Incidence and Risk Factors of Pneumonia in Hospitalized Patients with Seasonal Influenza A or B

  • Background: Most patients with influenza recover spontaneously or following treatment with an anti-viral agent, but some patients experience pneumonia requiring hospitalization. We conducted a retrospective review to determine the incidence and risk factors of pneumonia in hospitalized patients with influenza A or B. Methods: A total of 213 patients aged 18 years or older and hospitalized with influenza between January 2012 and January 2015 were included in this study. A reverse-transcriptase polymerase chain reaction assay was used to detect the influenza A or B virus in the patients' sputum samples. We collected demographic and laboratory data, combined coexisting diseases, and radiologic findings. Results: The incidence of pneumonia was higher in patients in the influenza A group compared to those in the influenza B group (68.6% vs. 56.9%), but this difference was not statistically significant. The presence of underlying respiratory disease was significantly associated with pneumonia in the influenza A group (adjusted odds ratio [OR], 3.975; 95% confidence interval [CI], 1.312-12.043; p=0.015). In the influenza B group, the white blood cell count (adjusted OR, 1.413; 95% CI, 1.053-1.896; p=0.021), platelet count (adjusted OR, 0.988; 95% CI, 0.978-0.999; p=0.027), and existence of an underlying medical disease (adjusted OR, 15.858; 95% CI, 1.757-143.088; p=0.014) were all significantly associated with pneumonia in multivariate analyses. Conclusion: The incidence of pneumonia was 65.7% in hospitalized patients with influenza A or B. The risk factors of pneumonia differed in hospitalized patients with influenza A or B.

Surface plasmon resonance 바이오센서를 이용한 재조합 B형 간염 표면항원의 정량분석 (Quantitative Assay of Recombinant Hepatitis B Surface Antigen by Using Surface Plasmon Resonance Biosensor)

  • We performed a basic experiment for rapid, on-line, real-time measurement of HBsAg by using a surface plasmon resonance biosensor to quantify the recognition and interaction of biomolecules. We immobilized the anti-HBsAg polyclonal antibody to the dextran layer on a CM5 chip surface which was pre-activated by N-hydroxysuccinimide for amine coupling. The binding of the HBsAg to the immobilized antibody was measured by the mass increase detected by the change in the SPR signal. The binding characteristics between HBsAg and its antibody followed typical monolayer adsorption isotherm. When the entire immobilized antibody was interacted, there was no additional, non-specific binding observed, which suggested the biointeraction was very specific as expected and independent of the ligand density. No significant steric hindrance was observed at 17.6 nm/$mm^2$ immobilization density. The relationship between the HBsAg concentration in the sample solution and the antigen bound to the chip surface was linear up to ca. $40\mu\textrm{g}$/mL, which is much wider than that of the ELISA method. It appeared the antigen-antibody binding was increased as the immobilized ligand density increased, but verification is warranted. This study showed the potential of this biosensor-based method as a rapid, simple, multi-sample, on-line assay. Once properly validated, it can serve as a more powerful method for HBsAg quantification replacing the current ELISA method.

The Potential Anti-HBV Effect of Amantadine in Combination with Ursodeoxycholic Acid and Biphenyl Dimethyl Dicarboxylate in HepG2 2.2.15 Cells

  • Experimental studies have demonstrated that the triple combination of amantadine (A)/ ursodeoxycholic acid (UDCA, U)/ biphenyl dimethyl dicarboxylate (DDB, D) might have a preferential antiviral effect compared with that observed in interferon-induced antiviral signal pathways, such as those of $STAT1\alpha$ and the 6-16 genes. To confirm the results, this study examined whether th signal transduction for the antiviral activity in HepG2 2.2.15 was induced dependently or independently of interferon. To accomplish this, the correlation between the $STAT1\alpha$ and 6-16 genes, and nitric oxide, for the mediation of the antiviral activity was assessed. The increase in nitric oxide in the UDCA groups suggests that the inhibition of viral gene replication was enhanced by the amantadine combinations (AU and AUD), and might be more effective if incubated for longer periods. It was found that $STAT1\alpha$ was activated by the amantadine combination, although to a lesser extent than that of $interferon-\alpha$, and the primary endpoints examined for the inhibition of gene expression (HBsAg and HBcAg) were remarkably well regulated. This suggests that the amantadine triple, or at least the double, combination had better clinical benefits than those of $IFN-\alpha$ and the nucleoside analogue single treatment. This demonstrates that the amantadine combination might be a substitute for the existing HBV therapy if the results of in vivo and in vitro studies concur.

