The purpose of this stuffy was to assess and compare the osseous responses to implanted particles of porous synthetic HA (Interpore $200^{(R)}$, Interpore International, U.S.A.), resorbable natural bovine derived HA (Bio-$oss^{(R)}$, Gestlich Pharma, Switzerland) and calcium carbonate(Biocoral $450^{(R)}$, Inoteb, France) in bone defects. Four calvarial defects of 2.5mm diameter were created in earth of 16 Sprague-Dawley rats. The experimental materials were subsequently implanted hi three defects, leaving the fourth defect for control purpose. Four animals were earth sacrificed at 3 days, 1week, 2weeks and 4 weeks after surgery. The tissue response was evaluated under light microscope. Overall, histologic responses showed that all the particles were well tolerated and caused no aberrent tissue responses. There were difference in the amount of newly formed bone at the experimental sites and control site. There was more new bone formation associated with calcium carbonate site. In addition, the calcium carbonate site displayed multinucleated giant cells surrounding calcium carbonate particles after the 1st week, and osteoid tissue within the particle after the 2nd week. After 4 weeks, calcium carbonate particles were resorbed and replaced with new bone. The healing of the natural bovine derived HA site was similar to that of porous synthetic HA, except that new bone growth between the two particles have progressed more in the former site after the 2nd week. In the natural bovine derived HA site, the particle was surrounded by newly formed bone after the 4th week. After 4 weeks, the control site showed more mature bone than other sites. In conclusion, the grafted site were better in new bone formation than non-grafted sites. In particular the calcium Carbonate site showed the ability of osteoinduction and natural bovine denver HA showed osteoconduction in rat calvarial defects. This suggest that calcium carbonate and natural bovine derived HA could enhance the regenerative potential in periodontal defects.
Purpose: Diabetes mellitus (DM) has been shown to alter the properties of the bone and impair bone healing around a titanium implant. The aim of this study is to investigate whether the low-intensity pulsed ultrasound (LIPUS), which has been known to stimulate the bone healing, improve the osseointegration of the titanium implant in tibia of DM-induced rats. Methods: 16 rats were received streptozotocin (60 mg/kg) for inducing diabetes. A total number of 32 titanium implants were placed bilaterally into both tibiae of these rats. The right tibia of each rat received LIPUS application (10 min/day) during 7 days post-operation, while the left side received no treatment. The study was carried on for six weeks and the rats were sacrificed at 1, 2, 4, and 6 weaks postoperatively (4 rats for each week) for histomorphometric and histologic analysis. Bone-implant contact and bone area were measured. Comparisons between the groups were made using statistical analysis on histomorphometric analysis. Results: The histomorphometry parameters showed that the bone-implant contact and the bone area values have decreased in the late osseointegration periods (4, 6 weeks) compared to the early osseointegration periods (1, 2 weeks) in both two groups. The bone-implant contact values of the LIPUS group were somewhat higher than those of controls at 1, 2 weeks, but the difference was not statistically significant. The bone area values of the LIPUS group were also higher than those of controls at 1, 2 weeks, but the difference was not statistically significant as well. Conclusion: Results of this study indicate that LIPUS may have positive effects on early osseointegration but could not improve the long term stability of dental implants.
The purpose of this study was to investigate effects of osteoporosis on extraction wound healing in the calcium deficient rat. In order to carry out this study, ten-week old Wistar strain rats weighing about 300 gms were selected. When the rats reached thirteen-week old, rats' mandibular first molars were removed. The rats were then divided into three groups: Group l(rats given a normal diet both before and after tooth extraction), Group 2(rats given a low calcium diet for three weeks before tooth extraction and a normal diet after tooth extraction), and Group 3(rats given a low calcium diet for three weeks before and after tooth extraction). The healing of extraction wounds, as assessed by microradiography, autoradiography, and histopathologic examination, were compared among these three groups. The obtained results were as follows : I. In Group 1, newly formed bone and active uptake of 45Ca around extraction wound were noted on the 3rd and the 7th day. On the 14th and the 21st day, the extraction wounds of this group showed the bone trabecular formation and active 4Ca uptake in the extraction wound and alveolar crest. The more prominent bone trabeculae with a less uptake of /sup 45/Ca were noted on the 42nd day. 2. In Group 2, newly formed bone and thinning of alveolar bone trabeculae with more extensive uptake of /sup 45/Ca than that in Group 1 were noted on the 3rd and the 7th day. On the 14th day, bone trabeculae were less thicker than that in Group 1. The prominent bone trabeculae in the extraction wounds and alveolar crest were noted on the 21st and the 42nd days. 3. In Group 3, newly formed bone was noted on the 3rd and the 7th day. Alveolar bone trabeculae and uptake of /sup 45/Ca were similar to that in Group 2. On the 14th and 21st day, bone trabeculae were less thicker than that in Group 2 and Group 3. The osteoporotic change with active uptake of /sup 45/Ca was markedly noted on the 42nd day.
