Preliminary Study for the Comparison of the Skin Temperature Changes by the Combustible Moxibustion and the Electronic Moxibustion Device (연소식 뜸과 전자식 뜸치료 시스템에 의한 체표 온도변화 비교를 위한 사전연구)

  • Objectives : The purpose of this study was to compare the skin temperature changes by the rechargeable and programmable electronic moxibustion device(EMD) with the traditional combustible moxibustion device(CMD). Methods : Skin temperature changes in six healthy volunteers induced by CMD and EMD were measured with digital infrared thermal imaging(DITI). Heat stimulation was applied on $LI_4$ and $TE_5$, and skin temperature changes on each point were measured at baseline and per minute for total 7 minutes, 2 minutes of heat stimulation and 5 minutes of observation. Results : There was no significant difference in the skin temperature changes between CMD and EMD. The temperature on $LI_4$ with EMD was $32.3{\pm}1.3^{\circ}C$ at baseline, $34.0{\pm}1.3^{\circ}C$ at 1 minute after heat stimulation start, $34.6{\pm}1.2^{\circ}C$ at 2 minutes after, and from 3 minutes after heat stimulation, it maintained $32.6{\sim}32.8^{\circ}C$. Conclusions : Methods for measuring skin temperature changes induced by heat stimulation of moxibustion were established, and the possibility of effectiveness of the newly developed electronic moxibustion device was raised with this preliminary study. This study can contribute to the development of clinical research methodology for traditional Korean medicine.

The Effect of Non-Invasive Sobu(H8) Point Stimulus by 'DONG CHU CHIM' on HRV (동추침(東樞鍼)에 의한 비침습적 소부혈(少府穴) 자극이 심박변이도에 미치는 영향)

  • Background : Recently a discussion about Qi including a study about the effect or the theory of acupuncture is getting prevailing in various angles. In most of studies about acupuncture stimulus, 'Filiform acupuncture'(毫鍼) is used. A study about Nine kinds of acupuncture(九鍼), except 'Filiform acupuncture'(毫鍼) has not been reported yet, and there is no study about using a special acupuncture made for controling Qi either. Objectives : 'DONG CHU CHIM' can be used for patients who are scared of a pain because it is a medical Qikong tool and non-invasive stimulus one. To assess a effect of Qikong operation using 'DONG CHU CHIM' objectively, we stimulated non-invasive to Sobu point of 30 normal adults using 'DONG CHU CHIM' and examined the result in the basis of high confirmed and repetitive HRV which is a functional assessment method of the autonomic nervous system. Method : This study has been proceeded in three periods. Total was 35min : 10min for the former and latter period of acupuncture stimulus, l5min for the acupuncture stimulus period. For each period, we measured 5min of 3 times(Stage I, II, III). Result : Comparing the changes of HRV during pre-stimulation and post-stimulation, HRT was significantly reduced, LOGTP, LOGVLF, LOGLF, LF/HF were significantly increased and SDNN, LOGHF were generally increased but did not show any significant changes. So we think that the stimulus of 'DONG CHU CHIM' affects on the sympathetic system and parasympathetic system, it activates the autonomic nervous system, and it makes the inequality of the sympathetic and parasympathetic nerve to be equal status. Conclusions : We can conclude that the non-invasive stimulus of 'DONG CHU CHIM' can affect significantly to the autonomic nervous system. So it can be used in clinic as a tool of Qikong operation, and it can be also used to weak patients or children because it doesn't give a lot of pain like 'Filiform acupuncture'(毫鍼) compared to invasive stimulus. By the basis of this study, more studies about the effect of 'DONG CHE CHIM' should be done in the future.

A Study on the Mean Variations of EEG for the Indirect Moxibustion Stimulation (간접 뜸 자극의 뇌파 평균 변화에 관한 연구)

  • In this paper, examined characteristics of EEG(electroencephalogram) variation when the stimulation was given to lung-wan(CV12), Shin-gwol(CV8) and Gwan-won(CV4) which were some of the acupuncture point, through indirect moxibustion and No stimulation. The EEG signals were measured before the stimulation, during the stimulation, and 1 hour after the stimulation. The measured time domain data were converted to the frequency domain data FFT(Fast Fourier Transform) and frequency power spectrum. Then the $\alpha,\beta,\delta$, and $\theta$ waves were analyzed for variation to the amplitude of vibration according to the stages of stimulation.

