A Case of Complex Treatment according to the Type of Korean Medicine Diagnostic System by Period of Recurrent Nummular Eczema on Whole-body (전신 재발성 화폐상 습진 시기별 한의 유형(습열형(濕熱型), 혈조형(血燥型))에 따른 한방 복합 치료 치험 1례)

  • Objectives : The purpose of this study is to report the Korean medicine treatment on whole-body nummular eczema. Methods : This case study was conducted with a 66-year-old female patient who has whole-body nummular eczema treated by Korean medicine. She was treated from 2021.3.30 to 2021.6.28. During the treatment, the two herbal medicines were changed depending on the patient's condition. We treated her with herbal medicine, herbal external preparations and acupuncture treatment. For the evaluation of the results, we used photo comparison, VAS, and DLQI. Results : After the treatment, the nummular eczema lesions of patient was remarkably improved. Also, the VAS grade of other symptoms such as pruritus, burning sense, oozing, crusting and erythema were decreased and DLQI score changed from 27 to 2. Conculsions : This study suggests that Korean medicine treatments are effective for patient with recurrent nummular eczema.

Effects of Korean Medicine in Idiopathic Pulmonary Fibrosis Patients - A Retrospective Chart Review (한방병원에 내원한 특발성 폐섬유화증 환자 82명에 대한 임상적 특징 분석; 후향적 차트 리뷰)

  • Objective: This study was designed to analyze the clinical features of idiopathic pulmonary fibrosis patients who attended a Korean medicine hospital and the treatment effects through retrospective chart reviews. Methods: The medical records of 82 outpatients who had been diagnosed with idiopathic pulmonary fibrosis and visited the Allergy, Immune, and Respiratory System Department of Kyung Hee Korean Medicine Hospital from 8 January 2015 to 8 January 2021 were retrospectively reviewed. To assess the treatment outcomes, we used the FVC (Forced Vital Capacity), DLCO (Diffusing capacity of the Lung for CO), 6-minute walk test, and HRCT (High Resolution Computed Tomography). Results: The study group consisted of 28 females and 54 males. The median age of the patients was 67.98±11.44 years. The chief complaints were cough (n=51) and dyspnea (n=49). Other frequent symptoms were general weakness (n=8), weight loss (n=4), and a fever (n=2). A total of 77 (93.90%) patients were prescribed Korean herbal medicine, and 52 (63.41%) patients were treated with acupuncture, moxibustion, cupping therapy, ICT, or pharmacopuncture. After treatments, FVC, DLCO, the 6-minute walk test, and HRCT were maintained or worsened slightly. Conclusions: This study presented the characteristics of idiopathic pulmonary fibrosis patients treated by Korean medical therapies, and further studies of Korean medical treatments for idiopathic pulmonary fibrosis patients would be valuable.

A Survey on Students' Perception of Clinical Performance Examination (CPX) in College of Korean Medicine Using Student Standardized Patients (학생표준화환자를 사용한 한의과대학 진료수행시험(CPX)에 대한 학생 인식 조사)

