A Proposal for a Classification System and Evaluation Standards for Ensuring the Safety of Transport Ventilator in Korea (국내 운반형인공호흡기 안전성 확보를 위한 분류체계 및 평가기준 제안)

  • Objectives : This study aims to present a new classification system using evaluation standards to ensure the safety of transport ventilators used in Korea. Due to the rapid advancement of technology, various types of ventilators have been developed and as the range of application increases, so does demand. With quick access to information and a growing economy, the technology used in Korean hospitals is equivalent to that of other countries; however, regulatory and safety consciousness are still lagging in Korea. Methods : Based on researching overseas cases, reference data and standards for improving the current system in Korea are proposed. Results : According to the review of transport ventilator use in Korea, it was found that concerns about safety is due to the absence of a standardized classification system for evaluating the safety and effectiveness of transport ventilators. Conclusions : In order to improve the safety of patients and the quality of medical care, it is essential to establish guidelines and assessment standards guaranteeing the safety of transport ventilators in Korea. Clear definitions and classifications for devices must precede the application of such standards. In addition, effective evaluation standards should be developed in order to resolve problems and improve upon the current system through continuous validation.

Comparative Study on the Sleep Patterns, Satisfaction of Sleep, and Sleep Enhancement Behaviors between Hospitalized and Non-hospitalized Elderly (입원노인과 재가노인의 수면양상, 수면만족도 및 수면증진행위 비교연구)

  • Purpose: This study was to compare and analyze sleep patterns, satisfaction of sleep, and sleep enhancement behaviors between hospitalized and non-hospitalized elderly. Methods: Subjects were 201 older adults, who were hospitalized patients or living in U-city, Gyung-gi province. Data was collected from June 10 to August 25, 2007, and was analyzed by the SAS program. Results: 1) Non-hospitalized elderly had better sleep patterns than hospitalized elderly patients. 2) There was a strong positive correlation between sleep patterns and satisfaction of sleep in both groups. 3) In hospitalized elderly, there were significant differences in sleep patterns and satisfaction of sleep by month. 4) In non-hospitalized elderly, there was a significant difference in sleep patterns by presence or absence of spouses. There was a significant difference in satisfaction of sleep by those living with others. There were significant differences in sleep enhancement behaviors by age, religion, length of time, and sponsors. Conclusion: To relieve sleep disturbances of elderly, comprehension of sleep of the aged by nursing care givers should be obtained. Also, improving environments and elder's self-esteem with religious consideration and preparation of financial conditions are needed to promote the sleep of hospitalized and non-hospitalized elderly.

Experience as a Vulnerable Elderly Individual with Diabetes (당뇨병을 가지고 살아가는 취약계층 노인의 경험)

  • Purpose: This phenomenological study was conducted to describe and understand the experience of vulnerable elderly individuals with diabetes by identifying the meanings and structures of the experience. Methods: The data were collected through in-depth interviews of six vulnerable elderly individuals with diabetes aged over 65 years. The interview data were audio-recorded, transcribed verbatim and checked for accuracy. The Colaizzi's method of phenomenology was used to analyze the data. Results: Four theme clusters were extracted as follows: 'Life adversity', 'Adaptability to the life', 'Replacement of absence of family with social support', 'Difficulty of self-care'. Conclusion: The results provide an in-depth understanding of life experiences of vulnerable elderly individuals with diabetes. The findings will be useful to nurses caring for this population.

Computer-aided proximal caries diagnosis: correlation with clinical examination and histology

  • Purpose: To evaluate the performance of the LOGICON Caries Detector using RVG-4 and RVG-ui sensors, by comparing results of each detector to the results of clinical and histological examinations. Materials and Methods : Pairs of extracted teeth were radiographed, and a total of 57 proximal surfaces, which included both carious and non-carious situations, were analyzed. The RVG-4 produced 8-bit images, while the RVG-ui unit produced 12-bit images, which were taken in the high sensitivity mode. The images produced by the LOGICON were evaluated by a trained observer using both automated and manual caries detection software modes. Ground sections of the teeth established the actual absence or existence of caries. Results: LOGIC ON-aided caries detection and depth discrimination of the RVG-4 and RVG-ui sensors were equally inconsistent irrespective of whether the LOGIC ON software was set to the automated or manual mode. Sensitivity ranged from 50% to 57% for caries penetration of the enamel-dentin junction. Conclusion: Care needs to be taken when using LOGIC ON in conjunction with RVG images as an adjunct for treatment planning dental caries. Even when applied by a trained observer, substantial discrepancies exist between the results of the LOGIC ON software-guided evalutations using RVG images and histologic examination.

