Is Axillary Dissection Necessary for Breast Cancer in Old Women? A Meta-analysis of Randomized Clinical Trials

  • Background: We performed this meta-analysis to assess the effectiveness and safety of axillary dissection in old women. Methods: The Cochrane Library, PubMed, EMBASE and Chinese Biomedical Literature Database were searched and all randomized controlled trials of axillary dissection in old women (at least 60 years old) were considered. Meta-analyses were completed using RevMan5.1. Results: Three eligible randomized controlled trials (RCTs) including 5,337 patients were considered. There was weak evidence in favour of axillary dissection (AD) in old women. The meta-analysis showed that the overall survival (OS) after 1, 3, 5 and 7 years and the disease free survival (DFS) after 1, 3 and 5 year were not statistically significantly different between AD and no AD groups. However, there was a difference in the 7 year DFS. Conclusions: Axillary dissection did not provide survival benefit to the old women with breast cancer analysed. Therefore, axillary dissection is not well-indicated in old women with breast cancer.

한국 지역사회 노인의 신체활동 프로그램이 수면에 미치는 효과: 체계적 문헌고찰 및 메타분석 (Effects of Physical Activity Programs on Sleep among Community-dwelling Elders in Korea: A Systematic Review and Meta-analysis)

  • 본 연구의 목적은 지역사회에 거주하는 한국 노인을 대상으로 한 신체활동 프로그램이 수면에 미치는 효과를 체계적이고 종합적으로 검증하기 위해 메타분석하는 것이다. 자료수집을 위해 PRISMA (Preferred Reporting Items for Systematic Reviews and Meta-Analysis) 가이드라인에 따라 2016년 11월 20일까지 출판된 연구물을 체계적으로 수집하고 각 기준에 따라 선별한 결과 총 7편이 메타분석 대상 논문으로 선정되었다. 자료 분석은 Comprehensive Meta-Analysis 3.0 프로그램을 이용하였으며, Q 검정을 통하여 연구들 사이의 동질성 여부를 검정한 후 개별 연구의 효과크기를 병합한 평균 효과크기를 산출하였다. 연구결과, 노인을 대상으로 한 신체활동 프로그램인, 힐링요가, 에어로빅댄스, 아쿠아로빅스, 한국무용, 베하스 운동, 운동프로그램, 타이치 운동이 수면에 미치는 총 효과는 Hedges' g=0.577(95% Confidence Interval (CI):0.337~0.817)로 유의한 효과크기를 보였다. 신체활동 프로그램 중 베하스 운동프로그램은 Hedges' g=0.758(95% CI:0.248~1.267)로 노인의 수면에 가장 큰 효과크기를 보였으며, 타이치 운동프로그램이 Hedges' g=0.697(95% CI:0.071~1.324), 운동프로그램은 Hedges' g=0.650(95% CI:0.141~1.159)의 순으로 효과크기를 보였다. 그러므로 지역사회 거주 노인의 수면의 질 향상을 위한 간호학적 중재를 적용함에 있어, 노인의 수면에 긍정적인 효과를 미치는 베하스 운동과 타이치 운동을 프로그램화 하여 적용하는 것이 도움이 될 것이다. 또한 노인의 수면에 유의한 효과를 미치지 않는 것으로 파악된 중재프로그램의 내용 및 특성, 문제점 등을 분석하여, 향후 노인을 대상으로 한 중재 개발 시 고려할 필요가 있다.

