CAM Zirconia 완전도재 구조물의 정밀 적합도에 관한 연구 (A STUDY OF PRECISE FIT OF THE CAM ZIRCONIA ALL-CERAMIC FRAMEWORK)

  • State of problem: Zirconia all-ceramic restoration fabricated with CAM system is on an increasing trend in dentistry. However, evaluation of the marginal and internal fits of zirconia bridge seldomly have been reported. Purpose: This study was to evaluate the at of margin and internal surface in posterior 3-unit zirconia bridge framework fabricated with CAM system(DeguDent, Germany). Material and Method: Preparations of secondary premolar and secondary molar on artificial resin model were performed for fabrication of 3-unit posterior bridge framework. Fits of 5 zirconia bridge framework were compared with 5 precious ceramo-metal alloy framework(V-GnathosPlus, Metalor, Switzerland), and prepared margins were designed to chamfer and shoulder finishing line. Each framework was cemented to epoxy resin model with reinforced glass ionomer(FujiCEM, GC Co., Japan), embedded in acrylic resin and sectioned in two planes, mesio-distal and buccolingual. Samples were divided into six pieces by sectioning and had two pieces of each surface(i.e mesial, distal, buccal and lingual surface) per abutment, so there were eight measuring points in each abutment. External gap was measured at the margin and internal gaps were measured at the margin, axial and occlusal surface. Gaps were observed under the measuring microscope(Compact measuring microscope STM5; Olympus, Japan) at a magnification of $\times100$. T-test were used to determine the statistic significance of the different gaps between zirconia and metal framework. Results and Conclusion: 1. External and internal marginal gaps of zirconia and metal framework were in clinically acceptable range. External marginal gaps were not different significantly between zirconia$(81.9{\mu}m)$ and metal $(81.3{\mu}m)$ framework and internal marginal gaps of zirconia $(44.6{\mu}m)$ were smaller than those of metal framework $(58.6{\mu}m)$. 2. Internal axial gaps of zirconia framework$(96.7{\mu}m)$ were larger than those of metal frame-work$(78.1{\mu}m)$ significantly and adversely, internal occlusal gaps of zirconia frame-work$(89.4{\mu}m)$ were smaller than those of metal framework $(104.9{\mu}m)$ significantly. 3. There were no significant differences in external and internal marginal gaps between chamfer and shoulder finish line when zirconia frameworks were fabricated.

Evaluation of translucency of monolithic zirconia and framework zirconia materials

  • PURPOSE. The opacity of zirconia is an esthetic disadvantage that hinders achieving natural and shade-matched restorations. The aim of this study was to evaluate the translucency of non-colored and colored framework zirconia and monolithic zirconia. MATERIALS AND METHODS. The three groups tested were: non-colored framework zirconia, colored framework zirconia with the A3 shade according to Vita Classic Scale, and monolithic zirconia (n=5). The specimens were fabricated in the dimensions of $15{\times}12{\times}0.5mm$. A spectrophotometer was used to measure the contrast ratio, which is indicative of translucency. Three measurements were made to obtain the contrast ratios of the materials over a white background ($L^*w$) and a black background ($L^*b$). The data were analyzed using the one-way analysis of variance and Tukey HSD tests. One specimen from each group was chosen for scanning electron microscope analysis. The determined areas of the SEM images were divided by the number of grains in order to calculate the mean grain size. RESULTS. Statistically significant differences were observed among all groups (P<.05). Non-colored zirconia had the highest translucency with a contrast ratio of 0.75, while monolithic zirconia had the lowest translucency with a contrast ratio of 0.8. The mean grain sizes of the non-colored, colored, and monolithic zirconia were 233, 256, and 361 nm, respectively. CONCLUSION. The translucency of the zirconia was affected by the coloring procedure and the grain size. Although monolithic zirconia may not be the best esthetic material for the anterior region, it may serve as an alternative in the posterior region for the bilayered zirconia restorations.

