Effect of initial placement level and wall thickness on maintenance of the marginal bone level in implants with a conical implant-abutment interface: a 5-year retrospective study

  • Purpose: Implant wall thickness and the height of the implant-abutment interface are known as factors that affect the distribution of stress on the marginal bone around the implant. The goal of this study was to evaluate the long-term effects of supracrestal implant placement and implant wall thickness on maintenance of the marginal bone level. Methods: In this retrospective study, 101 patients with a single implant were divided into the following 4 groups according to the thickness of the implant wall and the initial implant placement level immediately after surgery: 0.75 mm wall thickness, epicrestal position; 0.95 mm wall thickness, epicrestal position; 0.75 mm wall thickness, supracrestal position; 0.95 mm wall thickness, supracrestal position. The marginal bone level change was assessed 1 day after implant placement, immediately after functional loading, and 1 to 5 years after prosthesis delivery. To compare the marginal bone level change, repeated-measures analysis of variance was used to evaluate the statistical significance of differences within groups and between groups over time. Pearson correlation coefficients were also calculated to analyze the correlation between implant placement level and bone loss. Results: Statistically significant differences in bone loss among the 4 groups (P<0.01) and within each group over time (P<0.01) were observed. There was no significant difference between the groups with a wall thickness of 0.75 mm and 0.95 mm. In a multiple comparison, the groups with a supracrestal placement level showed greater bone loss than the epicrestal placement groups. In addition, a significant correlation between implant placement level and marginal bone loss was observed. Conclusions: The degree of bone resorption was significantly higher for implants with a supracrestal placement compared to those with an epicrestal placement.

  • The present study has been performed to evaluate Porphyromonas gingivalis (P.gingivalis) heat shock protein(HSP)60 as a candidate vaccine to inhibit multiple bacteria-induced alveolar bone loss. Rats were immunized with P.gingivalis HSP60 and experimental alveolar bone loss was induced by infection with multiple periodonto -pathogenic bacteria. Post-immune rat anti-P.gingivalis HSP IgG levels were significantly elevated and have demonstrated highly significant inverse relationship with the amount of alveolar bone loss induced by multiple bacteria. Results from PCR detection of subgingival bacterial plaque indicated that the vaccine successfully eradicated the multiple pathogenic species. We concluded that P.gingivalis HSP60 could potentially be developed as a vaccine to inhibit periodontal disease induced by multiple pathogenic bacteria.

  • We explored the role of reactive oxygen species (ROS) in LPS-induced bone loss. LPS was shown to increase the concentration of ROS in osteoclast precursors. The antioxidant decreased osteoclast formation by LPS. Furthermore, the antioxidant decreased NFATc1 expression by LPS, suggesting that ROS mediates NFATc1 expression in the regulation of LPS-induced osteoclast formation. Finally, the antioxidant decreased LPS-induced RANKL mRNA expression in osteoblasts. Taken together, these data indicate that LPS mediates ROS to induce bone loss.

  • Bone loss from the kerf of the sawblade may influence the final outcomes when employing three-dimensional-printed surgical guides. However, no studies have systematically addressed saw blade-induced bone loss. This study aims to quantify bone loss and propose a reduction guide to minimize the fracture gap. The postoperative gap tended to decrease as the amount of gap compensation increased. Osteotomy gaps can be attributed to the thickness of the saw blade, and the proposed methodology addresses this surgical error. Surgeons can proactively plan and design reduction guides with applied compensation using the method described in this study.

  • Background: The purpose of this retrospective study was to evaluate the clinical utility of an implant with a sandblasted, large-grit, acid-etched (SLA) surface and internal connection. Methods: Six patients who received dental implants in the Department of Oral and Maxillofacial Surgery, Chonnam National University Dental Hospital, were analyzed by factors influencing the success rate and marginal bone loss. Factors included patient's age, sex, implant installation site, whether bone graft was done, type of bone graft materials, approaching method if sinus lift was done, and the size of the fixture. In addition, the marginal bone loss was analyzed by using a radiograph. Results: All implants were successful, and the cumulative survival rate was 100 %. Average marginal bone loss of 6 months after the installation was 0.52 mm and 20 months after the functional loading was 1.06 mm. Total marginal bone resorption was 1.58 mm on average. There was no statistically significant difference in mesial and distal marginal bone loss. Conclusions: The short-term clinical success rate of the implant with an SLA surface and internal connection was satisfactory. Moreover, the marginal bone loss was also consistent with the implant success criteria.