Effect of Acupuncture-like Transcutaneous Electrical Nerve Stimulation on Plasma $\beta-endorphin$ and ACTH, Serum Cortisol and Urinary 17-OHCS Levels (저빈도-고강도 경피신경전기자극이 혈장 $\beta-endorphin$ 과 ACTH, 혈청 Cortisol 및 요중 17-OHCS 농도에 미치는 영향)

  • The purpose of this study was to examine the effect of electroanalgeia and $\beta-endorphin$ action by acupuncture-like (Lof/Hil) transcutaneous electrical nerve stimulation (TENS) applied to acupuncture points. Twelve healthy adult male aged between 19 ann 25 were randomly assigned to TENS group (n=6) and naloxone group (n=6). Subjects of both groups were strongly stimulated TENS with 4 pps and $200{\mu}s$ for 30 minutes on the LI 3 and LI 10 meridian points of dominant am. Naloxone group was injected naloxone hydrochloride before TENS application. The experimental pain threshold was measured by chronaxie meter CX-2 on the distal end of radius just before and after TENS application. The levels of plasma $\beta-endorphin$ and ACTH. serum cortisol and urinary 17-OHCS were analyzed by radioimmunoassay (RIA) kits before and after TENS application. In TENS group, there was a significant increase of experimental pain threshold (p<0.01), plasma $\beta-endorphin$ level (p<0.05), serum cortisol level (p<0,001) and urinary 17-OHCS levels (p<0.05) after TENS application. The plasma ACTH level was not significantly increased, but it showed an increasing tendency. In naloxone group, although there was a decreasing trend, ACTH and cortisol level did not show a significant change, but $\beta-endorphin$ and 17-OHCS level were significantly decreased (p<0.01). The result of this study stewed that acupuncture-like TENS induced analgesic effect, such that the levels of plasma $\beta-endorphin$, plasma ACTH, serum cortisol and urinary 17-OHCS were concomitantly increased with experimental pain threshold. It is suggested that the analgesic mechanism of the acupuncture-like TENS probably related to endogenous opioid component such as $\beta-endorphin$.

Effects of Kyeong points' Acupuncture of Three Hand Yang Collaterals on Facial Thermography of Healthy Human Beings (수삼양경 경혈 자침이 안면부의 체열변화에 미치는 영향)

  • Objectives : This study examined effects of kyeong points (經穴)' acupuncture of three hand yang collaterals (手三陽經) on the facial thermography in health subjects, because the kyeong point in Yang collaterals belongs to the characteristic of the five elements (五行)meaning fire (火). Methods : The volunteers who participating in this study rested for 20-30 min. atroom temperature (23-$25^{\circ}C$) before the examination and were informed to avoid smoking, drinking and use of any drugsfor the previous day. The thermography of the face was taken using Infra-Red Imaging System (IR 2000, MEDI-CORE Co., Korea) at time intervals of 15 minutes : at 15 min before, just before and after, 15 min after, 30 min after and 45 min after acupuncture stimulation. Acupuncture was applied to the left kyeong points (經穴)' of three hand yang collaterals (手三陽經) for 30 minutes. Results : The results showed that kyeong points (經穴)' acupuncture of three hand yang collaterals (手三陽經) decreased the temperature of all the areas of the facial surface comparing to those of the control group. Also, it was observed that the quantities of thermal changes following acupuncture of ST36 increased in the all ROIs (regions of interest) compared to those of the control group. Conclusions : Observing the thermography classified by ROI, it was clear that acupuncture of kyeong points (經穴) of three hand yang collaterals (手三陽經) could modulate thermogram of the facial area however, it is necessary to undertake more investigation supporting these results.

A Study on Acupuncture of Spring-point(형혈); Effect on Body Temperature and Central Immune System (형용자침이 발열 흰쥐의 체온하강과 중추성 면역에 미치는 영향)

