AGAPE-ET: A Predictive Human Error Analysis Methodology for Emergency Tasks in Nuclear Power Plants (원자력발전소 비상운전 직무의 인간오류분석 및 평가 방법 AGAPE-ET의 개발)

  • It has been criticized that conventional human reliability analysis (HRA) methodologies for probabilistic safety assessment (PSA) have been focused on the quantification of human error probability (HEP) without detailed analysis of human cognitive processes such as situation assessment or decision-making which are crticial to successful response to emergency situations. This paper introduces a new human reliability analysis (HRA) methodology, AGAPE-ET (A guidance And Procedure for Human Error Analysis for Emergency Tasks), focused on the qualitative error analysis of emergency tasks from the viewpoint of the performance of human cognitive function. The AGAPE-ET method is based on the simplified cognitive model and a taxonomy of influencing factors. By each cognitive function, error causes or error-likely situations have been identified considering the characteristics of the performance of each cognitive function and influencing mechanism of PIFs on the cognitive function. Then, overall human error analysis process is designed considering the cognitive demand of the required task. The application to an emergency task shows that the proposed method is useful to identify task vulnerabilities associated with the performance of emergency tasks.

Studies on the Safety Assessment of Red-ginseng Radix Extract Solution for Herb-acupuncture (III) (약침용 홍삼추출액의 면역독성, 발열성 및 용혈성시험에 관한 연구)

  • According to the Medical Product Safety Administration Guidelines for safety assessment guinea pigs, mice and rats were used for antigenicity test, rabbits for pyrogenic test and hymolytic test. The results were summarized as follows; 1. The antigenicity test of Red-ginseng radix extract solution for herb-acupuncture did not show any anaphylatic shock but produced IgE in high dose group. 2. The result of the pyrogen test of Red-ginseng radix extract solution for herb-acupuncture was negative. 3. The result of the hemolytic test of Red-ginseng radix extract solution for herb-acupuncture were negative in low and middle dose group but was positive in high dose group.

  • Based on the results of Probabilistic Safety Assessment(PSA) for a Nuclear Power Plant (NPP), Common Cause Failure(CCF) events have been recognized as one of the main contributors to the risk. Also, the CCF data and estimation method used in domestic PSA models have been pointed out as an issue with respect to the quality. The existing method of MGL and non-staggered testing even widely used were considered conservative in estimating the safety and had a limited capability in uncertainty analyses. Therefore, this paper presents the CCF estimation using a new generic data source and Alpha factor method. The analyses showed that Alpha factor and staggered method are effective in estimating the CCF contribution and risk insights of reference plant. This method will be a common bases for the optimization of new design for the construction plants as well as for the updating of safety assessment on the operating nuclear power plants.

This study is aimed to develop efficient and reliable safety management program to identify control measures for high risk activities by choosing and conducting proper risk assessment methodology that addresses Risk Priority Number(RPN) of adverse effects.

  • To ensure the reliability of the safety system so that handing large quantities of hazardous materials in chemical plant is considered basic information in chemical process design. However, the reliability of the production system may be reduced when the reliability of the safety system emphasized in order to ensure the safety of chemical process. It is necessary to balance the reliability of the production system and reliability of the safety. In this study, a quantitative risk assessment was performed by selecting the furnace process, which is widely used in the chemical plant in order to suggest a way to ensure the safety and productivity of chemical process, based on the quantitative data. Quantitative risk assessment methodology have been used directed graph analysis methodology. It is possible to evaluate the reliability of the safety system and the production system. In this study, the optimum system design requirement to improve the safety and the productivity of the furnace is two-out-of-three logic for TT and PT.

  • After train fire accident in Daegue, many research on train fire safety improvement have been carrying out. Since many alternative fire safety measures can be applied in our railway system, the effect of the each safety measure must be quantified prior to the safety investment. In order to estimate the effects of each safety measure quantitatively, fault trees and event trees are constructed in this study. Results can be applied for cost-benefit analysis or sensitivity analysis for safety measures in risk assessment process.

Safety Evaluation of Korean New Car Assessment Program - Side Impact (자동차 측면 충돌 안전도 평가 분석)

  • The number of vehicle accidents related to the side impact has increased since 1990 in Korea, thus the side impact test has been performed as one of the major vehicle evaluations of the Korean New Car Assessment Program(KNCAP) in 2003. A total of 77 vehicles of eight different types(compact, small, semi-midsize, midsize, and large sedans and small, midsize, and large sports utility vehicles) were tested in side impact and side pole impact conditions. In this study, the head and thorax injury values(HICs and chest deflections) of the side impact tests performed between 2003 and 2013 were investigated in terms of vehicle type, test year, and test condition. The recent vehicles showed better safety performances(lower injury values) and similar injury patterns were obtained between side impact and pole side impact tests.

  • Genetically modified (CM) crops with agricultural traits including herbicide resistance and insect tolerance have been commercialized. The safety testing strategies conducted for food and feed ingredients from GM crops differ from those applied to food ingredients in that they are conducted to demonstrate similarity between the CM food and the appropriate non-CM comparator rather than for quantitative risk assessment. However, there are similarities in the design and conduct of the safety assessment studies between these types of studies that should be readily recognized by toxicologists. The current presentation reviews some of the basic principles of safety assessment of typical dietary ingredients and compares and contrasts them with the testing strategies applied to CM foods and products obtained from them.

Review on the Application of Statistical Methods to Maritime Traffic Safety Assessment

  • For the maritime traffic safety assessment of vessels navigating in harbor or fairway, simulation techniques by using shiphandling simulator system have been traditionally used. When designing the simulation experiments and when analyzing the simulation results, however, there has been a little systematic method. Ship-handling simulations can be regarded as a kind of statistical experiment by using ship-handling simulator system, which means that shiphandling simulation conditions should be designed statistically and that the simulation results should be statistically analyzed as well. For the safe and economic design of harbor and fairway, reasonable decisions based upon the scientific analysis of shiphandling simulation results are indispensable. In this paper, various statistical methods, such as Bayes theorem, statistical hypothesis testing, and probability distributions, are reviewed with a view to application to maritime traffic safety assessment. It is expected that more reasonable decisions on harbor and fairway design can be made from shiphandlers' view point by using statistical methods.

  • The portion of embedded software in railway signaling system is increased by the advance of computer and communication technology. And the safely requirements for railway system are standardized by int'l std., such as IEC 62278 and IEC 62279. According to these circumstances for software safety problems, we are deduced the testing items for software safety test and assessment for railway signaling. Testing tools used for embedded software are surveyed to find a feasible safety assessment architecture. And the architecture of automatic test tool is proposed to test the deduced safely testing items in this paper.
