치매 입원환자의 낙상 영향 요인 (Factors Affecting Falls of Demented Inpatients)

  • 본 연구의 목적은 2012년부터 2015년까지의 질병관리본부의 퇴원손상심층조사 자료를 바탕으로 병원에 입원한 치매환자 중 낙상과 관련된 특성과 낙상 영향 요인을 파악하는 것이다. 한국표준질병사인분류의 질병 코드를 사용하여 치매로 진단 받은 60세 이상 환자를 선정하여, 낙상(W00-W19) 유무에 따라 낙상군과 비낙상군으로 구분한 총 1,732건을 최종분석에 사용하였다. 수집된 자료는 통계 프로그램 STATA를 이용하여 빈도분석, 교차분석(chi-square test)과 로지스틱 회귀분석을 실시하였다. 연구결과 전체 치매 입원환자 중 낙상은 8.0%에서 발생하였다. 낙상군과 비낙상군의 범주별 분석에서 통계적으로 유의한 차이는 연령에서 있었다. 질환 특성에서는 CCI(Charlson Comorbidity Index) 및 골밀도장애가 통계적으로 유의한 차이가 있었다. 낙상에 영향을 미치는 요인의 로지스틱 회귀분석 결과 60-69세 대상자를 기준으로 했을 때, 80세 이상의 노인은 낙상위험이 2.386배 높고, CCI가 0점인 대상자를 기준으로 했을 때, 3점 이상인 대상자의 낙상 위험이 0.421배로 낮으며, 골밀도장애가 없는 대상자를 기준으로 했을 때, 골밀도장애가 있는 대상자의 낙상위험이 3.581배 높았다. 본 연구의 결과 치매 입원환자는 연령이 80세 이상인 경우 약 2.3배, 골밀도장애가 있는 경우 약 3.5배 이상 낙상이 높을 수 있으며, 반면에 CCI가 3점인 경우 약 0.4배로 낙상이 낮을 수 있다. 따라서 본 연구 결과를 바탕으로 입원한 치매환자의 낙상 관련 영향 요인은 입원한 치매환자 및 돌봄자들에 대한 낙상 예방 교육에 도움이 되고 의료진의 치매환자 낙상 관리를 위한 의사결정에 기초자료로 사용되길 기대한다.

  • Fibrous dysplasia (FD) is an uncommon skeletal disorder in which normal bone is replaced by abnormal fibro-osseous tissue. Mainly, FD is found in children, and by adulthood it usually becomes quiescent. Our case showed FD of more than 14-year duration in the left maxilla. Our evaluation was that growth ceased in adulthood and had achieved the static stage. Because FD cases in elderly patients are rarely reported, we hereby present a monostotic FD case in a 65-year-old female. We presented sequential radiographic images and scintigraphic images of this case, and combined them with a literature review that emphasized the progression of the disease.

Eosinophilic Granuloma Presenting as an Epidural Hematoma and Cyst

  • Langerhans' cell histiocytosis (LCH) is a rare immunologic disorder characterized by histiocyte proliferation in multiple organ systems. Eosinophilic granuloma, a benign bone lesion, represents a focal form of LCH. We experienced a case of Langerhans' cell histiocytosis in a patient who presented with intracranial epidural hematoma and cyst on the midline of the frontal skull. A 10-year-old boy presented with a rapidly growing large scalp mass on the midline frontal area after mild head trauma. The scalp mass was painless and immobile. Plain skull x-ray showed a punched-out bone lesion. Computed tomography and magnetic resonance imaging showed a non-enhancing osteolytic lesion presenting with an epidural hematoma and cyst on the midline of the frontal skull. The lesion of the skull was completely resected and the patient's recovery was uneventful. The acute presentation of a solitary eosinophilic granuloma of skull with an epidural hematoma has been described in only five cases in the literature and we report the first case of LCH presenting as an intracranial epidural hematoma on frontal area.

