본 연구는 저소득 여성노인의 건강관련 삶의 질에 영향을 미치는 요인을 전기노인(65-69세), 중기노인(70-79세), 후기노인(80세 이상)으로 연령별로 구분하여 파악하고자 수행되었다. 자료는 B광역시의 5개 보건소의 맞춤형방문 보건사업에 등록된 기초생활수급자 여성 노인 383명으로부터 2010년 9월부터 11월까지 설문지를 통하여 수집되었으며 다중회귀분석으로 분석하였다. 저소득 여성노인의 건강관련 삶의 질, 생활 만족도, 인지기능, 허약수준은 연령별로 차이가 있었다. 저소득 여성노인의 건강관련 삶의 질에 영향을 미치는 요인은 연령별로 차이가 있었으며 생활 만족도, 인지기능, 허약수준, 주관적 건강수준, 만성질환개수를 포함한 모형은 전기노인의 건강관련 삶의 질을 42.8%, 중기노인의 28.9%, 후기노인의 31.5%를 설명하였다. 허약수준과 생활만족도는 저소득 여성노인의 전체 연령에 있어 건강관련 삶의 질에 영향을 미치는 요인으로 나타났다. 그러므로 저소득 노인여성의 건강관련 삶의 질 개선을 위한 건강증진 프로그램은 연령별 차이를 고려하여 개발되어 적용될 필요가 있다.
본 연구는 국민건강영양조사 제7기 제3년도(2018년)원시자료를 바탕으로 한국전기-후기노인의 안전 의식 및 건강요인이 손상 및 손상기전에 미치는 영향요인을 알아보고자 하였다. 연구 대상자는 1년간 손상발생 여부에 응답한 전-후기노인 1608명을 최종 대상자로 하였다. 분석은 빈도분석, 교차분석, 다중 로지스틱회귀분석을 실시하였다. 연구결과는 노인은 성별, 결혼상태, 안전의식, 건강요인이 손상발생률과 관련성이 있으며, 전기노인은 여성일수록, 결혼상태가 별거 및 이혼 등의 상태일 때, 안전의식이 낮을수록, 2주간 몸의 불편감, 1달간의 와병여부, 1년간의 입원여부, 2주간의 외래 진료여부, 연간 미충족 의료서비스 경험이 있을수록 손상 발생율이 높았다. 후기노인도 여성, 별거 및 이혼 상태, 안전의식 수준이 낮고, 와병 여부, 입원여부, 연간 미충족 의료서비스 경험자일수록 손상발생률이 높았다. 따라서 손상사고 발생율을 낮추는 예방 교육으로 사회활동이 많은 전기노인은 교통수단 이용증가에 따른 맞춤형 안전교육, 후기노인은 건강행태에 따른 맞춤형 안전의식 고취 교육을 선별적으로 강화하고, 다양한 운송수단에 대한 음주운행 시 법적 처벌 강화, 교육의 통합 시스템적인 접근이 요구된다 하겠다.
Objectives: This study aimed to identify the relationships of oral and systemic health-related characteristics with health-related quality of life (EQ-5D) in the elderly, and factors related to quality of life were compared according to age subgroups classified as either younger (young-old) or older (old-old) than 75 years of age. Methods: Data acquired by the Sixth Korean National Health and Nutrition Examination Survey(KNHANES) from 2013 to 2015 were used, and the research target was 3,124 people aged 65 years or older. A complex samples general linear model was used to identify health-related quality of life factors. Results: Education, economic activity, depression, stress, regular walking, self-rated oral health, pronunciation problems, and unmet dental care had significant effects on quality of life in both young-old and old-old participants. Marriage, income, number of systemic diseases, sleeping, and chewing inconveniences were significant factors for the young-old but not in the old-old participants. Instead, obesity and drinking were identified as significant factors in the old-old participants. Conclusions: We reaffirmed that factors affecting health-related quality of life in older adults differed by age group. We also confirmed the impact of oral health-related characteristics on this quality. Therefore, to improve quality of life for older adults, it would be efficient to divide groups by age and develop and implement programs that take relevant factors into consideration.
