From Patients and Caregivers 119 Rescue party's Experiences for Violence in the Pre-Hospital Emergency Medical Field (환자 및 보호자로부터 구급대원이 경험한 병원 전 응급의료현장 폭력실태)

  • The purpose of this study was to investigate 119 rescue party's violence experiences and to analyze their responses of violence experiences based on the pre-hospital emergency medical field. Data were collected from the self-administered survey of 226 rescue party in Jeollabuk-do area in Korea from January 1 through March 2, 2009. The data were analyzed by descriptive statistics and logistic regression analysis using SPSS Win 12.0. The results were as follows: first, all of 119 rescue unit in the pre-hospital emergency medical field reported that they got violence experiences more than once a week, and 62% of rescue team were exposed to physical violence. Patient's factors that caused violence were drug abuse or alcohol(56.2%), on the other hand rescue party's factors were shortage of human resources. Second, 42% of the respondents replied that they did not have any educational experiences for prevention and coping methods about violence within 5 years. 77.4% of those surveyed indicated that they wanted to receive specific educations which can prevent and cope with violence. Third, the mean value of total violent response scores was 2.53, and emotional response score was 3.2, social response score was 2.22, and physical response score was 2.17. Fourth, violent response score related to general characteristics proposed that physical(t=-2.08, p=o.38), emotional(t=-7.13, p=.006), and total responses(t=-4.764, p=.000) were statistically significant. And emotional(t=4.257, p=.000) and total responses(t=8.1330, p=.000) related to age were also statistically significant. Finally, among current tenure qualification scores, social response(t=9.987, p=.012) and total score(t=8.130, p=.000) were statistically significant. Between fire departments, violent response score suggested that physical(t=9.987, p=.000), emotional(t=2.433, p=.012), social(t=6.738, p=.000), and total score(t=5.943, p=.000) were statistically significant.

An Empirical Study on General Deterrence Effects of the On-site Investigation System in the Korean National Health Insurance (건강보험 현지조사제도에서 일반적 억제이론에 대한 경험적 연구)

  • Background: This study aimed to examine whether cases of punishing false claimants threat general physicians to check their medical cost claims with care to avoid being suspected, and identify empirically general deterrence effects of the on-site investigation system in the Korean National Health Insurance. Methods: 800 clinics were selected among a total of 15,443 clinics that had no experience of on-site investigation until June 2007 using a stratified proportional systematic sampling method. We conducted logistic multiple regression to examine the association between factors related to provider's perception of on-site investigation and high level of perceived deterrence referring to fear of punishment after adjusting provider's service experiences and general characteristics. Results: The probability of high perceived deterrence was higher 1.7 times (CI: 1.13-2.56), 2.73 times (CI: 1.68-4.45) each among clinics exchanging the information once or more per year or once or more for 2-3 months than among clinics no exchanging the information about on-site investigation. Also, the probability of high perceived deterrence was higher 2.27 times (CI: 1.28-4.45) among clinics that knows more than 3 health care institutions having experienced an on-site investigation than among clinics knowing no case. Conclusion: A clinic knowing more punishment cases by onsite investigation and exchanging more frequently information about on-site investigation is likely to present high perceived deterrence. This result will provide important information to enlarge preventive effects of on-site investigation on fraud and abuse claims.

Cognition and Experience about Sexual Harassment in Elementary School Students (초등학생의 성희롱에 대한 인식과 경험)

  • Purpose : Sexual abuse especially in younger group have been growing. This study was performed to propose the fundamental data for developing early education program to prevent damage from sexual harassment in elementary school students. Method : Questionnaire survey on cognitions and experiences about sexual harassment was done on 3125 elementary school students from seven elementary schools in Busan area from January to March, 2002. Data analysis was done with SPSS (ver 10.0). Results : Though 82% of the students have ever herd about sexual harassment previously, but self assessment of 'well-known' was only 5%. About 40% of them have ever experienced audio-visual sexual materials mainly through PC (18%), media (14%), etc. And, 60% of them have ever educated about sexual harassment. In cognitions, they agreed 'unwanted touch', 'intentional showing his (her) own body', 'send lewd mails and pictures', but disagreed 'ask massage from aged', 'gaudy joke' as one of sexual harassment. In attitudes, 'no response', 'defense with friends and family', 'express unpleasantness apparently' were desirable pattern, instead, 'his (her) own responsibility', 'problem only between person concerned', 'neglect is the best way' were undesirable. About 24% of them have ever experienced at least one of the fifteen types of sexual harassment with major types of lewd mails pictures (11%), hugging (5%), ask massage (4.7%) and unwanted touch (3.3%). The experience by gender was higher in girls (25.0%) than boys (22.4%) with no statistical significance. The mean level of cognition and attitude out of one hundred was $70.3{\pm}28.1$ and $73.5{\pm}15.9$, respectively. The mean frequency of experience was $0.4{\pm}1.1$ times. The level of cognition and attitude was higher in girls, 6th graders, and in ever educated group (p<0.001). Conclusions : The cognition level of elementary school students about sexual harassment assessed considerable. And, they also experienced several types of sexual harassment, though not too much. Therefore, compatible education program let them able to protect themselves from sexual harassment should be developed and applied even in early school age.