  • Objectives : The objective of this survey was to examine students' perception of the clinical performance examination (CPX) in college of traditional Korean medicine using student standardized patients. Methods : College of traditional Korean medicine students who completed the first-semester clinical practice education were selected as the subjects, and they participated in a survey asking questions about the following matters: satisfaction with CPX, self-evaluation, difficulties experienced during the CPX course, level of prior knowledge of clinical practice education, and usefulness of and intimacy with Student Standardized Patients (SSP). Results : Satisfaction with CPX was calculated to be 4.10 (5 being the perfect score), and self-evaluation of CPX was calculated to be 4.12. The subjects chose physical examination as the most difficult item relating to CPX. 80.5% responded positively to the question relating to diverse experiences in practice education, and 52.8% responded positively to the question relating to prior knowledge of diverse standardized patients. 55.6% responded positively to the question relating to performance proficiency of SSP, 63.9% responded positively to the question relating to usefulness of SSP to prepare for CPX, and 69.4% responded positively to the question relating to usefulness of SSP in evaluating CPX, and 55.6% responded positively to the questions relating to intimacy with SSP. It was confirmed that there was a strong quantitative correlation between prior knowledge of clinical practice education and usefulness of SSP, and that there was a strong quantitative correlation between performance proficiency of SSP and usefulness of SSP. It was confirmed that there was a middle-level correlation between performance proficiency of SSP and intimacy with SSP, and that there was a middle-level quantitative correlation between intimacy with SSP and usefulness of SSP. Conclusions : It was confirmed that senior students enrolled in college of traditional Korean medicine were mostly satisfied with the clinical performance examination in college of traditional Korean medicine using SSP, and that usefulness of SSP had a quantitative correlation with prior knowledge of diverse clinical practice educations, performance proficiency of SSP, and intimacy with SSP.

  • We studied the effect of Korean medicine treatment on the clearance of vancomycin-resistant enterococcus (VRE) colonization and on improvement in symptoms in a stroke patient. The patient was continuously followed up with stool VRE cultures while being treated with herbal medicine, Chunglijagam-tang, acupuncture, and oxygen therapy. We also checked the coccygeal pressure sore condition and the oxygen levels during therapy. No VRE colonization was found after treatment and the oxygen levels and pressure sores had improved. This study proved that Korean medicine treatment could be effective for the clearance of VRE colonization and improvement of symptoms.

Effects of Valerianae Radix et Rhizoma Methanol Extract on High-fat Diet-induced Hyperlipidemic Mice (힐초근(纈草根) 메탄올 추출물이 고지혈증 유발 생쥐의 지질대사에 미치는 영향)

  • Objectives: This study was designed to investigate the effects of Valerianae Radix et Rhizoma Methanol Extract (VRME) on serum lipid levels in a high-fat diet-induced hyperlipidemic mice. Methods: Each 8 C57BL/6 mice were randomly assigned to normal diet group, high-fat diet control group, high-fat diet plus 100 mg/kg/day of VRME group. In order to induce hyperlipidemia, high-fat diets were supplied to control group and VRME group for four weeks. Normal diet group were supplied with general feed for four weeks. After that control group supplied only high-fat diets as feed, VRME group received oral administration of VRME with high-fat diets for three weeks. and normal diet group were supplied with general feed for three weeks. After seven weeks, the changes in the body weight, the plasma levels of total cholesterol, triglyceride, high density lipoprotein-cholesterol, aspartate aminotransferase (AST), alanine aminotransferase (ALT), blood glucose in serum were measured. Results: In our results, VRME did not affects weight gain, serum AST and ALT in high-fat diet-induced hyperlipidemic mice. Oral administration of VRME lowered levels of total cholesterol and triglyceride, which were elevated by induction of hyperlipidemia. and oral administration of VRME lowered blood glucose significantly. Conclusions: These results suggest that VRME could act as a potent antihyperlipidemic in therapeutics for hyperlipidemia.

A Case Report on a Hypothyroidism Patient through Korean Medicine along with Whidam's Gokgol(CV2) Sugi Therapy (휘담식 곡골수기법을 병용한 갑상선기능저하증 1례 증례 보고)

