한국 노인의 연령 세분화에 따른 식사의 질과 주관적 건강 관련 인식 및 만성질환의 연관성 (Association of Dietary Quality with Subjective Health-Related Perception and Chronic Diseases According to Age Segmentation of Korean Elderly)

  • Objectives: This study examined the Korean elderly's dietary intake status, subjective health-related perception and chronic disease prevalence among age groups. Associations of dietary quality with subjective health-related perception and chronic diseases were also examined. Methods: Based on data from the 7th National Health and Nutrition Examination Survey, a total of 3,231 elderly were selected and categorized into 4 age groups of '65 ~ 69', '70 ~ 74', '75 ~ 79' and 'over 80'. Nutrient intakes, proportions of those with insufficient nutrient intakes, Korean Healthy Eating Index (KHEI), some subjective health-related perceptions and prevalence of major chronic diseases were compared according to the age groups. Differences in the subjective health-related perceptions and odds ratios of the chronic diseases according to the quartile levels of KHEI within the same age group were analyzed. Results: With the increase of age, several nutrient intakes (P < 0.001) and KHEI scores significantly decreased (P < 0.01). In women, activity restriction increased (P < 0.05), and EQ-5D score decreased with age (P < 0.001). Prevalence of hypertension (P < 0.0001), hypercholesterolemia (P < 0.05) and anemia (P < 0.01) significantly increased, while hypertriglyceridemia (P < 0.01) significantly decreased only in men. Obesity prevalence decreased, while underweight prevalence increased (P < 0.05). Subjective health status, EQ-5D score and PHQ-9 score significantly improved as KHEI score increased in certain age groups of women (P < 0.05). Odds ratio of hypercholesterolemia significantly increased with the increase of KHEI score in 65 ~ 69-year-old women. However, hypertension and anemia significantly decreased with the increase of KHEI score in 75 ~ 79-year-old women (P < 0.05). Conclusions: The study findings suggest that nutrition management and policy for the Korean elderly need to apply a segmented age standard that can better reflect their dynamic characteristics.

  • Vaccination in adults is recommended according to the age group and underlying diseases or risks of exposure. Patients with chronic diseases including chronic obstructive pulmonary diseases are susceptible to infectious diseases and related serious complications. They need risk-related vaccination along with age-related vaccination. Both influenza and pneumococcal vaccination are recommended in patients with chronic obstructive pulmonary diseases. They are additive if administered simultaneously. Pertussis vaccination is also needed in adolescents and adults. Although there is strong need for these vaccinations, the actual vaccination rate is low. Measures to effectively enhance the vaccination rate are needed.

Comparison of Waist-to-height Ratio (WHtR), Body Mass Index (BMI) and Waist Circumference (WC) as a Screening Tool for Prediction of Metabolic-related Diseases

  • The present study showed WHtR to be significantly better than BMI and WC for prediction of metabolic-related diseases in the middle-aged and older people in Korea, based on Bayesian ordered probit model analysis. The variations of WC, BMI and WHtR were compared according to the number of metabolic-related diseases such as hypertension, dyslipidemia, stroke, myocardial infarction, angina pectoris and diabetes. It was found that the three measures showed the similar variation except a very few extreme cases for age less than 40. For subjects over the age of 40, WC was not significant and WHtR gave more influence in greater variability than BMI on the number of metabolic diseases. Also, the rate of change for WHtR was higher than for BMI as the number of metabolic-related diseases increased. Specifically, the difference of the marginal effect of WHtR between no disease and only one disease was 1.81 times higher than that of BMI. Moreover, it was pointed out that the threshold value of WHtR for obesity should be considered differently by age.

  • 노화란 세포 및 생리 기능이 점진적으로 손상되는 복잡한 과정이다. 알츠하이머, 동맥경화 및 갱년기와 같은 노화와 관련된 질병은 노화가 진행이 되면서 발생된다. 노화와 관련된 질환은 다양한 원인에 의해 발생된다. 그 중 유전적인 변화 없이 유전자 발현을 조절하는 후성유전의 변화는 노화, 그리고 노화와 관련된 질환의 발생에 중요한 조절자로 알려져있다. 이 리뷰에서는 후성유전의 변화가 노화 및 노화와 관련된 질병의 발전과 진행에 어떠한 역할을 하는지에 대해 서술하였다. 노화 중에 일어나는 유전적 변화의 분자적 기전과 이러한 변화가 노화와 관련된 질병에 미치는 영향, 특히 노화와 관련된 질환과 관련된 유전자 발현 양식을 조절하는 RNA 메틸화, DNA 메틸화 및 miRNA에 대해 중점적으로 초점을 맞추었다.