  • 수직적, 수평적 골흡수가 심한 환자에서 임플란트를 이용한 전악수복의 경우 경조직과 연조직 이식을 통해서 임플란트를 원하는 위치에 식립할 수도 있지만, 치은과 치아의 기능과 심미를 회복할 수 있는 fixed detachable prostheses를 대체 술식으로 사용할 수도 있다. 이러한 증례에서 다양한 재료가 수복물의 제작에 사용 가능하지만, metal/acrylic 보철물에서는 레진치아의 파절 및 탈락이 일어날 수 있고, metal/ceramic 혹은 zirconia/ceramic 보철물에서는 도재의 chipping이나 파절과 같은 문제가 발생할 수 있다. 이에 최근에 심미적이면서도 기능적인 보철수복을 위해 zirconia에 도재를 축성하지 않고 임상적으로 적용가능한 monolithic zirconia framework이 출시되어 사용되고 있다. 본 임상 증례는 심미적인 요구도가 높은 완전 무치악 환자에서 임플란트를 식립하고 chipping이나 파절의 위험을 감소시키기 위해 monolithic zirconia framework을 이용해 만들어진 complete fixed detachable 보철물을 이용한 수복에 대해 보고하고자 한다. 이번 증례에서 보철물은 심미적, 기능적으로 만족스러웠으며, 2년 간의 정기검사에서 임상적인 합병증은 보고되지 않았다.

The effect of zirconia framework design on the failure of all-ceramic crown under static loading

  • PURPOSE. This in vitro study aimed to compare the failure load and failure characteristics of two different zirconia framework designs of premolar crowns when subjected to static loading. MATERIALS AND METHODS. Two types of zirconia frameworks, conventional 0.5 mm even thickness framework design (EV) and 0.8 mm cutback of full contour crown anatomy design (CB), were made for 10 samples each. The veneer porcelain was added on under polycarbonate shell crown made by vacuum of full contour crown to obtain the same total thickness of the experiment crowns. The crowns were cemented onto the Cobalt-Chromium die. The dies were tilted 45 degrees from the vertical plane to obtain the shear force to the cusp when loading. All crowns were loaded at the lingual incline of the buccal cusp until fracture using a universal testing machine with cross-head speed 0.5 mm/min. The load to fracture values (N) was recorded and statistically analyzed by independent sample t-test. RESULTS. The mean and standard deviations of the failure load were $1,170.1{\pm}90.9$ N for EV design and $1,450.4{\pm}175.7$ N for CB design. A significant difference in the compressive failure load was found (P<.05). For the failure characteristic, the EV design was found only cohesive failures within veneering porcelain, while the CB design found more failures through the zirconia framework (8 from 10 samples). CONCLUSION. There was a significant difference in the failure load between two designs, and the design of the framework influences failure characteristic of zirconia crown.

Considerations for the Survival of Complete Arch Implant-Supported Zirconia Restorations; Status of Antagonistic Arches and Stress Distribution on Frameworks: A Case Report

  • This report describes two cases of complete arch implant-supported restorations. The first patient had seven dental implants in each arch with monolithic zirconia frameworks. At four weeks' follow-up, the one-piece maxillary framework was fractured, which was re-designed and re-fabricated using laser-sintered cobalt-chrome alloy. The second patient had four implants in the mandible only. A mandibular monolithic zirconia framework and a maxillary conventional complete denture were fabricated and delivered. At five years' follow-up, the patient reported no significant discomfort. Careful consideration and monitoring of the status of antagonistic arches and stress distribution on zirconia frameworks were suggested for complete arch implant-supported fixed restorations.

  • 구강암에 대한 외과적 처치로 구강 내 광범위한 경조직, 연조직을 제거한 환자는 저작, 연하, 발음 등의 기능적 문제와 치열, 안모의 심미적 문제를 갖게 된다. 악골 재건 후에도 치조골 등 합병증으로 인해 가철성 보철물의 유지 및 지지를 받는데 제한적이다. 임플란트 지지 고정성 보철물은 이러한 환자들에게 적절한 보철수복 방법이 되어 왔다. 본 증례는 하악 재건 후 computer-aided design과 computer-aided manufacturing을 이용하여 지르코니아 프레임워크 상부에 단일 구조 지르코니아 크라운을 제작하여 현재 치열의 기능적, 심미적 회복을 하여 이에 보고하고자 한다.

