Objectives: The aim of this study is to analyze the utilization behaviour and influencing factors of complementary and alternative medicine (CAM) therapies among elderly people with chronic diseases. Methods: The study population was selected among 9 welfare facilities for the aged in 2 cities among Daegu Metropolitan City & Gyeongsangnam Province by two-stage cluster sampling. 250 senior citizens participated in a face-to-face interview using a structured questionnaire. Logistic regression analysis was performed to examine the association between the utilization of CAM therapies and various characteristics of the elderly. All statistics were analyzed using the PASW (ver 18.0). Results: Among 233 elderly peoples, 70.4% (164 persons) had used various kinds of CAM therapies (including nutritional methods, pharmacologic and biologic treatments, etc.) more than once during the last year. 48.8% (64 persons) ~ 60.7% (88 persons) of the elderly used CAM therapies without health and medical experts' counsel. The elderly with perceptions of CAM therapy used it 2 times more than those without knowledge of CAM therapies. The number of chronic diseases was more likely to increase the usage of nutritional methods (OR=2.92, 95% CI: 1.07-7.97), manipulative and body-based practices (OR=5.85, 95% CI: 1.97-17.34), pharmacologic and biologic treatments (OR=2.92, 95% CI: 1.49-8.17). Elderly with diabetes used nutritional methods 3.76 (95% CI: 1.49-9.47) more than elderly without diabetes. Conclusions: CAM therapies use in the aged with chronic diseases appears common. The findings suggest that the clinical efficacy and safety of CAM therapies on medical management of chronic disease may be investigated and that patient-physician communication need to be strengthened.
The purpose of this document is to explain the revisions of the Protection of Cultural Properties Act and its sub-laws which have been mad from Jan. 1999 to Sep. 2001. The Protection of Cultural Properties Act and its sub-laws have been revised three times from 1999 to 2001, before and after the Office of Cultural Properties was raised to Cultural Properties Administration on May 24, 1999. The main points of the revisions are as follows. First of all, the role of the local autonomous entities has been increased. The governor of the local autonomous entities is entitled to announce administrative orders related to the preservation of State-designated Cultural Properties. Also, the local autonomous entities has the authorities to examine whether the construction work which will be made in the outer boundaries, which is provided by regulations, of the protected area of the cultural properties might have any effect on preservation of cultural properties or not. Second, preventive actions to protect the cultural properties have been strengthened. If the scale of construction work is more than some scale, the preliminary survey of the surface of the earth to confirm the existence of buried cultural properties and their distribution is obligated. One who is promoting the development plan more than some scale must discuss the plan with the Administrator of Cultural Properties Administration in the process of planning. These actions would be effective to prevent the cultural properties from being damaged because of the development. Third, relaxation of the restrictions has been proceeded. On the basis of regulations which specify the actions to affect the preservation of cultural properties, negative system that does not limit the actions which are not specified in the regulations is introduced. The appropriateness of both protected structure and area should be regularly reviewed and adjusted. Also, most of the restrictions which was made only for administrative convenience and over-regulated the people's living have been revised. Finally, the number of cultural properties to be protected has been increased. Besides the State-designated Cultural Properties, the other cultural properties which are worthy to be protected as City-or-Province-designated Cultural Properties can be designated provisionally and protected. The system of registration and maintenance of the buildings and facilities which are not designated as the Modern Cultural Heritages is established. The penalty for damaging and stealing the cultural Properties which are not designated to be protected was strengthened. Even a dead natural monument can be acknowledged as an natural monument and it is limited to make a specimen or stuffing of the dead natural monument. All of these actions are fit to the high level of understanding of the public about the cultural properties and as the result of these actions, the number of cultural properties to be preserved has been increased. To sum up, the directions of revisions of the Protection of Cultural Properties Act and its sublaws which have been made from Jan. 1999 to Sep. 2001. are the localization of the protection of the cultural properties, the strengthening of protective actions, the relaxation of various regulations and the increasing of the number fo the protected cultural properties. Also, various problems raised in the processes of implementations of the laws have been reviewed and revised.

Shield TBM, since it was employed for Suyoungman Bay riverbed tunnel of Busan Subway in 2000,has been increasingly adopted in Korea, and in line with growing popularity, the study on Shield TBM has been expanded. However the studies mostly focus on ground condition in a bid to estimate the advancement rate and develop the model for calculating the excavation efficiency, whereas the efforts to analyze the cause of delay and to develop the improvement measures have been neglected. Thus the studies were mostly intended to analyze the schedule slippage focusing on ground conditions, while the study on schedule behind due to equipment itself and related facilities have yet to be attempted in earnest. This study hence was aimed at evaluating the troubles and schedule slippage caused by mechanical elements such as shield TBM equipment and tools and ground conditions, making use of FMEA approach so as to analyze the risk of schedule delay by such elements, thereby proposing the preventive measures to deal with high-risk factors. So, this study suggest the solution to highly ranked trouble factor for the purpose of enhance the efficiency on Shield TBM.
