Effects of soft occlusal appliance therapy for patients with masticatory muscle pain

  • Background: The options for stabilization appliance therapy for masticatory muscle pain include soft occlusal and hard stabilization appliances. A previous study suggested that hard stabilization appliance therapy was effective for patients with local myalgia who developed long facets on their occlusal appliances. The objective of this study was to identify patients in whom a soft occlusal appliance should be used to treat masticatory muscle pain by analyzing the type of muscle pain present and patient factors that influenced the effectiveness of this treatment. Methods: The study included 42 patients diagnosed with local myalgia or myofascial pain according to the Diagnostic Criteria for Temporomandibular Disorders Diagnostic Decision Tree. The analysis of patient factors included variables believed to be associated with temporomandibular disorders. First, a temporary screening appliance was used for 2 weeks to assess each patient for bruxism during sleep. Soft appliance therapy was then started. For each patient, the effectiveness of the appliance was evaluated according to the intensity of tenderness during muscle palpation and the treatment satisfaction score at one month after starting treatment. Results: Data from 37 of the 42 patients were available for analysis. Twenty-five patients reported satisfaction with the appliance. In logistic regression analysis, the odds ratio for reduction of facet length was 1.998. Nineteen patients showed at least a 30% improvement in the visual analog scale score. The odds ratio for local myalgia was 18.148. Conclusion: Soft appliance therapy may be used in patients with local myalgia. Moreover, patients who develop short facets on the appliance surface are likely to be satisfied with soft appliance therapy. Soft appliance therapy may be appropriate for patients with local myalgia who develop short facets on their occlusal appliance.

Buccal acrylic appliance의 임상적 적용 (THE CLINICAL APPLICATION OF BUCCAL ACRYLIC APPLIANCES)

  • 어린이의 공간유지와 교합유도의 치료를 위하여 가철성 장치는 소아치과 임상에서 일상적으로 사용되고 있다. 상악의 경우와는 달리 하악에서는 가철성 장치를 사용하는데 있어 유지력의 저하에 따른 치료효과의 감소, 환자의 불편감과 거부감으로 인한 미착용 등의 문제가 흔히 발생한다. 이것은 유치열기나 초기 혼합치열기 어린이의 경우, 하악구치부의 undercut양이 충분치 못함에도 불구하고, 여기에 협측 undercut으로부터 유지력을 얻는 각종 clasp를 적용하기 때문으로 사료된다. 또한 전통적인 가철성 장치들의 acrylic base plate가 설측에 위치하고 있어 설측 연조직이나 치조능 undercut과 많은 문제를 일으키는 현상을 볼 수 있다. 어린이들은 하악 장치의 작업모형을 위한 인상채득과정에서 술자의 지시대로 혀를 바르게 거상하지 못하는 예가 많은 것도 그 원인요소로 들 수 있다. 본 연구는 전통적인 하악의 Hawley acrylic plate로부터 clasp와 acrylic baseplate의 위치를 바꾸어 협측 base plate와 설측 clasp을 이용한 buccal acrylic appliance를 유치열기 및 초기 혼합치열기 어린이의 구강에 적용하여 그 장단점을 조사하고 환아들의 수용도와 효율성을 기존의 lingual acrylic appliance와 비교, 검토할 목적으로 시행되었다. 공간유지 및 치아이동을 필요로 하는 5개 증례를 대상으로 각 증례에 buccal acrylic appliance와 기존의 lingual acrylic appliance를 모두 적용하여, 유지력, 환자 수용도, 혀의 기능과의 조화, 치료 효과 등의 측면을 상호 비교해 보았다. 조사 결과, 조사 대상의 모든 증례에서 유지력과 환아의 수용도 측면에서 협측 장치가 설측 장치에 비해 월등히 우수한 것으로 나타났다. 또한 치료 효과의 측면에서도 협측 장치는 설측 장치와 유사한 것으로 나타났다. 본 연구를 통하여 하악에 가철성 장치를 사용할 경우, 전통적인 설측 장치와 관련된 각종 부작용을 해결하는 데에 buccal acrylic appliance가 훌륭한 대안으로서 임상적용을 추천할 만 하다고 사료되었다.

Modified Nance appliance를 이용한 매복치의 교정치료 (Impacted tooth treatment with modified Nance appliance)

  • Traction of impacted tooth often requires patient compliance to place elastics to surgically exposed impacted tooth. It can be very difficult and time-consuming. The aim of this article was to describe an impacted tooth tractor, Moon's appliance, which was modified from the Nance holding arch appliance. Moon's appliance eliminates patient compliance and generates a light continuous force. We achieved the desired treatment results for impacted tooth using this appliance. This new fixed appliance can be a reasonable alternative to conventional appliances.

  • The masticatory muscle relaxation appliance is developed to improve the malhabit of tooth clenching of the wearer. The repeated clenching of the tooth causes excessive attrition of tooth set. The intraoral appliance measures the tooth clenching pressure data and transmits the data to masticatory muscle relaxation appliance. The appliance compares the pressure data with the reference value and generates the warning signal. The relaxation appliance also stores the clenching pressure data for clenching habit analysis. The appliance is designed with a microprocessor, real time clock, nonvolatile read write memory and dual serial communication ports.

Does mini-implant-supported rapid maxillary expansion cause less root resorption than traditional approaches? A micro-computed tomography study

  • Objective: This study aimed to evaluate the volume, amount, and localization of root resorption in the maxillary first premolars using micro-computed tomography (micro-CT) after expansion with four different rapid maxillary expansion (RME) appliances. Methods: In total, 20 patients who required RME and extraction of the maxillary first premolars were recruited for this study. The patients were divided into four groups according to the appliance used: mini-implant-supported hybrid RME appliance, hyrax RME appliance, acrylic-bonded RME appliance, and full-coverage RME appliance. The same activation protocol (one activation daily) was implemented in all groups. For each group, the left and right maxillary first premolars were scanned using micro-CT, and each root were divided into six regions. Resorption craters in the six regions were analyzed using special CTAn software for direct volumetric measurements. Data were statistically analyzed using Kruskal-Wallis one-way analysis of variance and Mann-Whitney U test with Bonferroni adjustment. Results: The hybrid expansion appliance resulted in the lowest volume of root resorption and the smallest number of craters (p < 0.001). In terms of overall root resorption, no significant difference was found among the other groups (p > 0.05). Resorption was greater on the buccal surface than on the lingual surface in all groups except the hybrid appliance group (p < 0.05). Conclusions: The findings of this study suggest that all expansion appliances cause root resorption, with resorption craters generally concentrated on the buccal surface. However, the mini-implant-supported hybrid RME appliance causes lesser root resorption than do other conventional appliances.
