Body Composition Variations for Cerebral Infarction Patients Classified as Male and Female in Long-term Care Hospitals

  • Indicators to quantitatively assess the physical conditions can help optimize the effectiveness of rehabilitation therapy for stroke patients. The body composition variations in the paretic and non-paretic regions of stroke patients with cerebral infarction (7 males, 31 females) were analyzed using multi-frequency bioelectrical impedance. Specifically, resistance (R), lean mass (LM), fat mass (FM), extracellular water (ECW), intracellular water (ICW), R/LM relation, basal metabolic rate (BMR)/LM relation were utilized to evaluate the paretic and non-paretic regions of subjects with cerebral infarction. These values showed significant differences in gender and paretic/non-paretic regions. R and FM were high but LM and ICW were lower in female and paralysis. ECW was lower in females than males, but there was no considerable difference between paralysis and non-paralysis in both males and females. In addition, there was an inverse distribution between R and LM in paretic and non-paretic regions, with males on the upper left and females on the lower right. Furthermore, the relationship between BMR and LM showed excellent linearity (slope: 22.17 kcal/day/kg) irrespective of gender, paralysis, and non-paralysis. An easy, non-invasive and quantitative assessment using bioelectrical impedance would provide an useful tool for evaluating patients with cerebral infarction receiving rehabilitation therapy.

  • Indicators to quantitatively evaluate the body function may help to optimize the effectiveness of rehabilitation therapy for stroke patients. In this study, we analyzed the body composition in the paretic and nonparetic regions of stroke patients with hemiplegia caused by cerebral hemorrhage (7 cases) and cerebral infarction (13 cases) using multifrequency bioelectrical impedance. Specifically, we considered fat mass (FM), fat-free mass (FFM), FFMI index (FFMI), FM/FFM relation, body cell mass (BCM), basal metabolic rate (BMR), and BMR/FFM relation to evaluate the bodily function in the paretic and nonparetic regions. These values showed considerable differences according to grades determined by the stroke causes and the paralysis status. In the paretic regions, the FFM, FFMI, BCM, and BMR were low and the FM was high. In contrast, the nonparetic regions showed a high FFM and low FM. Furthermore, the paretic and nonparetic regions of all patients suitably fit a linear relation (slope: 22.17 kcal/day/kg) between BMR and FFM. Therefore, bio-electrical impedance measurements can be very useful to quantitatively assess paretic and nonparetic regions in hemiplegic stroke patients.

External cross-validation of bioelectrical impedance analysis for the assessment of body composition in Korean adults

  • Bioelectrical impedance analysis (BIA) models must be validated against a reference method in a representative population sample before they can be accepted as accurate and applicable. The purpose of this study was to compare the eight-electrode BIA method with DEXA as a reference method in the assessment of body composition in Korean adults and to investigate the predictive accuracy and applicability of the eight-electrode BIA model. A total of 174 apparently healthy adults participated. The study was designed as a cross-sectional study. FM, %fat, and FFM were estimated by an eight-electrode BIA model and were measured by DEXA. Correlations between BIA_%fat and DEXA_%fat were 0.956 for men and 0.960 for women with a total error of 2.1%fat in men and 2.3%fat in women. The mean difference between BIA_%fat and DEXA_%fat was small but significant (P < 0.05), which resulted in an overestimation of $1.2{\pm}2.2$%fat (95% CI: -3.2-6.2%fat) in men and an underestimation of $-2.0{\pm}2.4$%fat (95% CI: -2.3-7.1%fat) in women. In the Bland-Altman analysis, the %fat of 86.3% of men was accurately estimated and the %fat of 66.0% of women was accurately estimated to within 3.5%fat. The BIA had good agreement for prediction of %fat in Korean adults. However, the eight-electrode BIA had small, but systemic, errors of %fat in the predictive accuracy for individual estimation. The total errors led to an overestimation of %fat in lean men and an underestimation of %fat in obese women.

