Hemorrhagic fever with renal syndrome (HFRS), scrub typhus, murine typhus and leptospirosis have been the principal acute febrile diseases in Korea. To evaluate the seroepidemiologic patterns of acute febrile illness, sera collected from 2,423 patients in 1996 were examined for antibodies against Hantaan virus, Orientia tsutsugamushi, Rickettsia typhi, and Borrelia burgdorferi by indirect immunofluorescent antibody technique (IFA) and macroscopic agglutination test for Leptospira interogans. Seropositive cases against Otsutsugamushi, Rickettsia typhi, Leptospira interogans and Hantaan virus were 192 (7.9%), 193 (8.0%), 12 (0.5%) and 324 (13.4%), respectively. Male was more affected in HFRS and murine typhus contrasting to scrub typhus and leptospirosis in female. Most positive cases occurred during October and November for scrub typhus, and during November and December for HFRS. These results showed similar patterns with previous epidemical data for recent couple of years, and possibly implied no significant changes occurred in ecologic situations for acute febrile diseases in Korea.
Objectives: This study aims to describe the utilization of herbal formulas from Wenrejingwei by using network analysis and understand the treatment of acute exogenous febrile diseases. Methods: We constructed a matrix of high-frequency herbal combinations (HCs) from Wenrejingwei and Shang Han Lun and cluster networks based on cohesive analysis. Network analysis was performed to compare the results. Results: The results of the high-frequency HC network in Wenrejingwei showed cohesive patterns in three categories corresponding to dampness-heat and warm-fever treatment. Compared to the Shang Han Lun network, the Wenrejingwei network indicated a careful approach in the use of pungent and warm herbs such as Guizhitang. Moreover, the combination of Scutellaria baicalensis and Coptis chinensis along with the use of herbs strengthening yin, such as Ginseng Radix and Liriopes Radix, provide evidence of a holistic approach in the treatment of exogenous febrile diseases by considering the balance of the human body damaged by heat. Conclusion: The results of this study could help select appropriate herbal formulas and treatment methods for treating Onbyeong and modern acute febrile infectious diseases.
Tsutsugamushi disease (Scrub typhus) is an acute, febrile illness caused by Orientia tsutsugamushi, which is transmitted to humans through chigger bites. Leptospirosis, a febrile disease caused by various pathogenic Leptospira, and is acquired by exposure to contaminated water and soil. Both diseases have been the most common acute febrile diseases in the autumn in Korea for many years. Concomitant leptospirosis and scrub typhus is quite rare. We report a case of a coinfection with leptospirosis and scrub typhus in a 51-year-old male who presented with fever, abdominal pain and acute dyspnea. The patient was diagnosed with as acalculous cholecystitis, acute respiratory distress syndrome, and septic shock caused by the infection. This is the first case report of a coinfection with leptospirosis and scrub typhus in Korea.
Pneumocystis carinii pneumonia mainly occurs in immunocompromised patients and it is also known of major cause of death in children with acute lymphoblastic leukemia. After consolidation chemotherapy, acute lymphoblastic leukemia children is developed Pneumocystis carinii pneumonia frequently no an opportunistic infection but there were no controlled studies which have been performed to evaluate the usefulness of corticosteroid in Pneumocystis carinii pneumonia with acute lymphoblastic leukemia. We experienced a case of Pneumocystis carinii pneumonia in acute lymphoblastic leukemia with febrile neutropenic 6 years old girl. She was treated with trimethoprim-sulfamethoxazole and prednisone. We report this case with brief review of related literature.
Background: Monitoring appropriate medication categories can provide early warning of certain disease outbreaks. This study aimed to present a methodology for selecting and monitoring medications relevant to the surveillance of acute respiratory tract infections, such as influenza. Methods: To estimate correlations between acute febrile respiratory tract infection and some medication categories, the cross-correlation coefficient (CCC) was used and established. Two databases were used: real-time prescription trend of antivirals, anti-inflammatory drugs, antibiotics using Drug Utilization Review Program between 2012 and 2015 and physicians' number of encounters with acute febrile respiratory tract infections such as influenza outbreaks using the national level health insurance claims data. The seasonality was also evaluated using the CCC. Results: After selecting six candidate diseases that require extensive monitoring, influenza with highly specific medical treatment according to the health insurance claims data and its medications were chosen as final candidates based on a data-driven approach. Antiviral medications and influenza were significantly correlated. Conclusion: An annual correlation was observed between influenza and antiviral medications, anti-inflammatory drugs. Suitable models should be established for syndromic surveillance of influenza.
Sweet's syndrome, or acute febrile neutrophilic dermatosis, occurs in association with autoimmune diseases such as Hashimoto's thyroiditis but is rare in Graves' disease, in which all cases are induced by propylthiouracil (PTU). We report a case of Sweet's syndrome in a patient with Graves' disease treated with methimazole (MMI) during three weeks. A 34-year-old man presented with the acute onset of high fever, skin rashes on the whole body, arthralgia, and acroparesthesia. Laboratory results showed leukocytosis and elevated C-reactive protein. MMI first stopped and antibiotics and antihistamine therapy started, but his symptoms dramatically improved after oral prednisolone. Graves' disease has again been treated by MMI because of his aggravated ophthalmopathy. After one year of retreatment with MMI, there has been no recurrence of Sweet's syndrome, supporting that Sweet's syndrome in this case was not related to MMI exposure. To our knowledge, this is the first report of Sweet's syndrome associated with Graves' disease per se but not PTU or MMI use.
