Background : The Antimicrobial Stewardship Program promotes interdisciplinary interventions and targeted recommendations for the proper utilization of antibiotics. In particular, the aim of the program is to avoid indiscriminate use of broad-spectrum antibiotics based on the documented literature on the significant impact of unsystematic usage of antibiotics on the distribution of antibiotic-resistant microorganisms. To improve the care process for pneumonia treatment using antimicrobial agents, institution-level guidelines were established and disseminated at the Seoul National University Bundang Hospital in April 2016. In this study, we evaluated changes in the physicians'antibiotic prescribing patterns both before-and after-the implementation of the guidelines. Methods : The electronic medical records of inpatients who were prescribed with one or more antibiotics in May 2014 (Group A) and May 2016 (Group B) were reviewed. Data on demographic characteristics, clinical outcomes, and antibiotic prescriptions were collected and the prescription records were compared both before- and after- the implementation of the guidelines. Results : A total of 180 patients were included in the study: 77 patients in group A and 103 patients in group B. The baseline characteristics of the patients were not significantly different between the two groups. Community-acquired pneumonia was the most common diagnosis in both the groups and the difference was not significant (68.8% vs. 67.9%; p=0.67). The type of antibiotic prescriptions used for empirical treatment was not different between the two groups. The most commonly prescribed empirical antibiotics were cephalosporins, with no significant difference (p=0.31). One of the most inappropriately used antibiotics was piperacillin/tazobactam and the rate of prescription was similar in both the groups (p=0.68). The rates of appropriate empirical selection of antibiotics remained unaltered between the two groups (67.5% vs. 71.8%; p=0.53). Conclusions : Implementation of the guidelines only exhibited no significant effect on the antibiotic prescribing patterns of physicians for the treatment of pneumonia. To improve the adequate use of empiric antibiotics, more active interventions and closer monitoring of the feedbacks should be additionally considered and evaluated in future studies.
Antibiotic resistance in human pathogens is a major public health issue. Some of the resistance problem can be attributed to the transfer of resistant bacteria from animals to people and the transfer of resistance genes from animal pathogens and commensal bacteria to human pathogens. Control measures include improvements in food hygiene to reduce the spread of zoonotic bacteria to people via the food chain. However, to specifically address the issue, the medical profession must control misuse and overuse of antibiotics in hospitals and community practice. In addition, the livestock industries and their advisors must reduce and refine the use of antibiotics in animal production and replace antibiotics with alternative disease control measures as much as possible.
Objectives : Since cosmetics have been one of the help of life, unlike medicine, natural products have been used for cosmetics, generally giving the image of safety and relief compared to synthetic products. Among them, Houttuynia cordata has been known as a useful herbal medicine with antibiotic, anti-inflammatory and anti-oxidative activities. In this study, we were to use Houttuynia cordata extract on formulation of cosmetic emulsions. Methods : The effects of Houttuynia cordata extract on emulsion stability and viscoelastic properties of emulsion were measured using turbiscan and rheometer. And we assessed the anti-oxidative and antibiotic activities of Houttuynia cordata extract. Results : 1. The results of this investigation for emulsion stability showed that the stability emulsion containing Houttuynia cordata extract was decreased depending on concentration of Houttuynia cordata extract. 2. Super oxide dismutase activity was strongly dependents on concentration of Houttuynia cordata extract. 3. Houttuynia cordata extract showed good anti-microbial activity against Staphylococcus aureus. Conclusions : Houttuynia cordata extract can be effectively used in cosmetic emulsions when the relation between natural product extracts and formulation of cosmetics is elucidated.
Acceptance of antibiotic growth promoters (AGP) in agricultural animal production is rapidly disappearing. Both government regulations and consumer preference are driving this change. Producers in any country that seek export markets will be forced to give up AGP if they are to sell to the EU and many other markets. This report will first review the history of AGP use in the animal industry and the concerns about development of antimicrobial resistance. A description of the development and structure of the gut and how it is affected by AGP administration will conclude with results of studies to replace AGP with antimicrobial organic acids.
Vibrio spp. are aquatic bacteria that are ubiquitous in warm estuarine and marine environments. Especially, V. vulnificus and V. cholerae are currently known to cause potentially fatal infections in humans. This study investigated the distribution and antibiotic resistance of V. vulnificus and V. cholerae isolated from coastal areas of Gyeongsangnam-do in 2022. A total of 252 samples of water, shellfish and coastal sediment were collected from 7 locations along the coast, and 124 samples of fishery products were collected from markets. Among the 252 samples, forty-four V. vulnificus (11.7%) and fourteen V. cholerae non-O1/non-O139 (3.7%), none of which carried the ctx gene, were isolated. Out of the 124 samples, 6 (4.8%) tested positive for V. vulnificus and V. cholerae was not detected. The isolation rates of V. vulnificus and V. cholerae showed a significant correlation with environmental factors such as seawater temperature and salinity. In an antibiotic resistance test, V. vulnificus was susceptible to amikacin, gentamicin, imipenem trimethoprim/sulfamethoxazole, and ciprofloxacin, but resistant to cefoxitin (100.0%), followed by tetracycline (9.1%). Multidrug resistance was also observed. Continuous monitoring of Vibrio pathogens with water temperature and salinity is expected to help reduce the outbreaks, and rational use of antibiotic agents is needed to prevent the accession of antibiotic-resistant microorganisms in aquatic ecosystems.