Role of tea catechins in prevention of aging and age-related disorders

  • Tea polyphenols especially catechins have long been studied for their antioxidant and radical scavenging properties. Scientists throughout the world have investigated the usefulness of the regular green tea consumption in several disease conditions. In-vitro and in-vivo experiments on catechins especially epigallocatechingallate have revealed a significant role in many ways. Reactive oxygen species have been increasingly implicated in the pathogenesis of many diseases and important biological processes. Toxic effects of these oxidants, commonly referred to as oxidative stress, can cause cellular damage by oxidizing nucleic acids, proteins, and membrane lipids. Oxidative stress has been related to aging and age related disorders. It is found that in a wide variety of pathological processes, including cancer, atherosclerosis, neurological degeneration, Alzheimer's disease, ageing and autoimmune disorders, oxidative stress has its implications. Catechins have been reported to be useful in combating aging and age related disorders like cancer, cardiovascular disorders and neurodegenerative diseases. In this mini review we will discuss such studies done across the globe.

  • Cellular senescence is a process by which cells enter a state of permanent cell cycle arrest. It is commonly believed to underlie organismal aging and age-associated diseases. However, the mechanism by which cellular senescence contributes to aging and age-associated pathologies remains unclear. Recent studies showed that senescent cells exert detrimental effects on the tissue microenvironment, generating pathological facilitators or aggravators. The most significant environmental effector resulting from senescent cells is the senescence-associated secretory phenotype (SASP), which is constituted by a strikingly increased expression and secretion of diverse pro-inflammatory cytokines. Careful investigation into the components of SASPs and their mechanism of action, may improve our understanding of the pathological backgrounds of age-associated diseases. In this review, we focus on the differential expression of SASP-related genes, in addition to SASP components, during the progress of senescence. We also provide a perspective on the possible action mechanisms of SASP components, and potential contributions of SASP-expressing senescent cells, to age-associated pathologies.

Ginseng for an eye: effects of ginseng on ocular diseases

  • The sense of vision is the primary means by which we gather information from our surroundings, and vision loss, therefore, severely compromises the life of the affected individuals, their families, and society. Loss of vision becomes more frequent with age, and diabetic retinopathy, age-related macular degeneration, cataracts, and glaucoma are the major causes of vision impairment. To find active pharmacological compounds that might prevent or ameliorate the vision-threatening eye diseases, numerous studies have been performed, and some botanical compounds, including those extracted from ginseng, have been shown to possess beneficial effects in the treatment or prevention of common ocular diseases. In this review, we summarize the recent reports investigating the therapeutic effects of ginseng and ginsenosides on diverse ocular diseases and discuss their therapeutic potential.

노인성 만성 질환자의 특수영양 및 건강보조 식품의 섭취실태 (Supplement Use of Age-Related Chronic Disease Outpatients in Korea)

  • In order to investigate supplement use in middle-aged and elderly outpatients suffering from age-related diseases over the last year, a cross-sectional survey was conducted from July to December 2001. The subjects were 1702 (male 731, female 971) age-related disease outpatients over age 50. Data was collected using a standardized questionnaire and in-person interviews. The prevalence of supplement use and the different categories of supplements taken by the subjects were examined using SPSS statistical package. Supplements were used by 48.9% of the subjects. The mean number of kinds of dietary supplements taken by subjects was 1.7. Chinese medicine was the most commonly used supplements in both the male and female subjects. Among the reasons for using supplements, health promotion ranked as the most common. Most of the supplement users, however, did not know or only knew roughly about the health claims of their supplements. Subjects reported the information source on supplements as family, friends or relatives followed by doctors or pharmacists. These results may provide basic information for proper supplement use among Korean middle-aged and elderly outpatients with age-related diseases.

미국이민 중년 여성의 비만 관련요인과 만성질환에 미치는 영향 (Factors Related to Obesity of Mid-year Korean American Women and Their Prevalence of Chronic Diseases)

  • Purpose: The purpose of this study was to investigate factors related to obesity of mid-age women and to examine the relationship between obesity and chronic diseases including diabetes, hypertension, and heart disease. Methods: Using data from Korean American adults living in California, we analyzed the health behavior (diet, exercise, smoking, and binge drinking) and psychological stress of obese women with body mass index ${\geq}25$ by using chi-square test. Logistic regression was used to investigate independent effects of obesity on chronic diseases, after controlling for risk factors. Results: The prevalence of obesity appeared about 16.9%. The Obese group was less likely than the non-obese group to eat vegetables and more likely feel psychological stress. There were no significant differences in smoking, alcohol intake, and physical exercise between the two groups. Obesity was strongly related to chronic diseases (OR=4.642, 95% CI=1.328-16.222). Conclusion: This study suggests that obesity of mid-age women could be reduced by encouraging health behavior such as eating vegetables daily, performing physical exercise regularly, and taking care of emotional stress. Diet and physical activity interventions and emotional supports should be developed for weight loss and prevention of weight gain in mid-age women.
