Ankle osteoarthritis is a debilitating condition that causes severe pain associated with functional impairment and decreased activity. Ankle osteoarthritis, unlike that of the knee or hip joint, is rare in primary arthritis. Most cases are traumatic arthritis that occur after ankle sprain or fractures or chronic ankle instability. Although ankle fusion has been regarded as the standard treatment of ankle osteoarthritis in the past, total ankle arthroplasty (TAA) is increasing due to the development of the implant design and surgical techniques. TAA is biomechanically superior to ankle fusion by preserving the movement of the ankle joint. In particular, it is functionally superior to ankle fusion because it enables normal joint motion during gait. In addition, there is an advantage of preserving the movement of the hindfoot and reducing the abnormal stress applied to the adjacent joints after ankle fusion to prevent the occurrence of long-term adjacent joint arthritis. Although the short-term and mid-term results of TAA have been reported to be excellent, long-term follow-up has a relatively low survival rate and high complication rate compared to total knee or hip arthroplasty. Therefore, continuous and further research is needed.
Purpose: The purpose of this study was to investigate the effects of modified ankle movement patterns on participants' active dorsiflexion range of motion and leg muscle activity. Methods: This study recruited twenty-five participants, all of whom were healthy individuals with no abnormalities in the ankle or knee joints. The research methodology involved measuring the active dorsiflexion range of motion and muscle activity in each person's legs based on the presence or absence of toe extension while the subjects were in a comfortable, supine position. A statistical analysis was conducted using SPSS 25.0, and a paired samples t-test was employed. The significance level was set at 0.05. Results: When the participants demonstrated the modified ankle movement pattern with a controlled toe grip, there was an increase in their active dorsiflexion angle. However, during the proprioceptive neuromuscular facilitation technique without a controlled toe grip, a higher level of activity was observed in the leg muscles. Conclusion: The results of this study could be used as foundational data for establishing a rehabilitation exercise program designed to enhance range of motion and muscle activation in the ankle joint.
Purpose: This study was conducted to determine the effects of Mulligan's mobilization with movement (MWM) on changes in the talofibular interval in the sagittal plane in subjects with chronic ankle instability (CAI). Methods: Sixteen subjects with chronic ankle instability participated in this study. The talofibular intervals were measured from US images, and the weight-bearing lunge test was used to assess dorsiflexion of the ankle joint. Each dependent variable were measured on the both affected side and sound side in three trials in pre- and post-MWM. Dependent variables were examined with a two-way mixed-design analysis of variance (ANOVA). The two factors were side (sound side versus affected side) and intervention (pre- versus post-intervention). For post hoc analysis, paired t-tests were performed to compare the dependent variables. A p<0.05 was considered to indicate significance. Results: Dorsiflexion and talofibular interval differed significantly pre- and post-intervention (p<0.05). Post-hoc analysis revealed that the talofibular interval post-MWM was significantly less than that pre-MWM on the both the affected and sound side (p<0.05). The ankle dorsiflexion in the post-MWM group was significantly greater than that in the pre-MWM group on the affected side and the sound side (p<0.05). Conclusion: The Mulligan's MWM decreased the talofibular interval in subjects with CAI. These findings suggest that the MWM technique can change the position of the talus relative to the fibular in the weight bearing position.
Purpose : The purpose of this study was to examine the activity of the knee flexor and extensor with ankle plantar flexion and dorsiflexion. Methods : A total of 18 subjects(Male 6, female 12) performed 4 lower extremity patterns of PNF and the activities of the vastus medialis oblique, rectus femoris, vastus lateralis, biceps femoris, semitendinosus ipsilateral sides were measured using electromyography. Results : During 4 lower extremity patterns of PNF, knee flexor and extensor muscle activity were significantly difference. Knee extensors were shown to be higher knee extension and ankle dorsiflexion combined pattern. Knee flexors were shown to be higher knee flexion and ankle plantarflexion combined pattern. Conclusion : We suggest that it is efficient to strengthening of knee extensors with ankle dorsiflexion and to strengthening of knee flexors with ankle plantar flexion. Also, for the functioning as toe standing, we have to choice appropriate movement pattern.
The purpose of this study was to determine the effects of landing height on the lower extremity during a counter movement jump. Fourteen healthy male subjects (age: $27.00{\pm}2.94$ yr, height: $179.07{\pm}5.03$ cm, weight: $78.79{\pm}6.70$ kg) participated in this study. Each subject randomly performed three single-leg jumps after s single-leg drop landing (counter movement jump) on a force platform from a 20 cm and 30 cm platform. Paired t-test (SPSS 18.0; SPSS Inc., Chicago, IL) was performed to determine the difference in kinematics and kinetics according to the height. All significance levels were set at p<.05. The results were as follows. First, ankle and knee joint angles in the sagittal plane increased in response to increasing landing height. Second, ankle and knee joint angles in the frontal plane increased in response to increasing landing height. Third, there were no significant differences in the moment of each segment in the sagittal plane for the jumping height increment. Fourth, ankle eversion moment and knee valgus moment decreased but hip abduction moment increased for the jumping height increment. Fifth, Ankle and knee joint powers increased. In percentage contribution, the ankle joint increased but the knee and hip joints decreased at a greater height. Lastly, as jumping height increased, the power generation at the ankle joint increased. Our findings indicate that the height increment affect on the landing mechanism the might augment loads at the ankle and knee joints.