Compulsory Licensing as a price control and supply policy of patented drugs : Is it a possible alternative in South Korea? (특허신약의 가격통제 및 공급 정책으로서의 강제실시 : 한국에서의 가능성과 한계)

  • Korea has had problems with the price and supply of essential drugs such as Gleevec for leukemia, Fuzeon for HIV/AIDS, and Tamiflu for both avian flu and swine flu. The shortage or refusal of patented drugs supply is imposing a heavy burden in not only developing countries but also developed countries. Thinking over the serious results, we need to concern about the limited access to patented drugs by multinational drug companies' patent monopoly especially for pandemic and life threatening diseases. The effective response regarding to pandemic and life threatening diseases. The effective response regarding to pandemic situation requests collaborative and unbiased provisions of all countries in the world, however, sometimes patent monopoly may hinder the efforts. Compulsory licensing has been considered to be a useful alternative to the abuse of patent rights. However, the Korean experiences of compulsory licensing have left some controversial issues in connection with the availability of it in Korea. 'Flexibility' allowed in TRIPS and Doha Declaration has not come into effect in Korea for several reasons. Although the situation shows the limitations of compulsory licensing as a pharmaceutical supply policy, it is clear that compulsory licensing still has the possibilities of enhancing the access to medicines of all countries in need. Through searching the institutionalization process and experiments of compulsory licensing in Korea, this article explores the possibilities and the limits.

Effects of Emotional Labor and Workplace Violence on Physical and Mental Health Outcomes among Female Workers: The 4th Korean Working Conditions Survey (여성 근로자의 감정노동 및 작업장 폭력 피해 경험이 건강결과에 미치는 영향: 2014년 한국근로환경조사를 이용하여)

  • Purpose: We investigated the effects of emotional labor and workplace violence on various physical and mental health outcomes among female workers. Methods: We obtained data from 24,760 female workers who participated in the 4th Korean Working Conditions Survey (2014). Hierarchical logistic regression analysis was used to analyze the relationships. Results: Female emotional workers were more exposed to workplace violence than were female non-emotional workers. Verbal abuse was the most common type of workplace violence. Logistic regression analyses revealed that (1) emotional labor was significantly associated with higher odds of having musculoskeletal or abdominal pain (physical health outcomes), along with overall fatigue (a mental health outcome), and (2) workplace violence experiences were significantly associated with higher odds of musculoskeletal pain, headache/eye strain, abdominal pain, depression/anxiety disorder, overall fatigue, and insomnia/sleep disturbance, after controlling for covariates. Conclusion: This study demonstrated that both emotional labor and workplace violence have negative effects on physical and mental health. However, workplace violence experience has a stronger negative impact on health outcomes than does emotional labor alone. A management system to eradicate workplace violence and programs aimed at managing emotional labor are urgently needed at the organizational level.

A Study on a past pregnancy experiences of maternity with handicaped children (장애아를 가진 일부 모성의 임신기왕력에 대한 연구)

  • Ratio of physically and mentally handicapped people among the total population is about 3∼4%. Regardless of the extent of seriousness of handicaps, they have to sustain various difficulties in their life time and pose challenging problems to their homes, community and nation. In the spectrum of their problems, measures such as treatment, rehabilitation and welfare work all occupied important part but most significant measure is the prevention. Such handicaps items from health care of maternal and child, mostly causes throughout the process of pregnancy, delivery and upbringing. And then this study is focused on the pregnant history of the mothers with handicapped children. The objective of this study is to identify the basic factors which can create a handicapped child during pregnancy and is to prevent the birth of such a child. For this study, 301 mothers who have handicapped children were selected, now receiving various training at 5 rehabilitation and educational facilities. And questionnaires distributed to them during the period of Oct, 1983 to Dec, 14. The collected responses have been analized in terms of significant numbers, percentage and metic average. Their related results are as follows: 1. The sexual ratio between the male and female was 58.5% compare 41.5%. The average age of the handicapped children was 10.1 years old and that of mothers at the time of the children's birth was 28.9 old years. They were born as 2.14th in their family, the ratio of antenatal care for pregnant mothers was 36.2% and 22.9% were abnormally delivered. 37.9% out of the total were born from mothers with ages above 35 years and as their first baby. 2. The time of confirming the handicapped nature of children was during babyhood by 97.1% and discovery of these misfortunes were made by chance. As for causes of those-handicapped status 20.9% disease from complication, 15.3% from difficulties experience at the time of delivery, 11.3% from heredity and 10% were results of drug abuse during pregnancy, and 49.8% of the handicapped children were mentally retarded.

