MR Imaging of Carpal Tunnel Syndrome : The Usefulness of MRI in Treatment Decisions (수근관 증후군의 자기공명 영상 : 치료 결정의 유용성)

  • Backgrounds : Carpal tunnel syndrome (CTS) is a common condition that is usually diagnosed by electrophysiologic studies. However, CTS provide limited information to determine the causes of CTS and to choose the treatment method. We evaluated diagnostic sensitivity of MR imaging and treatment decisions by MR imaging in electrodiagnosed CTS. Methods : 14 patients (26 wrists) with electrodiagnosed CTS were studied using MR imaging. In 26 wrists for which axial T1 & T2 weighted images were obtained at 1.5T with a decided wrist coil. Previously described MR imaging of CTS such as increased median nerve signal, flattening of median nerve, reticular bowing, tenosynovitis and space occupying lesions were retrospectively evaluated. Degree of improvement was evaluated by global symptom score (GSS). The GSS rated symptoms from 0 (no symptoms) to 10 (severe) in each of five categories: pain, numbness, paresthesia, weakness/clumsiness, and nocturnal awakening. Subjects' GSS was recorded at baseline, 2 weeks, 1 month, 6 months after treatment. We decided to medical treatment that showed mainly inflammatory sign such as increased median nerve signal, tenosinovitis and to surgical treatment such as space occupying lesion, high canal pressure sign. Results : MR imaging showed that increased median nerve signal were in 20 wrists (77%), flattening of median nerve were in 6 wrists (23%), reticular bowing were in 3 wrists (12%), tenosynovitis were in 8 wrists (32%), decreased canal size in 2 wrists (7.6%), space occupying lesion were in 1 wrist (4%). A good outcome was revealed in 21 wrists by medical treatment that showed mainly increased median nerve signal, tenosynovitis. The mean GSS were 27.7 at baseline, 11.2 at 2 weeks, 11.0 at 6 months in medical treatment group. Another 5 wrist had surgical treatment shown by ganglion and high canal pressure sign such as median nerve flattening, reticular bowing, decreased canal size: 3 wrists had good prognosis, but 2 wrists (one patient) had no significant improvement due to small carpal tunnel size. Conclusions : Our results are in agreement with most previously described MR imaging signs of CTS. MR imaging plays an important role in several cases and especially in the assessment of failure of surgical treatment. Knowledge of MR findings may permit more rational choice of treatment.

  • Objectives : The aim of this study was to provide basic background information on stroke by evaluating various etiological factors on the basis that the onset time of cerebral infarction varies according to its cause. Methods : We studied hospitalized patients within 4 weeks after their ictus who were admitted at Kyunghee OMC, Kyungwon OMC, or Donguk Ilsan OMC from [month] $1^{st}$, 2005 to June $30^{th}$, 2007. We compared the general characteristics of acute stroke patients according to onset time. Results : Regardless of the onset time, the distribution of cerebral infarction patterns showed high SVO. The onset time did not show significant difference for average age, height, weight, BMI, waist measurement, hip measurement, or waist/hip ratio between onset during sleep and awakening. Patients whose stroke occurred while awake were more likely to have a past history of HTN, DM, ischemic heart disease, or atrial fibrillation but did not show significant difference according to their onset time. Conclusions : The above results show that the overall tendency of acute stage cerebral infarction patients varies according to their onset time. This study was carried out on the basis of previous findings that cerebral infarctions that occurred during sleep were more likely to have been caused by cerebral thrombosis, and strokes that occurred while awake were more likely to have been caused by cerebral embolism. However, there were no statistically relevant results, so a larger study group is needed to research the tendency of stroke patients.