  • Objective : A common treatment for hypothyroidism is to supplement the deficient hormones. However, taking hormone medication does not improve clinical symptoms or side effects, so it is necessary to develop other improved treatments. The purpose of this study is to report that the Korean medicine treatment along with Whidam's Gokgol(CV2) Sugi therapy has improved symptoms of hypothyroidism and related the level of TSH, free T4 after halting the use of levothyroxine. Methods : We employed Korean medicine treatment; herbal-medication(延齡固本丹 and 加減平胃散), acupuncture, and cupping therapy, and so forth. At the same time, we treated the patient with Whidam's Sugi therapy(especially, Gokgol Sugi therapy). We treated the patient more than five times a week for the first month and treated more than three times a week for the next three months. Since then, we have treated the patient once or twice a week. The patient took a total of four blood tests during her treatment in 2012 and 2017. Results : 1. Both TSH and free T4 levels and clinical symptoms were normalized for patient with hypothyroidism who stopped taking levothyroxine by treating the patient with Korean medicine treatment and Whidam's Sugi therapy. 2. The combination of Korean medicine treatment and Whidam's Sugi therapy was also used in the case of subclinical hypothyroidism patient. 3. The combination of Korean medicine treatment and Whidam's Sugi therapy for hypothyroidism lasted for five years, not just for a short period of time. Conclusions : The combination of Korean medicine treatment and Whidam's Sugi therapy(especially, Gokgol Sugi therapy) was effective in the treatment of hypothyroidism and it helped to normalize the level of TSH, free T4. It is hoped that further clinical research will continue in the future so that it can be used not only for hypothyroidism but also for other endocrine diseases.

The Current Research Status of Complementary and Integrative Medicine in Practice-Based Research Networks: A Systematic Review (개원의중심연구망에서 수행된 보완통합의학 관련 연구 현황: 체계적 문헌고찰)

  • Objectives : Practice-Based Research Networks (PBRNs), collaborations of practitioners and academic researchers, have provided platforms for conducting research to address clinical questions generated from daily routine care. This review aimed to critically analyse articles from PBRNs that are related to complementary and integrative medicine (CIM) and to suggest future directions for a PBRN which is appropriate for Korean Medicine (KM). Methods : PubMed, PBRN registries in Agency for Healthcare Research and Quality and relevant PBRN websites were searched up to November 2019 for research articles from PBRNs that focused on CIM regardless of study design. Methodological quality of the included studies was assessed. The included studies were read in full, classified and summarised according to their topics. Results : A total of 51 articles published from 1998 through 2020 were included in this review. They were categorised into three principal themes based on research questions and findings: health services research (embracing researches examining characteristics of patients and CIM practitioners/practices, and communication between patients and practitioners); effectiveness and safety of CIM practices/interventions; and feasibility studies of instruments and interventions in PBRN settings. The study designs varied including surveys (n=30), prospective observational studies (n=6), 2ndary analyses of existing studies (n=7), protocols (n=7), retrospective chart review (n=1) and qualitative study (n=1). Quality of the included studies greatly varied. Conclusions : PBRNs can serve as a feasible platform for conducting practice-relevant research on KM and CIM. Considering growing demands on evidence-base for routine practice of KM amid various stakeholders, a PBRN in KM community and further researches nested within PBRN designs are warranted.

Effect of Hominis Placenta Pharmacopuncture for a Patient with Mild Cognitive Impairment: A Randomized, Double-Blind, Placebo-Controlled, Multi-Center Trial

  • Objectives: Mild cognitive impairment (MCI) is condition of cognitive decline shown in transition from normal aging to dementia. Hominis placenta pharmacopuncture (HPP) is a treatment that combines effects of medication and acupuncture by injecting Hominis placenta into acupoints. The objective of this study was to evaluate the efficacy and safety of HPP for MCI. Methods: This was a randomized, double-blind, placebo-controlled, two-center clinical trial. Eligible patients were randomly allocated to either the HPP group or the placebo group. HPP or saline as placebo was administered to participants for eight weeks. Changes in symptoms were observed. The primary outcome was difference in mean change of Korean Version of the Montreal Cognitive Assessment (MoCA-K) score between the HPP group and the placebo group. Cognitive function, overall status of mood and sleep, and quality of life (QoL) were also assessed. Safety assessment and economic analysis were then conducted. Results: Thirty participants were enrolled. One participant in the placebo group dropped out. The score of MoCA-K increased after treatment. Its mean change was smaller in the HPP group than in the control group. HPP ameliorated Global Deterioration Scale and Korean Dementia Rating Scale subtests for attention, organization, and memory compared to the placebo. However, none of them was significantly different between the two groups. Mood, sleep, and QoL all improved more in the HPP group than in the placebo group, although differences between the two groups were not statistically significant. There was no adverse event probably related to the drug. HPP treatment needed KRW 345,000 more than the placebo group in improving Geriatric Quality of Life scale-Dementia score by one point for one year. Conclusions: Although HPP treatment did not significantly improve cognition, it changed behavioral and psychological symptoms in MCI.

