A Clinical study on the treatment of Atopic Dermatitis using Naesowhajungtanggamibang (내소화중탕가미방(內消和中湯加味方)의 아토피 피부염 치료에 대한 임상고찰)

  • Objective : The objective of this study is to assess the clinical effect of Naesowhajungtang Gamibang on atopic dermatitis. Method : A total of 30 patients with atopic dermatitis were evaluated for the symptoms and signs according to SCORAD index and measured for peripheral eosinophil counts and serum total IgE before and after treatment. Results : 1. Among 30 patients diagnosed as Atopic Dermatitis, 11, were male(37%) and 19 were female(63%). the ratio of male and female patients were 1:1.7. 2. Among 30 patients diagnosed as Atopic Dermatitis, 25 Have family history of allergic disease(83%) and 20 have past history of allergic discase. 3. There was statistically significant relationship between serum total IgE and SCORAD score(p=0.019). 4. There was statistically significant relationship between peripheral eosinophil count and SCORAD score(p=0.001). 5. We could find no statistically significant change of serum total IgE and peripheral eosinphil count before and after treatment. 6. There was statistically significant change of SCORAD score and subjective symptom before and after treatment.(p<0.05). Conclusion : Naesowhajungtanggamibang is effective in the treatment of atopic dermatitis. And peripheral eosinophil counts and serum total IgE were good serum maker reflection the severity of atopic dermatitis.

  • Atopic dermatitis (AD) usually develops in patients with an individual or family history of allergic diseases, and is characterized by chronic relapsing inflammation seen specially in childhood, association with IgE hyperproduction and precipitation by environmental factors. and wished to examine closely effect that Polygonum multiflorum isolated PM-E and PM-70M orally adminstration used to atopy dermatitis disease patient get in atopy eruption control experimentally. Atopic dermatitis is a chronically relapsing inflammatory skin disease. Animal models induced by relevant allergens play a very important role in the elucidation of the disease. This study was investigated the anti-allergic effect of PM-E and PM-70M on BMAC induced atopic dermatitis in NC/Nga mice. We summerized as the follow. PM-E and PM-70M significantly reduced the skin number of total cell number, CD4+ and CD11b+/Gr-1 cell compared with positive control and decreased the invasion of CD4+ cell in dorsal skin tissue compared with positive control group by using immunohistochemical staining and chemokine such as eotaxin and CCR3 compared with positive control group. PM-E and PM-70M markedly suppressed invasion and edema of leukocytes and mast cell in dorsal skin. Taken together, these findings suggested that PM-E and PM-70M has an anti-allergic activity and this might be useful for the clinical application to treat allergic diseases such as atopic dermatitis.

Component Analysis of Sweet BV and Clinical Trial on Antibody Titer and Allergic Reactions (Sweet BV의 성분분석과 항체역가 및 allergy 반응에 대한 임상적 연구)

  • Objectives : The aim of this study was to observe prevention of allergic reactions of Sweet Bee Venom (removing enzyme components from Bee Venom). Methods: Content analysis of Sweet Bee Venom and Bee Venom was rendered using HPLC method and characterization of Anti-Sweet Bee Venom in Rabbit Serum. Clinical observation was conducted for inducement of allergic responses to Sweet BV. Results : 1. Analyzing melittin content using HPLC, Sweet BV contained 34.9% more melittin than Bee venom pharmacopuncture at same concentration. 2. Observing chromatogram of HPLC, removal of the enzyme was successfully rendered on Sweet BV. 3. The anti-serum of Sweet BV showed high titers against melittin and bee venom and relatively low titer against phospholipase A2. 4. After conducting approximately 3,000 cases of Sweet BV administration, not a single case of generalized anaphylatic reaction occurred in clinical observation. 5. Mild compared to the bee venom pharmacopuncture, Sweet BV showed some acute hypersensitive reactions of edema, itchiness, and aching locally. 6. Sweet BV was administered on six patients with previous history of suffering from generalized acute hypersensitive reactions with the bee venom. None of the patients showed allergic reactions with Sweet BV, suggesting it can effectively prevent anaphylatic shock which may occur after the bee venom pharmacopuncture procedure. Conclusion : Summarizing above results, Sweet Bee Venom appears to be an effective measurement against allergic reactions from the bee venom pharmacopuncture especially against anaphylatic shock.

