Daily localization of partial breast irradiation patients with three-dimensional ultrasound imaging

  • Purpose: Accurate localization of the lumpectomy cavity during accelerated partial breast radiation (APBR) is essential for daily setup to ensure the prescribed dose encompasses the target and avoids unnecessary irradiation to surrounding normal tissues. Three-dimensional ultrasound (3D-US) allows direct visualization of the lumpectomy cavity without additional radiation exposure. The purpose of this study was to evaluate the feasibility of 3D-US in daily target localization for APBR. Materials and methods: Forty-seven patients with stage I breast cancer who underwent breast conserving surgery were treated with a 2-week course of APBR. Patients with visible lumpectomy cavities on high quality 3D-US images were included in this analysis. Prior to each treatment, X-ray and 3D-US images were acquired and compared to images from simulation to confirm accurate position and determine shifts. Volume change of the lumpectomy cavity was determined daily with 3D-US. Results: A total of 118 images of each modality from 12 eligible patients were analyzed. The average change in cavity volume was 7.8% (range, -24.1% to 14.4%) on 3D-US from simulation to the end-of-treatment. Based on 3D-US, significantly larger shifts were necessary compared to portal films in all three dimensions: anterior/posterior (p = 7E-11), left/right (p = 0.002), and superior/inferior (p = 0.004). Conclusion: Given that the lumpectomy cavity is not directly visible via X-ray images, accurate positioning may not be fully achieved by X-ray images. Therefore, when the lumpectomy cavity is visible on US, 3D-US can be considered as an alternative to X-ray imaging during daily positioning for selected patients treated with APBR, thus avoiding additional exposure to ionizing radiation.

  • The industrialization and globalization in the field of broadcasting are rapidly proceeded by extension of multilateral agreement as GATT and WTO, and by expansion of bilateral FTA. The broadcasting gets important in the industries according to the change of broadcasting environment grounded on industrial logic. As the broadcasting products become an important article of trade, broadcasting industry is the best bet in the cultural industries. In the international trade, the USA and Japan will treat cultural products the same as common goods and keep in the frame of free trade. On the contrary, the EU nations and Canada take a position that the cultural products are common goods and also public goods at the same time, and that therefore the cultural products will be excepted from the free trade. But this so called cultural exception, which is formed in the multilateral free trade agreement, is merely a temporal countermeasure, not a fundamental alternative especially in the present circumstances of DDA negotiation of WTO and of enlargement of FTA by the USA. So a nation shall carry out policies for cultural identity and cultural autonomy by the guarantee of Cultural Diversity Agreement of UNESCO, and organize a new cultural exchange order which substitutes the trade order by trade agreements.

Job Titles of Medical Technologist Managers in a Hierarchical System and Team System (계층제와 팀제에서 임상병리사 관리자 명칭)

  • In the 2000's, due to a change in hospital management strategy, the organizational structure shifted from a hierarchical system to a team system. While the hierarchical system is characterized by being activity centered, job title linked, and vertically managed, the team system is characterized by being competency centered, job title segregated, and horizontally managed. The job titles of medical technologist manager was surveyed three times in 1997, 2007, and 2017. It has been confirmed through staff members working at 24 hospitals in more than 500 beds in the metropolitan area. The results of job titles follow are as follow: "Team Leader; Part Leader" 14/24 (59%), "Chief Technologist; Area Head Technologist" 7/24 (29%), and "Chief" 3/24 (12%). The present authors propose an alternative name based on the team system to refine the three job titles currently used by medical technologists. First, the Chief Technologist is unclear if it refers to the Technologist General Manager or Technologist Manager. The Chief Technologist should be changed to "Team Leader". Second, given that Area Head Technologist or Section Chief are on the same position as Head Nurse, we suggest that Area Head Technologist or Section Chief should be changed to "Part Leader". Third, while the organization regulation is marked merely as Department of Laboratory Medicine according to the hierarchical system, it is marked as Laboratory Medicine Team according to the team system. Medical technologists come to have more belongingness, feeling of solidarity, and intimacy under the team system.

