  • Purpose: The aim of this study was to clarify the influence of training with a different kind of lesion on the performance of a target model. Materials and Methods: A total of 310 patients(211 men, 99 women; average age, 47.9±16.1 years) were selected and their panoramic images were used in this study. We created a source model using panoramic radiographs including mandibular radiolucent cyst-like lesions (radicular cyst, dentigerous cyst, odontogenic keratocyst, and ameloblastoma). The model was simulatively transferred and trained on images of Stafne's bone cavity. A learning model was created using a customized DetectNet built in the Digits version 5.0 (NVIDIA, Santa Clara, CA). Two machines(Machines A and B) with identical specifications were used to simulate transfer learning. A source model was created from the data consisting of ameloblastoma, odontogenic keratocyst, dentigerous cyst, and radicular cyst in Machine A. Thereafter, it was transferred to Machine B and trained on additional data of Stafne's bone cavity to create target models. To investigate the effect of the number of cases, we created several target models with different numbers of Stafne's bone cavity cases. Results: When the Stafne's bone cavity data were added to the training, both the detection and classification performances for this pathology improved. Even for lesions other than Stafne's bone cavity, the detection sensitivities tended to increase with the increase in the number of Stafne's bone cavities. Conclusion: This study showed that using different lesions for transfer learning improves the performance of the model.

  • The WHO separates odontogenic carcinomas into three groups : malignant ameloblastoma, primary intraosseous carcinoma(PIOC), and carcinomas arising from odontogenic epithelium including those arising from odontogenic cysts. In WHO criteria, primary intraosseous carcinoma is defined as a squamous cell carcinoma arising within the jaw, having no connection with the oral mucosa, and no developing from residues of odontogenic epithelium. This is a case of 52-year old man who had prolonged jaw pain and final diagnosis was primary intraosseous carcinoma(PIOC) on mandible. We obtained successful result after composite resection combined with hemimandibulectomy, RND, following reconstruction with latissmus dorsi myocutaneous flap, and postoperative radiation therapy.

  • This is to report a case of secondary reconstruction after partial mandibulectomy by using of marrow-cancellous bone and cortical bone harvested from the iliac crest in the case of an ameloblastoma on the mandible. According to the past experimental studies, the marrow and cancellous bone have the marked osteogenic potential of hematopoietic. And the cortical bone has the highest activity of bone induction, which is mediated through the action of bone morphogenic protein(BMP). This grafting technique, the combination of PMCB and cortical bone, has advantage in restoring lange defect of the mandible.

  • Decompression is considered an effective treatment for odontogenic cystic lesions in the jaw. A variety of decompression devices are successfully used for the treatment of keratocystic odontogenic tumors, radicular cysts, dentigerous cysts, and ameloblastoma. The purpose of these devices is to keep an opening between the cystic lesion and the oral environment during treatment. The aim of this report is to describe an effective decompression tube using a stainless steel tube and wire for treatment of jaw cystic lesions.

  • The occurrence of odontogenic epithelium in the well of a dentigerous cyst is a well known entity. This epithelium usually remains inactive and does not have clinical significance. However, these small inactive islands of epithelium may be stimulated, resulting in an ameloblastoma. Therefore correct diagnosis and proper treatment are very important. A 15 year-old boy came to the outpatient clinic on August 13, 1979. The Chief complaint was pain, difficulty in mouth opening and swelling of the right mandible of 1 month's durations. With the X-ray filming, it was revealed that dentigerous cyst had been originated from the third molar, occurred in posterior region of the right mandible involving the 1st and the and molar and the portion just beneath the sigmoid notch area. Pus discharged from the gingival sulcus distal to the 2nd molar and it was sure that the cyst had been infected. Enucleation performed with careful excision of all cyst wall was successfully carried out, and recovery and heading were rapid and uncomplicated.

  • Recently, computer-assisted surgery is popular for performing well-planned operations. Computer-aided navigation system is helpful in maxillofacial surgery with real time instrument positioning and clear anatomic identification. Generally, segmental mandibulectomy and reconstruction flap surgery have done by extra-oral approach such as, submandibular approach. This case report describes performing intra-oral segmental mandibulectomy and reconstruction with monocortical deep circumflex iliac artery (DCIA) flap and CT guided implant surgery by using computer-aided surgical guide and navigation for managing ameloblastoma in a 31 years old female patient.

  • The two recurrent ameloblastomas in mandibles of the 34 years old korean woman and 30 years old korean man were observed. We have treated the two recurrent ameloblastomas by means of hemimandiblectomies for radical operations and bone grafts having been obtained from each her and his iliac bone crests.

  • Surgical resection of the mandible due to presence of benign or malignant tumor is the most common cause of the mandibular deviation. Depending upon the location and extent of the tumor in the mandible, various surgical treatment modalities like marginal, segmental, hemi, subtotal, or total mandibulectomy can be performed. The clinicians must wait for extensive period of time for completion of healing and acceptance of the osseous graft before considering the definitive prosthesis. During this initial healing period prosthodontic intervention is required for preventing the mandibular deviation. This case report describes early prosthodontic management of a patient who has undergone a reconstructed hemi-mandibulectomy with modified mandibular guide flange prosthesis. The prosthesis helps patient moving the mandible normally without deviation during functions like speech and mastication.

  • Ameloblastic fibro-odontoma(AFO) is a rare mixed odontogenic tumor. It is composed of connective tissue characteristic of an ameloblastic fibroma and calcified tissue as a complex or compound odontoma. AFO usually presents itself as an asymptomatic swelling of jaw or failure of tooth eruption. The lesion usually occurs in individual less than 30 years old. The differential diagnosis of this tumor includes odontoma, ameloblastoma, and ameloblastic fibroma. This report describes an ameloblastic fibro-odontoma occurring in maxilla of sixteen-year-old female. The lesion was treated by surgical enucleation and curettage without extraction of the involved canine(#23). This patient has shown no sign of recurrence during postoperative 34 months. So we report our case with review of literatures
