Characterization of the Myxosporean Parasite Isolated from Emaciated Olive Flounders Paralichthys olivaceus on Jeju Island (제주도 여윔증상 넙치(Paralichthys olivaceus)로부터 분리한 점액포자충의 특성 분석)

  • To investigate the causes of emaciation in cultured olive flounder Paralichthys olivaceus in Korea. We performed histological examinations and polymerase chain reaction (PCR) with a new primer set. In most cases, the most severe emaciation was observed in the abdominal area Using PCR on extracted livers, kidneys, spleens, gills, brains, and intestines, we found that areas around the kidneys and intestines were as almost always positive. In significantly emaciated fish, PCR was positive in all internal organs except the gills. In addition, the homology of 812-bp nucleotide sequences of the 28S rRNA gene was more than 99% in emaciated fish. Partial homology with Myxobolus spp. and Cystodiscus axonis, whose data were obtained from GenBank was 86% and 88%, respectively. Histological examinations detected spores in kidneys and intestines but not in other organs. We also performed cohabitation experiments to determine whether infections could be exchanged among species or only within species. Uninfected olive flounder and red sea bream, Pagrus major, cohabitating with emaciated olive flounder showed 100% and 0% cumulative mortality, respectively. Thus the cause of emaciation in cultured olive flounder of Korea is likely due to a new parasite.

Case Report of a Lead Poisoning by Home-made Herb Pills (불법 제조된 환약 복용에 의하여 발생한 연중독 증례)

  • A 23-year-old female has visited university hospital for abdominal colic and constipation. She took home-made herb pills for 15 days as a traditional remedy for skin eczema. On clinical examinations, conjunctiva was pale, sclera was icteric and lead line was visible on gingiva. Laboratory examinations are as follows; hemoglobin was 8.6g/$d{\ell}$, reticulocyte was 4.2% and there was basophilic stippling of erythrocytes in peripheral blood smear. Blood level of lead was elevated as $69{\mu}g/d{\ell}$. The herb pills contained 3.32% lead, with cumulative dose of lead over 3.2g. The herb pills were manufactured by a 70-year-old man who was not aware of the health hazard of lead. We suppose that he made herb pills with the imported raw materials from China that contained a high percentage of lead.

Health Condition of Residents of Mental Health Sanatoriums and Policy Options (정신요양시설 거주인의 건강 실태 및 정책 대안)

  • Purpose : The purpose of this study is to directly understand the health condition of residents of mental health sanatoriums nationwide, which has been difficult to ascertain in surveys conducted to date. The study presents specific measures for improving the health of these residents. Methods : A "physical examination questionnaire for residents of mental health sanatoriums" was developed to check the basic physical condition of residents, and 20 out of 59 mental health sanatoriums nationwide were randomly selected. Medical personnel visited the sanatoriums, interviewing and examining the residents in person. A total of 396 health surveys were completed. Results : Many of the residents were underweight but had abdominal obesity. It was confirmed that chronic diseases among the residents were not diagnosed early or were not properly managed. Among the subjective symptoms complained of by the residents, musculoskeletal symptoms were the most common. Oral examinations revealed a serious level of oral health problems among the residents, including dental caries and missing teeth. Basic physical examinations found health problems that required additional examination or medical treatment. Blood pressure abnormalities made up the highest percentage of the health problems. Conclusion : Regular health surveys are needed to determine the health condition of residents of mental health sanatoriums. Access to and quality of primary medical services within the sanatoriums need to be dramatically improved. A delivery system for severe diseases and emergency medical care in the sanatoriums should also be specifically presented. The residents should be notified upon admission and during their stay that they have the right to the enjoyment of the highest attainable standard of mental and physical health. The issue of health rights should be addressed within a larger framework of reorganizing management plans for people in the community - not only residents - with chronic mental illness.