Effects of Drinking, Smoking and Drug Use Experience on Adolescents' Sexual Intercourse: Using the Data of the Korea Youth Risk Behavior Web-based Survey from 2010 to 2014 (청소년의 음주, 흡연, 약물사용 경험이 성경험에 미치는 영향: 2010~2014년 청소년건강행태온라인조사를 활용하여)

  • Purpose: The purpose of this study was to investigate the association between sexual intercourse and experience of alcohol, tobacco, and drugs use among adolescents in South Korea. Methods: This study used the data of the Korea Youth Risk Behavior Web-based Survey from 2010 through 2014. The subjects of this study were 367,562 students (Male 187,610, Female 179,952). The subjects' general characteristics and their sexual experiences according to their general characteristics were analyzed using frequency analysis. The relationship between sexual intercourse and the use of alcohol, tobacco, and drugs was analyzed through multiple logistic regression. The analyses were conducted using SPSS 18.0. Results: According to the study, the significant predictors of sexual intercourse were gender, grade, parents' educational level, residence type, economic status, and substance abuse. Adolescents who had used alcohol, tobacco, and drugs were more likely to have had sexual intercourse than their peers who hadn't use the substances. Conclusion: The results suggest that factors that increase the risk of having premature sex should be managed and prevented systematically. Among the factors, in particular, drinking, smoking, and drug use require intensive management.

Exposure to Violence and Coping Behavior during the Clinical Practice of Nursing Students (임상실습 중 간호대학생의 폭력 경험과 대처 행동)

  • This study was conducted to investigate violence experiences and coping behaviors of nursing students when they were doing clinical practice. Data were collected from 250 nursing students from 10th of November to 10th of December in 2014 using questionnaires. 231 questionnaires were analyzed by descriptive statistics, t-test, and one-way ANOVA. 98.7% of the subject have experienced violence. Most frequently mentioned setting where violences were experienced was psychiatric unit, abusers were patient. Most frequent violence type was verbal abuse(97.4%) followed by physical threatening(76.2%). Most frequent coping behavior was enduring(87.8%) followed by avoiding(83.2%). In this study, most of nursing students experienced violence during the clinical practice. They need to be better protected from violence during the clinical practice. It is needed various active effort and programs to prevent violence and to help students cope more against violence in their practice.

Comparative Study on Suicidal Ideation Factors between the Elderlies Living Alone and the Elderlies Cohabiting (독거노인과 비독거노인의 자살생각 영향 요인 비교)

  • The purpose of this study is to identify factors influencing suicidal ideation for the elderly living alone. Utilizing the identified factors as the base data, the study aims to develop community programs for suicide prevention. This research is a secondary analysis of the 2017 national survey, 'Survey on the Elderly,' conducting logistics regression analysis on the data collected on 10,299 elderly aged 65 or older living in 17 general residential facilities as of 2017. The analysis reveals that 10.5% of the elderly living alone have contemplated suicide. The frequency of suicidal ideation was attributed to such factors as gender, age, chronic disease, depression and experiences of abuse and discrimination. To prevent suicide among the elderly, therefore, active management from both public and private sectors are needed for early detection of chronic diseases and depression.

Risky Behavior Subtypes and Suicide Attempts in Adolescents with Depression (우울장애 청소년의 위험행동 유형과 자살시도의 관련성)

  • Objectives : The objective of this study was to identify the risky behavior subtypes in adolescents with depression, and the relationship between those subtypes and suicide attempt. Methods : Ninety adolescents who met the Diagnostic and Statistical Manual of Mental Disorders-4th edition criteria for major depressive disorder, dysthymia, or depressive disorder not otherwise specified participated in the study. Participants were interviewed about suicidal attempts, non-suicidal self-injuries. Smoking, drinking behaviors, and searching for the word "suicide" on the internet, negative life events including bulling-bullied experiences, and history of abuse were also included with questionnaires. Results : The results of cluster analysis showed that four risky behaviors were divided into three clusters : non-risky type, characterized by few risky behaviors ; delinquency type, which was characterized by smoking and drinking ; non-suicidal self-injury type. In non-suicidal self-injury type, percentage of girls was higher and clinical symptoms were more severe than delinquency type. In addition, delinquency type members could be a bully, while non-suicidal self-injury type members could be abused by a family member. Logistic regression analysis showed that both risky behavior subtypes were significant predictors of suicide attempt. Conclusion : These results suggest that two types of risky behaviors of depressed adolescents have an effect on the suicide attempts independently. Then, the implications and limitations of the study were discussed and directions for future studies were suggested.
