COMPUTERIZED EVALUATION OF OCCLUSAL CONTACTS IN CENTRIC CLOSURE IN DISTAL EXTENSION PARTIAL DENTURE CASES (유리단 국소의치 환자에서 T-scan system을 이용한 중심교합시에 교합접촉 양상 연구)

  • This clinical study was to analyze occlusal contacts in maximum intercuspation on distal extension partial denture and to compare tooth contact state between the denture teeth and abutment teeth by time mode and force mode using the T-scan system. The subjects ware twenty-one adult patients with upper natural teeth and lower distal extension partial denture. Conclusion : 1. The patient with simultaneous occlusal contacts both denture tooth and abutment and bilaterally in Kennedy Class I cases was one-sixth, but there was no one with symmetric occlusal and equal force among bilateral denture teeth and abutments. 2. The five-fifteenth of Kennedy Class I case patients resulted in simultaeous occlusal contacts bet-ween denture teeth and abutments but no one has the symmetric occlusal contacts and unifarm force between denture teeth and abutment teeth.

COMPARISON OF THE PHYSICAL PROPERTIES OF FOUR KINDS OF ACRYLIC RESIN DENTURE TEETH (4종 아크릴릭 레진 인공치의 물리적 성질에 관한 비교)

  • To compare the wear resistance of four kinds of commercial acrylic resin teeth [SR-Orthosit PosterioresR(Ivoclar Co., Liechtenstein), Endura PosteriorR(Shofu Inc. Japan), trubyte IPN teethR(Dentsply International Inc., York,), Trubyte BiotoneR(dentsply Inermational Inc. Brazil) by means of the toothbrush abrasion method, the artificial resin teeth were embedded in epoxy resin with the occlusal surfaces aligned in one plane for a total of 40 blocks. There after, each lock was mounted in the arm of the toothbrush abrasion machine(K 236, Japan). Wear measurements were made on the three preconditioned states. Those were as follows : no treatment specimens, thermocycled specimens, and thermocycled specimens which were immeresed applied load of 400g during the buring the brushing cycle. At the end of the 30,000-stroke cycle, each specimen was removed, and weighed. The microhardness of four kinds of commercial resin teeth were determined by means of microhardness tester. Microhardness tests were performed on te no treatment specimens, thermocycled specimens, and thermocycled specimens with immersion in the denture cleansing solution. Finally, the comparison of thermal properties were perfomed using differential scanning calorimeter(DSC-1500). The following results were obtained : 1. In the case of no treatment teeth, the wear amount of endura Posterior(EN) was the greatest among the others(p<0.01), and the wear amount of three kinds of artificial resin teeth was increased in the order of Trubyte IPN teeth(IN), Trubyte Biotone(BN), SR-Orthosit Posteriores(SN) but there was no statistic significance(p>0.01). 2. The wear amount of Trubyte IPN teeth(IT) and Trubyte Biotone(BT), was increased due to thermocycling effect, but that of Endura Posteriores(ET) was decreased conversely(p<0.01). 3. Except for the SR-Orthosit Posteriores(STC), the wear amount of three kinds of artificial resing teeth(that is, ETC, ITC, BTC) was increased due to denture cleansing solution$(Cledent^R)$, but there was no statistic significance(p>0.01). But the wear amount of the SR-Orthosit Posteriores(STC) was the greatest among the others(p<0.01). 4. The wear amount of toothbrush was the greatest in case of contact with occlusal surface of SROrthosit Posteriores resin teeth(p<0.01). 5. the microhardness values(KHN) of the SR-Orthosit Posteriores was the highest among the experimental artificial resin teeth(p<0.01). 6. There was no relationship between microhardness valuse(KHN) and wear amount of four kinds of experimental artificial resin teeth(p<0.01). 7. The differential canning calorimetric property of four kinds of artificial resin teeth did not show endothermal or exothermal peak in the range of $100^{\circ}C$

  • Supernumerary teeth are extra teeth or tooth-like structures which may have either erupted or unerupted in addition to the 20 deciduous teeth and the 32 permanent teeth. Clinicians sometimes confront unerupted supernumerary teeth where implants would be placed. Many clinicians consider immediate placement of implant into an extracted socket as an effective technique compared to the conventional method of waiting several months for bone healing, because of the advantages such as reduction of treatment period, preservation of alveolar bone and soft tissue, etc. We could hardly find the case of immediate placement of implant after extraction of impacted supernumerary teeth and permanent teeth together. Therefore we report the case of immediate implantation following extraction of impacted supernumerary teeth and poor periodontal conditioned permanent teeth.