대전지역 성인여성의 BIA와 신체계측치에 의한 체지방율 비교 연구 (Comparison of the Estimations of Body Fat by Bioelectrical Impedance Analysis and Anthropometric Measurements in Women in Daeieon)

  • Age-dependent changes in body fat can often be observed in normal population. A series of indirect body fat estimates, such as hydrodensitometry, Bioelectrical Impedance Analysis(BIA), and anthropometry equation for body fat, have been developed. The BIA made it possible to analyze body fat mass more related to hydrodensitometry than anthropometry. This study is to compare the body composition analysis between bioelectrical impedance analysis(BIA) and anthropometric measurements of women. The subjects were a group of Daejeon residents including 32 young-aged women($21.50{\pm}1.44$), 30 middle-aged women($50.33{\pm}5.27$), and 40 old-aged women($69.22{\pm}5.74$). We used BIA(inbody 3.0, Biospace Korea) to determine body fat and other body composition. We also measured weight, height, circumference for 12 parts, and skinfold thickness for 9 parts of all subjects' body. The results are as follows: The subjects' height by the age group were $161.74{\pm}0.94cm$ in the young-aged women, $154.16{\pm}1.09cm$ in the middle-aged, and $148.60{\pm}0.78cm$ in the old-aged respectively. BMI were, in order, $21.68{\pm}0.49,\;22.87{\pm}0.89,\;and\;23.85{\pm}0.55$. Relative body fat determined by BIA was, also in order, $29.06{\pm}0.92%,\;26.35{\pm}1.02%,and\;29.35{\pm}1.07%$. Circumference and skinfold that showed the highest correlation with body fat by BIA was waist in the young-aged(r=0.738) and bast in the middle- and old-aged(r=0.844, r=0.804), and triceps in the young-and old-aged(r=0.538, r=0.798), and subcostal in the middle-aged(r=0.872). Body fat Estimations by BIA were the highest correlation with Caucasian women's equation(r=0.588) in young-aged women, Siri's equation with Durnin & Womesley's body density measurement(r=0.875) in middle aged women and Caucasian women' equation(r=0.872) in old aged women. We need to develop specific anthropometric equations based on sex and age to determine body fat.

임피던스 분석을 활용한 사상인의 신체계측 연구 (Analysis on Physical Traits of Sasang Types Using Bioelectrical Impedance Analysis)

  • The purpose of this study was to measure circumference of four body parts of each Sasang groups with Bioelectrical Impedance Analysis (BIA), and evaluate its usefulness for Sasang type diagnostic test. We obtained the circumference measures of neck (CN), chest (CC), waist (CW), and hip (CH) of 91 college students using BIA and standardized by sex and age. The validity of Sasang group prediction with CC was evaluated. The Tae-Eum group (104.87${\pm}$5.19, 108.47${\pm}$4.96, 116.00${\pm}$9.39, 110.4${\pm}$5.29. neck, chest, waist and hip, respectively) has significantly (p<0.001) bigger circumferences than So-Yang (97.79${\pm}$3.82, 100.02${\pm}$4.21, 101.02${\pm}$7.88, 101.95${\pm}$4.06) and So-Eum (95.63${\pm}$4.67, 96.93${\pm}$4.80, 97.63${\pm}$7.72, 99.86${\pm}$4.40) groups at very four measures. Discriminant functions with CC can be used for Tae-Eum Sasang type diagnostic test with 91.2% of overall Percentage Correctly Predicted (PCP). Tae-Eum type-specific sensitivity was 73.3% and Tae-Eum type-specific specificity was 94.7%. The strength and weakness of measurement and standardization methods for Sasang type diagnostic test, and the methods for the standardization of physical traits with sex and age were discussed. Our study showed distinctive physical features of Tae-Eum type and the usefulness of BIA for the Sasang type diagnosis.

혈액투석 환자에서 Bioelectrical Impedance Analysis를 활용하여 측정한 과수분량과 근육량 감소와 사망률의 상관관계 (Relative Association of Overhydration and Muscle Wasting with Mortality in Hemodialysis Patients: Assessment by Bioelectrical Impedance Analysis)