Unilateral intraoral vertical ramus osteotomy and sagittal split ramus osteotomy for the treatment of asymmetric mandibles

  • In surgery for facial asymmetry, mandibles can be classified into two types, rotational and translational, according to the required mandibular movements for surgery. During surgery for rotational mandibular asymmetry, a bilateral sagittal split ramus osteotomy (BSSRO) may cause a large bone gap between the proximal and distal segments as well as condylar displacement, resulting in a relapse of the temporomandibular joint disorder, especially in severe cases. The intraoral vertical ramus osteotomy has an advantage, in this respect, because it causes less rotational displacement of the proximal segment on the deviated side and even displaced or rotated condylar segments may return to their original physiologic position. Unilateral intraoral vertical ramus osteotomy (UIVRO) on the short side combined with contralateral SSRO was devised as an alternative technique to resolve the spatial problems caused by conventional SSRO in cases of severe rotational asymmetry. A series of three cases were treated with the previously suggested protocol and the follow-up period was analyzed. In serial cases, UIVRO combined with contralateral SSRO may avoid mediolateral flaring of the bone segments and condylar dislocation, and result in improved condition of the temporomandibular joint. UIVRO combined with contralateral SSRO is expected to be a useful technique for the treatment of rotational mandibular asymmetry.

다골성섬유형성이상에서 발생한 골육종: 증례 보고 (Osteosarcoma Arising in a Polyostotic Fibrous Dysplasia: A Case Report)

  • 섬유형성이상은 골모세포의 비정상적인 분화로 정상 골수와 해면골이 미숙골과 섬유성간질로 대치되는 유전이 되지 않는 골질환이다. 섬유형성이상은 다양한 영상소견을 보이며, 일반적으로 우연히 발견되며 정밀검사가 필요하지 않다. 그러나 섬유형성이상에서 병적골절의 합병증을 보일 수 있고 드물게 악성변이의 합병증을 가진다. 다골성섬유형성이상을 가진 44세 남자환자의 대퇴골에서 방사선소견상 골피질의 파괴와 자기공명영상에서 연부조직종괴를 가진 악성변이의 소견을 보였고, 수술로 절제된 조직학적 검사에서 연골아세포골육종으로 진단 받았다.

Giant cell tumor of temporomandibular joint masquerading as temporomandibular joint pain dysfunction syndrome: a rare case report

  • Giant cell tumor (GCT) of the craniofacial bones has been reported but they are not common. This tumor occurs more often in women than in men and predominantly affects patients around the third to fifth decade of life. GCTs are generally benign but can be locally aggressive as well. We report a case of GCT involving the temporomandibular joint (TMJ), which was initially thought to be temporomandibular disorder (TMD). A 22-year-old female presented with swelling and pain over the right temporal region for 18 months associated with jaw locking and clicking sounds. On examination, her jaw deviated to the right during opening and there was a $2{\times}2$ cm swelling over the right temporal region. Despite routine treatment for TMD, the swelling increased in size. Computed tomography and magnetic resonance imaging of the brain and TMJ revealed an erosive tumor of the temporal bone involving the TMJ which was displacing the temporal lobe. Surgical excision was done and the tumor removed completely. Histopathological examination was consistent with a GCT. No clinical or radiological recurrence was detected 10 months post-surgery.

Natural Products as Sources of Novel Drug Candidates for the Pharmacological Management of Osteoarthritis: A Narrative Review

  • Osteoarthritis is a chronic degenerative articular disorder. Formation of bone spurs, synovial inflammation, loss of cartilage, and underlying bone restructuring have been reported to be the main pathologic characteristics of osteoarthritis symptoms. The onset and progression of osteoarthritis are attributed to various inflammatory cytokines in joint tissues and fluids that are produced by chondrocytes and/or interact with chondrocytes, as well as to low-grade inflammation in intra-articular tissues. Disruption of the equilibrium between the synthesis and degradation of the cartilage of the joint is the major cause of osteoarthritis. Hence, developing a promising pharmacological tool to restore the equilibrium between the synthesis and degradation of osteoarthritic joint cartilage can be a useful strategy for effectively managing osteoarthritis. In this review, we provide an overview of the research results pertaining to the search for a novel candidate agent for osteoarthritis management via restoration of the equilibrium between cartilage synthesis and degradation. We especially focused on investigations of medicinal plants and natural products derived from them to shed light on the potential pharmacotherapy of osteoarthritis.