Effect of Samul-Tang on the Allergic Inflammatory Response (사물탕(四物湯)이 알러지 염증반응에 미치는 영향)

  • Samul-Tang (SMT) has been used for nourishing of the blood, hematopoiesis as a herbal medicine history. The purpose of this study is to find out anti-allergic inflammatory reaction of SMT. To clarify the mechanism, the effect of SMT on vascular permeability of rat cutaneous tissue and histamine and cytokines (IL-6, IL-8, TNF-${\alpha}$) release from mast cells were observed. The results are the pretreatment with SMT significantly decreased the compound 48/80-induced degranulation and histamine release from RPMC, SMT also inhibited the anti-DNP lgE-induced increment of vascular permeability of rat cutaneous tissue. SMT significantly reduced the PMA plus A23187-induced increment of expression of IL-6, IL-8, and TNF-${\alpha}$ in HMC-1 Cell. The Present study provide evidence that SMT inhibits mast cell-derived inflammatory allergic reactions by blocking histamine release and pro-inflammatory cytokine expression, and suggest the mechanisms of action. Furthermore, in vivo and in vitro anti-allergic effect of SMT suggests a possible therapeutic application of this agent in inflammatory allergic diseases.

The Relationship between the Prevalence of Allergic Diseases and Urinary Cadmium Concentrations among School-age Children in Two Regions in Ulsan Metropolitan City (울산의 두 지역 초등학생의 알레르기 질환 유병과 요중 카드뮴 농도와의 관련성)

  • Objectives: The purposes of this study were to determine the relationship between the prevalence of allergic diseases and urinary cadmium concentration among children from six to 12 years of age, and to evaluate the association between cadmium in urine and other immunologically related factors in two elementary schools with different environmental conditions. Methods: Data on physician-diagnosed prevalence over the past 12 months and potential risk factors for allergic disease were collected from survey results drawn from 236 children living in different regions (central urban vs. suburban) from April to July in 2014 by an ISAAC questionnaire. We analyzed cytokine levels in serum through enzyme-linked immunosorbent assay and urinary cadmium concentration by use of inductively coupled plasma mass spectrometry. Results: Concentrations of urinary cadmium in suburban and central urban children were $0.11{\pm}0.11ug/L$, $0.14{\pm}0.17ug/L$, respectively (p>0.05). Results from the analysis of the t-test and chi-square test showed that urinary cadmium levels were positively associated with environmental factors (paternal smoking status, passive smoking, traffic volume and experiences of outdoor odors) and individual genetic factors (parental allergic disease). Cadmium concentrations in urine were also positively associated with the prevalence of allergic disease and immune-related cytokines (eosinophils, IgE, IL-5, IL-33, IL-17). Conclusion: This study suggests that genetic factors (parental history of allergic diseases), environmental factors, and regional status (suburban and central urban) should be considered as probable factors increasing the concentration of urinary cadmium, which has the potential to influence the allergic disease prevalence of school-age children.

  • Objectives The purpose of this study is to find the relationships between the allergy and weakness in children in Seong-Dong district. Methods 802 day care centers children were participated for survey on allergic disease, risk factors, and weakness. From the result, we analyzed the relationships between the aforementioned factors. Results 1. 802 children were participated for the survey. We eliminated survey answers with unclear answers, and selected 792 survey sheets as our final subject of study. The age of majority of subjects(720 children) were between one to six years old, and the others were under the age of one and over seven. 2. The prevalence rate of asthma in a lifetime was 6.69%, allergic rhinitis was 14.87%, atopic dermatitis was 28.82%, allergic conjunctivitis was 6.29%, food allergies was 9.58%, and drug allergies was 1.4%. In 2009, the prevalence rate of allergies during treatments is 6.69% for asthma, 13.41% for allergic rhinitis, 13.64% for atopic dermatitis, 4.13% for allergic conjunctivitis, 5.23% for food allergies, 0.96% for drug allergies. 3. The significant risk factors were types of dwelling, pet with fur, dirty environments without vacuum cleaners, the numbers of times bed covers were washed, frequency of air ventilation, changes in body after moving, family history. 4. The distribution of the number of children with weakness within the five visceras were respiratory weakness, digestive weakness, psychoneurologic weakness, neuromotor weakness, and urogenital weakness in the order of most to the least. More specifically, children who only has respiratory weakness were the majority, while children who have respiratory and digestive weaknesses, children who have respiratory, digestive, and psychoneurologic weaknesses, children who have respiratory and psychoneurologic were followed. 5. With the exception of children with urogenital weakness, Most of the children with respiratory weakness, digestive weakness, psychoneurologic weakness, and neuromotor weakness had been treated for allergies in 2009, while children who were diagnosed with allergic diseases at least once in their lifetime most likely had weak respiratory, digestive or psychoneurologic systems. Conclusions The results of this study showed that there was a correlation between children with weak viscera and children with allergic diseases. However, further research would be necessary.