This study investigated the effects of vitamin E supplementation on immune responses and antioxidant status in healthy Korean old and young women. Blood samples were obtained from 15 healthy old women (over 60 years old) and from 15 healthy young women(20 years old) before and 4 weeks after vitamin E( tocopherol acetate) supplementation(400IU/day). Daily nutrient intakes were calculated, and plasma vitamin E concentration, numbers and percentages of white blood cell and their subpopulation, percentages of lymphocytes and subpopulation, NK cell percentages, plasma immunoglobulin A, G, M and C3 concentration, proliferation of PMN with mitogen were measured. Also plasma TBARS concentration and radical scavenger activity of erythrocytes were investigated. Plasma vitamin E concentrations were significantly increased after supplementation in both groups. In elderly women, vitamin E supplementation restored the per centages of neutrophils, lymphocytes, and eosinophils which had been out of normal ranges before supple mentation. And after vitamin E supplementation, helper T cell percentages significantly increased in elderly. Plasma immunoglobulin and complement C3 concentrations were not affected by vitamin E supplementation in both groups. PMN proliferations with mitogen were significantly lower in old women than in young women, and there was no effect of vitamin E supplementation. Vitamin E supplementation significantly decreased plasma TBARS concentrations in old and young women. RSA of erythrocytes was increased in both groups, but the statistical significant was only found in young women group. Therefore, these results suggest that the moderate vitamin E supplementation in old women improves immune responses, especially nonspecific immunity and cell mediated immunity, via protection of oxidant stress.
This study is to research the use and the demand of the elderly generation that pursue economic ability and active lifestyle. The composition of the meals were "homemade meals" 65.3%, "homemade & convenience meals" 26.7% overall. For the "never miss a meal" category, 70-74 years old was 48.3%, over 80 years old was 60.0%. The elderly under 70 years old showed a high frequency in the use of alternative meals (p < 0.05) and the elderly with higher education used the alternative meals more frequently (p < 0.01). The rice-cakes, bread, tuna cans, Korean side-dishes, beverages, meat, cup-noodles and seafood as alternative meals are in the order of most frequency. Also, 96.0%, 59.0% and 46.5% of the elderly had experience in using "Ready to eat (RTE)", "Ready to heat (RTH)" and "Ready to cook (RTC)". The oldest group surveyed, the elderly at the age of 80 years old and above showed lower preference for "completely cooked state" but showed highest results in the "half-cooked state". This shows the difference between people's perception and the behavior that showed high frequency of 96.0% in the experience with "RTE". This can be inferred that the elderly needs the development of convenience food that is moderately packaged according to the newer lifestyle where the elderly live independent from their children. Moreover, there is a higher demand in the healthy convenience food, which means that there is a need for development in the quality of the convenience food that will allow more faith in convenience food by the elderly.
Background: As the population rapidly ages, older adults are increasingly likely to experience mobility problems. This study aims to explore the characteristics related to an elderly person's willingness to live in a nursing home if they have mobility problems Methods: This study analyzed data from 9,917 older adults (5,976 young-old and 3,941 old-old) obtained from the 2020 National Survey of Older Koreans. The dependent variable was the intended place of residence for older adults with mobility problems. Independent variables included various characteristics: (1) sociodemographic and social support, (2) health and functional status, and (3) residential environment. Rao-Scott chi-square tests and survey logistic regression analyses were performed for the young-old and old-old, respectively. Results: The intention to live in a nursing home was significantly different between the young-old (30.4%) and the old-old (34.7%) (p=0.009). According to fully adjusted multivariable analyses, for the young-old, the odds ratio of intending to live in a nursing home was significantly higher in social security benefit recipients (1.45; 95% confidence interval [CI], 1.06-1.97) compared to other individuals. The odds ratio was higher in unmarried (divorced, separated, widowed, or never-married) individuals for both young-old (1.41; 95% CI, 1.22-1.63) and old-old (1.34; 95% CI, 1.09-1.65) age groups, compared to their respective married counterparts. Conclusion: The results of this study suggest that in an aging society, health and social policies should be designed considering the different characteristics of the elderly to improve their health, function, and quality of life.
This study aimed to compare and evaluate nutritive conditions of the female elderly (2,893 people) based on a national health nutrition survey and health-related factors influencing nutritional intake. The results are as follows.- Average age of all subjects was $72.12{\pm}5.2$ old. BMI ($24.48{\pm}3.3kg/m^2$) and waist size ($83.9{\pm}9.5cm$) of urban elderly was larger ($23.74{\pm}3.3kg/m^2$, $82.28{\pm}9.9cm$) than that of rural elderly. MAR[4] and MAR[10] were also higher in urban elderly than rural elderly. The results of multiple regression analysis showed that factors influencing poor nutrition of elderly subjects were increasing age, smoking, and self-rated health. Old-old and current smokers were associated with a higher likelihood of poor nutrition (OR 1.82, 95%CI 1.43~2.30) and (OR 3.40, 95%CI 2.17~5.33) elderly female subjects in urban areas. Smoking (OR 2.29, 95%CI 1.33~3.95), poor self-rated health (OR 1.55, 95%CI 1.11~2.16), over nine hours of sleep per nighting (OR 1.72, 95%CI 1.17~2.53) and stress (OR 1.46, 95%CI 1.10~1.92) of elderly female subjects in rural areas were associated with higher likelihood of a poor nutrition. In conclusion, rural elderly are more influenced by poor health practice behaviors (self-rated health, sleeping stress) and psychological factors. Nutrition status of the rural elderly is worse than that of the urban elderly. To reduce nutritional risk of Korean elderly, especially rural elderly, active nutrition arbitration should include psychological environmental factors.
