The Effectiveness Evaluation of Helicopter Ambulance Transport among Neurotrauma Patients in Korea

  • Objective : Helicopter ambulance transport (HAT) is a highly resource-intensive facility that is a well-established part of the trauma transport system in many developed countries. Here, we review the benefit of HAT for neurosurgical patients in Korea. Methods : This retrospective study followed neurotrauma patients who were transferred by HAT to a single emergency trauma center over a period of 2 years. The clinical benefits of HAT were measured according to the necessity of emergency surgical intervention and the differences in the time taken to transport patients by ground ambulance transport (GAT) and HAT. Results : Ninety-nine patients were transferred to a single university hospital using HAT, of whom 32 were taken to the neurosurgery department. Of these 32 patients, 10 (31.3%) needed neurosurgical intervention, 14 (43.8%) needed non-neurosurgical intervention, 3 (9.4%) required both, and 11 (34.4%) did not require any intervention. The transfer time was faster using HAT than the estimated time needed for GAT, although for a relatively close distance (<50 km) without ground obstacles (mountain or sea) HAT did not improve transfer time. The cost comparison showed that HAT was more expensive than GAT (3,292,000 vs. 84,000 KRW, p<0.001). Conclusion : In this Korean-based study, we found that HAT has a clinical benefit for neurotrauma cases involving a transfer from a distant site or an isolated area. A more precise triage for using HAT should be considered to prevent overuse of this expensive transport method.

119 구급자동차의 내부 디자인 개발에 관한 연구 (Design of Fire-fighting Ambulance Interior)

  • 구급차는 병원 전 응급의료서비스의 중요한 요소이며, 119 구급대원의 업무능률 향상을 위한 중요한 공간이다. 구급대원들이 직무능력을 최대한 발휘할 수 있도록 구급차 내부의 환경을 최적의 공간으로 하여 구급대원들의 책임과 기능적 역할을 제공할 수 있도록 개선되어야 할 것이다. 따라서 본 연구에서는 구급차의 내부 공간에서 환자실에 탑재되어야 하는 응급의료 장비와 약제의 탑재 공간 확보 및 구급차 내에서의 응급처치에 따른 원활한 공간의 배치를 재구성하였으며, 환자의 위치와 응급처치 요원의 배치에 따른 공간 배정에 대한 분석 및 효율적 방법을 제시하고 있다. 또한 본 연구는 구급차 주행 중 처치공간에서 수행하는 119 구급대원 업무의 효율성을 향상시키기 위한 구급대원의 직무수행에 적합한 구급차 내부 수납공간의 디자인 설계 및 사용 환경에 대한 파악 및 문제점 분석을 통하여 응급의료서비스에 대한 안전성과 효율성을 제공하기 위한 새로운 환자실 공간 크기 및 디자인 안을 제시하고자 한다.

Using Shoulder Straps Decreases Heart Rate Variability and Salivary Cortisol Concentration in Swedish Ambulance Personnel

  • Background: Previous research has shown that paramedics are exposed to risks in the form of injuries to the musculoskeletal system. In addition, there are studies showing that they are also at increased risk of cardiovascular disease, cancer, and psychiatric diseases, which can partly be explained by their constant exposure to stress. The aim of this study is to evaluate whether the use of shoulder straps decreases physical effort in the form of decreased heart rate and cortisol concentration. Methods: A stretcher with a dummy was carried by 20 participants for 400 m on two occasions, one with and one without the shoulder straps. Heart rate was monitored continuously and cortisol samples were taken at intervals of 0 minutes, 15 minutes, 30 minutes, 45 minutes, and 60 minutes. Each participant was her or his own control. Results: A significant decrease in heart rate and cortisol concentration was seen when shoulder straps were used. The median values for men (with shoulder straps) at 0 minutes was 78 bpm/21.1 nmol/L (heart rate/cortisol concentration), at 15 minutes was 85 bpm/16.9 nmol/L, and at 60 minutes was 76 bpm/15.7 nmol/L; for men without shoulder straps, these values were 78 bpm/21.9 nmol/L, 93 bpm/21.9 nmol/L, and 73 bpm/20.5 nmol/L. For women, the values were 85 bpm/23.3 nmol/L, 92 bpm/20.8 nmol/L, and 70 bpm/18.4 nmol/L and 84 bpm/32.4 nmol/L, 100 bpm/32.5 nmol/L, and 75 bpm/25.2 nmol/L, respectively. Conclusion: The use of shoulder straps decreases measurable physical stress and should therefore be implemented when heavy equipment or a stretcher needs to be carried. An easy way to ensure that staff use these or similar lifting aids is to provide them with personalized, well-adapted shoulder straps. Another better option would be to routinely sewn these straps into the staff's personal alarm jackets so they are always in place and ready to be used.