Comparison between SNAP Canine Pancreas-Specific Lipase (cPL) Test Results and Pancreatic Ultrasonographic Findings in Dogs with Pancreatitis

  • The object of the present study was to compare abnormal serum canine pancreas-specific lipase results and pancreatic ultrasonographic findings in dogs with pancreatitis. Pancreatitis is a common disease in dogs that is difficult to diagnose. The noninvasive diagnostic procedures, including a serum canine pancreatic-specific lipase (cPL) test and ultrasonographic changes in the pancreas, can be considered for the diagnosis of canine pancreatitis in clinical practice. A retrospective study was performed to assess pancreatitis in dogs. Forty client-owned dogs were suspected to have pancreatitis, which was confirmed by abnormal serum SNAP cPL results. Abdominal ultrasound examinations were also performed. The present study investigated the distribution of clinical signs associated with pancreatitis, and the dogs were divided into two groups: group 1 (clinical signs compatible with pancreatitis; n = 30) and group 2 (no clinical signs; n = 10). Based on this study, an abnormal result on the SNAP cPL assay can still present as a normal pancreas through an ultrasonographic examination, and a normal health status can identify the status of pancreatic ultrasonographic abnormal lesions. Therefore, for dogs with suspected pancreatitis, it is important to repeat an ultrasonographic evaluation. There is no significant difference between clinical symptoms and ultrasonographic changes in the pancreas.

Pyometra and Serosal Inclusion Cysts of the Uterus in a Bitch (개에서 발생한 자궁축농증과 장막성 낭포 1예)

  • Pyometra is a diestrual, chronic disease process with acute manifestations in the adult, ovary-intact bitch. Serosal inclusion cysts develop during postpartum involution as mesothelium becomes trapped during rapid uterine contraction. A 10-year-old golden retriever bitch presented with lethargy, anorexia, tachypnea, abdominal distention, and abnormal vaginal discharge. Radiographic, ultrasound, and laboratory examinations were performed. On ultrasound examination, the uterus was distended by fluid containing echogenic "snow storm" particles; cystic structures containing anechoic fluid were found adjacent to the body of the uterus. Leukocytosis, neutrophilia, and anemia were diagnosed by a complete blood cell count. The initial diagnosis was pyometra, and an ovariohysterectomy was performed. Macroscopically, the uterine body and horns were expanded and partially adhered to the abdominal wall; numerous cysts containing clear fluid protruded over the entire surface of the uterus. Escherichia coli that was sensitive to enrofloxacin, was cultured from the lumen of the uterus. Histopathological assessment confirmed a final diagnosis of pyometra and serosal inclusion cysts of the uterus.

Analysis of Somatostatin-Secreting Gastric Delta Cells according to Upper Abdominal Symptoms and Helicobacter pylori Infection in Children

  • Purpose: Gastric delta cells (D-cells), which are somatostatin-secreting cells, are the main paracrine inhibitor of acid secretion. The number of D-cells was studied in children presenting with upper gastrointestinal (UGI) disease. Methods: We retrospectively investigated the number of D-cells in the gastric body and antrum through immunofluorescence examinations according to symptoms, endoscopic findings, and Helicobacter pylori infection in 75 children who visited Hanyang University Hospital Pediatrics. Results: The mean patient age was 12.2±3.3 years. The male-to-female ratio was 1:1.4. The mean D-cell number per high-power field in the antrum and body was 20.5 and 12 in children with substernal pain, 18.3 and 10.3 in vomiting, 22.3 and 6 in diarrhea, and 9.3 and 6 in abdominal pain, respectively (p>0.05). According to endoscopic findings, the mean D-cell number in the antrum and body was 14.3 and 6 with gastritis, 14 and 9.3 with reflux esophagitis, 16.7 and 8.7 with duodeno-gastric reflux, 19.3 and 12.7 with gastric ulcer, 16 and 13.7 with duodenitis, and 12.3 and 4 with duodenal ulcer, respectively (p>0.05). The D-cell number in the gastric body was 2.7 and 8.7 in children with current H. pylori infection and non-infected children, respectively (p=0.01), while those in the antrum were 15.5 and 14, respectively, with no statistical significance. Conclusion: The D-cell number was lower in the gastric body of children with current H. pylori infection. Further studies concerning peptide-secreting cells with a control group would provide information about the pathogenic pathways of UGI disorder.

Long-Term Management with Chlorambucil and Prednisolone in a Case of Feline Alimentary Lymphoma

  • A 14-year-old, castrated male, domestic shorthair cat was referred for gastrointestinal (GI) signs, including nausea, regurgitation, anorexia, and weight loss. Abdominal ultrasonography revealed thickening of the wall of the gastric and proximal duodenum, moderately enlarged mesenteric lymph nodes, and coarse echotexture of the splenic parenchyma. The results of the feline leukemia virus test were positive. Based on gastrointestinal endoscopic characteristics and histopathological examinations, low-grade alimentary lymphoma was identified in multiple regions of the gastrointestinal tract. The patient was treated with oral prednisolone and chlorambucil chemotherapy, and the clinical signs resolved gradually. During serial follow-up, ultrasonographic findings demonstrated decreases in the duodenal wall thickness and size of the abdominal lymph nodes over a period of 550 days. Survival time was 886 days with prednisolone and chlorambucil chemotherapy. This report describes clinical features, imaging findings, endoscopic characteristics, histopathological features, and long-term management with chlorambucil chemotherapy in a case of feline low-grade alimentary lymphoma.