Assessment of endodontically treated teeth by using different radiographic methods: an ex vivo comparison between CBCT and other radiographic techniques

  • Purpose: To compare different radiographic methods for assessing endodontically treated teeth. Materials and Methods: Root canal treatments were applied in 120 extracted mandibular teeth, which were divided into four groups: (1) ideal root canal treatment (60 teeth), (2) insufficient lateral condensation (20 teeth), (3) root canals filled short of the apex (20 teeth), (4) overfilled root canal treatment (20 teeth). The teeth were imaged using intraoral film, panoramic film, digital intraoral systems (CCD and PSP), CCD obtained with portable X-ray source, digital panoramic, and CBCT images obtained at 0.3 $mm^3$ and 0.2 $mm^3$ voxel size. Images were evaluated separately by three observers, twice. Kappa coefficients were calculated. The percentage of correct readings obtained from each modality was calculated and compared using a t-test (p<0.05). Results: The intra-observer kappa for each observer ranged between 0.327 and 0.849. The inter-observer kappa for each observer for both readings ranged between 0.312 and 0.749. For the ideal root canal treatment group, CBCT with 0.2 $mm^3$ voxel images revealed the best results. For insufficient lateral condensation, the best readings were found with periapical film followed by CCD and PSP. The assessment of teeth with root canals filled short of the apex showed the highest percentage of correct readings by CBCT and CCD. For the overfilled canal treatment group, PSP images and conventional periapical film radiographs had the best scores. Conclusion: CBCT was found to be successful in the assessment of teeth with ideal root canal treatment and teeth with canals filled short of the apex.

A CEPHALOMETRIC STUDY ON MESIODISTAL AXIAL INCLINATION OF POSTERIOR TEETH IN OPEN BITE AND DEEP BITE (개방교합과 과개교합에서 구치의 근원심 치축경사도에 관한 두부방사선계측학적 연구)

  • For the purpose of investigating mesiodistal axial inclination of posterior teeth in normal occlusion group, open bite and deep bite group and investigating the correlationship between the axial inclination of posterior teeth and overbite of anterior teeth, a cephalometric study was performed on the subjects consisted of normal occlusion group(40), open bite group(71 : Angle's Class I, Class II, division 1 25, Class III 25) and deep bite group(64 : Angle's Class I 23, Class II, division 1 21, Class III 20). Mesiodistal axial inclination of posterior teeth to occlusal, mandibular and palatal plane were measured. The findings of this study were as follows : 1. Upper and lower posterior teeth were more mesially inclined to occlusal plane in open bite group than in deep bite group. 2. Lower posterior teeth were more mesially inclined in deep bite group than in open bite group in Angle's Class II, division 1 malocclusion but there were no significant differences in Angle's Class I and Class III malocclusion. 3. There was no significant correlationship between the axial inclination of posterior teeth to each plane and overbite of anterior teeth in open bite group. 4. There was a significant correlationship between the axial inclination of upeer and lower second premolar to occlusal plane and overbite of anterior teeth in Angle's Class I, Class II, division 1 and Class III malocclusion.

DENTAL TREATMENT OF SHELL TEETH UNDER GENERAL ANESTHESIA: A CASE REPORT (Shell teeth 환자의 전신마취 하 치과치료 : 증례보고)

  • Dentinogenesis imperfecta is a hereditary disorder that causes abnormal dentin formation and tooth morphology. Affected teeth show very thin dentin and large pulp chamber. Immediately after their eruption, the teeth tend to be destroyed rapidly. Early dental intervention is recommended to improve the feeding capacity and quality of life for the child, to maintain vertical dimension, and to promote healthy and functional dentition. This case report describes a treatment for a 4-year-2-month-old girl whose primary and permanent dentition were affected by Dentinogenesis Imperfecta. Shell teeth were observed in the entire primary dentition. The pulp of most teeth was exposed and the patient complained of pain. Most paternal family members were transferred to the Dentinogenesis Imperfecta. She needed an extensive dental treatment, but her compliance could not be expected because she was very young and anxious. So we decided to treat her under general anesthesia. All molar teeth are restored with Stainless Steel crown. Maxillary anterior teeth were extracted and mandibular anterior teeth are restored with composite resin. The patient's pain disappeared and masticatory function was restored.

