The purposes of this study which was conducted by applying three projection formulae to the data from admission quota for paramedic of the Ministry of Education & Human Resources Development the number of ambulances the number of emergency medical centers of the Ministry of Health & Welfare and rate of successful candidates of annual report of the National Health Personnel Licensing Examination Board were to find out demand and supply of paramedic from 2002 to 2045 and to expand scope of practice of paramedic in Korea. The conclusions from this study were summarized as follows; (1) The simple formulae derived from the projection formula of the Economic Planning Board were applied under the present Emergency Medical Services Act including qualifying over 3-years experienced EMT-Basic for paramedic examinee, stationing paramedic or EMT-Basic or physician or nurse per ambulance, stationing paramedic or EMT-Basic per emergency medical center and under the amended Emergency Medical Services Act including qualifying only paramedic graduate for paramedic examinee, stationing 4.5 paramedics per ambulance, stationing 10 to 2 paramedics per emergency medical center. (2) It was estimated that on the American basis of 5.6 EMTs per 10,000 in 1996, the number of paramedics under the present act will reach the basis before 2020, the number of paramedics under the amended act will reach the basis about 2040. (3) It was estimated that on the basis of 22,000 paramedics demanded from the number of ambulances, the number of emergency medical centers in 2001, the number of paramedics under the present act will reach the basis before 2015, the number of paramedics under the amended act will reach the basis about 2030. (4) There was relationship between requirements for emergency medical centers scope of practice of paramedic in the act and demand-supply of paramedic, this necessitates surveys, studies, amendment of the act, legalization for expanded scope of practice of paramedic including EMD, instructor, teacher of safety. (5) This study which includes only expanded scope of practice of paramedic and projection for paramedic in the act needs complementary studies such as decision-making process in health manpower policy and so on.

Purpose: Recently, the incidence of malignant melanoma has been steadily increasing. Malignant melanoma is already known to have poorer prognosis than other primary skin cancers. Despite the poor prognosis, it is relatively less known to the public so that a number of patients visit hospital carrying advanced stage tumor. Yet, extensive study about malignant melanoma is currently insufficient, and specific guidelines and statistical figures in Korea are almost inexistent. Therefore, authors reviewed patients with malignant melanoma who have visited our hospital for last 14 years. Methods: Between January 1994 and January 2008, 62 patients were diagnosed with malignant melanoma at our hospital. A retrospective study was performed with data from patients' charts and biopsy results. Annual incidence, mean age of onset, gender, tumor location, tumor thickness, pathologic ulceration, clinicopathologic subtype, and clinical AJCC stage were evaluated. Analysis of factors associated with survival were performed using the Cox proportional hazard model. Kaplan - Meier method was used to generate survival curves. Results: Clinicopathologic features of 62 patients (32 male, 30 female) with average age of 57 years were evaluated. Most lesions were found in lower limb, and the most common subtype was acral lentiginous melanoma. We could also find that age, tumor thickness, and clinical stage were the only significant prognostic factors. Conclusion: Clinicopathologic features of malignant melanoma were analyzed in this study, but the result is not ready to be generalized because the number of cases is too small. Further study must be performed to report clinical guidelines for prognosis and treatment for malignant melanoma patients in Korea.