Effects of Lavandula angustifolia aroma on electroencephalogram in male adults with good sleep quality and poor sleep quality (라벤더향이 수면질이 좋은 남자 성인과 수면질이 나쁜 남자 성인의 뇌파에 미치는 영향)

  • This study investigated the effects of Lavandula angustifolia (L. angustifolia) aroma on the brain electrical activity evaluated by an electroencephalogram (EEG) in the male adults since many researches were performed with females and few with males. Sleep quality of 35 male adults were analyzed by Pittsburgh sleep quality index, and they were divided into two groups of good sleep quality and poor sleep quality. EEG electrodes were attached at the frontal (F3, F4), temporal (T3, T4), occipital (O1, O2), and parietal (P3, P4) regions according to the International 10-20 system. EEG was measured for 3 min per each period of before, during, and after L. angustifolia aromatherapy. Subjects with good sleep quality showed increases in the delta power at the parietal region of both cerebral hemispheres and in the theta power at the temporal region of right hemisphere (p<0.05), and a decrease in the alpha at the parietal region of both cerebral hemispheres (p<0.05). And subjects with poor sleep quality showed increases in the delta power at the frontal region of left cerebral hemisphere and in the theta power at the frontal region of both hemispheres (p<0.05). It is concluded that L. angustifolia aroma has effects on decreasing an awakening status related-brain wave and increasing the sleep status related-brain waves in the male adults with good sleep quality, and has also effects on increasing the sleep status related-brain waves in the male adults with poor sleep quality.

  • The movie WALL-E shows the Earth which has been overrun with garbage and devoid of plant and animal life, and humans who try to restore the ecosystem. The aim of this study is to find the implications of children's education with a detailed analysis of the movie's theme. The analysis method is the interpretive analysis model that is one of the qualitative methods of data analysis based on the concept of Wolcott. First, the Earth is an abandoned planet covered in trash: the Axiom is a haven for morbidly obese and indolent people. The main area of the earth's destruction, and the captain's cabin is the place to find a clue to awakening and change. Second, with the lapse of time, the Axiom becomes a ship allowing their return to earth. The Earth, recognized as their homeland, turns in to the land of hope when people return. Third, the cleaning robot Wall-E, as a main agent of change, has changed humans as well as itself. The results showed that we found nature-friendly development, destruction of nature, the criticism of modern civilization, the sanctity of life, ecological consciousness, coexistence, symbiosis and a win-win principle in our research.

The Effects of Posture and Sleep Deprivation on Heart Rate Variability (자세와 수면 박탈이 심박 변이도에 미치는 영향)

  • Autonomic nervous system (ANS) acts as a control system functioning largely below the level of consciousness, and controls visceral functions. The activity of the ANS has been assessed by means of the heart rate variability (HRV). It has been reported that HRV is dependent on sex, age, body mass index, and smoking, etc. However, the effects of posture and sleep deprivation on HRV have rarely been reported. Objective of our work was to find out which posture is appropriate for stable HRV. We measured the number of sleep deprivation and HRV using power spectrum in six stages for 30 minutes. Increased low frequency (LF) power and high frequency (HF) power indicate enhanced sympathetic and parasympathetic activity, respectively. We determined the LF/HF ratio to minimize individual difference. It was found that sleep deprivation by awakening up subjects was affected by posture, which resulted in changes of LF/HF. Although LF/HF varied with time, it was more stable in sitting than in supine. In conclusion, we recommend sitting posture when measuring HRV because of less sleep deprivation resulting in less variation in LF/HF.

Measurement of Temporal Job Stress for Hospital Nurses using Salivary Alpha-Amylase (타액-알파아밀라제를 이용한 병원간호사의 직무스트레스 측정)

