This study was performed to elucidate the effect of acupuncture and aqua-acupuncture in dogs. Each group was divided into control (18 dogs), PC-6 (6 dogs) and BL-21 (6 dogs) group for the antiemetic effect of acupuncture, respectively. And, the animals were divided into control (18 dogs), PC-6 (18 dogs) and BL-21 (18 dogs) group for the antiemetic effect of aqua-acupuncture with metoclopramide, respectively. 2% xylazine was injected to the all dogs to induce the emesis, and acupuncture and aqua-acupuncture treatment (0.5% metoclopramide) were applied to animals for 20 min. before xylazine injection, respectively. Emetic rates in PC-6 (33.3%) and BL-21 (16.7%) were lower than that of control group (44.4%) in acupuncture groups, respectively. Emetic rates in PC-6 (16.7%) and BL-21 (0.0%) were lower than that of control group (38.9%) in aqua-acupuncture groups, respectively. Based on these findings, it was considered that the antiemetic effects of acupuncture and aqua-puncture at BL-21 were more effective than those of PC-6 groups, and aqua-acupuncture with metoclopramide at BL-21 was the most effective among them.

Ginger (Zingiber officinale) belonging to the family Zingiberaceae is a perennial herb. It is widely distributed in tropical Asia. In India, it is cultivated mainly in Kerala, Andhia Pradesh, Uttar Pradesh, West Bengal and Maharashtra. It is one of the most common spices, which is in use since centuries for its versatile medicinal actions like antiemetic, stomachic, expectorant, anti-inflammatory, aphrodisiac etc in traditional system of medicine (Unani, Ayurveda, and Chinese medicine). It is useful for the treatment of various gastrointestinal, pulmonary, cardiovascular and sexual disorders. The phytochemical study of ginger showed the presence of many volatile oils and oleo-resins like gingerol, zinger one, zingiberol etc. Numerous experimental and clinical trials have proven ginger for its range of therapeutic activities such as antibacterial, antidiabetic, antiemetic, hypolipidaemic, hepatoprotective etc properties. The present article aims to explore traditional Unani and pharmacological activities of this herb reported till date.
Background: To evaluate the effect of prophylactic transdermal scopolamine in reducing nausea associated with postoperative epidural morphine. Methods: 30 healthy patients scheduled for cesarean section were given local anesthetics epidurally for surgical anesthesia. After delivery of the baby, transdermal scopolamine patch($Kimite^{(R)}$ Myung Moon Pharm. Co., Seoul, Korea) was applied to the study group(n=15) and placebo patch to the control group(n=15). Postoperative analgesia was provided soley with epidural morphine. Nausea was treated with metoclopramide. Results: During 24 hours postoperatively, the mean nausea score was significantly lower in the study group than in the control group. But the nausea incidence was not significantly different between the two groups. The mean number of times antiemetic drugs which were administered to patients were lower in the study group than in the control group($1.5{\pm}0.5$ vs $3.3{\pm}1.3$, p<0.05). Conclusion: Transdermal scopolamine patch provides antiemetic effect simply, continuously and safely, preventing nausea which could occur during administration of epidural morphine after cesarean section.
Background: To date, there are no controlled studies assessing the effect of metoclopramide administered epidurally to prevent nausea and vomiting associated with epidural morphine for postoperative analgesia. This study was undertaken to determine the effectiveness of continuous epidural infusion of metoclopramide, combined with epidural morphine, in reducing nausea or vomiting associated with epidural morphine and minimizing the side effects of metoclopramide. Methods: Sixty patients undergoing elective gynecologic surgery were randomly assigned to one of two study groups. Patients received continuous epidural morphine infusion (6.0 mg/day) following a bolus loading dose of 3.0 mg (Group A), or epidural mixture of morphine (6.0 mg/day) plus metoclopramide (20 mg/day) following a bolus loading dose (morphine 3.0 mg, metoclopramide 10 mg)(Group B). For the first 24 postoperative hours, incidence of nausea or vomiting, need for antiemetic therapy, level of sedation, degree of pain and pruritus, and adverse effects associated with metoclopramide were evaluated. Result: Incidence of nausea or vomiting and number of patients who required antiemetic therapy were significantly less in Group B, than in Group A (P<0.05). There were no significant differences between groups with regard to adverse effects associated with metoclopramide such as sedation, extrapyramidal reaction and other side effects (P=NS). Conclusion: We conclude simultaneous titration of morphine and metoclopramide via epidural continuous infusion following epidural bolus injection of the mixture reduces nausea or vomiting associated with epidural morphine while preventing side effects of metoclopramide.
In children and adolescents with acute gastroenteritis and other gastrointestinal disease, antiemetics are frequently used. But there are insufficient data about antiemetic use in children, so it should be used carefully. Despite some significant researches, treatment guidelines of ondansetron will be carefully presented through further investigation.
