An Ethnographic Research Study on Childbearing Process of Mother with Children in Korea (자녀를 둔 어머니의 출산과정 경험)

  • The childbearing process is a sociocultural phenomenon of a woman who gives birth to a child as well as a biological phenomenon. The purpose of this ethnographic research study was to explore the experience of childbearing process of mothers with children from pregnancy to the 3 months postpartum in Korea and to understand deeply the perspectives of childbearing women reflected on Korean sociocultural values. A convenient sample of 10 childbearing women were observed from January to October 2000 through field work in Seoul, Korea. Data analysis was accomplished under ongoing process. The results of this study were as follows : The mothers with children experienced self-reflection, family relation, and physical adaptation during pregnancy. In self-reflection, all mothers experienced universality and diversity in their self-discovering process. The universal experiences were maturation, life with family and priority on maternal value between being a mother and a woman. The diverse experiences were taking a dual role of working mother, emotional drift of a resigned mother, and disheartened life of a mother who has two daughters. In family relation, the foundation of the new marital relationship were attained during childbearing process and sexual life were changed for the benefit of a healthy mother and a healthy baby. All mothers established friendly relations with their mothers, but established friendly or conflicting or constraining relations with their mother-in-laws due to husband based family culture. In physical adaptation, the informants endured well the physical discomfort and recognized general appearance change. Also maternal-fetal interaction occurred and mothers realistically felt motherhood and accepted themselves as mother-to-be. The mothers prepared for the best delivery, look for a safe childbirth center, newborn goods, endorsed family coping during hospitalization and responded labor pain to make it more endurable, less painful, fast passed owing to labor recognition of the natural process to be a mother. After childbirth, they felt emancipation, satisfaction, accomplishment, more easiness, actually feeling as mother-to-be, emptiness, and showed response to the sex of newborn. Their Sanhujori practice was different according to the Sanhujori environment including provider, place, time in postpartum and reflected on Sanhubyung. The mothers felt actually mother-to-be and happiness during lactation regardless of feeding pattern. These mothers had a different maternal image about rearing subjecthood through their child-rearing experience. But all mothers felt need for family support and social support. The universal rearing response were actual feeling of mother-to-be, a strenuous experience, a pride on child-rearing, confusion, reflecting marital relationship, and wondering rivalry among children. In conclusion, mother of all with children went through self-discovery, self-reflection and made connections with the family as a mother and as a woman simultaneously during the childbearing process. Therefore it is suggested when harmony and balance between a mother and a woman is accomplished, the woman will lead a healthy and high quality of life. Also, this study sought to confirm the sociocultural factors affecting the childbearing process from the perspectives of the women with children. Therefore health care providers must understand deeply the childbearing women with children based on this finding of and try a integrative approach with new ideology of maternity with biocultural perspectives in a clinical setting.

Optimization of Hot-Water Extraction Condition of Garlic Using a Response Surface Methodology (반응표면분석법을 이용한 마늘 열수추출조건의 최적화)

  • Much research is conducted on the biological activities of medicinal herbs, traditional plants, and agricultural products, cultivated in Korea. This study focused on optimization of hot-water extraction methods for such products, by analyzing and monitoring extraction characteristics using a response surface methodology. We found that the total phenolics contents, electron-donating abilities, and nitrite-scavenging abilities of extracts were significantly affected both by the solvent used for extraction, and by the nature of the particular herb or plant under study. The extraction efficiencies of valuable ingredients such as alliin, allicin, and total thiosulfinate, were greatly affected by extraction temperature, but not by extraction time or the solvent used. We elicited a regression formula for each variable. We first entered the optimal values of all extraction conditions giving active ingredients into the model. Next, we entered the optimal values of all extraction conditions favoring the retention of valuable antioxidant characteristics. Finally, we entered processing factors into the model. Overall, the optimal extraction was at $80^{\circ}C$ for 3.5 hr with 8.5 ml of solvent/g of sample. The predicted values of each variable were similar to the actual values.

