Lack of Impact of Age on Acute Side Effects and Tolerance of Curative Radiation Therapy

  • Aim: The aim of this study was to determine the impact of age on the occurrence, severity, and timing of acute side effects related to radiotherapy. Materials and Methods: We analysed the data of 423 patients. Results: Of the patients, 295 (70%) were under the age of 65 (group 1) and 128 (30%) were over the age of 65 (group 2). The frequencies of radiotherapy-induced side effects were 89% in group 1 and 87% in group 2 (p=0.286). The mean times to occurrence were $2.5{\pm}0.1$ weeks in group 1 and $2.2{\pm}0.1$ weeks in group 2 (p=0.013). Treatment was ended in 2% of patients in group 1 and 6% of those in group 2 (p=0.062). Treatment interruption was identified in 18% of patients in group 1 and 23% in group 2 (p=0.142). Changes in performance status were greater in older patients (p=0.013). There were no significant differences according to the frequency or severity of side effects, except skin and genitourinary complications, between the groups. Conclusions: Early normal tissue reactions were not higher in older versus younger patients, though there was a tendency towards an earlier appearance.

방사선치료를 받는 암환자의 급성 부작용과 피로의 변화 (Changes in Side Effects and Fatigue of Cancer Patients Receiving Radiation Therapy)

  • Purpose : To observe changes in side effects and fatigue of the cancer patients receiving radiation therapy on the head/neck and the chest over a period from the start to the end of the therapy, and to analyze correlation between side effects and fatigue. Method : Twenty seven patients receiving radiation therapy 5 days per week for longer than 6 weeks participated in the present study. Fatigue was measured for when healthy, before-, and after-therapy. Side effects was surveyed by structured questionnaire on the last day of each therapy. Result : The results of this study were as follows. 1. Major side effects of the head/neck patients were skin irritation, change of taste, sore throat and xerostomia, while the chest patients experienced fatigue, skin irritation, anorexia, difficulty swallowing and cough increased with therapy. 2. Although fatigue was significantly changed for when healthy and before-therapy (F=60.25, p <.05) and also for before- and after-therapy, no statistical significance was demonstrated in fatigue of both the chest head/neck patients (p> .05). 3. Fatigue and side effects showed high correlation form the 4-th week with after the therapy Conclusion : The present results could be of great use to develop systematic intervention technique, leading to practical help for patients, since side effects and fatigue change to a large degree depending on the disease and the timing and technique of the therapy.

The Effects of Repetitive Transcranial Magnetic Stimulation on the Gait of Acute Stroke Patients

  • The aim of the present study was to examine whether repetitive transcranial magnetic stimulation (rTMS) can improve gait ability of acute stage stroke patients. This study was conducted with 39 subjects who were diagnosed as having a hemiparesis due to stroke. The experimental group included 20 subjects who underwent repetitive transcranial magnetic stimulation and the control group included 19 subjects who underwent sham therapy. The stroke patients in the experimental group underwent conventional rehabilitation therapy and rTMS was applied daily to the hotspot of the lesional hemisphere. The stroke patients in the control group underwent sham rTMS and conventional rehabilitation therapy. Participants in both groups received therapy five days per week for four weeks. Temporospatial gait characteristics, such as stance phase, swing phase, step length in affected side, velocity, and cadence, were assessed before and after the four week therapy period. A significant difference was observed in post-treatment gains for the step length in the affected side, velocity, and cadence between the experimental group and control group ( p < 0.05). However, no significant differences were observed between the two groups on stance phase and swing phase ( p > 0.05). We conclude that rTMS may be beneficial in improving the effects of acute stage stroke on gait ability.

Deep vein thrombosis and acute hepatitis after ChAdOx1 nCov-19 vaccination in a Charcot-Marie-Tooth patient: a case report

  • Monitoring of side effects after coronavirus disease 2019 (COVID-19) vaccination has become an important issue for health systems worldwide to ensure its safety. Recently cases of deep vein thrombosis (DVT), vaccine-induced immune thrombotic thrombocytopenia, and autoimmune hepatitis have been described: the underlying pathophysiological mechanisms are still debated. We report on a patient who presented with DVT and acute hepatitis 8 days after receiving the first dose of the ChAdOx1 nCoV-19 adenoviral vector vaccine against COVID-19. The patient is a 56-year-old male who was already affected by a rare form of axonal Charcot-Marie-Tooth disease linked to MME (membrane metalloendopeptidase) gene variation and associated with mild symptoms. His blood exams did not have any evidence of thrombocytopenia but D-dimer, troponin T, alanine transaminase, and aspartate aminotransferase were abnormal, suggesting the presence of a blood clot and acute hepatitis. The patient was treated with subcutaneous enoxaparin for 15 days and with rivaroxaban for the following 8 months: his symptoms improved and his exams showed recanalization of the veins and a healed liver. The pathogenesis of thrombosis and hepatitis after vaccination is still unclear, especially in subjects affected by rare comorbidities and this may affect the safety of vaccination in this type of population. We highlight the importance of careful monitoring of side effects after vaccination: clinical suspicion must rise when patients complain of symptoms that differ from their usual presentation.