Antioxidant and Immunoenhancement Activities of Ginger (Zingiber officinale Roscoe) Extracts and Compounds in In Vitro and In Vivo Mouse and Human System

  • Ginger (Zingiber officinale Roscoe) is traditionally used as appetite enhancer, improver of the digestive system, antithusive, anti-cold, antipyretic, analgesic, and antiinflammation. In vitro evaluation using human lymphocyte cultures showed almost similar indication with those in in vivo mouse study, NK cell lysing activity was improved significantly. Proliferation activity of B and T cells, and CD3$^{+}$ and CD3$^{+}$CD4$^{+}$T cell subset were better observed using oleoresin or gingerol and shogaol fractions. Although there were higher activities in gingerol, the improvement was almost equal to that by oleoresin. Shogaol did not show better improvement except at higher concentration. It could be concluded that treatment with single bioactive compound, such as gingerol, did not show significant effects compared to oleoresin, the crude extract. In human study, involving healthy male adult, the improvement of NK cell lysing activity was again demonstrated and even more apparent. The mechanism involved in the protection seemed to be through the antioxidant activity of gingerol. However, other mechanism underlying the improvement of NK cell lysing activity must be involved since this improvement seemed to be specifically toward NK cell activity. Since NK cells ave specific for the elimination of virus-infected cell and mutated cells, this positive effect on the immune system are very interesting. This work has also scientifically proved that the traditional beliefs that ginger had preventive effects on common cold appeared to be reasonable.

Hesperidin Induces Apoptosis by Inhibiting Sp1 and Its Regulatory Protein in MSTO-211H Cells

  • Hesperidin, a flavanone present in citrus fruits, has been studied as potential therapeutic agents that have anti-tumor activity and apoptotic effects in several cancers, but there is no report about the apoptotic effect of hesperidin in human malignant pleural mesothelioma through the specificity protein 1 (Sp1) protein. We investigated whether hesperidin inhibited cell growth and regulated Sp1 target proteins by suppressing the levels of Sp1 protein in MSTO-211H cells. The $IC_{50}$ value of hesperidin was determined to be 152.3 ${\mu}M$ in MSTO-211H cells for 48 h. Our results suggested that hesperidin (0-160 ${\mu}M$) decreased cell viability, and induced apoptotic cell death. Hesperidin increased Sub-$G_1$ population in MSTO-211H cells. Hesperidin significantly suppressed mRNA/protein level of Sp1 and modulated the expression level of the Sp1 regulatory protein such as p27, p21, cyclin D1, Mcl-1, and survivin in mesothelioma cells. Also, hesperidin induced apoptotic signaling including: cleavages of Bid, caspase-3, and PARP, upregulation of Bax, and down-regulation of Bcl-$_{xl}$ in mesothelioma cells. These results show that hesperidin suppressed mesothelioma cell growth through inhibition of Sp1. In this study, we demonstrated that Sp1 acts as a novel molecular target of hesperidin in human malignant pleural mesothelioma.

Phenotypic and Genotypic Detection of Metallo-β-Lactamase Producing Pseudomonas aeruginosa

  • This study was undertaken to evaluate phenotypic and genotypic methods for detection of Metallo-Beta-Lactamases (MBLs) among nosocomial Pseudomonas aeruginosa. Of the 50 P. aeruginosa isolates from clinical specimens, 20 were evaluated for carbapenem resistance and screened for MBL by double-disk synergy test and combined-disk test. Nineteen strains (95%) were found to be MBL producers among the 20 P. aeruginosa. MBL positives were further confirmed by Polymerase Chain Reaction (PCR). For the IMP and VIM types of MBLs, PCR analysis was performed on 19 of the 20, and 10 were positive for VIM MBL type. This study reports the validation of a simple and accurate MBL detection method that can be easily incorporated into the daily routine of a clinical laboratory. Early detection of MBL-carrying organisms, including those with susceptibility to carbapenems, is of paramount clinical importance, as it allows rapid initiation of strict infection control practices as well as therapeutic guidance for confirmed infection.Key Words : Hepatitis A virus (HAV), Anti-HAV, Hospital workers, Prevalence, Vaccination

MITRE ATT&CK 및 Anomaly Detection 기반 이상 공격징후 탐지기술 연구 (MITRE ATT&CK and Anomaly detection based abnormal attack detection technology research)

