  • Urodynamics describes a collection of tests designed to evaluate lower urinary tract function and can be performed using retrograde filling of the bladder within a room. In this study, we designed and calibrated the potable urodynamics monitoring system using DSP chip (TMS320VC33, Texas InstrumentTM, U.S.) and obtained signals of bladder(Pves) and bladder neck pressure(Pneck) and EMG using calibration kit (DPT9022K0122, MedtronicsTM, U.s,). This system monitor spontaneous urination during daily life and can make patients more comportable.

  • In this paper, we designed a Holter analysis system using the visual programming method. It differs from the existing analysis system in that the various signal processing algorithms represented by icons were designed by GUI concept which provide unskilled user with easy and convenient analysis environment. In order to analysis ECG signal, we only select the icon representing a algorithm to be applied by mouse and arrange the selected icons upon the order to be processed on screen. As a result it provides a convenient usage and flexibility of analysis. Finally, we can find the optimal algorithm for the ambulatory ECG analysis by comparing the several results obtained from the various analysis configuration.

Development of Wireless Ambulatory System Measuring 3-D Motion as Podiatric Diagnosis (족부 진단용 3차원 움직임 측정을 위한 무선 이동형 시스템 개발)

  • In this research, a wireless movable systems is introduced that is applicable for real-world clinical examination in order to resolve the inconvenience of wired motion analysis system which uses markers by measuring more accurately the angle of the 3D motion. The system is composed of bluetooth data transmission and 6 DOF inertial measurement system that is composed of 3-axis acceleration and 3-axis gyroscope. The samples were collected from 10 feet of 5 young children who were confirmed as patients having flat feet and achilles tendonitis by podiatric expert in pediatrics, and the validity of the system was evaluated by comparison with the opinion by the expert using the EMG data and the data measured by the developed system. ROM of ankle was measured by the difference between the maximum and the minimum peaks of 3-axis values (pitch, roll and yaw) measured at lateral malleolus bone, and based on this measured value, the eversion rate became relatively higher as flat feet state become worse, which is equivalent to the opinion from the podiatric experts. It was shown that there is a relation between the certain muscle activities and the abnormal motion of foot caused by disease.

Design of a Holter Monitoring System with Flash Memory Card (플레쉬 메모리 카드를 이용한 홀터 심전계의 설계)

  • The Holter monitoring system is a widely used noninvasive diagnostic tool for ambulatory patient who may be at risk from latent life-threatening cardiac abnormalities. In this paper, we design a high performance intelligent holter monitoring system which is characterized by the small-sized and the low-power consumption. The system hardware consists of one-chip microcontroller(68HC11E9), ECG preprocessing circuit, and flash memory card. ECG preprocessing circuit is made of ECG preamplifier with gain of 250, 500 and 1000, the bandpass filter with bandwidth of 0.05-100Hz, the auto-balancing circuit and the saturation-calibrating circuit to eliminate baseline wandering, ECG signal sampled at 240 samples/sec is converted to the digital signal. We use a linear recursive filter and preprocessing algorithm to detect the ECG parameters which are QRS complex, and Q-R-T points, ST-level, HR, QT interval. The long-term acquired ECG signals and diagnostic parameters are compressed by the MFan(Modified Fan) and the delta modulation method. To easily interface with the PC based analyzer program which is operated in DOS and Windows, the compressed data, that are compatible to FFS(flash file system) format, are stored at the flash memory card with SBF(symmetric block format).

Socioeconomic Determinants of Korean Medicine Ambulatory Services: Comparing Panel Fixed Effect Model with Pooled Ordinary Least Square (한방외래의료 이용의 사회경제적 결정요인 연구: 의료패널자료를 이용한 고정효과모형과 합동 Ordinary Least Square 모형의 비교)

  • Background: Korea is considered to have an integrative health system where both western medicine and Korean (traditional) medicine are officially recognized and provided. Although Korean medicine has been covered by National Health Insurance over 20 years, equity in the utilization of Korean medical care has rarely been examined. Methods: We examined medical care utilization and expenditure of outpatient Korean medicine using panel fixed effects model to remove selection bias. Then we compared it with pooled ordinary least square (OLS) model. This study used Korea Health Panel data, which provides accurate information on out-of-pocket health care payment, including non-covered medical services. Results: Principal findings indicate that the frequency of the utilization of Korean medicine is related with unobservable individual choices different from western medicine, so the panel fixed effect model is appropriate. But pooled OLS model is better fitted for the expenditure of Korean medicine, after controlling for western medical care expenditure. After adjusting for the selection bias, socioeconomic status (income, education) was significantly associated with the expenditure of Korean medicine, but not with the frequency of the utilization of Korean medicine. Conclusion: This study shows that expenditure of Korean medicine utilization is inequitable across socioeconomic groups, which implies that health insurance coverage of Korean medicine is not sufficient.