스테로이드제제 부작용을 동반한 만성 족부 한포진 치험 1례 (A Case Study of Chronic Plantar Pompholyx Including Steroids Side Effects)

  • Backgrounds : Pompholyx is a distinctive reaction pattern presenting as symmetric vesicular hand and foot dermatitis. Other than acute symptoms like itching and vesicles, chronic changes with scaling, fissure and pain may follow. As patients are treated by steroid for a long period, it tends to present side effects after quitting steroid treatment. Objectives : This study was carried out to investigate the effect of carbon arc lamp therapy and oriental medical therapy that venesection therapy, ointments and herbal medicine on chronic plantar pompholyx including steroids side effects. Methods : We treated one case of chronic plantar pompholyx which had continued about 10 years, including steroids side effects with venesection therapy, carbon arc lamp therapy, ointments and herbal medicine. To evaluate the results of this treatment, we categorized symptoms into six items which are vesicles, itching, fissures, pain, erosions and scales. Results : Except mild itching, vesicles and scales in a small area, all the symptoms were subsided. Conclusions : It is considered that various kinds of oriental medical therapy including carbon arc lamp therapy could replace steroid therapy which has many side effects.

Peripheral nerve blocks for acute trigeminal neuralgia involving maxillary and mandibular branches: a case report

  • Trigeminal neuralgia (TN) is neuropathic pain that affects the trigeminal nerve branches. Facial pain experienced by patients with TN is typically intense and excruciating. The second and third branches (maxillary and mandibular) are commonly affected. This case report focuses on the potential treatment options for acute TN attacks involving these branches. The proposed approach involves extra-oral peripheral blocks using local anesthetics. Pain levels were measured using a visual numeric scale (VNS) with potential side effects and other relevant documented information. The patients showed responses from high pain levels to almost complete remission (from 8 to 2 and from 10 to 2 on the final VNS), with no significant side effects. This technique provides immediate pain relief and complements oral medications by offering comfort and confidence until the desired drug effect is achieved.

  • Colchicine is a drug used for the treatment of acute gouty arthritis or various autoimmune diseases. Gastrointestinal adverse effects such as abdominal pain and vomiting are the common side effects of the drug, but rarely myopathy has been reported to occur particularly in renal recipients who were treated concomitantly with immunosuppressants. Herein, we report a case who presented with acute myopathy after treated with colchicine for acute gouty arthritis.

  • Epidural morphine is effective in the treatment of postoperative pain, but side effects, such as nausea, vomiting, pruritus and urinary retention commonly occur. Droperidol is frequently used as an antiemetic to prevent intraoperative and postoperative vomiting. But it has been reported to cause acute extrapyramidal effects including dystonia. We report one case of acute dystonia in young adult following the use of epidural droperidol.

급성 기계적 경부통 환자들의 관절가동술 적용 위치에 따른 통증과 가동범위와 치료 만족도의 즉각적인 효과 비교 (Comparison of Immediate Effects of Pain, Range of Motion and Treatment Satisfaction on Difference of Applying Joint Mobilization Levels in Patients With Acute Mechanical Neck Pain)

  • The purpose of this study was to apply the joint mobilization technique to the level of segments with pain and to the level of segments with hypomobility respectively and compare the immediate effects of the joint mobilization technique on the pain, the active cervical range of motion (ROM), and treatment satisfaction of patients with acute mechanical neck pain. After the baseline assessment, forty-two patients were randomized into two groups: a painful group ($n_1=21$) that received joint mobilization at the most painful cervical spine level and a hypomobile group ($n_2=21$) that received joint mobilization at the most hypomobile cervical level. The patients received an intervention that applied unilateral posterior-anterior gliding for 5 minutes and two repetitions of 10 times of active extension motion with distraction. In the Wilcoxon signed-rank test, the painful group and the hypomobile group were improved significantly in all pain variables (p<.001), while the painful group was improved significantly in the active cervical flexion (p<.001), extension (p<.001), left side-bending (p<.01), right side-bending (p=.001), left rotation (p<.001), and right rotation (p<.001). The hypomobile group was significantly improved in active cervical flexion (p=.001), extension (p<.001), left side-bending (p<.05), right side-bending (p=.001), left rotation (p=.001), and right rotation (p<.01) after intervention. In the Mann-Whitney U test, there was no significant difference in any of the dependent variables after the intervention between the two groups, but the painful group was slightly superior to the hypomobile group in all variables except for the right lateral flexion ROM and treatment satisfaction. These outcomes suggest that the cervical joint mobilization may be applied to either the level of painful segments or the hypomobile segments for the treatment of patients with acute mechanical neck pain.

종양괴사인자와 방사선이 세포자멸사에 미치는 영향 (The Apoptosis according to the Processing Irradiation and The Tumor Necrosis Factor)

  • 급성전골수구성 백혈병(Acute promyelocytic leukemia, APL)은 혈액암의 일종으로 치료의 성적이 좋지 않을 뿐 아니라 항암요법과 병행 하였을 경우 큰 효과를 보이는 것으로 알려져 있는 방사선 치료를 병행함에도 불구하고 정상세포에도 작용하여 부작용을 초래한다. 본 연구에서는 이러한 부작용을 감소시키기 위하여 감마선을 $TNF-{\alpha}$와 같이 처리하였을 경우 정상세포와 암세포의 세포 죽음에 어떠한 영향을 미치는지 확인하였다. HL-60 세포는 APL 세포주로서 사용하였고 DMSO를 처리하여 분화시킨 HL-60 세포는 정상과립구의 성질을 나타내어 정상대조군으로 이용하였다. 그 결과 $TNF-{\alpha}$와 함께 감마선을 처리한 HL-60 세포에서만 세포독성효과를 나타내었고 세포자멸사를 유도하여 세포가 죽음에 이르게 하였다. 결론적으로 $TNF-{\alpha}$는 항암치료의 부작용을 없애기 위해 저농도 감마선 치료 시 함께 사용하여 암 세포의 제거를 증가시켜 암의 치료효율을 높일 수 있는 유효물질로 사료된다.