  • 목적: 혈액투석 환자에서 체수분 상태는 중요한데 혈액투석 이후의 과수분 상태는 고혈압, 부종, 심혈관계 합병증을 유발할 수 있다. 혈액투석 환자의 근육량 평가 및 체수분 상태를 측정할 수 있는 BIA 검사를 이용하여 혈액투석 환자에서 이환율, 생존율과 관련된 인자를 알아보고자 하였다. 방법: 본 연구는 후향적 연구로 2010년 1월부터 2015년 5월 30일까지 BIA를 측정한 말기 신부전으로 진단받고 투석을 3개월 이상 시행한 환자를 대상으로 하였으며, 2017년 6월 30일까지 추적 관찰하였다. 혈액투석 환자에서 시행하는 기본 혈액 검사와 BIA data (ECW, ICW, TBW, SLM, FFM, SMM 등)를 참조하여 환자의 이환율, 생존율 등을 분석하였다. 결과: 사망군과 생존군에서 SLM, SMM, FFM의 차이는 없었다. 생존군에서 사망군보다 나이, C-반응단백 수치가 낮았고, 혈청 알부민, 부갑상샘호르몬, 인이 높았으며, 총 체액량(TBW) 중 세포외액량(ECW)의 비율은 사망군에서 좀 더 높은 결과를 보였다($0.39{\pm}0.01$ vs. $0.40{\pm}0.01$; p < 0.05). 과수분군을 ECW/TBW ratio > 0.4로 정의하였을 때 Kaplan-Meier 생존분석에서 과수분군에서 통계적으로 유의하게 사망률이 높았다. 결론: 혈액투석 환자에서 BIA로 측정한 근육량 감소과 과수분 상태는 환자의 사망률과 이환율과 관련된 중요한 인자로 생각되며, 근육량 감소보다는 과수분 상태가 사망률과 좀 더 밀접한 관련이 있을 것으로 생각된다.

Body composition: A predictive factor of cycle fecundity

  • Objective: To study the effect of body composition on reproduction in women with unexplained infertility treated with a controlled ovarian hyperstimulation and intrauterine insemination programme. Methods: This prospective observational study was conducted on 308 unexplained infertile women who were scheduled for a controlled ovarian hyperstimulation and intrauterine insemination programme and were grouped as pregnant and non-pregnant. Anthropometric measurements were performed using TANITA-420MA before the treatment cycle. Body composition was determined using a bioelectrical impedance analysis system. Results: Body fat mass was significantly lower in pregnant women than in non-pregnant women ($15.61{\pm}3.65$ vs. $18.78{\pm}5.97$, respectively) (p=0.01). In a multiple regression analysis, body fat mass proved to have a stronger association with fecundity than the percentage of body fat, body mass index, or the waist/hip ratio (standardized regression coefficient${\geq}0.277$, t-value ${\geq}2.537$; p<0.05). The cut-off value of fat mass, which was evaluated using the receiver operating characteristics curve, was 16.65 with a sensitivity of 61.8% and a specificity of 70.2%. Below this cut-off value, the odds of the pregnancy occurrence was found to be 2.5 times more likely. Conclusion: Body fat mass can be predictive for pregnancy in patients with unexplained infertility scheduled for a controlled ovarian hyperstimulation and intrauterine insemination programme.

체성분 분석용 칩 설계 (A Chip Design of Body Composition Analyzer)

  • 본 논문에서는 신체 임피던스 측정법(Bioelectrical Impedance Analysis, 이하 BIA)을 기초로 한 체지방 측정 칩 설계에 대한 내용을 서술하였다. 제안된 회로는 인체에 전류 신호를 인가하는 회로, 인체를 통해 나온 전압 신호를 측정하는 회로, 회로의 동작을 제어하는 마이크로 콘트롤러(Micom), 그리고 분석프로그램이 내장된 메모리(SRAM, EEPROMs) 의 모든 기능을 하나의 칩에 집적하였다. 특히 정밀한 인체 임피던스 측정을 위하여 다주파수 동작이 가능한 대역통과필터(Band Pass Filter, BPF)를 설계하였다. 또한, 설계된 대역통과필터는 weak inversion 영역에서 동작하기 때문에 면적과 전력소모를 줄일 수 있었다. 그리고 측정부분 회로의 성능을 개선하기 위해서 차동차이증폭기(Differential difference amplifier, DDA)를 이용한 새로운 전파정류기(Full wave rectifier, FWR)를 설계하였다. 또한 이 회로는 마지막 단에 연결될 아날로그-디지털 변환기(ADC)의 설계에 대한 부담을 덜어주는 장점도 있다. 이 칩의 시제품은 CMOS 0.35um 공정을 이용하였고 전력소모는 모든 주파수에서 6mW 이며 전원전압은 3.3V이다. 전체 칩의 크기는 $5mm\times5mm$ 이다.
