THE DIFFERENCE OF THE BEHAVIORAL PROBLEM AND MATERNAL REARING PATTERN BETWEEN ADOLESCENT PSYCHIATRIC PATIENTS IN PEER NEGLECT OR REJECT AND THOSE IN NON-NEGLECT - A PRELIMINARY STUDY - (외톨이 혹은 왕따 상태인 청소년 정신과 환자와 비외톨이 환자의 문제 행동 및 어머니의 양육 태도의 차이 - 예 비 연 구 -)

  • Objectives:The major goal of this study was to investigate and compare the behavior problems and mothers' rearing pattern between adolescent psychiatric patients either neglected or rejected by peer and those who were neither neglected nor rejected by peer. Methods:Adolescent psychiatric patients(N=25) performed peer neglect scale, peer reject scale. And their mothers performed the part of Child Behavior Checklist(CBCL), Maternal Behavior Research Instrument(MBRI), the scale for the maternal concern about the social life of their own patients, and the scale for maternal concern about the friendship of their own patients. According to the score of the peer neglect scale and the peer reject scale, we divided the patients into 'peer neglect-reject' group(n=15) and ‘on-loneliness’ group(n=10), and compared the scores of other scales between each group. Also, we investigated the correlation among all scales. Results:1) The scores of the part of CBCL(p<.05), depressed/anxious subscale of CBCL(p<.05) and thought problem subscale of CBCL(p<.01) in peer neglect-reject group were significantly higher than those in non-neglect group. The score of peer neglect scale was significantly correlated with that of the part of CBCL(r=.516), depressed/anxious subscale of CBCL(r=.483), thought problem subscale of CBCL (r=.651), social problem subscale of CBCL(r=.517). And the score of peer reject scale was significantly correlated with that of attention subscale of CBCL(r=.414), thought problem subscale of CBCL(r=.446), social problem subscale of CBCL(r=.531). 2) But, each group was not significantly different on the scores of MBRI, the scale for the maternal concern about the social life of their own patients, and the scale for maternal concern about the friendship of their own patients. And there were no significant correlation between the scores of peer neglect / peer reject scale and those of scales for maternal rearing pattern. Conclusion:The thought problem and depression/anxiety problem of adolescent psychiatric patients neglected or rejected by peer were estimated more higher than those of adolescent psychiatric patients neither neglected nor rejected by peer. But, from the view of maternal rearing pattern, each 2 groups seemed not to be different. Further research using more subjects will be needed.

THE RELIABILITY AND VALIDITY OF KOREAN CONNERS PARENT AND TEACHER RATING SCALE (한국어판 Conners 부모 및 교사용 평가 척도의 신뢰도와 타당도에 대한 예비적 연구)

  • Objectives:The Purpose of this study is to investigate the reliability and validity of Korean Parent and Teacher Conners' Rating Scale. Methods:Randomly selected 1st to 6th graders of the two elementary schools(N=1044) in Anyang City participated in the study. Children diagnosed with ADHD(N=23) at the child and adolescent clinic at a university affiliated hospital were included in the study for the analysis of clinical validity of the scales. Parent and teacher completed Korean Conners' Rating Scale and Korean-ADHD Rating Scale(KARS). In addition, parents completed Korean-Children Behavior CheckList(K-CBCL). Descriptive statistics, t-test, and analysis of variance were performed. Results:Scores of Korean Conners' Parents Rating Scale were significantly correlated with those of Korean Conners Teacher Rating Scale. High internal consistency reliability were demonstrated in both parent and teacher rating scales. There were significant correlations among sub-scales of Conners' Rating Scales, K-CBCL and K-ARS. Factor analyses revealed that the K-CTRS had three-factor structure (Inattention-Passivity, Hyperactivity, Conduct Problem) and the K-CPRS had five-factor structure(Impulsive-Hyperactive, Conduct Problem I, Anxiety, Psychosomatic, Conduct Problem II). Conners' Rating Scales effectively distinguish children with ADHD from children without ADHD. Conclusion:Korean Parent and Teacher Conners' Rating Scales are valid and reliable instruments that are useful for screening and identifying childhood problem behaviors. Future studies are required with a larger number of sample sizes including adolescents from various geographic regions.