  • Objective : There are many reports that acupuncture has thermoregulatory effects on human and animals. To investigate the effect and mechanism of antipyretic action of acupuncture, we observed the body temperature and cytokine expressions in the hypothalamus of rats. Methods : Lipopolysaccharide (LPS, i.p., 2.5mg/kg) was injected to conscious rats (Sprague-Dawley, male, n=4l) to cause hyperthermia and simple needling (stainless steel, 0.25 mm o.d., 5 mm insertion for 10 sec with no manipulation) was performed bilaterally with the measurement of rectal temperature. Next, we sacrificed rats to remove brain and determined the level of mRNA for interleukin-6 (IL-6), $interleukin-1{\beta}{\;}(IL-1{\beta})$, interleukin-2 (IL-2) and $interferon-{\gamma}{\;}(IFN-{\gamma})$ in the hypothalamus by using reverse transcriptase-polymerase chain reaction (RT-PCR). Resul1s : Needling on forepaw (acupoint HT8) and needling on hindpaw (acupoint BL66 and acupoint LR2) significantly inhibited LPS-induced fever of rats (P<0.01, 10 min after treatment respectively), but same treatment on proximal tail (non-acupoint) did not cause any change on fever. The levels of IL-6 and $IL-1{\beta}$ mRNA in the hypothalamus was significantly enhanced by LPS-injection, while the level of IL-6 and $IL-1{\beta}$ mRNA was markedly reduced after treatment on BL66 (P<0.01). Treatment on forepaw reduced it slightly, but not significantly. Equivalent stimulation on proximal tail did not cause any changes. Conclusions : Our results indicate that acupuncture stimulation on various body parts has differential thermoregulatory effects on LPS-induced fever of rats with site-specificity. And, we suggest that its antipyretic action might be accompanied with the suppression of hypothalamic production of pro-inflammatory cytokine of immune system, IL-6 and $IL-1{\beta}$.

The Protective Effects of Acupuncture on Oxidative Stress Caused by Cadmium in the Kidney (카드뮴으로 유발된 산화적 스트레스에 대한 침 자극의 신장 보호 효과)

  • Objectives : This study was performed to inquire into the protective effects of acupuncture on oxidative stress caused by cadmium accumulation in the kidney. Methods : Sprague-Dawley male($150{\pm}30g$) rats were stabilized for 1 week and divided into 5 groups: normal, control, $LR_3$ acupuncture, $BL_{23}$ acupuncture and sham acupuncture. For three days experimental groups received oral doses of cadmium 2 mg/kg twice a day. Acupuncture was applied bilaterally at each point 10 times for two weeks. The depth of stimulation was 1 mm at right angles and torsion of acupuncture was produced 2 times per second for 1 minute. The kidneys were extracted and weighed after two weeks, and renal function was confirmed through blood urea nitrogen(BUN). We measured reactive oxygen species of the serum and kidney, and compared expression levels of superoxide dismutase(SOD), catalase, glutathione peroxidase(Gpx), nuclear factor erythroid derived 2-related factor 2(Nrf-2), heme oxygenase-1(HO-1), nuclear factor-${\kappa}B$(NF-${\kappa}B)$, cyclooxygenase-2(COX-2), inducible nitric oxide synthase (iNOS), Bax and Cytochrome c. Results : The $LR_3$ acupuncture group and $BL_{23}$ acupuncture group experienced significantly increased kidney weight, and decreased BUN compared to control group. In terms of oxidative stress, the $LR_3$ acupuncture group and $BL_{23}$ acupuncture group experienced significantly reduced reactive oxygen species compared to the control group. Conclusions : The $LR_3$ acupuncture group and $BL_{23}$ acupuncture group experienced showed the effects of antioxidant, anti-inflammatory and apoptosis protection. The $BL_{23}$ acupuncture group was more effective than $LR_3$ acupuncture group.

Effect of Microcurrent Stimulation at Hand Somatic Acupuncture Points on Experimental Pain Threshold (미세전류가 수부체성경혈점의 실험적 통증 역치에 미치는 영향)

  • The purposes of this study were 1) to examine the effects of microcurrent electrical neuromuscular stimulation 2) to compare surface electrode with needle electrode at somatic acupuncture points on experimental pain threshold measured at the distal end of the radius 3) to determine the changes in effect over time. A total of sixty healthy adult male and female subjects were assigned randomly to one of two experimental group or to a control group. Group 1(n=20) received MENS(Microcurrent Electrical Neuromuscular Stimulation) with surface electrode. Group 2(n=20) received HENS with needle electrode. Group 3(n=20) received no MENS. It measured experimental pain threshold at the wrist on pretreatment, 0 min after treatment. 15 min after treatment, 30 min after treatment in two experimental group. The results were as follows: 1. Experimental pain thresholds were higher in males than females(P<0.01). 2. Only the experimental group exhibited a significant increased in pain threshold after MENS treatment(P<0.05). 3. Surface electrode group increased significant pain threshold 0 min, 15 min after treatment, but greatly decrease 30 min after treatment. 4. Needle electrode group increased significant pain threshold 0 min, 15 min after treatment, but decrease 30 min after treatment. The results suggest that MENS applied to appropriate somatic acupuncture point can increase pain threshold. Further research is needed to assess the effects of greatly variety intensity of MENS of pain sufferes.