In an attempt to figure out the relationship between zinc status and taste acuity of old and young women, dietary zinc intake, urinary zinc excretion, and taste acuity were determined for 118 women. Zinc intake was measured by 2-day food records and food frequency method. Urinary zinc excretion was measured from urine samples collected for twenty four hours. Body fat, lean body mass (LBM), and total body water were measured by bio-impedence. Average dietary zinc intake by food record was 4.15$\pm$1.33mg (=35% if Korean RDA) for the old women and 5.41$\pm$2.76mg (=25% of RDA) for young women. When zinc intake was measured by a frequency method, the average intakes of the old and young women were 3.5$\pm$1.7mg 4.5$\pm$1.9mg, respectively. It appears that dietary zinc intake of young women was significantly higher than that of the old women. Average urinary zinc excretion of the subjects was 0.27$\pm$0.16mg in the elderly and 0.24$\pm$0.13mg in young women, which indicated a marginal zinc status. However, zinc status was not significantly different between old and young women. Correlation analysis indicated that zinc intake and urinary zinc excretion were positively related to BMI and LBM in young women. The old women (m=49) showed significantly higher taste detection thresholds than young subjects (n=47) for both sweet and salty tastes. Recognition thresholds for sodium chloride and sucrose were not significantly different between old and young women. The lower the taste thresholds for salty taste, the higher the average dietary zinc intake. However, taste perception concentration was not related to the urinary zinc excretion level.
The basic premise of this study was that the elderly family would be one user type to live in apartment houses. This means that apartment houses should be designed by considering the elderly family as well as general young families. The purpose of this study was to carry out POE of apartment houses by the elderly resident and to suggest the design direction of apartment houses from the elderly's point of view. The subject was 150 elderly residents who lived in apartment houses in Jangyu new urban area, Kyungnam. The individual interview was done for this POE by six interviewers. They asked about how degree they were satisfied with 30 design elements of apartment houses. Those consisted of 8 elements related to outdoor environments, 16 ones to indoor environments, and 6 design evaluative elements for the elderly housing. The conclusions were as followings; 1) In general, the elderly subject was very satisfied with their apartment design. However, more considerations of the design evaluative elements for the elderly were needed in order to be apartment houses for the old generation. 2) Natural facilities such as small parks and walks, and local amenities should be arranged systematically to be used easily by the old residents. 3) The design solutions for reducing noise should be added. 4) The same quality of housing environment should be provided in all sizes of housing units.
주관적 기대여명(Subjective Life Expectancy)이란 자신의 수명에 대한 평가로, 주관적 인지평가에 그치지 않고, 건강한 삶을 위해 건강증진, 사회적 활동, 경제적 활동 등에서 실천적 행위를 하게 하는 기전으로 알려져 있다. 본 연구는 연령집단별 노인의 주관적 기대여명에 영향을 미치는 요인을 일반적 요인과 개입요인으로 나누어 파악하여 비교해보는 것에 목적이 있다. 연구대상자는 제 6차 고령화연구패널(KLoSA)을 이용하여 연령집단별로 연소노인(65세-74세) 2,106명, 중고령노인(75-84세) 1,803명, 초고령노인(85세 이상) 574명으로 총 4,483명이다. 일반적 요인은 통제변수로써 인구사회학적요인, 건강상태요인을, 개입요인은 독립변수로써 건강증진행위, 공적돌봄인지여부, 공식적 사회활동 참여, 일자리 및 소득보장 등에 해당하는 9가지의 변수를 설정하여 카이제곱검정, ANOVA, t-test, 위계적 다중 회귀분석을 실시하였다. 그 결과 주관적 건강상태와 기초연금은 모든 연령집단의 주관적 기대여명에 영향을 미쳤지만, 그 외의 변수들은 집단별로 차이가 있었다. 공식적 사회활동 참여, 일자리 및 소득보장요인은 연소노인과 중고령노인 집단에서 유의했고, 건강증진행위요인은 연소노인에서만 유의했다. 중고령노인은 다른 연령집단에 비해 개입요인보다 일반적 요인에서 유의미한 결과가 치중된 것으로 나타났다. 본 연구의 결과는 연령집단별 노인의 주관적 기대여명과 남은 노후를 잘 보내기 위한 실천적이고 개입 가능한 요인 탐색에 기초자료를 제공할 수 있을 것이다.