병원 전 전문심장소생술을 위한 기계적 가슴압박기의 효과 (The Effect of a Mechanical Chest Compressions for Out-of-hospital Advanced Cardiac Life Support)

  • 본 연구는 병원 전 환경에서 기계식 가슴압박기(LUCAS)와 수기심폐소생술의 비교실험을 통해 가슴압박의 질을 평가하고 기계적 가슴압박기를 사용한 효과적인 전문심장소생술을 제안하는데 그 목적이 있다. 병원 전 심정지상황을 가정하고 구급대원이 3인 1조로 현장에서 수기심폐소생술과 LUCAS를 이용한 심폐소생술을 적용하고, 구급차로 이송하며 수기심폐소생술과 LUCAS를 이용한 심폐소생술을 시행하였다. 연구결과는 다음과 같다. 첫째, 현장과 이송중의 수기심폐소생술의 비교결과 현장에서 수기심폐소생술이 평균압박깊이와 압박률, 이완율에서 유의한 차이가 나타났다(p<.001). 둘째, 현장에서 수기심폐소생술과 LUCAS를 비교한 결과 LUCAS가 압박률, 이완율에서 유의한 차이가 나타났다(p<.001). 셋째, 구급차로 이송중 수기심폐소생술과 LUCAS를 비교한 결과 평균압박깊이, 압박률, 분당압박횟수에서 유의한 차이를 보였다(p<.001). 위와 같은 결과로 보아 LUCAS는 적절한 압력으로 가슴압박을 수행할 수 있고, 그 동안 구급대원의 전문기도기 삽입, 정맥로 확보 등의 전문심장소생술을 추가적으로 수행할 수 있으며 환자의 소생률을 높이는데 기여할 것이다.

구급차량 내 전자파 방출에 관한 조사 (Research on the emission of electromagnetic waves in ambulance)

  • Purpose: The study aims to provide basic data to enhance the health of paramedics responsible for patient transport and treatment by analyzing the exposure level of paramedics to electromagnetic waves generated by electric devices used in ambulances. Methods: The study measured electromagnetic waves in ambulances in N region from July to December 2018. ME3030B produced by German Gigahertz Solutions was used to measure these waves and the maximum value was selected by moving it slowly in various directions. Each measurement part was selected and the mean value was calculated by repeatedly measuring at 10-minute intervals three times in total: $1^{st}$, $2^{nd}$, and $3^{rd}$ phase. Results: Among the electrical devices installed in the patient room of the ambulance measured at distances of 1 cm or 30 cm, results showed a high level of electric waves at the inverter ($26.25{\pm}39V/m$) and high level of electromagnetic waves ($564.00{\pm}31.75nT$) at the ozone sterilizer. According to measurements toward the front near the driver's seat, results indicated high levels of electric waves ($3.67{\pm}1.15V/m$) and electromagnetic waves ($450.00{\pm}19.52nT$) at the black box hard drive. Conclusion: Electromagnetic waves within the ambulance were stable and not beyond the range that might impact human health. However, in the case of the black box hard drive ($3.67{\pm}1.15V/m$, $450.00{\pm}19.52nT$) located under the passenger seat, it may have a direct effect on the human body and, thus it is necessary to move it to a storage area further away from the paramedics to minimized the impact.

COVID-19 기간 119구급차를 통해 일개 응급의료센터에 내원한 환자 중증도의 변화 (Change in the severity of patients visiting emergency medical centers through the 119 ambulance during the COVID-19 pandemic)

  • Purpose: Despite the fact that emergency medical centers (ED) are used by emergency patients, more than 50% of non-emergency patients have been reported to be admitted in EDs, of which more than 10% of them used the 119 ambulance. Therefore, this study investigated whether there was a change in the severity of patients visiting EDs during the first year of the COVID-19 pandemic. Methods: This was a retrospective study of patients who visited the ED through the 119 ambulance in 2019 (13,735) and 2020 (11,015). Patient data were analyzed using the medical information system. Results: There was an increase in non-emergency patients at the emergency departments (KTAS levels 4-5) in 2020 with a rate of 58.0% (6,393), as compared to the 2019 data with a rate of 44.9% (6,169). However, the 2020 median length of stay in the emergency department was 188 minutes (IQR: 0-5,909minutes) (p=.000), which was decreased as compared to the 2019 median length of stay of 231 minutes (IQR: 5-6,211minutes) (p=.003). Conclusion: Providing emergency ambulance guidelines for selecting proper hospitals and educating patients to refrain from using the ED for non-emergency patients should be taken into consideration to prevent overcrowding and construct a more effective emergency medical system (EMS).