A clinical study of allergic rhinitis (알레르기 비염에 관한 임상적 연구)

  • As recent developments of Immunology and Nuclear medicine, serum IgE and IgG values are helpful in the diagnosis and evaluation of the therapeutic effects of nasal allergies. But in Korea, air pollution and the increased use of food additives have become leading factors in nasal allergies, It seems to be induced by environmental change, especially industrialization and urbanization, so allergic rhinitis in our environment has changed in accordance with the changes made in the living environment. Therefore this study is attempted in order to observe a clinical analysis which places more importance on allergic rhinitis. We studied 200 patients who had visited Kyunghee Oriental Medical Center with allergic rhinitis from January 1, 1999 to December 31, 1999 The results were as follows: 1. The sex distribution was 114 males(57%) and 86 females(43%). In age distribution, the average age was 25. In males, ages ranged from 3 to 66 years old and the average was 23.81. In females, ages ranged from 4 to 67 years old and the average was 28.57. The peak age was 30~39 years old(24%); under 9 years old and 10~19 years old were each 18%; 20~29 years old was 22%; 40~49 years old was 11 %; over 50 years old 6.5%. The gulf between males and females showed a statistically significant difference(P<0.025). 2. In the age of onset, male' s maximum was 62.5, minimum was 0.25; female s maximum was 59.5, minimum was 0.2. Under 9 years old was the most with 34%(male 24%, female 10%), 10~19 years old was 18%, 20~29 years old was 22.5%, 30~39 years old was 13.50%, over 40 years old was 12%, The gulf between males and females were showed statistically significant difference.(p<0.014) 3. The average duration of the disease was 5.67 years. In male and female, the maximum was 30, the minimum was 0.05; under 5 years old was the most with 62%(male 34.50%, female 27.50%); 6~10 years old was 23%. So, under 10 years old was 85%. There was no statistically significant difference in the duration of disease. 4. Regarding type of residence, 47.50% of patients with allergic rhinitis lived in apartments, 52.50% lived in houses. In males, 29.50% lived in apartments, 27.50% lived in houses. In females, 18% lived in apartments, 25% lived in houses. There was no statistically significant difference in the residence by T-test and chi-test. 5. In the distribution of season, spring is the most with 29.5% of patients, winter 28%, fall 25.5%, and summer 17%. But there was no statistically significant difference. 6. After observing 200 patients with allergic rhinitis, classifying main symptoms into 5 types, sneezing was the main symptom in 177cases(88.50%), nasal obstruction in 176cases(88%), rhinorrhea in 169cases(84.5%), post nasal discharge in 87cases(43.50%), and itching in I04cases(52%). The Cumulus ration is 98.50% and symptoms overlapped with an average 3.57±0.1 times but in an analysis of variance of these symptoms, the gulf between males and females was not recognized as statistically significant by T-test and ANOVA. 7. Patients whose families have allergic diseases account for 90 cases(45%) : 49cases(24.50%) male and 41cases(20.50%) female. There were 4 cases (71.11 %) whose families have allergic rhinitis, 9cases(10%) of asthma, and 7.78% with allergic dermatitis. There were 61 (67.80%) cases of patients whose parents have allergic diseases; cases wherein the patient s child had allergic diseases numbered 13 (14.45%); and cases with a sibling with allergic diseases totalled 16cases (17.80%). There was no statistically significant difference in allergic disease regarding sex, parents, or siblings by chi-test. 8. Blood type: For males, type A is the most common, with 37cases(18.5%), followed by type B with 32cases(16%), type O 28cases(l4%) and type AB 13cases(6.5%). For females, type B is the most common, with 30cases(15%), followed by type O with 23cases(l1.5%), type A with 18cases(9%) and type AB with 13cases(6.5%). There was no statistically significant difference in blood type by chi-test. 9. In the selection of prefered food, most patients prefer cool food, with 98 such cases(49%), tepid food in 54cases(27%) and warm food in 48cases(24%). These showed a statistically significant difference in the selection of prefered food between males and females by chi-test(p<0.009). 10. The state of Past History was classified into II types. chronic hypertrophic rhinitis is the most common with 11cases (18.64%), tonsil and adenoid hypertrophy is 8cases(l3.56%), sinusitis is 6cases(10.17%), nasal septum deviation is 4cases, nasal polyp is 2cases, others are 10cases(l6.95%). No statistically significant difference in past history between males and females was shown, but a statistically significant difference was shown when males and females were compared with total cases by T-test(p<0.002, P<0.0008). 11. Regarding complications, 37 patients (28.91%) had sinusitis: 22cases(17.19%) in male, 15cases(11.72%) in female. Chronic hypertrophic rhinitis was found in 15cases(11.72%). Others are under 10%. There was no statistically significant difference in the type of complications between males and females, but a statistically significant difference was shown when males and females were compared with total cases by T-test(P<0.00l, P<0.007). 12. In the treatment, medication was used 1691 times, an average of 2.58 times. No.34 was used 370 times for 124 cases, an average of 2.98 times. No. 152 was used 318 times for 106 cases, an average of 3.00 times. No.151 was used 307 times for 97cases, an average of 3.16 times. No. 31 was used 117 times for 33 cases, an average of 3.55 times. No 25 was used 116 times for 33 cases, an average of 3.52 times. 13. In the duration of treatment, the most frequent is 1 week(69cases, 34.50%), the maximum is 20weeks, and the minimum is 1week. A treatment period of 2~3 weeks accounted for 32% of cases, a period of 4~5weeks accounted for 13.5%. The gulf between males and females showed a statistically significant difference in the duration of treatment.(p<0.01). There was a statistical significance when the males were compared with total cases by ANOVA(P<0.03). 14. A comparison between before-treatment and after-treatment showed a statistically significant difference in treatment by T-test (p<0.01) and F-test (p<0.0058).

