal surface roughness and surface chemistry of the anodized surface.

The effects of herbal medicine on travecular bone area were studied using ovariectomized rat as an animal model of Type I osteoporosis and SAM P6 as that of Type II. We counted red blood cells(RBC), hemoglobin(Hb), and hematocrit(Hct) using Couter`sR method. Each traditional boiling water extract of Achyrathis Radix, Psoraleae Fructus, Rehmanniae Radix Preparat, and Cornii Fructus and a systemic water extract of Astragali Ractiex was given 5g/kg/day, p.o., for 30 days in a group of 4-5 ovariectormized rats. One ml of blood was taken by tail vein at day 0, 7, 14, 21, and 30 days after administration of the extract. The traditional hot water extract of Cervi parvumn Corni (Cervi) was given the same dose as described above for 14 days in a group of 10 SAM P6 mice and systemic water extract of Astragali Radix was administered as the same dose as above for 30 days in 10 SAM P6 mice. Travecular bone area was measured 5 mcm decalcified and stained thin bone slice by image analysis using a digitalizer. In Type I, ovariectomized rats, administration of Astragali Radix, Rhemanniae Radix Preparat, and Corni Fructus decreased in RBC, Hb and Hct. In Type II, administration of Cervi increased in RBC and Hct and that of Astragali Radix was also elevated RBC. In Type I, any administration of herbal medicine used in this study did not elevate travecular bone area significantly except Corni Fructus showed a trend of increase in travecular bone area. However, Type II, Cervi and Astragali Radix increased in both mean and total travecular bone area. Thus, there are significant difference in response of herbal medicine in different types of osteoporosis.
The purpose of this study was to compare effects of the bioceramics on healing processes of the alveolar bone defects in dogs. Five adult dogs aged 1 to 2 years were used in this study. Experimental alveolar bone defects were created surgically with a #1/2 round bur at the furcation area of the buccal surface of the mandibular 3rd, 4th premolars and 1st molar. Fifteen experimental alveolar bone defects were devided into three groups according to the type of graft materials. The groups were as follows : 1) flap operation with dense hydroxyapatite( DHA group ) 2) flap operation with porous hydroxyapatite( PHA group ) 3) flap operation with natural coral ( NC group ) At 1, 2, 4, 6, and 12 weeks, dogs were serially sacrificed and specimens were prepared with Hematoxylin-Eosin stain and Mallory stain for light microscopic evaluation. The results of this study were as follows : 1. In every group, inflammatory cell infiltrations were seen at 1st weeks due to surgical trauma, however inflammatory response owing to graft materials were not seen. 2. In every group, the appearance of connective tissue around graft materials was loosely formed at the initial stages, however the connective tissue was densely formed at 2 weeks. 3. The presence of osteocytes were observed at 2 weeks in the natural coral group, however the osteocytes were appeared at 6weeks in the dense hydroxyapatite group. 4. A new bone was formed from the base and walls of the defect and gradually expanded toward the graft materials. 5. A resorption of the natural coral occurred irregularly at the periphery of the material, therefore the size and shape of the natural coral were reduced at 6 weeks. 6. At 12 weeks, the porous hydroxyapatite and natural coral were surrounded by newly formed bone most completely, however dense hydroxyapatite was surrounded by newly formed bone in part.