본 연구는 외과적으로 형성된 흰쥐 하악골 결손부에 S.mutan에서 분리 정제된 mutan을 주사하고, ${\mu}CT$로 촬영하여 골결손부의 치유에 미치는 영향을 3차원 골 미세구조 지표로 평가하기 위하여 실시하였다. 흰쥐 치조골에 임계 크기 결손부인 1.5 mm 지름의 원형 결손부를 형성하여 대조군에는 동일량의 saline을 투여하고, 실험군으로 LPS와, mutan을 주사하여 골치유에 미치는 영향을 관찰하였다. 수술 후 4주에 희생하고 치유 결과를 3차원 미세구조의 형태 계측학적 지표값을 비교 분석하여 다음과 같은 결론을 얻었다. 실험동물의 체중변화는 총 12회 투여 기간 동안 일정한 증가를 나타냈다. 8회차부터 mutan 처리군은 대조군과 비교하여 유의한 차이를 보였으며(p<0.05), 9회차에는 mutan 처리군이 대조군과 LPS 처리군과 비교하여(p<0.05), 10~12회차에는 LPS 처리군과 비교하여 유의한 체중증가를 나타냈다(p<0.01). Mutan이 생체 내에서 하악의 골치유 효과를 알아보기 위해 ${\mu}CT$를 통해 형태계측학적 지표 분석 결과, 대조군에 비해 LPS 처리군에서 뚜렷한 골결손부 치유 지연 및 골흡수 양상을 보였으며, mutan 처리군 은 LPS의 작용보다 미약하지만 골치유를 지연시키는 것으로 확인되었다. ${\mu}CT$를 통해 골밀도 분석 결과, 대조군과 비교하여 LPS와 muatn 처리군에서 낮은 골밀도치를 나타냈고, mutan은 LPS 처리군보다 유의하게 높은 골밀도치를 보였다(p<0.01).
This study was performed to identify the effect of constant direct current and to give us methods which can be applied easily in clinic. Six rabbits was used at this experiment. After each animal was fractured at left fibula, divided into experimental group(n=3) and central group(n=3). Experiment duration of electrical stimulation on experimental rabbits was 35 days. Direct current from fifteen microampere to twenty microampere was passed continously through the placed electrode between fracture area and thigh. Negative electrode was placed at fracture area and positive at thigh. Roentgenography was used to observe bone-healing progression wet three times-at 15days, 25days and 35days after electrical stimulation. The results obtained are as followings: 1. Both experimental group and control group do not obtain callus formation on the first roentgenography(15 days after ES). 2. On the second roentgenography(25 days after ES), experimental group achieves above $70\%$ on fracture-healing, but control group achieves about $20-30\%$ on fracture healing. 3. On the third roentgenegraphy(35 nays after ES), experimental group achieves above $85-95\%$ on fracture healing and control group achieves about $60-70\%$ of bone union. Thus, statistically significance(independent t-test) was occured ie the second and third roentgenography between experimental group and control group.

Objective: The purpose of this study was to determine whether cyst enucleation would be done after marsupialization of large cystic lesion on the jaws or not, and if so, when it should be done. Patients & Methods: 12 patients with cystic lesion treated by marsupialization only and 10 patients with cystic lesion treated by marsupialization followed by enucleation were examined in this study. Postoperative clinical and radiographic examinations were performed at 1, 4, 7, 10, 13, 16, 19 months on 22 patients. Bone regeneration and reduction rate of the residual cystic cavities and bone density were evaluated with a analysis of digital panoramic radiographs. Also histological evaluation of the healing process was performed on 1 patient. Results: Uneventful healing and spontaneous bony filling of the residual cavities were observed in all cases. Postoperative radiographs showed that the size of the lesions was reduced for a few months, but the reduction rate of the residual cavity was minimized for 13~16 months after marsupialization. The bone density was increased 22.5 % after 19 months. Conclusion: This results suggest that the appropriate timing to perform enucleation would be 13~16 months after marsupialization. The state of healing process could be confirmed by histological examination and radiographic evaluation of bone density. Enucleation after marsupialization could be applied appropriately to reduce the periods of bone healing in large cystic lesion on the jaws.
Purpose: Osteoporosis, is a major health problem for the elderly and post-menopausal women and shown to alter the properties of bone as well as impair bone healing around titanium implants in both human and animals. The objective of this study was to examine the effect of LIPUS with adipose-derived stem cells on the healing process around a titanium implant in rats with osteoporosis. Methods: Sixteen osteoporosis-induced rats were divided into two groups: an adipose-derived stem cell injected with Low Intensity Pulsed Ultrasound (LIPUS) application group and a control group. Titanium screw implants (diameter, 2.0 mm: length, 3.5 mm, Cowell Medi, Korea) were placed into both tibia of 16 rats, on 8 rats as the control group and the other 8 rats as the experimental group. Rats were sacrificed at different intervals from 1, 2, 4 and 8 weeks after implantation for histopathologic and immunohistochemical analyses. Results: Histopathological analysis revealed newly formed bone in experimental group earlier than that in control group. Especially at 1 week after implantation, more amounts of new bone matrix and collagen around the implant in the experimental group were seen compared with the control group. Immunohistochemical analysis showed that the levels of osteoprotegerin (OPG) expression in the experimental group were increased at early stages compared with that of control group until 2 weeks after implantation. But after 2 weeks, the expression level of OPG similar in both groups. The expression levels of receptor activator of nuclear factor kB ligand (RANKL) were stronger in the experimental group than the control group until 2 weeks after implantation. After 4 weeks, expression of RANKL in experimental group was similar to the control group. Conclusion: The results of this study suggest that LIPUS with Adipose-Derived Stem Cells in implantation could promote bone healing around titanium implants in rats with osteoporosis.