Background: Robotic surgery is an alternative to minimally invasive surgery. The aim of this study was to report on current trends in robotic thoracic and cardiovascular surgical techniques in Korea. Methods: Data from the National Evidence-based Healthcare Collaborating Agency (NECA) between January 2006 and June 2012 were used in this study, including a total of 932 cases of robotic surgeries reported to NECA. The annual trends in the case volume, indications for robotic surgery, and distribution by hospitals and surgeons were analyzed in this study. Results: Of the 932 cases, 591 (63%) were thoracic operations and 340 (37%) were cardiac operations. The case number increased explosively in 2007 and 2008. However, the rate of increase regained a steady state after 2011. The main indications for robotic thoracic surgery were pulmonary disease (n=271, 46%), esophageal disease (n=199, 34%), and mediastinal disease (n=117, 20%). The main indications for robotic cardiac surgery were valvular heart disease (n=228, 67%), atrial septal defect (n=79, 23%), and cardiac myxoma (n=27, 8%). Robotic thoracic and cardiovascular surgeries were performed in 19 hospitals. Three large volume hospitals performed 94% of the case volume of robotic cardiac surgery and 74% of robotic thoracic surgery. Centralization of robotic operation was significantly (p<0.0001) more common in cardiac surgery than in thoracic surgery. A total of 39 surgeons performed robotic surgeries. However, only 27% of cardiac surgeons and 23% of thoracic surgeons performed more than 10 cases of robotic surgery. Conclusion: Trend analysis of robotic and cardiovascular operations demonstrated a gradual increase in the surgical volume in Korea. Meanwhile, centralization of surgical cases toward specific surgeons in specific hospitals was observed.

This report presents the findings of a one-year monitoring effort to empirically characterize and evaluate the nature of near-ground winds for structural engineering purposes. The current wind engineering practice in the United States does not explicitly consider certain important near-ground wind characteristics in typical rough terrain conditions and the possible effect on efficient design of low-rise structures, such as homes and other light-frame buildings that comprise most of the building population. Therefore, near ground wind data was collected for the purpose of comparing actual near-ground wind characteristics to the current U.S. wind engineering practice. The study provides data depicting variability of wind speeds, wind velocity profiles for a major thunderstorm event and a northeaster, and the influence of thunderstorms on annual extreme wind speeds at various heights above ground in a typical rough environment. Data showing the decrease in the power law exponent with increasing wind speed is also presented. It is demonstrated that near-ground wind speeds (i.e., less than 10 m above ground) are likely to be over-estimated in the current design practice by as much as 20 percent which may result in wind load over-estimate of about 50% for low-rise buildings in typical rough terrain. The importance of thunderstorm wind profiles on determination of design wind speeds and building loads (particularly for buildings substantially taller than 10 m) is also discussed. Recommendations are given for possible improvements to the current design practice in the United States with respect to low-rise buildings in rough terrain and for the need to study the impact of thunderstorm gust profile shapes on extreme value wind speed estimates and building loads.
Introduction: Design and implementation of screening programs in each country must be based on epidemiological data. Despite the relatively high incidence of CRC, there is no nationwide comprehensive program for screening in Iran. This study was designed to investigate national CRC data and help to determine guidelines for screening. Methods: Incidence data used in this study were obtained from Iranian annual of National Cancer Registration report. Age standardized rates (ASR)were calculated using world standard population and were categorized by age, sex, anatomic subsite and morphology of tumor. Data were analyzed using SPSS.V.13 and Open Source Epidemiologic Statistics for Public Health software (OpenEpi v.2.3.1). Results: A quarter of cases were less than 50 years of age. The majority of tumors were detected in the colon. The overall ASR in the four years period was 38.0 per 100000 and was higher for men compared women (P<0.05). Incidence rate of colorectal cancer increased with age. Conclusion: Results of present study indicated that incidence of colorectal cancer is relatively high in Iran. Incidence of CRC in people under 50 years and in rectum were reported higher than other countries that related etiologic factors should be investigate in further studies. According to the increasing of ASR after age 50 years, it seems that onset of screening at age 50 would be appropriate.