A Clinical Observation of the Nasal Acupunture Therapy on Nasal Obstruction (비침이 비색증에 대한 영향)

  • We studied 32 patents, who had visited Kyung Hee downtown oriental hospital with nasal obstruction. They has been observed their nasal cavity through anterior rhinoscopy. We had paten't middle and inferior turbinte bleeding by acupuncture. This symptom had been disappeared after this treatment. The result were as follows. 1. Sex and age distribution: The males were 18($56.2\%) and Female were 14($43.8\%$). The most common occurrence was found between 1-5 years old. 2. The most common duration of disease was between 2-6 months. 3. In decreasing order, the common complication & past history of otolaryngologic or allergic disease were allergic rhinitis, atopic dermatitis, chronic sinusitis, adenoid or tonsil hypertrophy, otitis media with effusion and asthma. 4. In descending order, distribution of symptoms were rhinorrhea, sneezing, dry sensation, sputum, frontal headach, postnasal drip and cough. 5. The improvement rate in symptom were $31.3\%\;excellent\;and\;53.1\%$ good.

A Clinical Study of Urticaria and an Assessment Guideline (두드러기에 대한 임상 연구 및 평가 지침)

  • Objectives: This study was designed to assess traditional Korean medical therapy for urticaria and establish a clinical assessment guideline. Methods: Duration of symptoms, personal and family history of allergic diseases, serum IgE level, suspected causes and Provocation factors were investigated in 56 patients. Frequency of wheals, duration of wheals, number of wheals, itch severity and antihistamine counts on a 6-point (0-5) scale were assessed in 34 patients. The change of total scores and each criterion between baseline and follow up were analyzed using paired t-test (p<0.05). Results: 1. Acute urticaria within 6 weeks was $26.8\%$ and chronic urticaria over 6 weeks was $73.2\%$. The mean duration of symptoms was $19.2\pm38.8$ months. 2. $57.1\%$ of patients had a personal history of allergic diseases and $50\%$had a family history. $29.1\%$ had over 200 IU/mL serum IgE level. 3. The suspected causes were none $(39.3\%)$, foods $(32.1\%)$, drugs $(12.5\%)$, contact $(5.4\%)$ and maternity$(5.4\%)$. 4. The suspected provocation factors were foods $(41.1\%)$, none $(26.8\%)$, fatigue $(12.5\%)$, heat and sweating $(12.5\%)$, pressure $(10,7\%)$ and stress $(7.1\%)$. 5. The total scores between baseline and follow up of 34 patients were lowered from $15.38^{\circ}\;{\ae}3.75\;to\;8.82^{\circ}\;{\ae}5.21$, which was significant statistically (p=0.000). 6. Itch severity, frequency of wheals and number of wheals were the most significantly lowered of all criteria. Conclusions: Traditional Korean medical therapy was proven to be effective for urticaria, while itch severity, frequency of wheals and number of wheals should be assessed clinically with greater priority.

An Epidemiologic Study on Allergies in the Community (지역사회 알레르기 질환의 역학적 연구)

