Allergic to Pool Water

  • To identify the allergy problem of a 36-year old swimming instructor, who experiences heavy itching and rashes whenever she comes in contact with pool water. Patch tests were performed with European standard series and materials from the work floor. A positive patch test to aluminum chloride and flocculant was observed. Occupational dermatitis is, based on a contact allergy to aluminum chloride in the flocculant.

Knowledge of atopic dermatitis and food allergies, as well as health information literacy, among North Korean refugee mothers: a descriptive survey study

  • Purpose: This study investigated knowledge of atopic dermatitis and food allergies, as well as health information literacy, among North Korean refugee mothers with preschool-age children and explored how these variables were related to participants' characteristics. Methods: A descriptive study design was used. Data from 130 North Korean refugee mothers were collected between January and March 2023. Results: The mean scores were 14.48 of 30 for atopic dermatitis knowledge; 2.77 of 10 for food allergy knowledge, and 56.95 of 80 for health information literacy. Significant differences were observed in knowledge of atopic dermatitis based on breastfeeding duration (F=4.12, p=.009), and in knowledge of food allergies based on mixed feeding (F=3.11, p=.049). Health information literacy showed significant relationships with education level (F=3.76, p=.026), occupation (F=3.99, p=.021), checking nutritional information (t=2.91, p=.004), mixed feeding (F=4.50, p=.014), and atopic dermatitis diagnosis (t=6.86, p=.001). Significant positive correlations were observed between atopic dermatitis knowledge, food allergy knowledge, and health information literacy. Conclusion: Personalized educational programs should be implemented to improve awareness of allergies and health information literacy among North Korean refugee mothers, which would help them find, evaluate, and understand health-related information. These programs should focus on providing nutrition and dietary education to promote healthy growth in children and prevent diseases.

아토피피부염 자녀를 둔 어머니의 질환관리 효능감 관련 요인 (Factors associated with Disease Specific Self-efficacy of Mothers of Children with Atopic Dermatitis)

  • Purpose: The disease specific self-efficacy of mothers caring children with atopic dermatitis is a crucial factor for adherence to recommended treatments. This study investigated factors associated with disease specific self-efficacy of mothers of atopic dermatitis children. Methods: A cross-sectional study was used. One hundred twenty five mothers recruited from the out-patient clinic of three hospitals completed self-administrative questionnaires, including the disease specific self-efficacy, general parenting efficacy, and severity of atopic dermatitis. Descriptive statistics, ANOVA, and multiple regression analysis were performed. Results: The degree of disease specific self-efficacy of mothers was significantly different according to the disease severity, and parenting efficacy. 26.0% of the variance of disease specific self-efficacy was explained by the severity of disease and parenting efficacy. Conclusions: Future studies need to focus on the development of educational programs that will help to improve mothers' disease specific self-efficacy as well as parenting efficacy.

프로바이오틱스에 의한 알레르기 질환 제어 및 안전성 (Control of Allergy with Probiotics and Its Safety)

  • Allergic diseases, including atopic dermatitis, asthma, allergic rhinitis, and food allergies, could be caused by dysbiosis that results in an immune system imbalance. The incidence of allergic diseases has been increasing and they are now one of the most common diseases throughout the world. Recently, probiotics have been suggested as an alternative intervention for the prevention and treatment of allergic diseases. Probiotics are endogenous microflora with functional effects within hosts. They have various clinical and immunological capacities and have recently been considered as a supplement for the treatment and prevention of allergic diseases. Probiotic bacteria modulate immune cells such as Th1, Th2, and regulatory T cells that are correlated with protection against atopic dermatitis, however, safety concerns for the use of probiotics have been raised. Therefore, further research is needed to clarify the efficacy and safety of probiotics in the treatment of allergic diseases.

유아기 아토피 피부염에 있어서 식품 알레르기의 역할에 대한 고찰 (The Role of Food Allergy in Infantile Atopic Dermatitis)

  • Infantile atopic dermatitis(AD) may be developed by food allergens due to immature intestinal epithelium and its mechanism, which may have no clear-cut evidence, is thought to be IgE mediated immediate and late phase hypersensitivity. It is not easy to diagnose AD caused by food allergens exactly so it is likely to be underestimated more than it be. But we must consider it as a major factor of not only infantile AD but childhood and adult AD. We can see similar theory at previous Oriental medicine. Allergens can be transmitted to a fetus through the placenta and infantile AD is inflammatory condition by food allergens and immature function of intestines. So we must consider those factors at infantile AD treatment. We expect a new model of infantile AD treatment combining the conventional therapy with the diet therapy based on the Oriental medical theory.

  • Atopic dermatitis, caused by immune hyper-responsiveness, is wide spread in humans as well as in the dogs, especially in industrialized condition. Pet dogs are generally exposed to the same environment as their owners, and a significant portion of these animals are also known to suffer from this allergic disease. However, diagnosis and treatment methods of atopic dermatitis in animals have not been well established. We explored the possibility of using recently developed PG102 for the treatment of atopic dermatitis in the canine population. PG102 is a water soluble extract prepared from Actinidia arguta, and has been shown to produce significant therapeutic effect in variable allergy animal models. After oral administration of PG102 at a dose of 12.5 mg/kg daily for 4 weeks, severity of disease was greatly improved. IgE is one of representative members used to diagnose allergic diseases in humans. However, it is not well established whether there is any correlation between the serum level of IgE and atopic dermatitis. Our data indicated that dogs diagnosed to have atopic dermatitis contained higher level of serum IgE than the normal dogs and that treatment of dogs with PG102 significantly lowered the serum level of IgE. Taken together, this study demonstrated that PG102 treatment yielded significant amelioration of canine atopic dermatitis and down-regulation of serum IgE and that the serum level of IgE can be used as a convenient member for diagnosis of atopic dermatitis in dogs.