  • As contemporary society has become more complicated, specialized, and segmented, people are experiencing more diverse types of stress. In particular, while several factors associated with job stress have been examined among nurses, who belong to a professional group, the existing research has made no quantitative assessments of stress that reflect temporal differences in individuals. Therefore, the aim of this study is to understand the effects of job stress on alpha-amylase with regard to the working hours of nurses, to assess the variations in jobs stress over time, and provide basic data to improve the quality of nursing services. Ninety nurses working in three shifts in general, emergency, and intensive care wards of a university hospital in D City participated in this study. Salivary alpha-amylase (SAA) was extracted and analyzed at two-hour intervals from 07:00 to 15:00 from nurses on the day shift and from 23:00 to 07:00 from those working the night shift. The SAA level was highest between 23:00 and 01:00 for nurses in general wards ($mean{\pm}S.D.\;39.00{\pm}14.88$) and between 11:00 and 13:00 for those in both intensive care units and emergency wards ($mean{\pm}S.D.\;67.50{\pm}62.93$ and $mean{\pm}S.D.\;39.67{\pm}35.96$, respectively). The characteristic variation in SAA was significant between 23:00 and 01:00 (p < 0.01) and for those in their fifties or older (p < 0.01). The activation ratio of alpha-amylase, a stress reactant, showed an increase when the sympathetic nervous system was activated by mental stress; in addition, job stress was manifested with the effect of awakening at different time segments and at different ages among the nurses. With the aim of raising the level of service based on the nurses maintaining their mental health, it is necessary to focus sharply on the time segment for critical control and to conduct repetitive studies to determine the divisions of eustress critical values as well as to expand the population.

A Comparative Study on the Secondary School Mathematics Education of South and North Korea (남북한 중등학교 수학교육의 통합방안 모색)

  • There have recently been increasing exchanges between South and North Korea in many areas of society, involving politics, economics, culture, education. In response to these developments, research activities are more strongly demanded in each of these areas to help prepare for the final unification of the two parts of the nation. In the area of mathematics education, scholars have started to conduct comparative studies of mathematics education in South and North Korea. As a response to the growing demand of the time, in this thesis we compared the secondary mathematics education in South Korea with that in North Korea. To begin with, we examined the background of education, in North Korea, particularly predominant ideological, epistemological and teaching theoretical aspects of education in North Korea. Thereafter, we compared the mathematics curriculum of South Korea with that of North Korea. On the basis of these examinations, we compared the secondary school mathematics textbooks of South and North Korea, and we attempted to suggest a guideline for researches preparing for the unification of the mathematics curriculum of South and North Korea. As a communist society, North Korea awards the socialist ideology the supreme rank and treats all school subjects as instrumental tools that are subordinated to the dominant communist ideology. On the other hand, under the socialist ideology North Korea also emphasizes the achievement of the objective of socialist economic development by expanding the production of material wealth. As such, mathematics in North Korea is seen as a tool subject for training skilled technical hands and fostering science and technology, hence promoting the socialist material production and economic development. Hence, the mathematics education of North Korea adopts a so-called "awakening teaching method," and emphasizes the approaches that combine intuition with logical explanation using materials related with the ideology or actual life. These basic viewpoints of North Korea on mathematics education are different from those of South Korea, which emphasize the problem-solving ability and acquisition of academic mathematical knowledge, and which focus on organizing as well as discovering knowledge of learners' own accord. In comparison of the secondary school mathematics textbooks used in South and North Korea, we looked through external forms, contents, quantity of each area of school mathematics, viewpoints of teaching, and term. We have identified similarities in algebra area and differences in geometry area especially in teaching sequence and approaching method. Many differences are also found in mathematical terms. Especially, it is found that North Korea uses mathematical terms in Hangul more actively than South Korea. We examined the specific topics that are treated in both South and North Korea, "outer-center & inner-center of triangle" and "mathematical induction", and identified such differences more concretely. Through this comparison, it was found that the concrete heterogeneity in the textbooks largely derive from the differences in the basic ideological viewpoints between South and North Korea. On the basis of the above findings, we attempted to make some suggestions for the researches preparing for the unification in the area of secondary mathematics education.