A Study on the Creation and Maintenance of Laboratory Notebooks in Scientific Laboratories of a University (자연과학분야 대학실험실에서의 연구노트 작성 및 관리에 관한 연구)

  • This study investigated the perceived significance of laboratory(lab) notebooks and the creation and maintenance of them from the perspectives of graduate students and professors in science fields. Semi-structured interviews of three professors and four graduate students were conducted. The interviewees were from three university laboratories of physics, chemistry and biological science. While professors perceived administrative continuity to be a reason for creating lab notebooks, students valued lab notebooks as a memory, and emphasized learning professional knowledge and skills through the recordkeeping practice. Students understood the ownership of lab notebooks and professors wanted to keep lab notebooks until they retire. Interviewees focused more on the content of lab notebooks rather than the form and the structure. They were also not willing to adopt an electronic lab notebook system. Students learned how to create lab notebooks through a course, their current or former academic advisors, seniors in the lab or well-written sample lab notebooks. In particular, students have learned how to create lab notebooks within their laboratories, which are considered to be communities of practice. In this sense, professors responsible for managing laboratories play an important part in teaching students the recordkeeping practice. University archives should also be more concerned with managing and preserving lab notebooks created from their universities.

Enhancement of Antioxidation Effect of Platycodon grandiflorum with Vitamin C on the DLPC Liposomes (DLPC Liposome에 미치는 도라지 추출성분의 비타민 C 첨가에 의한 항산화력 상승효과)

  • The effect of antioxidant activity of Platycodon grandiflorum (PG) on the liposomal phospholipid membrane was investigated by spectrophotometry. Membrane oxidation causes damage to the membrane fluidity and permeability. It brings further destruction to the sustenance of biological homeostasis. In addition, it is related to several diseases, aging and carcinogenesis. The sample PG was extracted and fractionated to five different types; butanol (PGMB), ethylacetate (PGMEA), ethylether (PGMEE), hexane (PGMH) and methanol (PGMM). The oxidation indices of PGMEA and PGMEE fractions in oxidized dilinoleoylphosphatidylcholine (DLPC) liposomes had stronger antioxidant activities than that of ${\alpha}$-tocopherol and were similar to antioxidant activities compared with butylated hydroxy toluene (BHT), a well-known potent antioxidant, in oxidized DLPC liposomes. The oxidation indices of PGMM extract, PGMB and PGMH fractions exhibited weak antioxidant activity compared with ${\alpha}$-tocopherol in oxidized DLPC liposomes. The oxidation indiex of PGMEE fractions added with vitamin C showed even strong antioxidant activity in the oxidized DLPC liposomes. The oxidation activity of BHT with vitamin C also proved to be stronger than BHT without vitamin C. Therefore vitamin C evidently helps to improve the effect of antioxidant in DLPC liposomes. These results indicate that potentially bioactive substances in PGMEE fraction has a function as potent antioxidant against phospholipid membrane oxidation.

Characteristics of Constructed SPR (Surface Plasmon Resonance) Sensor System for the Detection of Salmonella and hIgG Antigen-Antibody Reaction. (살모넬라와 면역글로블린(hIgG)의 항원-항체반응 감지를 위한 표면 플라즈몬 공명형 센서시스템의 특성)

  • Surface Plasmon Resonance (SPR) sensor system, has rapid response and high sensitivity, can be applicable for detecting reaction times of many biospecific interactions. A SPR sensor system was constructed to detect the antigen-antibody reactions of salmonella and hIgG (human immunoglobulin G). Sensor chips made of gold thin film were used for detecting biological bindings of antigen and antibody reactions. The antigen and antibody reactions for salmonella and hIgG were carried out with various time intervals to observed characteristics of these reactions using SPR sensor system. The resonance angle shift changes were clearly observed at the time of salmonella or hIgG antibody injection into sample cell since each antibody was self-assembled on gold chip surface of the sensor. It was found that the antibodies of salmonella and hIgG reacted with its sensor chip surface in 10 minutes and 60 minutes respectively. And the antigens of both salmonella and hIgG were bound to its antibody within 1 minute.