Purpose: The aims of this study are to examine clinical characteristics, patterns of medical care utilization, and factors which determine medical care utilization of elementary school children with recurrent abdominal pain (RAP), to find posssible factors influencing the onset and the course of the disorder. Method: We performed questionnaires in Kwangju on children from two primary schools from June,1 1998 to June 30 and carried out statistical analysis. Result: 1) Total number of questionnaires were 1417. 715 were male and 702 were female and the ratio of male to female was 1.02:1. Average age was 10.3 years. 2) 268 children had RAP (18.9%), boys 132 (18.4%), girls136 (19.2%). 3) The duration of the pain within 10 minutes was 68.5%. 178 children with RAP (66.3%) visited the doctor. The utilization pattern of medical facilities of the pupils with RAP; the most frequently utilized medical facility was pediatrics (35.2%) and the order ran as internal medicine (31.5%), and pharmacy (29.25). The utilization pattern of medical facilities for the older students; the utilization rate of pediatrics decreased, but internal medicine increased. The major factors affecting the selection of the medical facility were geographic accessibility, kindness of the personnel, good results and traffic convenience. 4) Symptoms which were accompanied with abdominal pain were headache (44.5%), chest pain (28.2%), dizziness (26.6%), vomiting (9%), and 119 children (44.5%) had no accompanied symptoms. 5) In 95 children (35.3%) abdominal pain, occured at postprandial time, in 55 children (20.5%) before meal and in 39 children (14.7%) at school. The highest incidence rate of RAP was observed on Monday (21.4%), and the lowest on Saturday (8.7%). 6) The most frequent involved part of the abdomen was periumbrical area (38%) and the order ran as epigastrium and suprapubic area. The most frequent characteristics of abdominal pain were burning pain (36.9%) and the order ran as dull, cramping and colicky pain. Conclusion: RAP is a frequent disease entitiy in children. Too many times children with RAP are treated by other departments instead of Pediatrics. A child has a peculiar growth and development which is different to those from an adult with advancing years. So, it is necessary to choose special medical care and an adequate medical facility.
Background: We aimed to evaluate the role of genetic polymorphisms in tobacco carcinogen-metabolizing genes and their interactions with smoking in a hospital-based case-control study of Japanese subjects. Materials and Methods: We examine the associations of pancreatic cancer risk with genetic polymorphisms in GSTM1, GSTT1 and GSTP1, phase II enzymes that catalyze the conjugation of toxic and carcinogenic electrophilic molecules. The study population consisted of 360 patients and 400 control subjects, who were recruited from several medical facilities in Japan. Unconditional logistic regression methods were used to estimate odds ratios (ORs) and 95% confidence intervals (CIs) for the associations between genotypes and pancreatic cancer risk. Results: Among the control subjects, the prevalence of the GSTM1-null genotype and the GSTT1-null genotype was approximately 56% and 48%, respectively. Cases and controls were comparable in terms of GSTM1 and GSTT1 genotype distributions. Neither of the deleted polymorphisms in GSTM1 and GSTT1 was associated with the risk of pancreatic cancer, with an age- and sex-adjusted OR of 0.99 (95%CI: 0.74-1.32) for the GSTM1-null genotype, and 0.98 (95%CI: 0.73-1.31) for the GSTT1-null genotype. The OR was 0.97 (95%CI: 0.64-1.47) for individuals with the GSTM1 and GSTT1-null genotypes compared with those with the GSTM1 and GSTT1- present genotypes. No synergistic effects of smoking or GST genotypes were observed. Conclusions: Our results indicate no overall association between the GSTM1 and GSTT1 deletion polymorphisms and pancreatic cancer risk in the Japanese subjects in our study.