A Case Report of a Patient with Obsessive-Compulsive Disorder (OCD) Treated by Korean Medical Treatments and Psychotherapy (한방치료와 심리치료를 통한 강박장애 환자 치험례)

  • Objectives The purpose of this study is to report that a patient with obsessive-compulsive disorder (OCD) significantly responds to Korean medical treatments and psychotherapy.Methods We treated the patient with Korean medical treatments (acupuncture and herbal medication) and Melonis Calyx Vomiting Therapy. We also used psychotherapy including Li-Gyeung-Byun-Qi therapy. The patient was diagnosed by DSM-IV diagnostic criteria for OCD. We used Obsessive-Compulsive Inventory-Revised-Korean (OCI-R-K), Visual Analogue Scale (VAS), Beck’s Depression Inventory (BDI), and Beck’s Anxiety Inventory (BAI) to evaluate the patient.Results OCI-R-K score was decreased from 39 points to 9 points. BDI score was decreased from 14 points to 6 points. BAI score was decreased from 25 points to 13 points.Conclusions This study suggests that Korean medical treatment is an effective way to treat patients who suffer from OCD.

Presenteeism and Absenteeism According to Health Problems on Nurses (간호사의 건강문제에 따른 프리젠티즘과 결근율의 관계)

  • Purpose: The purpose of this study was to examine the presenteeism and absenteeism in relation to the health problems of nurses. Method: Informed consent was provided before study. Data was collected from January 7 to March 6, 2008 by Stanford Presenteeism Scale-13. Data was collected from 3 hospitals in Seoul Gyeonggi area. The response rate of the questionnaires was 96.2%. Incomplete data was analyzed using SPSS program. Results: Shoulder, back and neck pain were the highest reported health problems and primary health Problem of nurses. Impaired presenteeism was ranked in this order: depression or anxiety, cold, and uterus or ovary disorder. Absenteeism was ranked in this order: uterus or ovary disorder, cold, and dermatological problem. Impaired presenteeism was significantly increasing according to the number of health problems. But Absenteeism was not. Presenteeism was significantly different by age, educational level, religion, whether or not the worker lived with their family, marital status, whether or not the worker has children, time spent in the workforce, experience of turnover, shift work, night work, and level of satisfaction with salary. But absenteeism was not significantly different given these criteria. Conclusions: Health problems of nurses can negatively effect the quality of nursing service and productivity at hospital. Therefore the hospital administrator should control the presenteeism and absenteeism through the management of health problems of nurses.

The Impact of Building Types on Fire Damage by Month

  • Statistics show that while the number of fires has decreased over the last decade, losses of human life and economic property due to fire have increased. Moreover, the number of large-scale fires that have occurred in recent years has resulted in heightened public anxiety. This study aims to identify a specific period of the year most vulnerable to fire, and fire trends, such as damage of fire to humans, to the economy, and different building types. For this purpose, we analyzed human and economic damages using statistics related to fire from 2007 to 2017 and provided a monthly distribution of fire damages both to humans and to the economy by building type. We also identified the relationship between the human damage and the economy damage, and compared the economic losses per casualty by building type. The human damage in residential buildings occupied the highest portion, whereas the economic damage of industrial buildings represented more than a half of all economic damage due to fire. The economic damage per casualty was shown highest for industrial buildings and has also increased rapidly in recent years.

Trends and Issues of Adolescent's Smart Phone Addiction Research in Korea (청소년 스마트폰 중독관련 국내 연구 동향)

  • Objectives: The purpose of this study is to examine the trend of adolescent's smart phone addiction researches from 2012 to 2014 in Korea, and to propose further study for prevention of adolescent's smart phone addiction. Methods: Subjects were 169 researches about adolescent's smart phone addiction from 2012 to 2014 in Korea, and the theme was summarized as following: variables associated with adolescent's smart phone addiction, prevention and intervention strategies of it, the status and development of adolescent's smart phone addiction scale, awareness and prevention of smart phone addiction. Results: As a result of this study, 63.3% were as the most source of researches in 2014, and master's thesis was 74.6 percent. According to the theme of research, the most effects on smart phone addition is self-control and parenting and the most relational variables with smart phone addition are anxiety and parenting. The most affected by smart phone addiction is adaption of school life and conversational breakdown. The studies about prevention of smart phone addiction and intervention examine the effect of therapy program and group consultation. Conclusions: The educational program for using smart phone should be made and applied for preventing adolescent's smart phone addiction.