Association between the DICER rs1057035 Polymorphism and Cancer Risk: Evidence from a Meta-analysis of 1,2675 Individuals

  • Background: DICER, one of the microRNA (miRNA) biogenesis proteins, is involved in the maturation of miRNAs and is implicated in cancer development and progression. The results from previous epidemiological studies on associations between DICER rs1057035 polymorphism and cancer risk were inconsistent. Thereforewe performed this meta-analysis to summarize possible associations. Materials and Methods: We searched all relevant articles on associations between DICER rs1057035 polymorphism and cancer risk from PubMed, EMBASE, Chinese Biomedical Literature and Chinese National Knowledge Infrastructure until August 2014. Odds ratios (ORs) with 95% confidence intervals (CIs) were calculated to assess any associations. Heterogeneity tests, sensitivity analyses and publication bias assessments were also performed in this meta-analysis. All analyses were conducted using STATA software. Results: Seven case-control studies, including 4,875 cancer cases and 7,800 controls were included in the meta-analysis. Overall, the results indicated that the C allele of DICER rs1057035 polymorphism was significantly associated with decreased cancer risk in allelic comparison, heterozygote and dominant genetic models (C vs T: OR=0.88, 95%CI 0.81-0.95, p=0.002; TC vs TT: OR=0.85, 95%CI 0.77-0.93, p=0.001; CC/TC vs TT: OR=0.86, 95%CI 0.78-0.94, p=0.001). In the subgroup analysis by ethnicity, a significantly decreased cancer risk was found in Asian but not Caucasian populations. Conclusions: The present meta-analysis suggests that the C allele of the DICER rs1057035 polymorphism probably decreases cancer risk. However, this association may be Asian-specific and the results should be treated with caution. Further well-designed studies based on larger sample sizes and group of populations are needed to validate these findings.

Association Between XPD Asp312Asn Polymorphism and Esophageal Cancer Susceptibility: A Meta-analysis

  • Objective: To investigate the association between xeroderma pigmentosum group D (XPD) Asp312Asn polymorphism and esophageal cancer (EC) susceptibility by meta-analysis. Methods: We searched PubMed up to April 9th, 2012, to identify relevant papers, and 8 published case-control studies including 2165 EC patients and 3141 healthy controls were yielded. Odds ratios (ORs) with relevant 95% confidence intervals (CIs) were applied to assess the association between XPD Asp312Asn polymorphism and EC susceptibility with the Comprehensive Meta-Analysis software, version 2.2. Results: Overall, the meta-analysis results suggested the XPD Asp312Asn polymorphism to be significantly associated with EC susceptibility [(Asn/Asn+Asp/Asn) vs. Asp/Asp: OR=1.20, 95%CI=1.05-1.36, p=0.01; and Asp/Asn vs. Asp/Asp: OR=1.15, 95%CI =1.01-1.31, p=0.04]. In the subgroup analysis by ethnicity and cancer type, significantly associations were found for Caucasian populations [(Asn/Asn+Asp/Asn) vs. Asp/Asp: OR=1.26, 95%CI =1.08-1.47, p<0.001; Asp/Asn vs. Asp/Asp: OR=1.19, 95%CI =1.02-1.40, p=0.03] and esophageal squamous cell carcinoma [(Asn/Asn+Asp/Asn) vs. Asp/Asp: OR=1.19, 95%CI=1.01-1.41, p=0.04]. There was no heterogeneity and no publication bias existed. Conclusions: This meta-analysis shows that the XPD Asp312Asn polymorphism may be a risk factor for developing EC, especially for Caucasian populations and esophageal squamous cell carcinoma.