We reconstructed the community disaster risk and patterns of damage magnitude using 'Annual Report of National Disaster Data'. And we developed the inspection index for damage reduction capacity of each facility or each damage prevention measure. The inspection indices include climatic and hydrological vulnerabilities, and disaster probability components. Also we considered all indices and components for DB construction on the GIS system. Then we constructed 'Inspection for Safety Degree of Community System(ISDCS)' according to the facility's criteria from the combination of damage reduction capacity of facility and damage magnitude. The safety index has designed a system that encourages communities to carry out a list of activities in the raw from natural disaster. The activities ultimately saves lives, reduce property damage, and protect disaster facilities. Damage patterns were also analyzed according to damage type using pattern analysis and GIS.
This study purposed to develop GIS model for evaluating hospital location in a metropolitan city. Secondly, the suitability score of hospital was compared with the number of outpatient/inpatients of hospitals. This study used administrative data collected from the annual report of the 5 ward offices from Daejeon in Korea in 2004. 7 Acute care hospitals operated in Daejeon in 2004 were selected for the analysis. The suitability of hospitals in a metropolitan city was evaluated by the economic factors which were assumed to influence the health care demand. Each criterion was measured from each Dong. Correlation coefficients between the suitability score of hospital and the number of patients were varied depending on the buffering size. The relationship showed positive relationship with the number of inpatient (0.15) and outpatients (0.10) on the 0.5km buffering size. However, the correlation coefficients were changed to a negative direction or decreased when the buffering size was increased to 1.0km, 1.5km, and 2.0km.
The purpose of this study was to compare differences in role conflict, self efficacy, job satisfaction, and job involvement perceived by school nutrition teachers and dietitians. A total of 335 female school nutrition teachers and dietitians from Incheon area were surveyed by a self-report questionnaire in November 2010. Collected data were processed using SPSS 15.0. The reliability of the questionnaire was tested and differences between the two groups were analyzed using the Student's t-test, the chi-square test, and a two-way analysis of variance (ANOVA). The job satisfaction level of nutrition teachers was higher than that of dietitians. The effect of role conflict and job satisfaction for nutrition teachers and dietitians was different according to age. In addition, self-efficacy, job satisfaction, and job involvement of nutrition teachers and dietitians were influenced by their annual salaries. The frequency of meals served also affected role conflict, job satisfaction, and job involvement of the nutrition teachers and dietitians, and the effect of job involvement by the nutrition teachers and dietitians was different according to meal service type. Therefore, it is necessary to establish strategies for resolving role conflicts among school foodservice specialists and improve their self-efficacy for enhancing consumer's satisfaction with the general quality of school food service. These efforts may contribute to job satisfaction and job involvement of food service specialists and ultimately to the productivity of food service and the establishment of food service specialist roles.
Background: Stomach cancer is the fourth most common cancer and the second leading cause of cancer-related death through the world. It is predicted that the number of new cancer cases will be more than 15 million cases by 2020. Regarding the lack of studies on this topic in the country, we have thoroughly examined the patho-epidemiology of stomach cancer in Iran. Materials and Methods: In this cross- sectional study data were collected retrospectively reviewing all new stomach cancer patients in Cancer Registry Center report of health deputy for Iran during a 6-year period (2003-2008). The study also examined the morphology of common stomach cancers. Trends in incidence and morphology underwent joinpoint regression analysis. Results: During the six-year period, a total of 35,171 cases of stomach cancer were registered. Average age standardized rate for females and males were equal to 7.1 and 15.1 per 100,000 persons, respectively. Most common histological type was adenocarcinoma, NOS with 21,980 cases (62.50%). The annual percentage change (APC) in age-standardized incidence rate (per 100,000) was increase in both females and males at 11.1 (CI: 4.3 to 18.3) and 9.2 (CI: 5.2 to 13.4), respectively. Conclusions: According to our results, the incidence of gastric cancer is increasing in Iran, so further epidemiological studies into the etiology and early detection are essential.

This study developed the optimization method to correct the measured traffic volume of the expressway that minimizes the measurement error and satisfies the traffic balancing with TCS. For this purpose, the model constructed in this study was compared and verified with the true traffic volume. Verification result of the model, it was found that the measurement error is reduced when the measured traffic volume is corrected for the traffic volume balance. As a result of applying it to 40 links of the Kyoungbu expressway, the measured traffic volume was corrected by -8.1%~9.6% and the measurement error was decreased as much as the corrected traffic volume. This research is meaningful in improving the accuracy of the measured traffic volume of the expressway, while the scale and role of the expressway are increasing.