Bone Loss Preventing Effect of Sophorae Fructus On Ovariectomized Rats

  • The preventive effects of Sophorae Fructus extracts (I: hot water extract and II: combination product using I) on bone loss in ovariectomized (OVX) rats were investigated. Sophorae Fructus extracts were orally administrated to OVX rats for 9 weeks. Ovariectomy caused the increase of body weight and deoxypyridinoline (Dpd: bone resorption marker) and decrease of calcium (Ca: bone formation marker) level in serum. Dpd level were significantly decreased and Ca levels were elevated at 9 weeks in Sophorae Fructus extracts administered groups after ovariectomy at a dose of 0.556 g/kg/day compared with control group. In administered groups, trabecular bone area (TBA) in the tibia and lumbar were also increased compared with control group in histomorphological analysis. The preventive or treatment effects of Sophorae Fructus extracts on bone loss in OVX rats appears to be due to suppression of bone turnover.

Palm Vitamin E Prevents Osteoporosis in Orchidectomized Growing Male Rats

  • Testosterone deficiency increased bone resorption, giving rise to osteoporosis. Testosterone deficiency also increased lipid peroxidation and free radical formation. Free radicals have been shown to be toxic to osteoblasts as well as to activate osteoclasts. In this study, the effects of giving an antioxidant, i.e. vitamin E-rich extract from palm oil on bone mineral density and calcium content was studied. Palm vitamin E prevented the loss in bone mineral density due to orchidectomy, seen in the whole femur, proximal amd midshaft regions, as well as L4 vertebra. Similar observations were seen in bone calcium content of the L5 vertebra. Giving palm olein also prevented the loss in bone mineral density in the femoral midshaft and L4 vertebra; and bone calcium content in the L5 vertebra. In conclusion, vitamin E-rich extract from palm oil was effective in preventing the loss in bone mineral density and calcium content of orchidectomized male rats. This action is probably due to its role as an antioxidant.

Multiple Ossicular Dislocation Including Stapediovestibular Dislocation Presenting with Conductive Hearing Loss

  • Temporal bone trauma can cause hearing loss and in case of prolonged conductive hearing loss, traumatic ossicular injury should be considered. Separation of the incudostapedial joint is the most common lesion, and stapediovestibular dislocation is relatively rare but can easily cause perilymphatic fistula. Here, we report a very rare case of external stapediovestibular dislocation after trauma, ending up with successful surgical outcome. A 27-year-old man with non-progressive hearing loss on the right side since childhood visited the clinic. Audiogram showed a conductive hearing loss with air-bone gap of 55 dB on the right side. Temporal bone CT revealed the disruption of ossicular chain. An exploratory tympanotomy identified multiple ossicular disruptions including external stapediovestibular dislocation with shiny fibrous membrane sealing the oval window. Ossicular chain reconstruction was performed using the total ossicular replacement prosthesis of titanium. A postoperative audiogram showed a recovery of air-bone gap less than 10 dB. To the best of our knowledge, this is the first case of external long-standing stapediovestibular dislocation, with oval window completely sealed with fibrous membrane, ending up with successful hearing recovery by surgery. This case would help dealing with such condition which can be encountered in the clinic.

디지털 공제술을 이용한 platform switching 임플란트의 변연골 소실에 대한 연구 (Evaluation of marginal bone loss around platform-switched implants by digital subtraction radiography)

  • 목적: 본 연구는 platform matching, platform switching을 시행한 임플란트의 변연골 소실을 비교하여 platform switching 개념의 임상적 의의를 평가하고자 한다. 연구 재료 및 방법: 원광대학교 치과병원 임플란트 센터에서 치료를 진행한 환자를 대상으로 임플란트 식립 시기, 직경, 길이, 지대주 연결방법 및 보철물 연결고정 유무를 조사하였다. 변연골 소실량을 측정하기 위해서 Emago advanced v5.6 프로그램을 사용하여 임플란트 식립 시와 가장 최근 내원 시의 치근단 방사선 사진을 비교하였다. 결과: 환자 82명의 임플란트 150개를 대상으로 6 - 63개월 동안 관찰한 결과 platform matching 임플란트는 $1.16{\pm}0.54mm$, platform switching 임플란트는 $0.68{\pm}0.27mm$의 변연골 소실을 나타내었다. 결론: Platform switching을 시행함으로써 임플란트의 변연골 소실을 줄이는 효과가 있는 것으로 사료된다.