PRIDE 3D Simulator for Virtual Verification of Remote Handling Procedures in Processing Cell (PRIDE 3D 시뮬레이터를 통한 공정셀 내부의 원격작업 가상검증)

  • The Korea Atomic Energy Research Institute (KAERI) constructed the PyRoprocessing Integrated inactive Demonstration facility (PRIDE) to carry out experiments on engineering scale pyroprocessing. PRIDE has a large processing cell that human workers are not allowed to access, and thus the equipment inside is operated from outside using remote handling systems. It is therefore essential to examine the operability and maintainability of the equipment in view of remote handling systems, and the equipment is thoroughly examined in a mockup cell before it is installed in the processing cell. If the equipment is tested in a virtual mockup rather than in a mockup cell, the development cost can be significantly reduced. The PRIDE 3D simulator was integrated for virtual verification of equipment that will be installed in the processing cell. All remote handling devices in the actual PRIDE were also virtually installed in the PRIDE 3D simulator. The 3D model of the equipment was loaded and located in the exact position in the virtual processing cell. A scenario to replace an actual electrode was implemented in the PRIDE 3D simulator. The design of the equipment and the working procedures in the scenario were successfully evaluated. The results demonstrated that the PRIDE 3D simulator can be used successfully as an alternative to actual mockup testing.

The Influence of Low Cost Airline's Flexible Fare Policy on Consumers' Perceptions of Price Fairness (저가항공사의 유동적 요금 전략이 소비자의 가격공정성 지각에 미치는 영향)

  • Purpose - The purpose of the study is to reexamine the price fairness as practiced by low cost airlines, as a consumer has to experience such inconveniences as inferior airport transportation, extra fees on in-flight meals, and non-negotiable seats, and consumers evaluate such experiences keeping in mind their total costs. This evaluation includes price fairness and allows a reasonable and overall consideration of factors of low cost airlines. It tries to set up a measurement of the indicators consumers' perceptions of price fairness academically as it adapts price fairness to airline services which are renowned for price volatility. Research design, data, and methodology - The research proposes an alternative pricing strategy for the long term profit of low cost airlines after going over conflicts between the traditional theory of consumers' price perception mechanism and flexible fair policy of low cost airlines. It was meaningful when it relates to the early stage of the business, while it enhances the risks relating to the long term survival of low cost airlines. In addition, it is significant as it highlights the negative influences on consumers' perceptions of price fairness, as low cost airlines run on extremely low cost perspectives. Results - The results of the research provide insight into four perspectives, as consumers' perceptions of price fairness are influenced by the frequency and range of price changes and services. The first perspective is that it would lead to positive price evaluation when a low cost airline cuts prices frequently with little changes than one big change. It also would lead to the same result when it comes to necessary services. The second perspective is that one big increase of price would rather undermine the negative aspects of price changes than those of several smaller ones. The third perspective is that additional services would be good to consumers' perceptions of price fairness as compared to discount benefits with respect to the cost. Finally, a low cost airline should consider that consumers will change airlines or defer their flight schedule if the flight fares increase beyond their limits. Conclusions - Low cost airlines should reconsider their pricing policies for services that were provided free earlier. A consumer would not like discount benefits when made to pay for services that were, for long, free of charge. If a low cost airline can provide services with no charge, it should improve volumes if the costs are standardized and, moreover, should consider the charging fees. Alternatively, a consumer can choose between services and fair discount. Low cost airlines are implementing sales promotion strategies, as the competition is more intense than it used to be. In these days, they should regard services over sales promotion, as consumers may prefer to spend money on good premium services. Some differentiation in services could create a good market position for the airlines and, hence, good financial performance.

Difference in Rooting in the Scion from Different Node of Ligusticum chuanxiong Hort (토천궁(土川芎)의 삽수(揷穗) 채가(採歌) 부위(部位)에 따른 발근차이(發根差異))

  • Scions from the one- and two-years old and the nodes of Ligusticum chuanxiong stem were used to study the rooting ability with the treatment of plant growth regulators planted to different bed soils as the alternative propagation for the present method by rhizome. The rooting ratio in the scion from the one- and the two-years old plants showed any difference, but that was ranged from 94% to 100% in the first node and from 56% to 64% in the second node from the bottom of the stem. The scion of first node from both the plants showed more than 90% in the root ratio and 16 in the number of rooting and growed more than 31cm in the root length. The Rootone-F and 250ppm of NAA were most effective for the promotion of rooting. Rooting ratios in the sand and in the mixture of vermiculite and perlite as bed soils were about 18%, but that in the cultured soil considerably decreased.

