Purpose: This study was done to identify types of smoking decision making-temptation in adolescents and characteristics related to type among student smokers. Method: Data collection was done from March to July. 2006. A survey was administered to 275 students in 13 high schools and 15 middle schools in Seoul, South Korea. To identify types and characteristics smoking decision making-temptation in adolescents, cluster analysis using the K-mean method was employed. Characteristics of the influential variables according to the identified types of adolescent smokers were evaluated using ANOVA. Results: Four types of smoking pattern in adolescents were identified: habitual craving (17.7%), nicotine dependence (35.8%), feeblemindedness (28.4%), and self control (18.1%). The score for nicotine dependency was higher in the habitual craving type than any other type (F=11.79, p=.001), while the score for self efficacy for smoking abstinence was higher in the self control type (F=23.06, p=.000). Conclusions: Findings from this study suggest that effective interventions for smoking cessation in adolescents require not only active implementation of nicotine replacement therapy but also development of individualized approaches for each person targeting change in the social environment that may lead to positive smoking decisional balance.

The aim of this paper is to report the rate of current and ever cigarette smoking and explore correlates of current cigarette smoking among adolescents aged 13-15 in Viet Nam. This analysis was derived from GYTS survey, which comprised of 3,430 adolescents aged 13-15, conducted in 2014 in 13 cities and provinces of Viet Nam. We calculated the weighted rates of current and ever cigarette smoking and reported patterns of smoking behavior. We also performed logistic regression to explore correlates of current cigarette smoking behavior. The weighted rate of ever cigarette smoking was 9.5% (95% confidence interval (CI): 8.5 %-10.5%), in which the weighted rate among males (15.4%; 95% CI: 13.6%-17.0%) was higher than that among females (4.2%; 95% CI: 3.3%-5.1%). The weighted rate of current cigarette smoking was relatively low at 2.5% (95%CI: 2.0%-3.0%) with higher weighted rate among males (4.9%; 95% CI: 3.8%-5.9%) compared to the corresponding figure among females (0.2%; 95% CI: 0.0 %-0.5%). Current cigarette smoking was significantly higher among males than females, in students aged 15 versus 13 years old, and in students who had several or all close friends smoking and students with daily observation of smoking at school. For greater smoking reduction outcomes, we recommend that tobacco interventions for adolescents should consider targeting more male students at older ages, establish stricter adherence to school-based banning of cigarette smoking, engage both smoking and nonsmoking adolescents and empower adolescents to resist peer smoking influence as well as changing their norms or beliefs towards smoking benefits.
Background: Smoking exposure may be objectively assessed through specific biomarkers. The most common biomarker for smoking is cotinine concentration in urine, and setting an optimal cut-off point can accurately classify smoking status. Such a cut-off point for Korean adolescents has never been studied. Objectives: The aim of this study was to determine a cut-off point for urinary cotinine concentration for the discrimination of smoking in adolescents. Methods: Participants were adolescents aged 13~18 years who participated in the third cycle of the Korean National Environmental Health Survey. We used urine samples to confirm the level of cotinine concentrations. Smoking status was determined by self-reported questionnaire. We identified the optimal cotinine cut-off point for discriminating smoking status using receiver operating characteristic curve analysis. Results: Of the 904 participants, 28 (3.1%) were smokers, among whom 20 (71.4%) were male. The median urinary cotinine concentrations in smokers was 218 ㎍/L (male: 215 ㎍/L, female: 303 ㎍/L), and that in non-smokers was 1.31 ㎍/L (male: 1.46 ㎍/L, female: 1.18 ㎍/L). We found significant differences in urinary cotinine concentration according to smoking status and sex (p<0.001). Urinary cotinine concentrations performed well for identifying smoking adolescents [area under the curve: 0.954 (male: 0.963, female: 0.908)]. The cut-off that optimally distinguished smokers from non-smokers was 39.85 ㎍/L (sensitivity: 89.3%, specificity: 97.4%). Male [39.85 ㎍/L (sensitivity: 90.0%, specificity: 94.9%)] had a different optimal cut-off point than female [26.26 ㎍/L (sensitivity: 87.5%, specificity: 99.6%)]. Conclusions: This study determined a cut-off point for urinary cotinine of 39.85 ㎍/L (male: 39.85 ㎍/L, female: 26.26 ㎍/L) to distinguish smokers from non-smokers in adolescents.