  • 연구 배경 최근 한국 및 중국에서 근골격계 질환에 대한 도침술의 활용이 늘고 있다. 하지만 현재까지의 임상연구는 대부분 증례보고 형태에 그치거나, 충분한 근거가 확보되지는 않은 실정이다. 우리는 요천추신경병증환자의 치료에 있어 도침치료와 통상한의치료와의 비교를 통해 도침치료의 유효성, 안전성, 경제성 평가를 시행하기 위해 본 연구를 기획하게 되었다. 연구 방법 본 연구는 예비임상 연구로, 다기관에서 진행되며, 무작위대조군, 활성 대조군을 통한 2개군 병행집단 연구로 평가자 맹검을 시행하게 된다. 이 연구에서는 총 50명의 요천추신경병증 환자를 대상으로 2개군으로 균등하게 분배하여 도침술 또는 통상한의치료를 시행하게 된다(각군당 25명). 도침술 그룹의 경우 협척혈에 도침치료를 시행 받게 되며, 통상 한의치료군은 연구자 판단하에 도침술을 제외한 적절한 한의치료를 시행 받게 된다. 두 개의 그룹은 주당 2회씩 총 3주간 치료를 받게 된다. 일차 유효성 평가는 요통으로 인한 기능장애를 평가하기 위한 Oswestry disability index를 통해 시행한다. 이차 유효성 평가지표로는 numeric rating scale, European Quality of Life 5-Dimension 5-Level, short-form McGill Pain Questionnaire, Roland-Morris Disability Questionnaire scores를 시행하게 된다. 고찰 본 예비임상연구의 결과는 추후 있을 요천추신경병증에 대한 도침치료 및 한의통상치료 비교효과 연구의 유효성, 경제성평가 본 임상연구를 위한 기초 정보 및 가능성을 확인하고 적절한 대상자수 산정에 도움이 될 것이다.

Korean Medicine Treatment for Anterior Inferior Cerebellar Artery Infarction: A Case Report (어지럼증을 주소로 하는 전하소뇌동맥경색 환자에 대한 한방치료 증례보고 1례)

  • Objective: The aim of this study is to report the effectiveness of Korean medicine for a patient with dizziness following anterior inferior cerebellar artery infarction. Methods: The patient was treated with traditional Korean methods including acupuncture, herbal therapy, moxibustion, and vestibular rehabilitation exercise during an admission period of seven days. The patient's dizziness was assessed using a numeric rating scale (NRS) and the Korean Dizziness Handicap Inventory (K-DHI), Korean Activities-specific Balance Confidence (K-ABC), Korean Vestibular Disorders Activities of Daily Living (K-VADL), the modified Rankin scale (mRS), and the Korean version of the Modified Barthel Index (K-MBI). Results: After seven days of combined treatment with traditional methods and vestibular rehabilitation, the patient's dizziness was reduced from NRS 6-7 to NRS 2. In addition, K-DHI decreased from 84 to 22; K-ABC improved from 52% to 78.125%; K-VADL reduced from 175 to 37; the mRS score changed from 4 to 1; and the K-MBI score increased from 86 to 98. No adverse events were observed during treatment. Conclusion: This study suggests that combined therapy of Korean medicine and vestibular rehabilitation can be effective treatment for anterior inferior cerebellar artery infarction patients.