Background: To determine the effectiveness of continuous epidural infusion of droperidol, combined with epidural morphine, in reducing nausea or vomiting associated with epidural morphine and minimizing the side effects of droperidol, 48 patients undergoing elective thoracic surgery were randomly assigned to one of two study groups. Methods: Patients received continuous infusion of epidural morphine(6.0 mg/day) following a bolus loading dose of 3.0 mg(Group A), or epidural mixture of morphine(6.0 mg/day) plus droperidol(5.0 mg/day) following a bolus loading dose(morphine 3.0mg, droperidol 1.5 mg)(Group B). For the first 48 postoperative hours, the incidence of nausea or vomiting, the need for antiemetic therapy, level of sedation, and adverse effects associated with droperidol were evaluated. Results: The incidence of nausea or vomiting and the number of patients who required antiemetic therapy were significantly less in Group B than in Group A(P<0.05). There were no significant differences between groups with regard to the adverse effects associated with droperidol such as mental depression, respiratory depression and abnormal movements(P=NS). Conclusion: We conclude that simultaneous titration of morphine and droperidol via epidural continuous infusion following epidural bolus injection of the mixture reduces nausea or vomiting associated with epidural morphine while it prevents the side effects of droperidol.
Background: Antiemetic triplet therapy including dexamethasone (DEX) is widely used for patients receiving highly emetogenic chemotherapy (HEC). In Japan, the appropriate dose of DEX has not been established for this combination. Materials and Methods: To assess the efficacy and safety of increased-dose DEX, we retrospectively examined patients receiving HEC with antiemetic triplet therapy. Results: Twenty-four patients (fosaprepitant group) were given an increased-dose of DEX (average total dose: 45.8mg), fosaprepitant, and 5-HT3 antagonist. A lower-dose of DEX (33.6mg), oral aprepitant, and 5-HT3 antagonist were administered to the other 48 patients (aprepitant group). The vomiting control rates in the fosaprepitant and aprepitant groups were 100% and 85.4% in the acute phase, and were 75.0% and 64.6% in the delayed phase. The incidences of toxicity were similar comparing the two groups. Conclusions: Triplet therapy using an increased-dose of DEX is suggested to be safe and effective for patients receiving HEC.
Despite a considerable improvement in antiemetic chemotherapy and the use of antiemetic prophylaxis for decades, there are still numerous cancer patients suffering from anorexia and nausea after chemotherapy, which forces them to postpone or refuse treatment. This case report is about a 66-year-old female patient diagnosed with malignant lymphoma of the small intestine who underwent right hemicolectomy. After the tumor and the colon adjacent to it were surgically removed, the patient continued to undergo chemotherapy. Chemotherapy-induced anorexia and nausea appeared but were well treated with administration of Dokhwaljihwang-tang-gamibang. During the treatment, no adverse effect related to this Korean medicine was reported. The outcome was evaluated by the functional assessment of anorexia/cachexia therapy (FAACT); the total score increased from 85 to 130 within 12 days, and the symptoms were alleviated. This improvement might suggest the feasibility of Dokhwaljihwang-tang-gamibang to help cancer patients suffering from chemotherapy-induced anorexia and nausea.
Clebopride malate(Cm) is a new benzamide drug which has a potent central antidopaminergic activity possessing antiemetic and anxiolytic properties. A purpose of this study was to assess the feasibility of formulating sustained release preparation by dispersing a drug in hydrophilic polymeric matrices and double layered tablets(DLT), using HPMC, carbopol, PEO, PVP/VA and other polymers as a rate controlling barrier. The matrix and DLT showed optimum dissolution pattern up to 8 hours and the contents of polymer were optimized at 30% level in this preparation. After an oral administration in beagle dog, Cm concentration was determined by use of GC-ECD and pharmacokinetic parameters were calculated by Vallner's method. The AUC of DLT showed similar results and the duration of action was increased 55% compared to that of regular release dosage form. The calculated absorption rate effectiveness(ARE) and controlled release effectiveness(CRE) for DLT increased 50% compared to that of matrix, the overall effectiveness(E) of this product appears to be about 70%. in vivo effectiveness test, DLT showed significant differences from control on antiemetic action of Cm. In consequence, it was possible to conclude that double layered tablet might be a good candidate for the sustained release dosage forms.
임신시 자연 발생의 임신 구토와 철분제제 복용으로 인하여 구토 현상을 경감시키고자 항구토제를 복용하는 경우가 있다. 본 연구에서는 임신시 항구토제가 태아의 기형 발생에 영향이 있는가를 조사하기 위하여 시판 항구토제를 5 및 10ml/kg/day의 용량으로 흰쥐 태아의 기관형성기인 임신 7일에서 17일까지 매일 1회씩 투여하여 본 결과다음과 같은 성적을 얻었다. 1. 모체의 체중, 임상증상 및 황체의 수에서 대조군과 약제투여군 사이에 별다른 차이를 나타내지 않았으며 사망한 것도 없었다. 2. 태아의 체중 및 평균 태자수에서 대조군과 약제투여군 사이에 차이를 나타내지 않았으나 태아의 재흡수에서는 약제투여 농도가 높을수록 다소 증가하였다. 그러나 사망한 태아는 없었다. 3. 약제투여군에서 두정골(parietal bone), 부전골(metatarsal bone), 완전골(metacarpal bone) 및 anterior phalanges ossified center에서 골화지연(delayed ossification) 현상이 나타났다.
