EVALUATION OF AN ACCIDENT MANAGEMENT STRATEGY OF EMERGENCY WATER INJECTION USING FIRE ENGINES IN A TYPICAL PRESSURIZED WATER REACTOR

  • Following the Fukushima accident, a special safety inspection was conducted in Korea. The inspection results show that Korean nuclear power plants have no imminent risk for expected maximum potential earthquake or coastal flooding. However long- and short-term safety improvements do need to be implemented. One of the measures to increase the mitigation capability during a prolonged station blackout (SBO) accident is installing injection flow paths to provide emergency cooling water of external sources using fire engines to the steam generators or reactor cooling systems. This paper illustrates an evaluation of the effectiveness of external cooling water injection strategies using fire trucks during a potential extended SBO accident in a 1,000 MWe pressurized water reactor. With regard to the effectiveness of external cooling water injection strategies using fire engines, the strategies are judged to be very feasible for a long-term SBO, but are not likely to be effective for a short-term SBO.

  • A major issue for all nuclear stakeholders is to keep the probability of circumstances that could lead to core damage as low as possible. In addition, for NPP, appropriate accident management provisions are to be implemented to limit the consequences associated with an accident. Development and application of L2 PSA is a structured way to demonstrate that such objectives are achieved. The paper presents the efforts recently done in Europe to harmonize some best-practices in that field, from research area to risk assessment. The Fukushima Daiichi accident reiterated the importance of these activities and the need to efficiently reinforce the NPP safety based on risk assessment conclusions. New perspectives in Europe are briefly presented.

  • Risk assessment of a railway system should be periodically conducted managing a large amount of accumulating accident/incident data and scenarios, which generally requires enormous time and efforts. Therefore, special information management system is essential for railway risk assessment, where data needed for decisions on managing the railway safety could be promptly supported. The objective of this study is to develop a railway accident analysis program for risk assessment. The program is application running on the web which links railway accident analysts throughout the railway industry to a central database. Data entered, together with associated code tables. is stored on MS-SQL database. The program uses the concepts of accident, safety events, causes, related factors(vehicle, person, infrastructure, tool/equipment), recommendations to bring together the various elements of railway accidents. The program will be useful in finding hazard conditions, accident scenarios, quantitatively assessing the risk, and providing pertinent risk measures, eventually serving to prevent railway accidents and reduce severities of railway accidents.

  • The safety accident of construction industries occur variously in other industries, including other industries, resulting in significant losses of human and material losses. In particular, General worker represents the highest safety accident rate each year, and the various types of accidents are the ones that show the greatest interest in the field, which is the most interesting job in the field. This study aims to identify trends in safety hazards and to analyze the accident severity for major types and influence factors.

  • It is a problems in industrial accidents that the knowledge for industrial accidents is obtained by experience, not by experiment. This experiential knowledge is obtained by Investigating accident cases and utilizing those for safety education. Therefore, in this paper, the situation about the serious accident of construction is analyzed by occupation, a kind of construction, time group, season, type of accident, and accidental cause. And the mutual · relations of these factors are studied. The most frequent type of the serious accidents of construction Is the falling accident. It happenes most frequently at apartment construction among kinds of construction and to structural worker, finishing worker, normal worker in order among occupations. And it is found that the most critical causes of the falling accident are the imperfection of safety facilities and unwearing of protection equipments, so a number of accidents can be reduced by the expansion of safety facilities and wearing of protection equipments absolutely. The counterplan of prohibition of accidents and the direction of government policy are presented by a series of nalyses for accident cases.

  • Forensic Engineering is the area covering the investigation of products, structures that fail to perform or do not function as intended, causing personal injury or damage to property. To investigate explosion accident of the centrifugal casting machine in terms of the forensic engineering, in this paper, the computing simulation using ADINA FSI has performed to investigate that the effect of the Check-Pin fracture by the flow phenomena and molten metal weight and the mechanical properties test of the accident Check-Pin has performed using the instrumented indentation technique. Through these studies, the safety accident that may occur in centrifugal casting machine can be minimized by performing specialized and systematic investigation of the accident cause in terms of the forensic engineering.

  • There are two to three helicopter accidents every year in Korea, representing 5.7 deaths per 100,000 flights. In this study, an analysis was conducted on helicopter accidents that occurred in Korea from 2005 to 2017. The accident analysis was based on the aircraft accident and incident report published by the Aircraft and Railway Accident Investigation Board. This Research analyzed the characteristics of accidents occurring in Korea caused by human error by pilots. Accident analysis was done by classifying the organization, flight mission, aircraft class, flight stage, accident cause, etc. Pilot's huan error was classified as Skill-based error, decision error and perceptual error in accordance with the HFACS taxonomy. The accidents caused by pilot's human error were classified into five categories: powerlines collision, loss of control, fuel exhaustion, unstable approach to reservoir, and elimination of tail rotor.

Machine learning-based categorization of source terms for risk assessment of nuclear power plants

  • In general, a number of severe accident scenarios derived from Level 2 probabilistic safety assessment (PSA) are typically grouped into several categories to efficiently evaluate their potential impacts on the public with the assumption that scenarios within the same group have similar source term characteristics. To date, however, grouping by similar source terms has been completely reliant on qualitative methods such as logical trees or expert judgements. Recently, an exhaustive simulation approach has been developed to provide quantitative information on the source terms of a large number of severe accident scenarios. With this motivation, this paper proposes a machine learning-based categorization method based on exhaustive simulation for grouping scenarios with similar accident consequences. The proposed method employs clustering with an autoencoder for grouping unlabeled scenarios after dimensionality reductions and feature extractions from the source term data. To validate the suggested method, source term data for 658 severe accident scenarios were used. Results confirmed that the proposed method successfully characterized the severe accident scenarios with similar behavior more precisely than the conventional grouping method.

  • The construction industry has a higher disaster rate than other industries, so safety education and management are highly important. In order to reduce the construction accident rate, it is necessary to study the key safety management factors reflecting the characteristics of the construction industry, where there are differences in processes and manpower input for each process, and a small number of managers. Therefore, in this study, key safety management factors for each Process of construction were derived through cross-analysis between safety accident types and accident occurrence objects through disaster case data. The extracted key safety management factors are expected to provide useful information for safety education and supervision of construction sites.

Analysis on Comparison of Highway Accident Severity between Weekday and Weekend using Structural Equation Model (구조방정식 모형을 이용한 주중과 주말의 고속도로 사고심각도 비교분석)

  • In order to identify and understand the crucial factors to induce traffic accident, causal relationships between diverse factors and traffic accident occurrence have been investigated continuously. It is one of most important issues all over the world to reduce the number of traffic accidents and deaths by them. Korea government is also stepping up their effort to reduce the number of traffic accidents and mitigate the severity of the accidents by establishing various traffic safety strategies. By introducing the five-day work week and increasing concern of leisure activities, the differences of trip characteristics between weekday and weekend is getting greater. According to this, the patterns and crucial factors of traffic accident occurrence in weekend appear differently from those in weekday. This study aims to understand major different factors affecting accident severity between weekday and weekend using 12,042 incident data occurred on freeways of Korea from 2006 to 2011. The model developed in this study estimated relationships among various exogenous factors of traffic accident by each type using SEM(Structural Equation Model). The result provides that road factors are related to the accident severity for weekday model, while environment factors affects on accident severity for weekend.