  • The result of Bibliographic studies on the pathological mechanism of the sudden coma, we got the conclusion like this. 1. The sudden coma is an acute syndrome that refers to be a sudden fainting, an unconsciousness, an aphasia or a cold clammy limb, and immediately awakes or dies, and awakes in a short time, and if we awake, it doesn't leave over and above a sequela. 2. The clinical presentation of the sudden coma can be summarized as follows : The 1st is a disease raising the sudden death due to unconsciousness accompanied by wry mouth & sudden syncope with coma. The 2nd is simply the state of cold limbs. The 3rd is the meaning of the physique and symptomes of the six meridians. The last is the ancient method of expression in contrast of the beriberi. 3. The pathological mechanism of the sudden coma consists of the toxoid from outside, Qi and Xie, fatigue, damp phegm, the damage from seven emotions and the damage from five mental elements, especially the mental disorder due to the angry energy, causes the problems when the fleming-up of liver fire and the depressed of liver qi raise the physiological disorder. 4. Therapeutic methods of sudden coma are soothing the liver and remove stasis, soothing depression and circulating of the qi, calming the liver and suppressing yang. When that is early stage, at first, we must checking upward adverse flow of the qi after promoting the circulation of qi and awakening, and then, we must regulate excessive deficiency of yin yang by therapy that is based on differentiated in symptoms according to heat & cold, deficiency & excess, and use invigorating herb medicine for supporting vigour.

The Significance of Independence Park in Korean Landscape Architectural History (독립공원의 조경사적 의의)

  • This paper investigates the historical significance of Independence Park in Korean landscape architecture by examining the idea and goal, master plan and scheme, and meaning and limitation of the park The construction was announced in July and began with the Independence Arch in the middle of September, 1896. Dr. Philip Jaisohn (Suh Jai-Pil), Counselor of The Privy Office and president of the Independence newspaper, played an important role in park construction. He formally advised the arrangements and general planning of the park, but he actually played a leading role in the park as much as he designed and superintended the erection of the arch. He had the conception of a productive park for cultivating and experimenting with a variety of trees surrounding the monument. In terms of the history of Korean landscape architecture, Independence Park is important in that it is the first modem city park that was tried independently as part of the modernizing-Seoul project and the only park of which object and scheme were revealed. It also strengthens the roots of Korean modem landscape architecture by pushing Korean history of public parks into the prior time to Japanese colonial period and enriches the contents of Korean modem landscape architecture. Independence Park is the original realization of a public park because it was constructed with participation and donations from the planning stage to the construction and use. In addition, it is the goal and means to inspire the spirit of national independence and patriotism in Korean people and lead them to voluntary awakening, enlightenment, and participation. Independence Park, however, was not constructed according to Dr. Jaisohn's original intent due to the lack of funds and the rudimentary level of landscape construction technology. In addition, the location was too far from downtown and accessibility was very difficult. For these reasons, many do not consider the park as having been constructed. However, this kind of view must be changed. Other parks of the time were not detailed and decorated like typical western parks, but were left as public spaces. In other words, these parks should not be judged by comparing them to western parks. This is the same concept as that of the first parks of modem Korea being called 'park land.' These parks were more natural environments than planned gardens.

Systematic Review on the Sasang Type-specific Pathophysiological Symptoms of Sleep (사상체질별 수면 소증(素證)에 대한 체계적 고찰연구)

  • Objectives: The purpose of this study was to review previous clinical studies on underlying mechanisms of sleep-related type-specific pathophysiological symptoms among the Sasang types. Methods and Procedure: We reviewed seven research databases from December 2003 to August 2015 with the keywords Sasang typology, constitution and sleep. The Sasang type-specific sleep-related symptoms were analyzed based on seven categories, including subjective sleep quality, sleep latency, sleep duration, habitual sleep efficiency, sleep disturbances, use of sleep medications and daytime dysfunction. Results: Total of twelve studies were included in the analysis. The Tae-Yang type showed low subjective sleep quality, long sleep latency and frequent awakening, and the So-Yang type showed long sleep latency. The Tae-Eum type presented high subjective sleep quality, short sleep duration, frequent snore, toss and turn during the sleep, and insomnia and low frequency of dream. The So-Eum type showed long sleep latency in male and high frequency of dream. The Eum type combining Tae-Eum and So-Eum types had higher subjective sleep quality, longer sleep duration and higher frequency of dream than the Yang type combining Tae-Yang and So-Yang types.Conclusions This study reviewed type-specific sleep-related characteristics and discussed possible pathophysiological mechanisms involved. Differences in sleep characteristics among the Sasang types might stem from type-specific temperaments and require further study.