Comparison of Brain Activation Images Associated with Sexual Arousal Induced by Visual Stimulation and SP6 Acupuncture : fMRI at 3 Tesla (시각자극과 삼음교 자침으로 유발된 성적 흥분의 대뇌 활성화 영상의 비교 : 3 테슬라 기능적 자기공명영상법)

  • Purpose : This study was performed not only to compare the brain activation regions associated with sexual arousal induced by visual stimulation and SP6 acupuncture, but also to evaluate its differential neuro-anatomical mechanism in healthy women using functional magnetic resonance imaging (fMRI) at 3 Tesla (T). Subjects and methods : A total of 21 healthy right-handed female volunteers (mean age 22 years, range 19 to 32) underwent fMRI on a 3T MR scanner. The stimulation paradigm for sexual arousal consisted of two alternating periods of rest and activation. It began with a 1-minute rest period, 3 minutes of stimulation with either of an erotic video film or SP6 acupuncture, followed by 1-minute rest. In addition, a comparative study on the brain activation patterns between an acupoint and a shampoint nearby GB37 was performed. The fMRI data were obtained from 20 slices parallel to the AC-PC line on an axial plane, giving a total of 2,000 images. The mean activation maps were constructed and analyzed by using the statistical parametric mapping (SPM99) software. Results : As comparison with the shampoint, the acupoint showed 5 times and 2 times higher activities in the neocortex and limbic system, respectively. Note that brain activation in response to stimulation with the shampoint was not observed in the regions including the HTHL in the diencephalon, GLO and AMYG in the basal ganglia, and SMG in the parietal lobe. In the comparative study of visual stimulation vs. SP6 acupuncture, the mean activation ratio of stimulus was not significantly different to each other in both the neocortex and the limbic system (p < 0.05). The mean activities induced by both stimuli were not significantly different in the neocortex, whereas the acupunctural stimulation showed higher activity in the limbic system (p < 0.05). Conclusions : This study compared the differential brain activation patterns and the neural mechanisms for sexual arousal, which were induced by visual stimulation and SP6 acupuncture by using 3T fMRI. These findings will be useful to understand the theory of traditional acupuncture and acupoint channel in scientific point of view.

The Effect of Low Frequency Electro-acupuncture at ST39 on Intestinal Motility in Rats (하거허 상응부위 저주파 전침자극이 흰쥐의 장운동에 미치는 영향)

  • Objectives : The aim of this study was to investigate the effect of low frequency electro-acupuncture at ST39 on intestinal motility in rats. Methods : Intestinal hypermotility and hypomotility in rats were induced by oral carbachol ingestion and loperamide injection. Rats were divided into seventeen experimental groups including the normal and holder groups. The rats were induced with intestinal hypermotility and hypomotility and divided into pre and post-treatment groups. I also carried out acupuncture (needle retention) and low frequency electro-acupuncture at ST39 or the sham point. I fed charcoal to rats after the treatment and calculated its distance travelled in the gastrointestinal tract, which was compared by groups so as to determine which treatment was more effective in increasing or decreasing intestinal motility. Results : 1. In normal rats, low frequency electro-acupuncture at ST39 showed no significant effect on intestinal motility. 2. Pre-treatment with acupuncture (needle retention) at ST39 on intestinal motility over-activated with carbachol significantly decreased intestinal motility in rats. 3. Pre-treatment with low frequency electro-acupuncture at ST39 on intestinal motility over-activated with carbachol significantly decreased intestinal motility in rats. 4. Pre-treatment with acupuncture (needle retention and low frequency electro-acupuncture) at ST39 showed no significant effect on intestinal hypomotility in rats that was induced by loperamide injection. Conclusions : These results suggest that acupuncture (needle retention) and low frequency electro-acupuncture at ST39 have preventive effects on intestinal hypermotility. Regardless of the stimulation method, ST39 showed an effect on intestinal motility. Further study is required to confirm other effects of ST39.