연구배경: 쯔쯔가무시병, 렙토스피라병, 신증후출혈열은 우리나라에서 흔한 급성열성질환으로 최근 발병과 유행성의 보고가 있음에도 불구하고 열성질환에 있어서 폐침범에 대한 연구는 부족한 상태이다. 열성질환에 있어서 폐합병증이 있을 때 임상적으로 더 심한 경과를 보이고 CRP 수치도 더 높게 측정되는 경우가 있어 입원 시 측정한 CRP값이 조기에 적극적인 치료 대상에 대한 예측인자가 될 수 있는지 알아보고자 하였다. 방 법: 2002년 1월부터 2008년 5월까지 한림대학교 춘천성심병원에서 혈청학적 검사에서 양성이거나 임상적으로 진단을 받았던 환자를 대상으로 후향적 연구를 하였다. 결 과: 총 105명의 환자가 대상이 되었고 쯔쯔가무시병 63명(63%), 신증후출혈열 32명(30.5%), 렙토스피라병 10명(9.5%)이었고 폐침범은 42명(40%)에서 관찰되었으며 폐부종 20명(19%), 가슴막삼출 20명(19%), 간질폐렴 2명(2%)이었다. CRP는 평균 8.87${\pm}$7.29 mg/dl였고 20 mg/dl 이상의 높은 값을 보였던 환자도 8명 있었고 폐침범 중 간질폐렴 형태에서 가장 높은 평균값을 보였다(p=0.027). 폐합병증을 보였던 환자군에서 임상적으로 심한 경과를 보였으며 중환자실 입원치료와 인공호흡기 치료를 동반하고 있었고 더 높은 CRP값을 보였다(p=0.0073). 결 론: 급성열성질환에 있어서 폐합병증을 보인 군에서 더 높은 CPR 수치가 측정되었으며 입원 시 CRP측정을 통해 조기에 더 적극적인 치료가 필요한 환자군을 선택할 수 있고 폐침범에 의한 사망률과 치명률의 감소에도 도움이 될 것으로 추정된다.
Biomarkers are playing an increasingly important role in antimicrobial stewardship. Their applications have included use in algorithms that evaluate suspected bacterial infections or provide guidance on when to start or stop antibiotic therapy, or when therapy should be repeated over a short period (6-12 h). Diseases in which biomarkers are used as complementary tools to determine the initiation of antibiotics include sepsis, lower respiratory tract infection (LRTI), COVID-19, acute heart failure, infectious endocarditis, acute coronary syndrome, and acute pancreatitis. In addition, cut-off values of biomarkers have been used to inform the decision to discontinue antibiotics for diseases such as sepsis, LRTI, and febrile neutropenia. The biomarkers used in antimicrobial stewardship include procalcitonin (PCT), C-reactive protein (CRP), presepsin, and interleukin (IL)-1β/IL-8. The cut-off values vary depending on the disease and study, with a range of 0.25-1.0 ng/mL for PCT and 8-50 mg/L for CRP. Biomarkers can complement clinical diagnosis, but further studies of microbiological biomarkers are needed to ensure appropriate antibiotic selection.
목 적 : 최근 요배양이 음성이거나 기준이하의 세균 집락이 있었던 경우에서도 신피질 손상이나 요역류가 있음이 보고되고 있으며, 실제 소변 배양검사에서 낮은 세균 집락을 보인 경우에도 요로 감염이 확인된 연구 보고들이 있다. 이에 저자들은 발열과 농뇨가 있는 영아에서 전형적인 요로 감염의 특성을 분석하고, 요로 감염이 의심되나 소변 배양 검사가 음성인 경우에도 영상 및 기능 검사상 이상 여부를 파악하여 관련 인자를 확인하고자 하였다. 방 법 : 2001년 1월부터 2003년 2월까지 가톨릭대학교 의과대학 성모자애병원 소아과에 발열을 주소로 내원하여 일반 요 검사상 농뇨 소견을 보여 입원한 12개월 이하의 환아 136명을 대상으로 소변 배양 검사를 실시하고, 배양 검사 결과와 상관없이 요로 감염 의심 시 신장의 급성 감염 상태를 방영하는 DMSA scan과 요 역류 검사를 시행하여 이상 여부와 관련 인자를 분석하였고, 동시에 임상특성과 원인균도 분석하였다. 결 과 : 136명 중 53명의 환아에서 소변 배양 검사에서 세균이 배양되었으며 57균주가 동정되었다. 단일 균주에 의한 감염 빈도가 92.5%(49/53)이었고, E. coli가 49주(86%)로 가장 많았으며 E. faecalis, M. morganii, Proteus, P. aeruginosa, S. aureus, E. fergusonii 순으로 분리되었다. 이들 균주는 대체적으로 cephalosporins 및 aminoglycosides에 감수성이 높았으나, aminopenicillins에는 감수성이 낮았다. DMSA scan과 요 역류 검사는 배양검사 양성인 경우와 음성인 경우에서 유사한 비율로 이상 소견을 보였다. 소변 배양 검사 양성 유무와 상관없이 농뇨를 보인 영아에서 발열 기간이 48시간 이상인 경우, 연령이 증가할수록, CRP가 높을수록 급성기에 시행한 DMSA scan 검사 이상 유무에 유의성이 있음을 확인하였다. 결 론 : 요로 감염 균주에 대해 1세대, 3세대 cephalosporins와 amikacin에 감수성이 높아 요로 감염 영아에서 이들 항생제의 병합요법이 고려될 수 있다. 또한 발열과 농뇨가 있는 영아에서, 소변 배양 검사가 음성이더라도 나이가 3개월 이상이고 48시간 이상의 발열을 보인 경우, C-반응 단백 수치가 높은 경우에는 급성기의 DMSA scan 검사와 요역류 검사를 시행하여 신장 이상 유무를 확인하고 조기 치료 방침을 결정하는 것이 이후에 신장실질 질환으로의 이환을 막는데 도움을 줄 수 있을 것이다.