Meta-analysis for Efficacy and Safety of Propofol during Dental Sedation

  • Background: Dental sedation reduces fear and phobia during dental treatment and helps patients get quality treatment by inducing adequate consciousness control. Propofol has recently grabbed the spotlight, but no meta-analysis for efficacy and safety of propofol in dentistry has yet been performed. Thus, the purpose of this study was to perform meta-analysis to verify the efficacy and safety of propofol for use in dental sedation. Methods: Articles published between 1980 and 2010 were searched in the web sites, journals and medical database including The Cochrane Library, MEDLINE and EMBASE. And a total of 22 studies were selected among the randomized controlled trials (RCTs) that compared the use of propofol with other sedatives (control group). The data was collected from these studies and meta-analysis for efficacy and safety was performed using Comprehensive Meta-Analysis 5.0 (CMA 5.0). Results: The patient recovered significantly faster and discharged significantly earlier in the propofol group (SMD = -1.442, P < 0.001). The satisfaction of patient and that of operator was higher in the propofol group (P < 0.05). The incidence of arrhythmia and apnea/ hypoventilation was significantly lower in the propofol group (OR = 0.071, P < 0.05), and there was no significant difference in the other side effects. On the level of sedation, although the sedation score was significantly lower in the propofol group (SMD = -0.430, P < 0.05). Conclusions: The present analysis showed that the use of propofol resulted in high satisfaction levels on the part of the patients and operators, a shorter recovery time, and faster hospital discharge. The incidence of complications, however, was lower in the propofol groups or not much different between the propofol and control groups. Thus, the adequate use of propofol in dentistry is believed to be helpful for the effective and safe sedation of the patients.

Red Meat Intake and Risk of Non-Hodgkin Lymphoma: A Meta-Analysis

  • Background: While the incidence of non-Hodgkins lymphoma (NHL) has been rising worldwide, the reasons remain undefined. Recent research has focused on effect of red andf processed meat intake as a risk factor, but with inconclusive results. We therefore conducted a meta-analysis of data published to date, to ascertain the overall association between intake and NHL. Materials and Methods: A published literature search was performed through Pubmed, Cochrane Library, Medline, and Science Citation Index Expanded databases for articles published in English. Pooled odds ratios (ORs) and 95% confidence intervals (95%CIs) were calculated using random or fixed effects models. Heterogeneity was assessed using Chi-square and I2 statistics. Dissemination bias was evaluated by funnel plot analysis.We performed a formal meta-analysis using summary measures from these studies. Results: In total, 11 published studies were included in the final analysis. The combined analysis revealed that there was significant association between the red meat and NHL risk (OR=1.10, 95%CI: 1.02 to 1.19, p=0.01). Additionally, there was showed significance association between processed red meat and NHL risk (OR=1.17, 95%CI: 1.06 to 1.29, p=0.001). In subgroup analysis, a statistical significant association was noted between diffuse large B-cell lymphoma (DLBCL) (OR=1.20, 95%CI: 1.04 to 2.37, P=0.01) and red meat intake. Conclusions: In this meta-Analysis, there was evidence for association between consumption of red meat, or processed meat and risk of NHL, particularly with the DLBCL subtype in the red meat case.

숲 체험 활동이 유아의 사회성 발달의 효과에 관한 메타분석 (A Meta-Analysis on Effects of Infant's Sociality Development in Forest Experience Activities)

  • This study aims to examine the effects of infant's social development forest experience activities through meta-analysis. The final nine studies(total of 165 in the experimental group and 159 in the control group) were selected as a method of systematic review. Meta-analysis on overall effect size estimation, chi-square test, significance analysis, publication bias analysis, and subgroup analysis was performed using the R program. The overall effect size of 9 studies was 1.59, indicating a large effect size. As a result of subgroup analysis of the sub-factors of sociality, autonomy showed the largest effect size at 1.47, the adjusted effect size of cooperation was 1.34, the effect size adjusted for peer interaction was 1.29, and the adjusted effect size for perspective-taking ability was 0.97. All were found to have a statistically significant effect. To analyze the moderating effect, a meta-regression analysis was conducted on the participation period(4, 5~6, 7~8weeks), the number of sessions(6~10, 11~15, 16~20), the frequency per week(1, 2, 5), and the participation time(40, 60, 90, 120, 150min), but there was no statistical difference. Although not statistically significant, the effect size was larger when the participation period was 4 weeks, the number of sessions was 16 to 20, the frequency was 2 times per week, and the participation time was 40 minutes. This results can be usefully utilized by policy makers and forest commentators related to the vitalization of forest education through forest experience activities.