Study on the effect of 3 point belt on chest compression

  • 본 연구는 이송 중 심장정지 환자에게 가슴압박의 질을 높일 수 있는 방법을 제안하였다. 병원 전 단계에서 심장정지 환자에게 심폐소생술을 제공 할 때 구급차에 고정되어 있는 3점식 벨트를 사용하여 가슴압박의 질을 높일 수 있다. 119구급대원이 가슴압박 방법과 더불어 3점식 고정벨트를 착용한 경우 가슴압박의 질이 더 높아졌다. 또한 구급대원의 위험에 노출될 가능성도 적어졌다. 따라서 이송 중 구급차 안에서 3점식 고정벨트를 착용한 경우 119 구급대원은 안전을 확보할 수 있게 되고 심장정지 환자에게는 고품질의 가슴압박을 제공 할 수 있다.

119구급대원의 업무 효율성 향상을 위한 구급차 내 처치공간 개선에 관한 연구 (A study on the improvement of first-aid activity space within the ambulance for the efficiency of 119 EMT's work)

  • Purpose: The purpose of this study is to provide the basic data for the ergonomic design of first-aid activity space(FAS) for a future ambulance suitable for the performance of 119 EMT. Methods: Questionnaire was filled out by the 124 EMT in 119 centers. The questionnaire consisted of 3 sub factors including the satisfaction and compatibility of the FAS. The satisfaction and compatibility was analyzed based on characteristics of the subjects. Results: The satisfaction rate of the FAS within the ambulance was an average of 2.23, and the compatibility category got an overall average score of 2.26. The lowest ranked criteria in the satisfaction and compatibility factor was the functionality of storage space showing 1.93 and 1.88, respectively. The best position of the paramedic seat was the top side of the patient, near the head, which is suitable for patient assessment and treatment. Conclusion: During patient transportation of 119 EMTs, for efficient work process, a layout design of the FAS should be based on ergonomics that satisfied the performance function.

IT 융합 기술을 적용한 응급관리 정보 시스템의 신뢰성 향상 방안에 관한 연구 (Research on Enhancing Reliability of IT Convergence Technology Applied Emergency Management Information System)

  • 응급 상황이 발생하면, 주어진 응급관리 정보 시스템(EMIS)에 의하여 응급차가 출동한다. 그러나 기존 EMIS의 문제점은 전문 의료인과 응급차 간의 신뢰성 높은 의사소통에 주안점을 두지 않아서 불필요한 사망자를 초래했다는 점이다. 본 논문에서는 이와 같은 단점을 해결하기 위해 고신뢰성 응급관리 정보 시스템인 HEMIS(Highly-reliable Emergency Management Information System)를 제시한다. 평가 결과, HEMIS는 기존의 EMIS에 비해 데이터 압축, 데이터 안전성 그리고 QoS면에서 우수하다는 것을 알 수 있었다. 그러므로 HEMIS는 응급차 내에서 의료 서비스 질을 높임과 동시에 응급차로 이동하면서 사망자 수를 줄일 것으로 기대된다.

효율적인 응급의료 정보전달매체로서의 119구급활동일지 분석 (An Analysis of the 119 EMS System using the Standardized Record on the Efficient Emergency Medical Information Delivery Media)

  • 이 연구를 위해 2009년 1월1일부터 2009년 2월 8일 까지 일개 종합병원에 119구급대로 내원한 모든 환자의 구급활동일지 255부를 조사하였다. 구급활동일지의 전체 기재율 62.1%, 가장 높은 항목은 환자 인수자에 대한 기록으로 100.0%, 가장 낮은 항목은 의사지도에 관한 항목으로 0.4%로 나타나 기대치에 미치지 못한 것으로 조사되었다. 효율적인 응급의료 정보전달매체로서의 119구급활동일지의 기재율을 높이기 위해서는 전문 인력의 확충과 의료진의 적극적인 관심 및 피드백, 구급활동일지의 항목 배열의 규칙성, 기록의 중요성에 대한 지속적인 교육이 필요할 것으로 사료된다.