A Clinical Study of Treating Autism Spectrum Disorder in Childhood (자폐 스펙트럼 장애 환아 1례에 대한 증례 보고)

  • Objectives The purpose of this study is to report the effectiveness of Korean medicine on treating autism spectrum disorder (ASD) in childhood. Methods We treated an 8-years-old patient who was diagnosed with autism (grade II) with herbal medicine, acupuncture and electric moxibustion. Every 11 months, Korean-Childhood Autism Rating Scale (K-CARS) was used to assess the patient's symptom progression. Results After 21 months of Korean medicine treatment, the child's overall health was improved. Moreover, language ability, emotional control, memory and cognitive abilities got better, as well as anxiety, agitation, and aggressive behavior related to ASD were also decreased. K-CARS score at the initial stage of the treatment was 39.5 points, which can be considered as severe autism. After 11 months of the treatment, the K-CARS score was 34.5 points which is mild-moderate autism. Conclusions Korean medicine treatment can be effective in improving symptoms of ASD. However, further studies are needed to demonstrate the treatment benefits to ASD.

  • Objectives: Attention Deficit Hyperactivity Disorder (ADHD) is a psychiatric disorder, characterized by the primary symptoms of inattention and/or hyperactivity and impulsivity. The aim of this study is to investigate the clinical efficiency of the oriental medical therapy combined with art therapy in the treatment of ADHD. Methods: This study is clinical reports of 3 patients with ADHD who are being treated with the oriental medical therapy (Herb-med, acupuncture) and art therapy. K-ARS (Korean ADHD Rating Scale) was compared before and after 1 month of treatment. Results: The Oriental medical therapy including herb-med and acupuncture is effective in improving ADHD patients. Also, art therapy is effective in decreasing patients' anxiety and tension and helpful for patients themselves to make an objective observation. Therefore, it is practically useful in the treatment of ADHD. Conclusions: The results show the oriental medical therapy in combination with art therapy is effective in the treatment of ADHD.

A Clinical Study of Art Therapy for Children Who Witnessed a Tragic Accident (충격적 사고를 목격한 아동에 대한 미술치료 사례)

  • Purpose: This study aims to examine what can be seen in children's paintings, their common characteristics, and what role art therapy plays in diagnosing the post-traumatic stress disorder in children who experienced trauma, through an art therapy program among elementary school students that witnessed the death of parents from a fire drill accident. Methods: A program of three times of art therapy was progressed among 34 children who witnessed a fire accident. Revised Children's Manifest Anxiety Scale (RCMAS) was used for comparative analysis of art therapy results. Results and Conclusion: First, children who had the possibility to develop post-traumatic stress disorder were found through art therapy. Second, an opportunity to express themselves were given to children who refused psychological tests or treatment through art therapy.

The Effects of an Active Pain Management with Structured Physiotherapy on Postoperative Pulmonary Complications and Pain among Children (안위를 고려한 통증관리중재가 수술 후 아동의 폐합병증 발생, 통증, 안위에 미치는 효과)

  • Purpose: The purpose of the study was to compare the active pain management (APM) with structured physiotherapy (SPT) with the conservative care on postoperative pulmonary complications, pain, and comfort in children under three year. Method: A non-equivalent control group, non-synchronized design study was used. A total of 64 children participated in the study. The children in the experimental group (n=32) received APM with SPT after surgical operation. After transferred to the general unit, the parents were instructed to hold the child for 30 minutes to relieve anxiety and have him/her sleep comfortably for 2 hours. Scheduled 20 minutes chest percussion was performed by the parents for 2 days: twice every 4 hours, one in 6 hours, then one every 8 hours for the rest of two days. Analgesic was administered as needed. Pain and comfort were observed and recorded by nurses using the FLACC and COMFORT Behavior Scale. Results: One child in the control group was diagnosed with postoperative pneumonia. The children in the experimental group who were received the APM with SPT reported higher scores in comfort and lower scores in pain than those in the control group. Conclusion: The findings suggest that APM with SPT can help prevent postoperative pulmonary complications and pain.