Objective: The aim of this study was to evaluate the patterns of helicopter emergency medical services (HEMS) in Ullung Island. Methods : The authors reviewed the records from emergency room diaries and the lists of helicopter transfers in the Ullung Public Health Medical Center over the 5-year period from Jan 1, 1997 to Dec 31, 2001. Results : One hundred thirteen cases were transferred by helicopters in 88 flights. According to year, the number of flights was 13(14.8%) and the number of cases was 15(13.3%) in 1997; 17(19.3%) and 21(18.6%) in 1998; 18(20.5%) and 20(17.7%) in 1999; 17(19.3%) and 20(17.7%) in 2000; and 23(26.1%) and 37(32.7%) in 2001. According to the kind of helicopter, the number of flights was 46(52.3%) and the number of cases was 60(53.1%) by Maritime police; and 19(21.6%) and 28(25.1%) by 119 rescue. According to time zone, there were no night flights. According to sex and age, there were 75 male cases(66.4%) and 28 cases(28.3%) of patients aged sixty years and over. The number of flights was 11(12.5%) and the number of cases was 15(13.3%) in November; 10 flights(11.4%) and 14 cases(12.4%) in March; and 7 cases(8.0%) in each of September, October and April. The most common season of helicopter transfer cases was autumn. According to transfer area, there were 48 cases (42.5%) in Pohang city, Gyeonsangbukdo; 35(31.0%) in Gangnung city, Gangwondo; and 17(15.0%) in Daegu metropolitan city. According to condition, there were 27 cases(23.9%) of cerebro-vascular accident, 13(11.5%) of fracture and 11(9.7%) of head injury. According to admission department, there were 42 cases(37.2%) in Neurosurgery, 21(18.6%) in Internal Medicine and 13(11.5%) in Orthopedic Surgery. According tothe Korea Standard Classification of Disease(3-KSCD), circulatory systemic disease(IX) and injury, intoxication and others (XIX) were the two most frequent categories with 34 cases(30.1%) each, followed by digestive system disease (XI) with 23 cases(20.4%). Conclusions : HEMS in Ullung Island leave much to be desired. Helicopters cannot make a night flight and are not equipped with medical facilities. HEMS in islands such as Ullung Island are essential. We hope that night flights, equipment-monitoring systems for emergency patients in the helicopters, and a law related to HEMS in the island will all be established.

Objectives: This study was conducted to identify factors that affect medical service performance in rural health sub-centers and to develop a method to improve health sub-center performance that takes advantage of these insights. Methods: This study included 1,242 South Korean health sub-centers that had been in operation at least since December 31, 2009 as units of analysis. After adjusting for population differences between areas, the performance of medical service among health sub-centers was analyzed according to medical services performed per person. We performed t-tests, ANOVA, Scheffe's tests and multiple regression analyses. Results: The following were significant variables affecting the medical service performance of rural health sub-centers: number of hospitals and clinics, presence of community health practitioner posts, distance from health sub-centers to main public health centers, distance from health sub-centers to the nearest emergency medical facilities, and proportion of the local population aged 65 and over. In contrast, the proportion of the local population between ages 0-4 and the placement of public health doctors that had already completed their internship were not significant variables. Conclusions: The medical service performance of health sub-centers located in rural areas is significantly affected by local population and health care environment characteristics, and therefore, it is imperative to develop strategies to provide differentiated service based on these factors.

This study examined the relationship between the physical activity levels and health care utilization in Korean elderly based on the data from The fifth Korean National Health and Nutrition Examination Survey (K-NHANES V). As a result, physically active participants were 0.54 times (OR;95% CI, p<0.001) less likely to use inpatient health care and 0.73 times (OR;95% CI, p=0.040) less likely to use outpatient health care and 0.75 times (OR;95% CI, p<0.007) less likely to use drugstores than physically inactive participants, and the frequency of health care use was lower. In addition, physically minimal active participants were 0.64 times (OR;95% CI, p<0.001) less likely to use inpatient health care than physically inactive participants; the frequency of other health care facilities was lower. In conclusion, the relationship between being physically active and lower health care utilization among Korean elderly identified in this study. This suggests that being physically active is helpful to the elderly, not only in health promotion and improving quality of life, but also in reduced medical expenditure.
The purpose of this study was to develop a Eco smoking booth that can effectively reduce hazardous pollutants generated during smoking and evaluate the efficiency and effectiveness of removing hazardous pollutants. The design and manufacture of an eco-friendly automatic smoking booth equipped with deodorizing facilities, such as inlet - HEPA filter - electrostatic precipitator (EP) - impregnated activated carbon - exhaust port, etc., and the efficiency of removing hazardous pollutants from inside and outside was measured and evaluated. The complex odor removal efficiency was 95.37% inside the smoking booth, and 97.38% at the exit of the preventive facility. The carbon monoxide removal efficiency was 94.25% in the inside and 98.32% in the outlet. In addition, the removal efficiency of particulate matter, (PM1, PM2.5, and PM10) inside the smoking booth was 98.59%, and 98.85% at the outlet. The total volatile organic compounds (TVOCs) decreased from $26,000{\mu}g/m^3$ to $5,203{\mu}g/m^3$ in the smoking booth, resulting in 79.99% removal efficiency. After the ventilator was operated, the measured effluent concentration was $5,019{\mu}g/m^3$, and the removal efficiency was 80.70%. Therefore, the smoking booth designed and manufactured through this study can be applied to the removal of harmful pollutants even in the small working environment in the future.