부모효능감 관련변인에 대한 메타분석 (A Meta-Analysis of Variables Related to Parenting Efficacy)

  • The purpose of this study is to synthesize research findings on the relationship between parenting efficacy and the variables related to it, as well as to produce results using meta-analysis. The following questions guide this study : Which variables are frequently conducted in research related to parenting efficacy? What is the effect size of each variable? The 65 studies whose results are analyzed in this paper were collected via a search for investigations focusing on 'parenting efficacy'. The studies were used for meta-analysis in which eleven variables were selected. The results of the analysis indicate that a large effect size is produced by the parenting stress in a negative direction. A higher than medium effect size occurs on the basis of parenting beliefs in a negative direction. A higher than medium effect size is also caused by the father's involvement in child care and social support. A medium effect size is produced on the basis of children's temperament, and a small effect size is the result of the mother's academic background, parenting knowledge, and parent's income. There is no relationship between parenting efficacy and children's gender.

Effects of Advance Care Planning on End-of-Life Decision Making: A Systematic Review and Meta-Analysis

  • Purpose: The purpose of this systematic review and meta-analysis was to investigate the effects of advance care planning on end-of-life decision-making. Methods: Databases including RISS, KISS, KMbase, KoreaMed, PubMed (MEDLINE), Embase, and CINAHL were searched for studies that examined the effects of advance care planning interventions. The inclusion criteria were original studies in English or Korean; adults ≥18 years of age (population); advance care planning (intervention); completion of advance directives (AD) or advance care planning (ACP) (outcomes); and randomized or non-randomized controlled trials (RCTs and non-RCTs, respectively) (design). Study quality was measured using the checklists of the Joanna Briggs Institute. Meta-analyses were conducted with the Comprehensive Meta-Analysis program. Results: Nine RCTs and nine non-RCTs were selected for the final analysis. The effect sizes (ES) of the outcome variables in nine RCTs were meta-analyzed, and found to range from 0.142 to 0.496 for the completion of AD and ACP (ES=0.496, 95% CI: 0.157~0.836), discussion of end-of-life care (ES=0.429, 95% CI: -0.027~0.885), quality of communication (ES=0.413, 95% CI: 0.008~0.818), decisional conflict (ES=0.349, 95% CI: -0.059~0.758), and congruence between preferences for care and delivered care (ES=0.142, 95% CI: -0.267~0.552). Conclusion: ACP interventions had a positive effect on the completion of AD and ACP. To apply AD or ACP in Korea, it is necessary to develop ACP interventions that reflect aspects of Korean culture.

성인 혈우병 환자와 운동: 체계적 문헌고찰과 메타분석 (Exercise and adults with hemophilia: a systematic review and meta-analysis)

  • Purpose: This study aimed to evaluate the effectiveness of exercise in patients with hemophilia. Methods: We carried out a systematic review and meta-analysis in accordance with the Preferred Reporting Items for Systematic Review and Meta-analysis (PRISMA) guidelines. A literature search was conducted of published randomized controlled trials for exercise interventions from January 1, 2014 to March 15, 2023. To estimate the size of the effects of exercise, a meta-analysis was performed using the R package "meta." Results: Five databases were searched to obtain articles published in Korean or English. Of 1,150 articles reviewed, 13 were included in the systematic review and 9 in the meta-analysis. The risk of bias was assessed using RoB 2.0. The overall effect sizes of exercise interventions, calculated as the standardized mean difference, were -0.11 (95% confidence interval [CI] = -1.41 to -1.20) for pain, -2.13 (95% CI = -3.33 to -0.93) for joint health, 9.96 (95% CI = 7.51 to 12.28) for physical activity, and 0.59 (95% CI = -0.39 to -1.56) for quality of life. Conclusion: These findings suggest that exercise is useful for improving the joint health and physical activity of patients with hemophilia. Thus, it is necessary to develop and apply exercise interventions for patients with hemophilia to reduce their pain and improve their quality of life.