Colon Cancer Mimicking Physiologic FDG Uptake : with Using of Negative Oral Contrast (네거티브 경구 조영제를 이용한 PET/CT 촬영시 나타난 종양성 섭취와 유사한 생리적 장관 섭취)

  • A 64-year-old female with glioblastoma multiforme (GBM) was assigned to our department for whole body PET/CT scan. She ingested 1 liter of pure water as negative oral contrast just before PET/CT examination. FDG-PET/CT images showed a very intense hypermetabolic, focal lesion in the abdominal cavity around descending colon. The SUVmax of the lesion was 17.2. But there was no abnormal lesion corresponded to the area of PET scan in the combined contrast enhanced CT scan. We suggested considering a malignant lesion due to very intense glycolytic activity. Conventional abdominal CT scan & colonoscopy were accomplished within one week after PET/CT evaluation. There was no abnormality in both examinations. We executed follow-up PET/CT evaluation after 1 month and couldn't find any abnormality around the corresponding area. So we concluded the hypermetabolism was colonic physiologic uptake. A colonic physiologic uptake is a well known cause of false positive finding. Nuclear physicians should be considered the possibility of malignancy when interpret focal colonic uptake, especially incidental finding. There are a few reports that using of negative oral contrast is able to reduce gastrointestinal physiologic uptakes. But as we can see in this case, although we used negative oral contrast, intense physiologic uptake is detected and maxSUV is able to up to 17.2. So, it is important to keep a fact in mind. Even though there is a colonic physiologic uptake in PET/CT image, it may be able to show very intense hypermetabolism regardless of using negative oral contrast.

Idiopathic Arterial Thromboembolism(ATE) in a Turkish Angora Cat (터키산 앙고라(Turkish Angora) 고양이에서 발생한 특발성 동맥 색전혈전증(arterial thromboembolism: ATE) 1례)

  • A 2-year-old, female, Turkish Angora cat was presented due to hindlimb paralysis with 2-day history of vomiting and depression. Physical examination revealed weak femoral pulse and cyanotic footpads of hind limbs. Chest auscultation, thoracic radiography, and electrocardiogram were no remarkable. Abdominal ultrasonography showed suspected hyperechoic thrombus in abdominal aorta. Underlying causes of the arterial thromboembolism were not found on multiple diagnostic examinations and the cat was diagnosed as idiopathic saddle thromboembolism. Despite 4-week regimen with heparin sodium, aspirin, and diltiazem, the hind limbs were progressively and irreversibly necrotized. Therefore, coxofemoral amputation was performed. After surgery, the clinical condition of this cat returned to normal without further complications.

Doses of Pediatric and X-ray Examination Assistants according to Changes in Pediatric X-ray Exposure Conditions (소아 X선 촬영조건 변화에 따른 소아 및 촬영보조자 선량)

  • Although pediatric X-ray examinations are continuously increasing, there are not many studies on the radiation exposure to children and X-ray examination assistants according to X-ray Exposure conditions. Accordingly, we measured the radiation exposure dose of pediatric and X-ray examination assistants according to the standard guidelines and clinical average X-ray Exposure conditions when X-ray examination 10-year-old children. The effective dose and organ dose to pediatric were measured using an Dose area production meter and Monte Carlo-based PCXMC program, and the exposure dose of X-ray examination assistants was measured using an ion-chamber. When performing abdominal supine AP projection, the effective dose to children was up to 2.38 times higher under clinical average X-ray Exposure conditions than the standard guidelines. In addition, during abdominal supine AP projection, the radiation exposure dose to the X-ray examination assistants was highest on the hands at 0.0148 ~ 0.0709 mSv, and exposure dose could be reduced by up to 35% when wearing protective gloves. In conclusion, because the X-ray Exposure conditions used in clinical are unnecessarily high, unnecessary medical radiation exposure could be reduced if appropriate X-ray Exposure conditions and the radiation field area were minimized and the assistant wore shielding gloves.