Optimization of SELDI-TOF MS for Peptide Profiling of Sorghum Seed (수수종자의 펩타이드 분석을 위한 SELDI-TOF MS 최적화 연구)

  • For accurate analysis of low molecular peptides using SELDI-TOF MS (surface enhanced laser desorption/ionization time of flight mass spectrometry), the optimized analytical conditions should be established for a specific biological sample. This study was conducted to optimize SELDI-TOF MS analytical conditions for profiling low molecular peptide below 10 kDa presented in sorghum seeds. Analytical conditions were as follows; (1) protein chips: CM10 (weak cation exchanger) and Q10 (strong anion exchanger), (2) dilution factors of binding buffer: 1/2, 1/5, 1/10, 1/20, 1/50, 1/100, and 1/200, (3) the stringency of Q10 binding buffer: 10 mM and 100 mM, and (4) protein extraction buffers: sodium borate, sodium borate + acetone, phenol, and TCA buffers. Optimum dilution factors were selected as 1/20 and 1/50 in both protein chips, CM10 and Q10. Low stringency of Q10 binding buffer (10mM) detected more peptide peaks than high stringency (100 mM). Selected protein extraction buffers of sorghum seed for SELDI-TOF MS analysis was the sodium borate buffer in the range of 2~10 kDa, while the phenol buffer was more suitable in the range of 10~20 kDa.

Diversity and Biomass of Benthic Diatoms in Hampyeong Bay Tidal Flats (함평만 갯벌 저서규조류의 다양성과 생물량)

  • The diversity and biomass distribution of benthic diatom flora at tidal flats of Hampyeong Bay were studied from 2006 to 2013 as a part of KLTER Program. A total of 83 species (77 strains in 2006, 65 strains in 2007, 41 strains in 2008, 45 strains in 2009, 54 strains in 2010, 55 strains in 2011, 56 strains in 2012 and 40 strains in 2012) were identified as benthic diatoms of Hampyeong Bay tidal flats. The most dominant species were Paralia sulcata and Cyclotella litoralis. Cyclotella sp., Diploneis sp., Entomoneis alata, Gyrosigma sp., Navicula abunda, Navicula gregaria, Navicula spp. and Nitzschia palea were the other common species which contributed to the high benthic diatom biomass in the Bay. The diversity of benthic diatoms varied according to the sample stations and seasons. The highest diversity was observed in August samples. The range of chlorophyll- a concentration in sediments of tidal flats for 8 years was 21~65 mg $m^{-2}$. The standing crops of benthic diatoms varied according to the studied stations and sampled seasons. The number of taxa and standing crops showed decreasing tendency year after year from all sampled stations. The distributions of standing crops and cell volumes of benthic diatoms showed similar pattern. The taxa and biomass of benthic diatoms showed low correlation coefficients with temperature variables expressed as the following equations $Y=-0.0208X^2+0.5264X+19.529(r^2=0.0269)$ and $Y=-0.9181X^2+27.011X+310.07(r^2=0.0797)$ respectively.

User experience of MV2000-MT (SU:M2)® as a Mechanical Ventilator: A Comparative Clinical Study on Usability, Safety, and Medical Staff Satisfaction

  • In the present study, we aimed to demonstrate that MV2000-MT(SU:M2)® (MV, MEK-ICS, Paju, Korea), a domestic ventilator, is not inferior in terms of usability, safety, and medical staff satisfaction as compared to Hamilton G5 (G5, Hamilton Medical AG, Rhäzuns, Switzerland). A total of 39 patients who applied MV (group M) or G5 (group H) were included in the study sample. Usability was evaluated by the following factors: the number of alarm errors, replacement requirement of breathing circuit, replacement requirement of a right-angle connector, and ease of ventilator weaning. For safety evaluation, the number of ventilator replacements due to malfunction of the ventilator was evaluated. Items for medical staff satisfaction survey were as follows: the number of MV and G5 uses, hardware, and software assessment. In the usability evaluation, the replacement requirement of the right-angle connector was lower in Group M than in Group H (mean ± standard deviation, Group M: 7.39 ± 6.72, Group H: 14.19 ± 10.24, p = 0.021); however, the evaluations of other parts were not significantly different between the two groups. The number of ventilator replacements due to a malfunction of the ventilator did not differ between two groups. The number of MV and G5 uses was 3.0 [3.0-4.0] and 10.0 [5.0-10.0] (median [interquartile range], p < 0.001). Overall, the mean medical staff satisfaction score of Hamilton G5 was higher than that of MV2000-MT(SU:M2)®. The usability of MV is comparable to that of G5. However, medical staff satisfaction with Hamilton G5 was higher than that with MV2000-MT(SU:M2)®, and this difference could be due to the difference in the number of uses. In order to improve the penetration rate of the domestic mechanical ventilator, it is necessary to find ways to increase familiarity of medical staff with domestic mechanical ventilators.