Functional Anatomy of the Temporomandibular Joint and Pathologic Changes in Temporomandibular Disease Progression: A Narrative Review

  • The temporomandibular joint (TMJ) is one of the most unique joints in the human body that orchestrates complex movements across different orthogonal planes and multiple axes of rotation. Comprising the articular eminence of the temporal bone and the condylar process of the mandible, the TMJ integrates five major ligaments, retrodiscal tissues, nerves, and blood and lymph systems to facilitate its function. Cooperation between the contralateral TMJ and masticatory muscles is essential for coordinated serial dynamic functions. During mouth opening, the TMJ exhibits a hinge movement, followed by gliding. The health of the masticatory system, which is intricately linked to chewing, energy intake, and communication, has become increasingly crucial with advancing age, exerting an impact on oral and systemic health and overall quality of life. For individuals to lead a healthy and pain-free life, a comprehensive understanding of the basic anatomy and functional aspects of the TMJ and masticatory muscles is imperative. Temporomandibular disorders (TMDs) encompass a spectrum of diseases and disorders associated with changes in the structure, function, or physiology of the TMJ and masticatory system. Functional and pathological alterations in the TMJ and masticatory muscles can be visualized using various imaging modalities, such as cone-beam computed tomography, magnetic resonance imaging, and bone scans. An exploration of potential pathophysiological mechanisms related to the TMJ anatomy contributes to a comprehensive understanding of TMD and informs targeted treatment strategies. Hence, this narrative review presents insights into the fundamental functional anatomy of the TMJ and pathological changes that evolve with TMD progression.

편측성 측두하악관절장애 환자에서 골스캔을 이용한 교합안정장치 치료효과 예측 (Prediction of Splint Therapy Efficacy Using Bone Scan in Patients with Unilateral Temporomandibular Disorder)

  • 목적: 골스캔이 측두하악관절장애의 예후 예측에 유용한지 알려져 있지 않다. 이 연구의 목적은 편측성 측두하악관절장애 환자에서 예후 예측 인자로서 유용한 골스캔 소견은 무엇인지 알아보는 것이었다. 대상 및 방법 : 2005년 1월부터 2007년 7월에 걸쳐서 편측성 측두하악관절장애로 진단받고 치료 전 골스캔을 시행한 55명의 환자(남:여=9:46; 나이, $34.7{\pm}14.1$세)들을 대상으로 하였다. 양측의 측두하악관절과 두정골 영역에서 $13{\times}13$ 픽셀 크기의 정사각형 관심영역을 그려서 Tc-99m HDP섭취를 정량화하였다. 측두하악관절 섭취비는(측두하악관절 카운트 - 배후영역 카운트) / 배후영역 카운트로 계산하였고 이환된 측두하악관절에 대한 비대칭지표는 이환된 관절과 비이환된 관절 섭취비의 비로 정의하였다. 교합안정장치 시행 6개월 후 임상적 평가를 통해 호전군과 비호전군으로 나누고 골스캔 소견이 이러한 예후에 따라 어떻게 다른지 평가하였다. 결과: 46명의 환자가 호전되었고 9명의 환자는 증상의 호전이 관찰되지 않았다. 호전군과 비호전군간에 이환관절의 측두하악관절 섭취비, 반대편 정상관절의 측두하악관절 섭취비,그리고 비 대칭지표의 유의한 차이는 관찰되지 않았다(p>0.05). 그러나 소그룹 분석에서 이환 측두하악관절에 Tc-99m HDP의 섭취가 증가한 환자들에서는 비대칭지표에 의해 환자군이 구분되었다. 즉, 호전군의 비대칭지표가 비호전군에 비하여 유의하게 증가되어 있었다($1.32{\pm}0.35$ vs. $1.08{\pm}0.04$, p=0.023). 결론: 편측성 측두하악관절장애 환자에서 Tc-99m HDP 골스캔은 시각 분석에서 이환 관절의 섭취가 증가되어 있는 경우 교합안정장치 치료에 대한 예후를 예측할 수 있는 것으로 보인다.