This paper describes the possibility of analyzing gait pattern from the changes of the lower leg electrical impedance. This impedance was measured by the four-electrode method. Two current electrodes were applied to the thigh, knee, and foot., and two potential electrodes were applied to the lateral, medial, and posterior position of human leg. The correlation coefficients of the joint angle and the impedance change from human leg movement was obtained using a electrogoniometer and 4ch impedance measurement system developed in this study. We found the optimal electrode position for knee and ankle joint movements based on high correlation coefficient, least interference, and maximum magnitude of impedance change. The correlation coefficients of the ankle, knee, and the hip movements were -0.913, 0.984 and 0.823, respectively. From such features of the human leg impedance, it has been made clear that different movement patterns exhibit different impedance patterns and impedance level. This system showed feasibility that lower leg movement could be easily measured by impedance measurement system with a few skin-electrodes.
Purpose: The purpose of this study is to determine the effects of visual information on movement time and each angular velocity of trunk and lower extremity joints while healthy adults are in sitting and squat motion. Methods: Participants consisted of 20 healthy male and female adults; movement time and each angular velocity of trunk, pelvis, hip, knee and ankle of sitting and squat motion according to common vision, visual task and visual block were analyzed using a three dimensional motion analysis system. Results: Each angular velocity of the trunk, pelvis, hip, knee and ankle in phase 2 of the sitting showed significant difference according to the types of visual information (p<0.05). Movement time and each angular velocity of pelvis and hip in phase 2 of squat motion showed significant difference according to the types of visual information (p<0.05). According to the common vision, each angular velocity of knee and ankle in phase 1 was significantly fast in sitting (p<0.05). According to the common vision, each angular velocity of trunk, pelvis, hip, knee, and ankle in phase 2 was significantly fast in sitting (p<0.05). Conclusion: Visual information affects the angular velocity of the motion in a simple action such as sitting, and that in more complicated squat motion affects both the angular velocity and the movement time. In addition, according to the common vision, visual task and visual block, as angular velocities of all joints were faster in sitting than squat motion.
Purpose: The aim of the present study was to investigate the effects of proprioceptive neuromuscular facilitation (PNF) and taping interventions on balance ability, joint position sense, and ankle joint strength. Methods: Thirty subjects who had experienced an ankle sprain within the previous 3 months participated in this study. The subjects were randomly assigned to a PNF group (n=15) or a taping group (n=15). Before and after the interventions, ankle dorsi-flexion and plantar-flexion strength, joint position error, and total center of pressure movements in one leg while in a standing position were measured. Results: Regardless of the group allocation, ankle dorsi-flexion and plantar-flexion strength significantly improved after the interventions (p<0.05). Compared to preintervention measurements, joint position errors were significantly reduced postintervention (p<0.05). The PNF intervention significantly decreased the total lateral movement of the center of pressure in the one leg standing condition (p<0.05). Conclusion: Both PNF and taping interventions improved joint position sense and ankle joint strength. In common with the findings of a previous study, the PNF intervention improved balance ability. Further study is required to investigate the effects of various PNF and taping interventions on ankle performance in subjects with chronic ankle sprains.
Purpose: The purpose of this study was to compare the changes in the muscle activation of the quadriceps muscle (rectus femoris, vastus lateralis, vastus medialis) during the stand-to-sit (StandTS) movement according to the plantar flexion angle of the ankle joint. Methods: A total of 22 healthy young females participated in this study. During the StandTS under the three conditions (plantarflexion angle 0°, 20°, and 45° of the ankle), electromyography (EMG) data (% maximum voluntary iso¬metric contraction) of the rectus femoris, vastus lateralis, and vastus medialis were recorded using a wireless surface EMG system. Results: There was a significant difference in the muscle activation of rectus femoris, vastus lateralis, and vastus medialis according to the plantar flexion angle (0°, 20°, and 45°) of the ankle. The muscle activation of the quadriceps was the highest at a 45° angle of plantarflexion and the lowest at 0°. One-way repeated ANOVA was used to analyze the muscle activation data of the lower extremity muscles according to the angle of the ankle joint. Conclusion: Based on the results of our study, it was confirmed that the muscle activity of the quadriceps can be increased even in the StandTS movement, which involves the eccentric contraction of the quadriceps muscle. This suggests that maintaining a plantar flexion posture for a long time, say by wearing high-heeled shoes, can quickly cause muscle fatigue in the lower-limb muscles, which can cause a decrease in balance ability leading to falls.
The purpose of this study was to determine the effect of ankle joint mobilization with movement (MWM) on the range of motion (ROM) in the ankle, on the muscle strength of lower extremities, and on spatiotemporal gait parameters in chronic hemiplegic patients. Fifteen subjects with chronic stroke were divided into two groups: an experimental group (8 subjects) and a control group (7 subjects). Both groups attended two or three sessions of physical therapy each week. The experimental group also attended additional MWM training sessions three times a week for five weeks. For both groups, the ROM of the ankle, the muscle strength of the lower extremities, and the spatiotemporal gait parameters in paretic limbs were evaluated before and after the training period. The results showed that the experimental group experienced more significant increases than did the control group in terms of passive (6.10%) and active (21.96%) ROM of the ankle, gait velocity (12.96%), and peak torque, of the knee flexor (81.39%), the knee extensor (24.88%), and the ankle plantar flexor (41.75%)(p<.05). These results suggest that MWM training in patients with chronic stroke may be beneficial in increasing ROM in the ankle, muscle strength in the lower extremities, and gait speed.