Extensive use of antibiotics over recent decades has led to bacterial resistance against antibiotics, including gentamicin, one of the most effective aminoglycosides. The emergence of resistance is problematic for hospitals, since gentamicin is an important broad-spectrum antibiotic for the control of bacterial pathogens in the clinic. Previous study to identify gentamicin resistance genes from environmental samples have been conducted using culture-dependent screening methods. To overcome these limitations, we employed a metagenome-based culture-independent protocol to identify gentamicin resistance genes. Through functional screening of metagenome libraries derived from soil samples, a fosmid clone was selected as it conferred strong gentamicin resistance. To identify a specific functioning gene conferring gentamicin resistance from a selected fosmid clone (35-40 kb), a shot-gun library was constructed and four shot-gun clones (2-3 kb) were selected. Further characterization of these clones revealed that they contained sequences similar to that of the RNA ligase, T4 rnlA that is known as a toxin gene. The overexpression of the rnlA-like gene in Escherichia coli increased gentamicin resistance, indicating that this toxin gene modulates this trait. The results of our metagenome library analysis suggest that the rnlA-like gene may represent a new class of gentamicin resistance genes in pathogenic bacteria. In addition, we demonstrate that the soil metagenome can provide an important resource for the identification of antibiotic resistance genes, which are valuable molecular targets in efforts to overcome antibiotic resistance.
Polymicrobial nature of diabetic foot infection has been well documented in the literature. Initial antibiotic therapy of diabetic foot infection is usually empiric until reliable culture data is shown. This study was carried out to determine the common bacteriological flora of diabetic foot infection and antimicrobial sensitivity pattern in order to enhance possible empiric treatment. The specimens were obtained from wounds of 207 cases of diabetic foot ulcer, and the bacteriological isolation, and antimicrobial susceptibility tests of the isolates were carried out by standard microbiological methods. Staphylococcus aureus was the most common isolate, with 46.2% of recover rate among total bacterial isolated cases. Among gram-negative organisms, Pseudomonas aeruginosa was most common. Gram-positive organisms showed significant susceptibility to clindamycin, trimethoprim/sulfamethoxazole, and levofloxacin, besides vancomycin. Cefoperazone, piperacillin/tazobactam, and amikacin in addition to imipenem were most effective agents compared to gram-negative organisms. Diabetic foot infection requires use of combined antimicrobial therapy for initial management. Our results indicate that the most effective antibiotic combination for diabetic foot infection of Korean patients is clindamycin plus cefoperazone.
The purpose of this study was to investigate the antibiotic resistance pattern of E. coli and Salmonella spp. isolated from chicken feces. One hundred and forty-seven E. coli isolates showed resistance to tetracycline (95.2%), erythromycin (89.2%), ampicillin (70.1%), streptomycin (59.2%), cephalothin (56.5%), sulfamethoxazole/trimethoprim (53.7%), ciprofloxacin (57.1%), enrofloxacin (59.2%) and norfloxacin (57.1%). The multiple resistance was seen in 144 isolates (97.9%) and the rate of five, six and seven drugs resistance pattern were 20.4%, 18.4% and 16.3%, respectively. Also, the multiple resistance of E. coli to twelve drugs were seen in 1 isolates (0.7%). Fourteen Salmonella spp. showed resistance to ampicillin (50.0%), streptomycin (57.1%), erythromycin (64.3%) and tetracycline (57.1%) and the rate of two and three drugs resistance pattern were 4 isolates (28.6%), respectively. The prevalence of resistant organisms in Korea probably reflects lack of proper antibiotic policy resulting in prolonged and indiscriminate use of antimicrobial agents.

Coagulase-negative staphylococci (CoNS) are a typical group of microorganisms, and the recent advances in laboratory technology and medicine has dramatically modified their significance in medical practice. CoNS, which were previously classified as normal bacterial flora, have recently been reported to be associated with serious infectious diseases, such as surgical wound infection or periprosthetic joint infection. Representative CoNS include Staphylococcus epidermidis, S. haemolyticus, and S. saprophyticus, which are known to cause serious problems in biomaterial-based and prosthetic device infections, as well as to cause simple urinary tract infections in sexually active women. Over the last decade, the clinical isolation rate of CoNS has been increasing, and antibiotic resistance has also been occurring. This review aimed to investigate the incidence of CoNS infection and to use the results as basic data for the management of CoNS, with a focus on the isolation rate and antibiotic resistance in clinical surgery.
Amp. B의 세포막 독성을 낮추기 위하여 egg phosphatidylcholine를 사용한 리포좀계를 이용하였다. 리포좀에 포획 된 Amp.B는 Candida albicans에 대해 free drug보다 향상되거나 동일한 항생효과를 가지면서도 동시에 적혈구에 대한 세포막 독성은 현 저히 감소된 것으로 나타났다. 이러한 현상은 Ca$\pi$dida albicans의 ergosterol이나 적혈구의 ch이est terol사이에 리포좀이라는 제3의 이중막이 존재할 경우 이들 사이에서 Amp. B가 재분배하게 되어 상대적으로 적혈구에 대한 독성은 줄어드나 리포좀보다는 ergosterol에 대한 친화력이 크므로 항생효과의 면에서는 큰 변화가 없는 것으로 보인다. Candiida의 세포벽과 결합할 수 있는 효소를 리포좀 표면에 삽입하기 위하여 ${\beta}$-glucuronidase를 이용하였으나 약물전달의 상승효과는 크게 나타나지 않았다.