Introduction: Diabetes mellitus, as a major health problem for the elderly has been shown to alter the properties of the bone and impair bone healing around a titanium implant in both humans and animals. The aim of this study was to examine the effect of adipose-derived stem cells on the healing process around a titanium implant in streptozotocin-induced diabetic rats. Materials and Methods: Thirteen rats were divided into two groups: adipose-derived stem cells injected group and a control group. A titanium screw implant (diameter: 2.0 mm, length: 3.5 mm) was placed into both tibia of 13 rats: 13 right tibia as the control group and 13 left tibia as the experimental group. The rats were sacrificed at different intervals (1, 2, and 4 weeks) after implantation for histopathology observations and immunohistochemistric analysis. Results: The histopathological findings revealed earlier new formed bone in the experimental group than the control group. In particular, at 1 week after implantation, the experimental group showed more newly formed bone and collagen around the implant than the control group. In immunohistochemistric analysis, osteoprotegerin (OPG) expression in the experimental group increased early compared to that of the control group until 2 weeks after implantation. However, after 2 weeks, OPG expression in the experimental group was similar to OPG expression in the control group. The receptor activator of nuclear factor ${\kappa}B$ ligand (RANKL) expression in the experimental group increased early compared to that of the control group, and then decreased at 2 weeks. After 2 weeks, the level of RANKL expression was similar in both groups. Conclusion: These results suggest that adipose-derived stem cells in implantation can promote bone healing around titanium, particularly in diabetes mellitus induced animals.

Objectives: Dental implants installation in patients with diabetes remains controversial as altered bone healing around implants has been reported. And little is known about the biological factors involved in bone healing around implants. The present study aimed to investigate the biological markers around immediately placed implants in rats with controlled and uncontrolled diabetes. Materials and Methods: Twenty rats (40 sites) were divided into the control, insulin-treated and diabetic groups. The rats received streptozotocin (60 mg/kg) to induce diabetes; animals in the insulin-treated group also received three units of subcutaneous slow-release insulin. Two threaded titanium alloy implant ($1.2{\times}3mm$) were placed in the extraction socket of the both maxillary first molars and allowed for healing. Bone blocks including implant were harvested at 3 days, 1, 2 and 4 weeks. The levels of bone morphogenetic protein (BMP)-4, transforming growth factor (TGF)-${\beta}1$, osteocalcin (OC) and osteonectin (ON) were measured in the peri-implant osseous samples by RT-PCR. Results: The BMP-4 level increased immediately in all groups by day 3, then decreased abruptly in the control and the insulin-treated groups. However, by week 4, all groups showed mostly the same amount of BMP-4 expression. The level of TGF-${\beta}1$ also instantly increased by day 3 in the insulin-treated group. This level elevated again reaching the same values as the control group by week 4, but was not as high as the diabetic group. In addition, the expression of OC and ON in the control and insulin-treated groups was higher than that of the diabetic group at 2 weeks and 4 weeks, indicating active bone formation in these groups. Conclusion: The immediate placement of titanium implants in the maxilla of diabetic rat led to an unwanted bone healing response. Conclusively, the results of this study suggest that immediate implant insertion in patients with poorly controlled diabetes might be contraindicated.
Purpose: Fibrillar collagens like type I collagen, are the major constituent of the extracellular matrix and structural protein of bone. Also, it can be a scaffold for osteoblast migration. The purpose of this study is to estimate the effects of absorbable atelo-collagen sponge (Teruplug$^{(R)}$, Terumo biomaterials Co., Tokyo, Japan) insertion in tooth extraction sites on periodontal healing of the second molar, healing of the fractured mandibular bone and new bone formation of third molar socket after the extraction of the impacted third molar with mandibular angle fracture. Methods: In our study of six cases of mandibular angle fractures, all of them underwent the extraction of the third molar tooth & absorbable atelo-collagen sponge insertion in tooth extraction site. Three of them had a intraoral infection & oral opening to fracture site, two of the six had dental caries, and only one had reduction problem due to third molar position. Six consecutive patients with noncomminuted fractures of the mandibular angle were treated by open reduction and internal fixation using one noncompression miniplates and screws placed through a transoral incision. Results: All of the patients have showed good postoperative functions and have not experienced complications requiring second surgical intervention. There was well healing of the mandibular bone and the most new bone formation of third molar socket after the extraction of the impacted third molar with mandibular angle fracture. Conclusion: The results of this study suggest that absorbable atelo-collagen sponge is relatively favorable bone void filler with prevention of tissue collapse, food packing, and enhance periodontal healing. Thus, the use of atelo-collagen sponge and one noncompression miniplate seems to be relatively easy, safe, and effective for the treatment of fractures of the mandibular angle and third molar extraction.