Successful management of absent sternum in an infant using porcine acellular dermal matrix

  • Congenital absent sternum is a rare birth defect that requires early intervention for optimal long-term outcomes. Descriptions of the repair of absent sternum are limited to case reports, and no preferred method for management has been described. Herein, we describe the use of porcine acellular dermal matrix to reconstruct the sternum of an infant with sternal infection following attempted repair using synthetic mesh. The patient was a full-term male with trisomy 21, agenesis of corpus callosum, ventricular septal defect, patent ductus arteriosus, right-sided aortic arch, and congenital absence of sternum with no sternal bars. Following removal of the infected synthetic mesh, negative pressure wound therapy with instillation was used to manage the open wound and provide direct antibiotic therapy. When blood C-reactive protein levels declined to ${\leq}2mg/L$, the sternum was reconstructed using porcine acellular dermal matrix. At 21 months postoperative, the patient demonstrated no respiratory issues. Physical examination and computed tomography imaging identified good approximation of the clavicular heads and sternal cleft and forward curvature of the ribs. This case illustrates the benefits of negative pressure wound therapy and acellular dermal matrix for the reconstruction of absent sternum in the context of infected sternal surgical site previously repaired with synthetic mesh.

Rigid Bronchoscopy for Post-tuberculosis Tracheobronchial Stenosis

  • The healing process of tracheobronchial tuberculosis (TB) results in tracheobronchial fibrosis causing airway stenosis in 11% to 42% of patients. In Korea, where pulmonary TB is still prevalent, post-TB tracheobronchial stenosis (PTTS) is one of the main causes of benign airway stenosis causing progressive dyspnea, hypoxemia, and often life-threatening respiratory insufficiency. The development of rigid bronchoscopy replaced surgical management 30 years ago, and nowadays PTTS is mainly managed by bronchoscopic intervention in Korea. Similar to pulmonary TB, tracheobronchial TB is treated with combination of anti-TB medications. The indication of rigid bronchoscopy is more than American Thoracic Society (ATS) grade 3 dyspnea in PTTS patients. First, the narrowed airway is dilated by multiple techniques including ballooning, laser resection, and bougienation under general anesthesia. Then, most of the patients need silicone stenting to maintain the patency of dilated airway; 1.5 to 2 years after indwelling, the stent could be removed, this has shown a 70% success rate. Acute complications without mortality develop in less than 10% of patients. Subgroup analysis showed successful removal of the stent was significantly associated with male sex, young age, good baseline lung function and absence of complete one lobe collapse. In conclusion, rigid bronchoscopy could be applied to PTTS patients with acceptable efficacy and tolerable safety.

Experiencing cardiac arrest during surgical exploration in hemodynamically stable patients with multiple stab wounds, including lower extremity in Korea: a case report

  • Stab wounds, particularly those affecting multiple body regions, present considerable challenges in trauma care. This report describes a case of sustained self-inflicted stab injuries to the abdomen and thighs of a 23-year-old male patient. Although the patient's vital signs were stable and bleeding was minimal from thigh wounds without overt signs of vascular injury, the patient experienced a sudden, profound hemorrhage from the right thigh, leading to cardiac arrest. Successful resuscitation was followed by surgical repair of a right superficial femoral arterial injury accompanying a resuscitative endovascular balloon of the aorta. Subsequent lower extremity computed tomography angiography revealed no additional vascular abnormalities. The patient was discharged in stable condition on the 12th postoperative day. This case underscores the unpredictability of stab wound trajectories and the potential for hidden vascular injuries, even in the absence of immediate life-threatening signs. It also emphasizes the critical role of advanced imaging modalities, such as computed tomography angiography, in identifying concealed injuries, and the importance of strategic intraoperative techniques, including resuscitative endovascular balloon occlusion of the aorta, in achieving favorable patient outcomes.