  • Hyperdontia or supernumerary teeth without associated syndrome is a rare phenomenon, as supernumerary teeth are usually associated with cleft lip and palate or other syndromes such as Gardner's syndrome, cleidocranial dysplasia, and so on. Five patients with supernumerary teeth visited our department. They had no familial history or other pathology, certain treatment protocols was modified due to the presence of supernumerary teeth. Non-syndromic supernumerary teeth, if asymptomatic, need to have periodical radiographic observation. If they showed no variation as they impacted in the jaw, careful examination is necessary because they may develop into pathological status such as dentigerous cysts. The importance of a precise clinical history and radiographic examination for patients with multiple supernumerary teeth should be emphasized.

  • Developmental dental morphological anomalies are fusion, gemination, twinning, concrescence and etc. They may cause many problems in conservative, periodontal and esthetic aspects. Fusion is a condition where two separate tooth buds unite at some stage in their development to form a bifid crown. If tooth contact occurs early, at least before the start of calcification, the two teeth may be completely unites to form a single large tooth. If tooth contact occurs after the time when a portion of the tooth crown has completed its formation, there may be union of the roots only. In fusion the dentin is always confluent. Fusion teeth is more common in the deciduous than in the permanent dentition. Fused teeth are relatively rare, and are mostly mandibular anterior teeth. Fusion of normal and supernumerary teeth or between normal teeth may occurs. The exact etiology factor of fusion is unknown, but genetic and environmental factors seems to be related. A variety of complications and subsequent treatments have been suggested on this teeth. Periodontal conditions may arise due to a groove formed at the line of fusion of the two teeth. Crowding or Protrusion with potential for malocclusion or delayed eruption of adjacent teeth. Fusion teeth appear in the anterior region, they usually cause esthetic problems. Treatments vary depending on the problem, the location, and the extent of fusion. Treatment of fused teeth has been reported from endodontic, orthodontic, periodontic, surgical and multidisciplinary. This report presents the esthetic improvements by separation of two clinical crowns. Bone reduction or endodontic treatments are not required.

Restoration after endodontic treatment with Endocrown (임상가를 위한 특집 3 - Endocrown을 이용한 근관치료 후 수복)

  • Successful treatment of a badly broken down tooth with pulpal disease depends not only on good endodontic therapy, but also on good prosthetic reconstruction of the tooth after endodontic therapy is completed. The ideal treatment of endodontically treated teeth has been widely and controversially discussed. Endocrown is a restorative option for endodontically treated teeth. Endocrown design incorporates the core and short post into the crown as a single restoration. The preparation of endocrown consists of a circular equigingival butt-joint margin and central retention cavity of the entire pulp chamber instead of employing intraradicular posts. This design significantly increases the surface area of the preparation available for cementation. It is particularly useful in young patient teeth for long-term provisional restoration and in teeth with short clinical crowns. This technique represents a promising and conservative method for the treatment of endodontically treated teeth that require long-term protection and stability. Endocrown can be considered as a feasible alternative to full crowns or composite overlays for the restoration of non vital teeth.

STATISTICAL STUDY OF THE FUSED TEETH IN THE PRIMARY DENTITION IN KOREAN CHILDREN (한국인아동의 유합유치에 관한 통계학적 연구)

  • This study was designed to find out the prevalence d the fused teeth in the primary dentition and their successors. The clinical and panoramic radiographic examination was undertaken of 5226 children (male 2812, female 2414) attended department of Pedodontics, College of Dentistry, Yon Sei University, Seoul, Korea. The results were as follows; 1. The prevalence of the fused teeth in the primary dentition was 1.03 percent and female (1.2%) affected more frequently than male (0.89%). 2. Compare with the maxilla and mandible, the prevalence of the fused teeth were higher in the mandible than in the maxilla. 3. The fusion of the primary teeth were occured between central with lateral incisor in the maxilla, and central with lateral and lateral with canine in the mandible. 4. There were 33 cases (61%) of succeding teeth missing among 54 case of the rused teeth.