A Study on Community Service Order : It's Effective Introduction, Operation, and Establishment in Korea (우리나라 사회봉사명령제도의 효과적 도입, 운영 및 정착에 관한 연구)

  • Community Service Order is a program that holds a significant place in the system of probation. With its important role in the probation, there is an equally high expectation of the intervention from the program among the people which it serves. The problem lies in that people's understanding of the program is minimized to merely a volunteer-based social activity, which cause a division on how the program is to be run. The purpose of this research is to come up with a method that can efficiently organize the administrative plan to maximize its potentiality of the program. To adopt such a plan, "Sentencing to Service," an effective community service order program in Minnesota, U. S. A. was examined. An interview with the administrative staff, as well as a survey, using systematic sampling, with the participants was conducted to study the effective process and intervention of the program that has existed for fifteen years. The result concluded that "Sentencing to Service" is a successful program with the components of success being that of a positive support from the local community and high motivation level of the participants. In order for the program to be a positive experience for the participants, it is highly contingent upon the crew leaders. On the basis of the research findings, the following suggestions are made. First, a structured guideline for those who are involved in the program is necessary. Second, thorough training provided for the crew leaders are required and a social worker is recommended for the position of crew leader. Lastly, Community Service Order should not widen the net, but ought to serve as an alternative to incarceration.

The Influence of Unattended Distractors on the Identification of Targets (주의하지 않은 방해자극이 표적의 식별에 미치는 영향)

  • Negative repetition effect (NRE) refers to the phenomenon that the accuracy of report is impaired when a target was flanked by the same distractor than when by alternative distractor. To probe the nature of NRE, this study introduced attention window(s) indicating the positions where a target might be presented, and non-attention window(s) where a target could not be presented. Attention windows are supposed to help participants detect targets readily. Two among three positions are indicated by attention windows in Exp. 1, and a single large attention window encompassing central two positions among four positions was used in Exp. 2, and either large or small attention window was used depending on the number of target candidates in Exp. 3. In the result of three experiments, NREs were consistently observed when both positions of a target and a distractor were indicated by previous attention windows. However, NREs (including its tendency) and its opposite, PREs were observed when a distractor was presented in the non-attention position, depending on its distance from the target and the size of attention window. It seems that this pattern of repetition effects is hard to be explained by repetition blindness hypothesis (Kanwisher, 1991), positional uncertainty hypothesis (Keren & Boer, 1985), and inhibitory attention capture hypothesis (Kwak et al., 1993). Instead it was proposed that shifting of spatial attention should be considered accordingly with the structure of stimulus display. The promising role of this task was discussed in studying the relation of attention and perception.

The Measurement of Ho-166 Absorbed Dose for the Endovascular Irradiation with a Balloon Angio Catheter Using a GafChromic Film (GafChromic 필름을 이용한 Ho-166 의 혈관내 방사선조사를 위한 선량분포 측정)

  • The GafChromic film was used for the dosimetry of a balloon angio catheter filled with the radioisotope HO-166 for endovascular irradiation. The balloon angio catheter was 2 cm long and 3 mm in diameter when inflated. The isotope, Ho-166, was produced by the neutron bombardment using the research reactor in Korea Atomic Energy Research Insititute. Co-60 teletherapy beam was used for making H-D curve for the Gaf-Chromic film. The film dosimetry was measured with a videodensitometer. The radial dose distribution indicated that the absorbed dose dropped to about 20% of the surface dose at the 1 mm away from the balloon surface and at 5 mm position the dose decreased to below 1% of the surface dose. The result also shows that with the specific activity of Ho-l66, 250 mCi/ml it takes 230 seconds to deliver 1200 cGy to the region where is 1mm away from the balloon surface. The concentric isodose curves were also presented. The Ho-166 is an another alternative for endovascualr irradiation to prevent restenosis after PTCA (Percutaneous Trans Coronary Angioplasty)

Siamese Cats - Analysis of Six Thai Independent Animators

  • As an animation educator at Mahidol University International College, Bangkok, for eight years I have been in a privileged position to be educating a small section of a new generation of Thai animators. Thailand has had little animation experimentation of their own and pre-internet had limited access to such diversity so I have attempted to bring some of the wealth of animation history, in particular British and European, as this is my background, into the classroom. In particular my teaching aim has been to introduce various possibilities to the choices of medium, styles and textual narratives that early (and current) experimental and independent animators have explored. Since the release of the first commercial feature film, Khun Kluay 2006, the Thai animation industry has gone through various highs and lows in the commercial sector, building a skilled workforce, many of whom trained and worked abroad then returned. Now there is strong evidence of an independent animation starting to emerge. In a continuation to my research on Thai Animation this paper will analyze selected independent works in more detail, exploring the themes, style and technology used. Whilst also acknowledging the possible mis- interpretation as that this is coming from the perspective of an outsider whose cultural language and interpretation may transcend the creators' given experiences and add alternative interpretation. The aim will be to contextualize the content, ideas and cultural perspective and bring more Thai works into the gaze of animation studies academics