Exploring the Direction of Home Economics Education in Preparation for the Generalization of a One-Person Household (1인 가구 시대의 가정과교육 방향 탐색)

  • This study explores the social phenomenon of the universalization of one-person households through a literature analysis and text mining in order to explore a future direction for Home Economics Education(HEE) development in the one-person household era. From 2010 to 2018, texts from newspaper articles and book content of one-person households were analyzed through R program. The results of the study are as follows. In order to develop students' competency to live a happy life in the one-person household era, it is necessary to: (1) expand the preemptive and collaborative research of HEE, (2) develop and operate a curriculum to raise the living competency to live alone, (3) expand opportunities for secondary school students as well as off-campus youth, middle-aged, and elderly students, and (4) develop various HEE's elective curriculum focusing on the ability to live as one-person household. Also, (5) in order to overcome the psychological and social poverty and isolation of one-person households, HEE should strengthen the learner's ability to form relationships through self-esteem, care of others, community life, communication and conflict resolution education. In conclusion, HEE's independent living competency, relationship formation competency, and practical problem solving competency are all necessary competencies to live in one-person households. In this study, it is meaningful to suggest a future direction for HEE and to use new research methods such as word cloud techniques in the absence of HEE's previous research in relation to the increase of one-person households.

Idiopathic acute eosinophilic pneumonia in a 14-month-old girl

  • Idiopathic acute eosinophilic pneumonia (IAEP), characterized by acute febrile respiratory failure associated with diffuse radiographic infiltrates and pulmonary eosinophilia, is rarely reported in children. Diagnosis is based on an association of characteristic features including acute respiratory failure with fever, bilateral infiltrates on the chest X-ray, severe hypoxemia and bronchoalveolar lavage fluid >25% eosinophils or a predominant eosinophilic infiltrate in lung biopsies in the absence of any identifiable etiology. We present a 14-month-old girl who was admitted to our pediatric intensive care unit because of acute respiratory distress. She had a fever, dry cough, and progressive dyspnea for 1 day. Chest X-ray showed multifocal consolidations, increased interstitial markings, parenchymal emphysema and pneumothorax. IAEP was confirmed by marked pulmonary infiltrates of eosinophils in the lung biopsy specimen. Most known causes of acute eosinophilic pneumonia, such as exposure to causative drugs, toxins, second-hand smoking and infections were excluded. Her symptoms were resolved quickly after corticosteroid therapy.

A Qualitative Study on the Experiences of Mothers Who Have Abandoned Their Additional Birth Plans (후속 출산을 포기한 한 자녀 어머니들의 임신·출산 및 양육경험에 대한 질적 분석)

  • Objective: The present study aimed to explore the experiences of mothers who have abandoned their additional birth plans since their first child was born. Methods: During in-depth interviews, mothers were asked to recall their own experiences of motherhood. The interview data were analyzed using the grounded theory. Results: The central phenomenon was 'The burden of raising a child is too heavy for mothers to carry alone.' The causal conditions were 'First experience: Unpredictable life', 'Physical and psychological difficulty', and 'Spouse's absence.' The contextual conditions were 'Expectations and reality of parenting' and 'Workplace conditions.' The mothers used interaction strategies of 'Defusing conflict only on a surface level: Giving up', 'Reliance on grandmothers', and 'Downgrading career aspirations.' These strategies were mediated by intervening conditions, 'Stereotypes and experiences of non-family nanny', 'Child's development', and 'Family conflict.' The results were 'Endless guilt', 'Ambivalence between independence versus interdependence' and 'Unfairness: Why only me?' Finally, the process of psychological conflict was identified to account for the steps leading mothers to give up on their birth plans. Conclusion/Implications: During the first experience of motherhood, mothers' conflict gets escalated despite their struggles. These experiences might discourage their additional birth plans. These results provide implications for policies to increase fertility rates.