Objectives: The purpose of this study was to investigate the smoking status and related factors among adolescents in Korea. Methods: We used data from the 5-7th Korea National Health and Nutrition Examination Survey. The distribution of smokers and smoking status according to general characteristics was analyzed using complex chi-square tests. Multiple logistic regression was used to analyze the relationship between smoking experience and general characteristics by calculating the 95% confidence intervals. Results: There were 618 subjects who had experience smoking one or two puffs, the starting age of one or two puffs on smoking was 14.37 years old, the number of smoking days per month was 12.42 days, and the amount of smoking per day in a month was 7.59. Gender, residential area, income level, drinking experience, subjective health status, perceived stress level, continuous depression for more than 2 weeks, suicidal thoughts within a year, and age were significantly related to smoking experience. Conclusions: It is necessary to understand the smoking behavior of adolescents. It is important to lower the smoking rate among adolescents through the development and operation of systematic smoking prevention and smoking cessation policies at the national level, as well as the efforts of youth themselves to quit smoking.
Purpose: This study has examined effectiveness of a secondhand smoking prevention program for adolescents. Methods: The study was done in a nonequivalent control group pretest/posttest design. The subjects of the current study were 198 middle school students from K city in the Gyeongbuk province. The students were assigned to an experimental group (100) and participated in the secondhand smoking prevention program or to a control group (98) and did not take part. Data collection was done from June to July 2012. A Chi-square test, independent t-test, paired t test and ANCOVA were used with the SPSS 18.0 program for data analysis. Results: There were significant differences between the groups in knowledge of short-term influence of secondhand smoking (F=9.65, p<.005), knowledge of long-term influence of secondhand smoking (F=15.53, p<.001), verbal coping skills for secondhand smoking prevention (F=16.35, p<.001), behavioral coping skills for secondhand smoking prevention (F=8.49, p<.005), and assertiveness of secondhand smoking prevention (F=17.30, p<.001) measurements. Conclusion: The secondhand smoking prevention program delivered to the adolescents is an effective method of encouraging secondhand smoking prevention and can be utilized as an effective nursing intervention for adolescents.
Purpose: The purpose of this study was to analyze the smoking behaviors and to identify the impact of depression, temperament, and characteristics of smoking experience on rural adolescents. Method: A survey using a self-administered questionnaire was done of 310 rural high school students. Using SPSS 12.0, Pearson correlation coefficients and multiple regression were used to analyze the data. Results: The smoking rate of rural high school students was higher than that of the national rate for this age group. Serious depression was observed in the rural high school students. There was a positive correlation among smoking experience, school records, novelty seeking and a negative correlation between smoking and cooperativeness. The significant predictors of smoking experience were type of school, satisfaction with school, school records, novelty seeking, reward dependence and cooperativeness. These factors accounted for 38.9% of variances in the smoking experience of rural adolescents. Conclusion: These results suggest that adolescents who exhibit factors identified in this study are regarded as having the potential to smoke, and this result proposes the basis for program development for the prevention of smoking and smoking cessation.
Objectives: This study aimed to investigate the effects of the amount and duration of smoking on gingival symptoms in adolescents. Methods: We used the web-based survey data of youth health behaviors from 2016 conducted by the Korean Centers for Disease Control and Prevention. The subjects of the study were 67,983 middle and high school students. Data analysis was performed using the SPSS Ver 20.0 program, and complex sample multivariate logistic regression analysis was performed to analyze gingival symptoms according to short- or long-term smoking habits. Results: In subjects with short-term smoking habits, there was no significant difference in gingival symptoms according to the smoking amount. However, after adjusting for socioeconomic characteristics and oral health behaviors in subjects with long-term smoking habits, a heavy smoking amount (${\geq}0.52PY$) had a 1.25 times higher effect than a light smoking amount (<0.52 PY) on gingival symptoms. Long-term heavy smoking (${\geq}0.52PY$) and low daily tooth-brushing frequency in high school students were factors affecting gingival symptoms. Conclusions: When conducting a project on the oral health of adolescents, it is argued that specific investigations and measures should be set up regarding health behavior factors, such as the amount and duration of smoking among adolescents.