Novel Early Predictor of Acute Kidney Injury after Open Heart Surgery under Cadiopulmonary Bypass Using Plasma Neutrophil Gelatinase-Associated Lipocalin

  • Background: Open heart surgery using cardiopulmonary bypass (CPB) is considered one of the most frequent surgical procedures in which acute kidney injury (AKI) is a frequent and serious complication. The aim of the present study was to evaluate the efficiency of neutrophil gelatinase-associated lipocalin (NGAL) as an early AKI biomarker after CPB in cardiac surgery (CS). Methods: Thirty-seven adult patients undergoing CS with CPB were included in this retrospective study. They had normal preoperative renal function, as assessed by the creatinine (Cr) level, NGAL level, and estimated glomerular filtration rate. Serial evaluation of serum NGAL and Cr levels was performed before, immediately after, and 24 hours after the operation. Patients were divided into two groups: those who showed normal immediate postoperative serum NGAL levels (group A, n=30) and those who showed elevated immediate postoperative serum NGAL levels (group B, n=7). Statistical analysis was performed using Statistical Package for the Social Sciences version 18. Results: Of the 37 patients, 6 (6/37, 16.2%) were diagnosed with AKI. One patient belonged to group A (1/30, 3.3%), and 5 patients belonged to group B (5/7, 71.4%). Two patients in group B (2/7, 28.5%) required further renal replacement therapy. Death occurred in only 1 patient (1/37, 2.7%), who belonged to group B. Conclusion: The results of this study suggest that postoperative plasma NGAL levels can be used as an early biomarker for the detection of AKI following CS using CPB. Further studies with a larger sample size are needed to confirm our results.

Does performing high- or low-risk coronary artery bypass graft surgery bias the assessment of risk-adjusted mortality rates of hospitals? (관상동맥우회로술의 위험 수준이 병원내사망률 평가 결과에 미친 영향 분석)

  • The purpose of this study was to analyze whether nonemergency, isolated coronary artery bypass graft (CABG) surgery for high- or low-risk patients biases the assessment of the risk-adjusted mortality rates of hospitals. This study used 2002 National Health Insurance claims data for tertiary hospitals in Korea. The study sample consisted of 1,959 patients from 23 tertiary hospitals. The risk-adjustment model used the patients' biological, admission, and comorbidity data identified in the claims. The subjects were classified into high- and low-risk groups based on predicted surgical risk. The crude mortality rates and risk-adjusted mortality rates for low-risk, high-risk, and all patients in a hospital were compared based on the rank and the four intervals defined by quartile. Also, the crude mortality rates of the three groups were compared with their 95% confidence intervals of predicted mortality rates. The C-statistic (0.83) and Hosmer-Lemeshow test ($X^2$=11.47, p=0.18) indicated that the risk-adjustment model performed well. Presenting crude mortality rates with their 95% confidence intervals of predicted rates showed higher agreements among the three groups than using the rank or intervals of mortality rates defined by quartile in the hospital performance assessment. The crude mortality rates for the low-risk patients in 21 of the 23 hospitals were located on the same side of their 95% confidence intervals compared to that for all patients. High-risk patients and all patients differed at only one hospital. In conclusion, the impact of risk selection by hospital on the assessment results was the smallest when comparing the crude inpatient mortality rates of CABG patients with the 95% confidence intervals of predicted mortality rates. Given the increasing importance of quality improvements in Korean health policy, it will be necessary to use the appropriate method of releasing the hospital performance data to the public to minimize any unwanted impact such as risk-based hospital selection.