  • 공격자의 무기가 점차 지능화 및 고도화되고 있어 기존 백신만으로는 보안 사고를 막을 수 없으므로 endpoint까지 보안 위협이 검토되고 있다. 최근 endpoint를 보호하기 위한 EDR 보안 솔루션이 등장했지만, 가시성에 중점을 두고 있으며, 이에 대한 탐지 및 대응 기술은 부족하다. 본 논문에서는 보안 관리자 관점에서 효과적인 분석과 분석 대상을 선별하기 위해 실 환경 EDR 이벤트 로그를 사용하여 지식 기반 MITRE ATT&CK 및 AutoEncoder 기반 Anomaly Detection 기술을 종합적으로 사용하여 이상 공격징후를 탐지한다. 이후, 탐지된 이상 공격징후는 보안 관리자에게 로그정보와 함께 alarm을 보여주며, 레거시 시스템과의 연계가 가능하다. 실험은 5일에 대한 EDR 이벤트 로그를 하루 단위로 탐지했으며, Hybrid Analysis 검색을 통해 이를 검증한다. 따라서, EDR 이벤트 로그 기반 언제, 어떤 IP에서, 어떤 프로세스가 얼마나 의심스러운지에 대한 결과를 산출하며, 산출된 의심 IP/Process에 대한 조치를 통해 안전한 endpoint 환경을 조성할 것으로 기대한다.

  • 최근 AV 벤더들의 악성코드 동향 보고서에 따르면 신종, 변종 악성코드의 출현 개수가 기하급수적으로 증가하고 있다. 이에 따라 분석 속도가 떨어지는 수동적 분석방법을 대체하고자 기계학습을 적용하는 악성코드 분석 연구가 활발히 연구되고 있다. 하지만 지도학습기반의 기계학습을 이용할 때 많은 연구에서 AV 벤더가 제공하는 신뢰성이 낮은 악성코드 패밀리명을 레이블로 사용하고 있다. 이와 같이 악성코드 레이블의 낮은 신뢰성 문제를 해결하기 위해 본 논문에서는 새로운 레이블링 기법인 "Unified Labeling"을 소개하고 나아가 Fine-grained 방식의 특징 분석을 통해 악성 행위 유사성을 검증한다. 본 연구의 검증을 위해 다양한 기반의 클러스터링 알고리즘을 이용하여 기존의 레이블링 기법과 비교하였다.

데이터 마이닝 기법을 이용한 소규모 악성코드 탐지에 관한 연구 (A Study on Detection of Small Size Malicious Code using Data Mining Method)

  • 최근 인터넷 기술을 악용하는 행위로 인하여 경제적, 정신적 피해가 증가하고 있다. 특히, 신규로 제작되거나 변형된 악성코드는 기존의 정보보호 체계를 우회하여 사이버 보안 위협의 기본 수단으로 활용되고 있다. 이를 억제하기 위한 다양한 연구가 진행되었지만, 실제 악성코드의 많은 비중을 차지하는 소규모 실행 파일에 대한 연구는 미진한 편이다. 본 연구에서는 기존에 알려진 소규모 실행 파일의 특징을 데이터마이닝 기법으로 분석하여 알려지지 않은 악성코드 탐지에 활용할 수 있는 모델을 제안한다. 데이터 마이닝 분석 기법에는 나이브베이지안, SVM, 의사결정나무, 랜덤포레스트, 인공신경망 등 다양하게 수행하였으며, 바이러스토탈의 악성코드 검출 수준에 따라서 개별적으로 정확도를 비교하였다. 결과적으로 분석 파일 34,646개에 대하여 80% 이상의 분류 정확도를 검증하였다.

브랜드 액티비즘의 구현방식 연구: 브랜드 담당자의 심층 인터뷰를 중심으로 (Studies on Executions of Brand Activism: Focused on In-depth Interviews with Brand Managers)

  • 코로나 바이러스의 확산으로 인한 불확실성의 시기를 겪으며 소비자들은 예전보다 기업에 대해서 적극적인 사회공헌활동을 요구하게 되었다. 즉, 기업 브랜드에게 공익 추구 성향을 기대하는 경향이 두드러지고 있다. 소비자들은 사회공헌활동에서 한단계 넘어서서 회사가 사회문제라고 인식하는 것에 대해서 적극적으로 해결하려 하고 이를 추구하는 브랜드에 대해서 적극적인 지지와 소비의사를 나타내자 기업들도 이에 화답하여 브랜드 액티비즘(brand activism)이라는 새로운 사회적 마케팅을 적극적으로 시도하고 있다. 부연하면, 기업들은 브랜드 자체를 하나의 인격체로 설정하여 이슈가 되는 사회문제에 대해서 해결방식을 제시하고 지지를 이끌어내는 브랜드 전략을 사용한다 하겠다. 이번 연구에서는 브랜드 액티비즘 관련 브랜드 캠페인 수행 경력이 있는 다섯명의 브랜드 담당자와 인터뷰를 통해서 브랜드 액티비즘이 가장 잘 나타나있는 사례를 선정하고 이에 대해서 어떠한 특징이 소비자의 적극적인 지지를 이끌어냈는지에 대해서 논의하였다. 그 결과, 소상공인에 대한 지원, 인종차별 반대, 젠더 다양성과 평등, 지속가능성의 특징이 도출되었다.