  • In order to find out the sex- and age-related prevalence rate of allergic diseases in the community. as well as the causes of those diseases. questionnaires were given to 2.415 subjects (1.206 males and 1.209 females) from May to June. 1998. As a second stage. skin tests were given to 452 of the original subjects between July and August. 1998. Finally. from March to November 1999. the results of the tests were compared with those of other countries to make a comparative analysis of epidemiological research on allergic diseases. The results of this study were as follows: The frequency of allergic symptoms by sex was 50.1% in males and 56.7% in females. respectively, which displayed significant difference between males and females(p<.01). By age, the frequency of allergic symptoms in males was highest for those under the age of 9 at 60.6%, whereas in females the highest frequency was between the ages of 30 to 39 at 75.1 %. The relationship between economic level of the subjects and their frequency of allergic diseases showed significant difference (P<.05). There was a high degree of correlation between family history and the frequency of allergic diseases(P<.01). In allergic symptoms, the highest prevalence in both males and females occurred with urticaria, itch and skin rash (33.6% with males and 38.0% with females). Rhinitis, nasal blocking and sneezing had the second highest frequency in both sexes (11.8% of males and 14.0% of females). With regard to the causes of allergic symptoms among the people with allergies, the rate in males was 30.1% in food. 25.0% in pollen, and among females the rate was 30.7% in food, 26.6% in pollen. The frequency of food allergies by sex was 25.6% in males and 25.8% in females, with little difference between males and females, and by age, males of the ages between 40 to 49 and females of the ages between 30 and 39 showed the highest frequency, 31.8% and 32.6% respectively. Among 585 people with food allergies, the sympoms occurring with the allergy in males were 81.5% with urticaria, itch and skin rash. 5.9% with diarrhea and stomachache, and 5.2% with rhinitis, nasal blocking and sneezing. Among females with food allergies, the symptoms were 82.6% with urticaria, itch, skin rash, 9.1% with diarrhea and stomachache, and 4.7% with rhinitis, nasal blocking and sneezing. The type of food most often causing allergy in males was mackerel (6.6%) and females peaches (7.4%). In respect to a positive rate in the skin test by sex, of the 379 males taking the skin test, 21.4% were positive for allergies, whereas 14.7% of the 346 females tested were positive. Among food items found to cause positive allergic reactions, hops had the highest frequency in males (58.9%) and whole eggs had the greatest effect on females (36.4%). The estimated prevalence of food allergies in terms of skin tests was 5.0% of 1.206 males and 3.6% of 1.209 females. In the estimated frequency of food allergy by age group, males of the ages between 40 and 49 and females of the ages from 30 to 39 had the highest rate (6.5% with males and 4.5% with females). In a comparative analysis of epidemiological research on allergic diseases by country group, allergic diseases occurred in 18-50% of each population studied, and the occurrence of allergic diseases when categorized according to symptoms displayed the following as such: allergic rhinitis (3.8-52%), allergic asthma (1.6-17%), and atopy dermatitis (0.33-20.4%). The prevalence of food-related allergies was 6.6-64.9% and the foods causing allergies frequently included milk, eggs, eggs, peanuts, fish, etc.