  • Food allergy is an important public health problem affecting 5% of infants and children in Korea. Food allergy is defined as an immune response triggered by food proteins. Food allergy is highly associated with atopic dermatitis and is one of the most common triggers of potentially fatal anaphylaxis in the community. Sensitization to food allergens can occur in the gastrointestinal tract (class 1 food allergy) or as a consequence of cross reactivity to structurally homologous inhalant allergens (class 2 food allergy). Allergenicity of food is largely determined by structural aspects, including cross-reactivity and reduced or enhanced allergenicity with cooking that convey allergenic characteristics to food. Management of food allergy currently focuses on dietary avoidance of the offending foods, prompt recognition and treatment of allergic reactions, and nutritional support. This review includes definitions and examines the prevalence and management of food allergies and the characteristics of food allergens.

마행의감탕(麻杏薏甘湯)으로 호전된 알레르기 질환을 동반한 안면부 포진상 피부염 치험 1례 (A Case Report of Facial Dermatitis Herpetiformis with Allergy Diseases Improved by Administration of Mahaengeuigam-tang)

  • This study aims to report a case of facial dermatitis herpetiformis with allergy diseases improved by administration of Mahaengeuigam-tang (Maxingyigan-tang). Multiple blisters appeared on the patient's face, Erythema, papules, and scales were also observed on the neck, back, and knee areas of the patient. The patient has had cough, sputum, and dyspnea symptoms since she was 5-6 months old. After diagnosing the patient's condition as dermatitis herpetiformis, atopic dermatitis, and bronchial asthma through history taking and symptom observation, Mahaengeuigam-tang (Maxingyigan-tang) was prescribed for about 30 days. As a result of evaluation through our own criteria, dermatitis herpetiformis symptoms improved from 18 to 2 points. Atopic dermatitis and asthma symptoms also decreased from 12 to 0 and 20 to 3 points, respectively. Since this study has proven the efficacy of Mahaengeuigam-tang (Maxingyigan-tang) for dermatitis herpetiformis patient, this prescription may be fully utilized as the treatment for other inflammatory skin diseases.

The Importance of Food Open Challenge Test in Atopic Dermatitis: The Comparison of Allergy History, Skin-Prick Test, and Specific IgE Detection

  • Food allergies play an important role in Atopic Dermatitis (AD). Dietary manipulation is essential in the management of AD. However, there has been a paucity of data reporting the prevalence of food allergies in AD patients in Korea. In this study, the Food Open Challenge Tests (FOCT) were conducted to investigate food allergies in AD patients. The skin-prick test and the detection of specific IgE, as well as allergy history of patient were used to detect food allergies in all AD patients. Elimination diet was conducted for two weeks prior to FOCTs. The prevalence of food allergies by FOCT is as follows: milk (67.3%); chicken (64.2%); pork (62.8%); eggs (61.0%); beef(55.4%) ; wheat (52.0%) and soybean (45.2%). Allergenic food items in Korean AD patients were different from people in other foreign countries. The seven major foods those tested positively by FOCTs were completely eliminated from the replaced diets for two weeks, and were subsequently reintroduced one at a time. Results from FOCTs were not comparable with allergy history or skin-prick tests or specific IgE detection. The sensitivity and specificity of skin-prick tests and specific IgE detection were lower than FOCTs. Allergy history, skin-prick tests, and specific IgE detection are useful for the identification of food allergen but its clinical significance differed according to food items. Therefore, we conclude that even though a 10-day delay was necessary after food challenge, FOCT is a useful and valid method to confirm food allergies and may be essential for the effective control of food allergies for treatment of AD.

HaCaT 세포에서 Lactobacillus 혼합배양액 추출물이 아토피관련 케모카인 발현에 미치는 효과 (The Effect of Lactobacillus Mixture Culture Fluid Extracts on Atopic Dermatitis Chemokine Expression of in HaCaT Cells)

  • Objectives : Recently the case of lactobacillus mixture culture fluid appliment was reported. In this study, anti-inflamation effects and anti-allergy effects were studied by stimulus of lactobacillus mixture culture fluid extracts in HaCaT cells. Methods : The atopic dermatitis were induced by TNF-${\alpha}$ and interferon-${\gamma}$ in HaCaT cells. TARC/CCL17, MDC/CCL22, RANTES/CCL5 and ROS production were investigated to explain anti-inflamation and allergy effects of lactobacillus mixture culture fluid with cell-enzyme-linked Immunosorbent assay in 450 nm, 485 nm, 535 nm with spectro-fluorometer. Results : The extracts of lactobacillus mixture culture fluid were decreased TARC/CCL17, MDC/CCL22, RANTES/CCL5 expressions and ROS production with a concentration dependent manner. Conclusions : The effects mechanism of Lactobacillus mixed culture fluid for atopic dermatitis symptoms were considered to be explain anti-inflamation and allergy effects via control of cytokine, chemokine and ROS production, and the fluid could be applied in skin cells directly. But classified AD symptom degrees reported in clinical case before as Reaction Period, Reduction Period, Effect Period, Reproduction Period and Rebound Period could not be explained. Further study will be expected.
