  • Percutaneous vertebroplasty is a relatively easy and minimally invasive procedure used in treating vertebral fractures. However, the procedure has many complications, one of which is bone cement leakage, which happens frequently. Leakage to the paravertebral venous system, in particular, may lead to especially devastating consequences. Here we report a case of a 65-year-old male patient with an intracardiac foreign body (bone cement) that generated a perforation on the right ventricle, and result in hemopericardium after percutaneous vertebroplasty. We performed open heart surgery to remove the foreign body.

  • Bony lesions of the glenoid and Hill-Sachs lesions are the most common injuries after a first-time traumatic shoulder dislocation. However, fracture of the coracoid process after traumatic shoulder dislocation is rare. A single, open surgical procedure could be performed by a Latarjet procedure using a fractured fragment of the coracoid process. If a fracture of the coracoid process is associated with a traumatic anterior shoulder dislocation, the Latarjet procedure may be the most appropriate surgical option.

  • In assisted reproductive techniques, monozygotic twinning is not influenced by the trial of reducing the number of embryos transferred. We present extremely rare case of monochorionic triamniotic triplet pregnancy following in vitro fertilization and two-embryo transfer. During the first trimester, the crucial ultrasound findings of monochorionic triamniotic triplet pregnancy are three distinct amniotic sacs forming a triangle or ipsilon zone within a single chorionic cavity. After prudential counseling about the potential risk of a monochorionic triamniotic triplet pregnancy with the patient, selective reduction of two fetuses via radiofrequency ablation was performed. We report a case of selective reduction for monochorionic triamniotic triplet pregnancy using radiofrequency ablation at 12 weeks' gestation and obstetric progression after radiofrequency ablation was uneventful until 20 weeks' gestation.

  • Primitive neuroectodermal tumor (PNET) arising primarily in the lung is an extremely rare and aggressive malignancy with poor chances of patient survival. We present a case of long-term survival by a 29-year-old woman with PNET diagnosed after a hertological and immunohertochemical examination of a biopsy specimen obtained by performing video-assisted thoracic surgery. The patient underwent a left lower lung lobe lobectomy and 6 cycles of adjuvant chemotherapy. The patient has been free of any symptoms of the recurrence of the disease for 6 years after treatment completion.

  • We report a 54-year-old woman who presented with a well-defined, homogeneous, and non-enhancing mass in the retrobronchial region of the bronchus intermedius. The patient underwent endobronchial ultrasound-guided transbronchial needle aspiration (EBUS-TBNA) for histological confirmation. Serous fluid was aspirated by EBUS-TBNA. Cytological examination identified an acellular smear with negative microbiological cultures. The patient was finally diagnosed with bronchogenic cysts by chest computed tomography (CT) and EBUS-TBNA findings. However, 1 week after EBUS-TBNA, the patient developed bronchogenic cyst rupture and pneumonia. Empirical antibiotics were administered, and pneumonia from the bronchogenic cyst rupture had resolved on follow-up chest CT. To our knowledge, this is the first reported case of pneumonia from bronchogenic cyst rupture after EBUS-TBNA.

  • Idiopathic interstitial pneumonia is a group of lung disease classified by clinical, radiological and pathologic findings. Steroid is described in many reports as an effective treatment for the idiopathic interstitial pneumonitis. For the treatment of steroid non-responders, immuno-suppressive or cytotoxic agents are used as second line drug of choice. We experienced a case of nonspecific interstitial pneumonitis who was treated initially with glucocorticoid and antibiotics without effects, but later did respond to cyclosporin treatment. This 63 year-old patient was admitted due to dyspnea, cough, and sputum production. He was started on glucocorticoid and antibiotics after he was diagnosed as having nonspecific interstitial pneumonitis by open lung biopsy. The initial steroid treatment was not effective, however, after cyclosprin treatment, the patient showed clinical and radiologic improvements.

  • This paper seeks to find out the policies of management and layout of middle school rooms for the community people's use. This paper surveys community's needs with respect to both the use of school rooms before, during, and after classes and preferences of use of school rooms. This paper adopts two experimental case studies to find out the models. It uses t-test analysis of the statistics to find out community people's preferences for the use of school rooms between two communities in an urban area, and uses simple and multiple regression analyses to develop models concerning community people's uses of school rooms before, during, and after classes. It also uses cluster analysis to find out the cluster among community people's preference of school rooms. It found, first, that community people's use of school rooms after class can be influenced by the uses of a play ground, a music classroom, an audio visual classroom, and a gymnasium. The use during regular classes is related to the uses of the fine arts classroom, a general classroom, a home economics classroom, a gymnasium, and a playground. The use before class is affected by the uses of a fine arts classroom, a playground, and a library. It also found that, with respect to community people's preferential use of school rooms, the rooms can be clustered as a cluster of laboratories such as a general classroom, a music room, a fine arts classroom, a science classroom, a home economics classroom, and a technique classroom, a cluster of athletic areas such as a gymnasium and a playground, and a cluster of supporting facilities such as a library, an audio visual classroom, and a computer classroom. Those clusters can also be clustered in more detail, i. e., that both a general classroom and playground can be apart from a cluster of laboratories or a cluster of supporting facilities; that an audio visual classroom can be fostered into a cluster with a home economics affairs classroom and a technique classroom. Finally this paper suggests policies of management and layout of school rooms.

  • Among the treatment options for BRCA mutation carriers, risk reducing surgery is the most effective. However, this procedure has been rarely performed in Korea. Interestingly, our case showed double heterozygosity for BRCA1 and BRCA2 mutations. The patient was diagnosed with left renal cancer and left breast cancer at 45-years-of-age, 4 years before risk reducing surgery. The patient received left radical nephrectomy and left partial mastectomy with axillary lymph node dissection. After pretest counseling, the patient underwent genetic testing that identified BRCA1 and BRCA2 mutations. After post-test counseling, the patient decided on intensive surveillance. At 49-years-of-age, the patient was newly diagnosed with contralateral breast cancer. Treatment options were discussed once again. We performed bilateral total mastectomy with immediate reconstruction and prophylactic bilateral salpingo-oophorectomy after multidisciplinary discussion. The patient has been satisfied with the results of surgery. We think this procedure is a recommendable treatment option for BRCA mutation carriers.

  • Objectives: This study is to identify the effect of sex education by school doctor of Korean medicine on elementary school students from fourth to sixth-grade. Methods: Eighty three students who received four sex eduction lectures by school doctor of Korean medicine were assigned as experimental group while 132 students who did not receive any sex education were assigned as control group. This study observed the changes in students’ sexual knowledge and sexual attitude before, after and three months after the sex education program. Results: Experimental group showed positive changes on sexual knowledge (male p=0.000, female p=0.046) and sexual attitude (male⋅female p=0.000). Three months after the conclusion of sex education program, the follow-up evaluation showed decline in effect of education, which suggests a need for continuous sex education. As students showed high level of satisfaction, we expect school doctor of Korean medicine can improve knowledge on health and further increase familiarity in Korean medicine by this kind of sex education Conclusions: Sex education by school doctor of Korean medicine for elementary school students from fourth to sixth grade has effectively changed sexual knowledge and attitude. Further, it is necessary to research the effect of sex education on elementary students over long-term period.