Effect of span length on the fit of zirconia framework fabricated using CAD/CAM system

  • PURPOSE. The purpose of this study was to evaluate the effect of the span length on the fit of zirconia framework fabricated using CAD/CAM system. MATERIALS AND METHODS. Abutments for single, 4-unit and 6-unit fixed partial prostheses were fabricated. Ten zirconia frameworks were fabricated for each group. The marginal and internal gap were presented by means of replica technique and measured by measuring microscope ($AXIO^{(R)}$, Carl Zeiss, Rochester, NY) and software (I-$solution^{(R)}$, IMT i-solution Inc., Vancouver, BC, Canada). The results were statistically analyzed by multivariate analysis test and Dunnett T3 test for post hoc test (${\alpha}$=.05). RESULTS. There were statistically significant differences at 2, 4, 7, 8 points (mesio-distal section) and b, d, e, f, g (labio-lingual section). In some marginal reference points of 6-unit group (P<.05), the marginal gap were larger than 120 ${\mu}m$. CONCLUSION. Span length of zirconia core may have an influence on marginal and internal fit. Within the limitation of this study, the increase of span length of zirconia framework of 6 or more-unit fixed partial denture may decrease the marginal and internal fit.

Effect of different veneering techniques on the fracture strength of metal and zirconia frameworks

  • PURPOSE. To determine whether the fracture strengths and failure types differed between metal and zirconia frameworks veneered with pressable or layering ceramics. MATERIALS AND METHODS. A phantom molar tooth was prepared and duplicated in 40 cobalt-chromium abutments. Twenty metal (IPS d.SIGN 15, Ivoclar, Vivadent, Schaan, Liechtenstein) and 20 zirconia (IPS e.max ZirCAD, Ivoclar) frameworks were fabricated on the abutments. Each framework group was randomly divided into 2 subgroups according to the veneering material: pressable and layering ceramics (n=10). Forty molar crowns were fabricated, cemented onto the corresponding abutments and then thermocycled ($5-55^{\circ}C$, 10,000 cycles). A load was applied in a universal testing machine until a fracture occurred on the crowns. In addition, failure types were examined using a stereomicroscope. Fracture load data were analyzed using one-way ANOVA and Tukey HSD post-hoc tests at a significance level of 0.05. RESULTS. The highest strength value was seen in metal-pressable (MP) group, whereas zirconia-pressable (ZP) group exhibited the lowest one. Moreover, group MP showed significantly higher fracture loads than group ZP (P=.015) and zirconia-layering (ZL) (P=.038) group. No significant difference in fracture strength was detected between groups MP and ML, and groups ZP and ZL (P>.05). Predominant fracture types were cohesive for metal groups and adhesive for zirconia groups. CONCLUSION. Fracture strength of a restoration with a metal or a zirconia framework was independent of the veneering techniques. However, the pressing technique over metal frameworks resisted significantly higher fracture loads than zirconia frameworks.

Preparation of a Composite of Sulfated Zirconia/Metal Organic Framework and its Application in Esterification Reaction

  • A porous metal-organic framework (MOF), MIL-101, was synthesized in the presence of sulfated zirconia (SZ) to produce acidic SZ/MIL-101 composites for the first time. The composites were characterized with XRD, nitrogen adsorption, FT-IR, scanning electron microscope, chemical analysis and so on. The composites (SZ/MIL-101s) were successfully applied in a liquid-phase esterification for a high yield of ester. This catalytic result of SZ/MIL-101, compared with that of pure SZ or MIL-101 (showing a negligible yield of ester), suggests that the SZ in the composite is highly active in the acid catalysis probably because of the well-dispersed active species of SZ. Moreover, the esterification is catalyzed in heterogeneous mode as confirmed by negligible esterification after filtration of the catalyst. Finally, microwaves can be efficiently applied both in the synthesis of the composites and the esterification reaction to accelerate the two processes of synthesis and esterification by about 5 times.

The effects of repetitive firing processes on the optical, thermal, and phase formation changes of zirconia

  • PURPOSE. The aim of this study was to investigate the effect of different numbers of heat treatments applied to superstructure porcelain on optical, thermal, and phase formation properties of zirconia. MATERIALS AND METHODS. Forty zirconia specimens were prepared in the form of rectangular prism. Specimens were divided into four groups (n = 10) according to the number of firing at heating values of porcelain. Color differences and translucency parameter were measured, and X-ray diffraction (XRD) analysis and differential scanning calorimetry (DSC) were performed. Data were analyzed with analysis of variance (ANOVA). RESULTS. There were no statistically significant differences in ∆E, TP, L, a, and b value changes of the zirconia specimens as a result of repetitive firing processes (P>.05). CONCLUSION. Although additional firing processes up to 4 increase peak density in thermal analysis, additional firing processes up to 4 times can be applied safely as they do not result in a change in color and phase character of zircon frameworks.