Purpose : Adolescence is a period of growth and development in body structure and physiologic, psychologic, and social functioning. Smoking and drinking in this period are very harmful and may lead to other forms of substance abuse. We surveyed the actual aspects of smoking and drinking among Korean adolescents and analyzed associated factors. Methods : A survey was performed among adolescents using a questionnaire about smoking and drinking. Data were collected from students who visited pediatric clinics for routine school examinations from July to November 2007. Results : A total of 2,546 adolescents (smoking 1,512; drinking 1,034) participated in the study. The overall smoking and drinking rates were 29.2% and 48.2%, respectively, and the mean age at the first instance of smoking and drinking was 13.8 and 14.1 years, respectively. Among drinking adolescents, 30.1% were offered a drink by their parents, although most adolescents were encouraged to smoke or drink by their friends. Dissatisfaction with family and school life, economic status, and school performance was associated with a higher smoking and drinking rate (P<0.001). Broken families were associated with a high rate of smoking, but peer satisfaction was not related to smoking or drinking. The frequency and amount of smoking and drinking in adolescents were as high as those in adults. Social sanctions seemed insufficient to keep adolescents from smoking and drinking. Conclusion : Adolescent smoking and drinking are highly influenced by family and school environments, so more active social sanctions are required, including parental involvement and legislation preventing adolescents from smoking and drinking.
Purpose: This study was to investigate the smoking related social influence, refusal skill and nonsmoking related self-efficacy among adolescents. Method: The subject for this study consisted of 3,000 students of middle and high school located in Chonbuk province. The data were collected by self-report questionnaire from Aguest 19 to September 14, 2002. Result: The smoking rate was 3.3% in middle school students and 13.6% in high school students. The subject have had 61.1% of family members, 42.7% of same sex friends, 39.4% of object sex friend, 64.9% of peer group and 85.9% of teachers were being smoke. The smoking related social influence and smoking status showed significant relationship; current smokers' smoking related social influence level was much higher than former and never smoker. Friends', teachers' and family members' influences were important to adolescents' smoking. Adolescents had social influence made by many smokers. 27.7% of subject were suggested to smoke by near people, 40.4% of which them were smoked or tried to smoke. The mean refusal skill score of subject was 39.13 points per hundred. Adolescents have showed very low level of refusal skill to smoke. Current smokers' refusal skill level was significantly lower than former and never smoker. The mean average nonsmoking related self-efficacy score of current smoker was 44.46 percents per hundred, it showed that current smoker had very low level of self-efficacy to ceasing smoke. Conclusion: For the purpose of adolescents smoking prevention and ceasing, the educational program must designed to reduce smoking related social influence, to promote the refusal skill and nonsmoking related self-efficacy.

Purposes: The purpose of this study was to examine the association between risk behaviors and smoking in Korean adolescents. Methods: This study used data from online survey of youth health behavior in 2017. Data from a total of 54,411 people (27,139 male, 27,272 female) were included in the analysis. chi-square test, simple logistic regression, multiple logistic regression were performed using SAS 9.4. Findings: Multiple logistic regression analysis showed that risk behaviors such as drinking alcohol experience, sexual experience, drug use experience and high caffeine energy drinks intake experience had a significant effect on smoking. Adolescents with drinking experience were more likely to smoking than those who had no experience(OR=8.58, 95% CI: 7.67~9.60). Adolescents with sexual experience were more likely to smoking than those who had no experience(OR=4.47, 95% CI: 3.91~5.11). Adolescents with drug use experience were more likely to smoking than those who had no experience(OR=2.32, 95% CI: 1.63~3.32). Also, adolescents with high-caffeine energy drinks intake experience were more likely to smoking than those who had no experience(OR=1.37, 95% CI: 1.23~1.53). Practical Implications: All the risk behaviors were significantly associated with smoking rates. Results of this study suggest that physicians and health workers in medical institutions and health centers should simultaneously serve education and consultation for the smoking cessation as well as for the prevention of risk behaviors.