Falls in the General Hospital Inpatients: Incidence, Associated Factors (일개 종합병원 입원환자의 낙상 실태 및 관련 요인)

  • Background : To estimate fall incidence rate and associated factors in inpatients from a general hospital. Method : The data were collected from 104 fall incident reports developed by the patient safety committee in a general hospital in Seoul from 01 January 2007 to 31 December 2008. Information included general characteristics of patients, factors related to fall, types, places, circumstances and outcomes of fall. Result : The incidence rate of fall, which was 4.4 per 1,000 total discharged patients and 0.5 per 1,000 patient-days, was much lower than that of several hospitals in the United States. The difference may reflect the different incidence reporting system of each hospital. Fall-prone patients were, in general, $$\geq_-$$65 years of age, had an alert mental status, were ambulatory with some assistance, and were dependent on and ambulatory device. High incidence of falls was associated with patients with circulatory disease. The majority of fall events usually occurred in bed or at the bedside in the patient's room, and occurred more often during the night than during the day or evening. Risk factors of fall were use of drugs (antihypertensive or neuropsychiatric drugs) and environmental factors (e.g., overly high bed height, surrounding objects, inadequate fitness shoes and slippery floor). Physical injury occurred in 43.3% of fall events, which typically required diagnosis of injury and treatment such as suturing. Risk factors for repeated falls were use of a neuropsychiatric drug (odds ratio=13.9) and gait disturbance (odds ratio=91.2). Risk factors for fall-related injury were alert mental status (odds ratio=3.3 times more likely to fall than those who were drowsy or in a stupor) and general weakness(odds ratio=3.3 times more likely to fall than those who were not generally weak). Conclusion : Medical and nursing staff should be aware of the fall risk factors of hospitalized patients and should intensively pursue preventative strategies. Development of fall prevention education based on these results is recommended.

Study of the Residential Environment and Accessibility of Rehabilitation for Patients with Cerebral Palsy (뇌성마비 환자의 주거 환경과 재활 접근성에 관한 연구)

  • Purpose: This study examined the residential environment and accessibility of rehabilitation for cerebral palsy (CP) to identify the problems with residential laws pertaining to the disabled and provide basic data on the health legislation for the rights of the disabled. Materials and Methods: The literature was searched using three keywords: residence, rehabilitation, and accessibility. Two items were selected: residential environment and rehabilitation accessibility. The questionnaire included 51 items; 24 were scored using a Likert scale and 27 were in the form of multiple-choice questions. Results: This study included 100 subjects, of which 93 lived at home and seven lived in a facility. Of these 93 subjects, 65% were living in apartments, usually two or more floors above ground, and 40% of them were living without elevators. According to the Gross Motor Function Classification System, subjects with I to III belonged to the ambulatory group and IV, V were in the non-ambulatory group. Subjects from both groups who lived at home found it most difficult to visit the rehabilitation center by themselves. In contrast, among those who lived at the facility, the ambulatory group found it most difficult to leave the facility alone, while the non-ambulatory group found it most difficult to use the toilet alone. Moreover, 83% of respondents thought that rehabilitation was necessary for CP. On the other hand, 33% are receiving rehabilitation services. Rehabilitation was performed for an average of 3.6 sessions per week, 39 minutes per session. Conclusion: There is no law that ensures secure and convenient access of CP to higher levels. Laws on access routes to enter rooms are insufficient. The disabled people's law and the disabled person's health law will be implemented in December 2017. It is necessary to enact laws that actually reflect the difficulties of people with disabilities. Based on the results of this study, an investigation of the housing and rehabilitation of patients with CP through a large-scale questionnaire will necessary.

Wireless Wearable GRF Sensing System for Continuous Measurements (연속적 데이터 획득을 위한 착용형 무선 지면 반력 측정 시스템)

  • This paper presents a wireless ground reaction force (GRF) sensing system for ambulatory GRF recording. The system is largely divided into three parts: force sensing modules based on optical sensor, outsole type frame, and embedded system for wireless communication. The force sensing module has advantages of the low height, robustness to the moment interference, and stable response in long term use. In simulation study, the strain and stress properties were examined to satisfy the requirements of the GRF sensing system. Four sensing modules were mounted on the toe, ball, and heel of foot shaped frame, respectively. The GRF signals were extracted using Micrpcontroller unit and transferred to the smart phone via Bluetooth communication. We measured the GRF during the normal walking for the validation of the continuous recording capability. The recorded GRF was comparable to the off the shelf stationary force plate.

  • In this study, we developed E.C.G telemetry system by using a digital communication method. This system is composed of a number of Ambulatory Telemetry System(ATS)'s that use for the same frequency and one center monitoring system(CMS) utilized the personal computer. The purpose of the ATS is, Therefore, to present for acqusition ECG data and transmitting to CMS. The ATS hardware is based on one chip microprocessor (8096) included A/D convert, ECG pre amplifier, ID-decoder, TRX Gateway and FM TRX module. Using the PC can be convinet to processing and deposite for long terms continued patient data.

  • The real-time monitoring about the activity of the human provides useful information about the activity quantity and an ability. This study developed a system for human physical activity assessment in ambulatory monitoring using portable sensing device combining a tri-axial accelerometer and wireless sensor node. This real-time system is able to identify several postures, posture transitions and movements with classification algorithm. In addition, this system also features fall detection capability. The results of the assessment for evaluating the performance of the system show high identification accuracy.
