Impact of Multiple Cardiovascular Risk Factors on the Carotid Intima-media Thickness in Young Adults: The Kangwha Study

  • Objectives: Although risk factors for coronary artery disease are also associated with increased carotid intima-media thickness (IMT), there is little information available on the asymptomatic, young adult population. We examined the association between multiple cardiovascular risk factors and the common carotid IMT in 280 young Korean adults. Methods: The data used for this study was obtained from 280 subjects (130 men and 150 women) aged 25 years who participated in the Kangwha Study follow-up examination in 2005. We measured cardiovascular risk factors, including anthropometries, blood pressure, blood chemistry, carotid ultrasonography, and reviewed questionnaires on health behaviors. Risk factors were defined as values above the sex-specific 75th percentile of systolic blood pressure, body mass index, total cholesterol/ high-density lipoprotein cholesterol ratio, fasting blood glucose and smoking status. Results: The mean carotid IMT${\pm}$standard deviation observed was $0.683{\pm}0.079mm$ in men and $0.678{\pm}0.067mm$ in women (p=0.567) and the evidence of plaque was not observed in any individuals. Mean carotid IMT increased with an increasing number of risk factors(p for trend <0.001) and carotid IMT values were 0.665 mm, 0.674 mm, 0.686 mm, 0.702 mm, and 0.748 mm for 0, 1, 2, 3, and 4 to 5 risk factors, respectively. The odds ratio for having the top quartile carotid IMT in men with 3 or more risk factors versus 0-2 risk factors was 5.09 (95% CI, 2.05-12.64). Conclusions: Current findings indicate the need for prevention and control of cardiovascular risk factors in young adults and more focus on those with multiple cardiovascular risk factors.

  • Surgery to increase cerebral blood flow is one of the treatment methods of cerebral infarction. However, invasive methods, such as surgery, may result in postoperative complications or side effects. In order to supplement this invasive method, non-invasive devices have been introduced that use human blood pressure to pressurize the extremities to increase cerebral blood flow. However, the problem of poor speed and accuracy was raised. In this paper, the perfusion index of each arm was measured by applying pressure to both arms using Blood Oxygen Level Sensor to improve the accuracy of measurement and measurement time. The pressure applied to the arm by 75% of the moment when it falls to the leg and the pressure calculated by using the pressure value obtained from the arm. Like the existing blood pressure measuring cerebral blood flow increasing device, the blood flow can be increased by more than 20% and the measurement time can be shortened, so that it can be selectively used for the patient with cerebral infarction.

Evaluation of Appropriateness of Blood Order Based on Crossmatching to Transfusion Ratio in Elective Surgery (선택적 수술에서 교차시험/수혈비를 활용한 혈액요청의 적정성 평가에 관한 연구)

  • The over-preparation of blood for elective surgery causes some problems such as returning of blood, inefficient task and loss of reagents. In recent blood transfusion practice, there has been changing patterns of blood use in elective surgery as various side effects of transfusion have become known and operation techniques are developed. This study was performed to evaluate the optimal utilization of blood for various elective and/or emergency surgery. We surveyed the quantity of blood transmitted to wards, returned to blood bank, and the number of crossmatching test done for the elective surgery of the patients at the D. University Hospital, the tertiary teaching hospital from August 1, 2001 to October 31, 2001. Crossmatched to transfusion ratio (C/T ratio) was calculated, in which C means the unit of accomplished crossmatching test and T means the unit of transfused blood component. The unit of transfused blood was analysed in terms of issuing time, sex, age, clinical department, and blood component type. The usage of bloods for the emergency surgery was also analyzed. In our study, C/T ratio was highest on Monday (1.54) and Tuesday (1.53), and higher in female patients(1.54) than in male patients (1.32). No significant relationship was observed between age groups. Among clinical departments, thoracic and cardiovascular surgery showed the highest C/T ratio (1.54). From above results, we could be sure that the management of transfusion practice was relatively appropriate in recent years, although the entire introduction of type and screen (T&S) method was desirable. If the T&S method is performed, the C/T ratio would be almost 1.00 and it would also relieve the duty of blood bank.

It also help circulate of the blood flow and vigor. The study expects the related researches to improve the various treatments through this treatment. The researcher encountered many problems with the lack of concerned materials and former studies but expects this study to be a study to retard aging the skin and prevent the diseases through the study of the blood vessel pressure massage.

A Study on Blood Lipid Levels, Nutrient Intakes, and Oxidation and Inflammation Markers of Overweight and Obese Adults according to Blood Cholesterol Levels in Korea (과체중 이상 성인에서 혈중 콜레스테롤 수준에 따른 영양소 섭취량, 혈중 산화 및 염증 관련 지표에 관한 연구)

  • This study was designed to investigate the relationships among blood lipid levels, nutrient intakes, oxidation and inflammation markers of overweight adults(23$\leq$BMI<25) and obese(BMI$\geq$25) in Korea. The subjects were classified as control, borderline hyperlipidemia. and hyperlipidemia groups based on The Korean Guidelines of Hyperlipidemia Treatment for the Prevention of Atherosclerosis. The study was conducted through questionnaires, anthropometric checkups, 2-days of 24 hr recalls, and blood biomarker analyses. Systolic blood pressure(SBP) was significantly increased in the hyperlipidemia group(p=0.0464). Intakes of nutrients were not significantly different among the three groups. Blood oxidized-LDL levels were significantly increased in the hyperlipidemia group(p<0.0001). Blood triglyceride(TG) levels were positively associated with BMI(p=0.0498), SBP(p=0.0158), and diastolic blood pressure(DBP; p=0.0076). Blood total cholesterol levels were positively associated with SBP(p=0.0005), and blood HDL-cholesterol levels were negatively associated with body fat (p=0.0408). Blood LDL-cholesterol levels were negatively associated with height(p=0.0207), and blood VLDL-cholesterol levels were positively associated with SBP(p=0.0011) and DBP(p=0.0490). Intakes of protein(p=0.0257) and dietary fiber (p=0.0094) were positively associated with blood HDL-cholesterol levels. Frap levels were positively associated with TG levels(p=0.0001) and VLDL-cholesterol levels(p=0.0077). Oxidized-LDL levels were positively associated with LDL-cholesterol levels(p=0.0135). These results suggest that oxidation and inflammation markers may be related to hypercholesterolemia progress, and dietary fiber intake may play a role in preventing hyperlipidemia in overweight and obese adults.

A Multicenter Survey on the Current Status of Pediatric Blood Cultures in Korea (소아 대상 혈액배양검사 현황 파악을 위한 국내 다기관 설문조사)

  • Purpose: Blood culture is an essential diagnostic tool and requires clear indications, proper techniques, and quality control. We aimed to investigate whether blood cultures in children are appropriate for indications, are performed correctly, and receive proper quality control. Methods: We conducted an online survey targeting pediatric infectious diseases (ID) specialists working in general hospitals and neonatologists (Neo) working at hospitals operating a neonatal intensive care unit in Korea. Results: Approximately 81.1% (30/37) of pediatric ID specialists and 72.2% (52/72) of Neo responded to the survey. Some of the respondents (33.3% of ID and 59.6% of Neo) performed blood culture as a regular test irrespective of the indication. Approximately 40% of ID and 65.4% of Neo ordered only one set of blood culture in patients suspected with bacteremia. The most commonly used disinfectant for skin preparation was povidone-iodine, while the skin preparation method varied by institution. Approximately two-thirds of the institutions were monitoring the blood culture contamination rate, whereas relatively few provided staff with feedback on that rate. In addition, less than half of the institutions were providing regular staff training on blood culture (40% of ID and 28.8% of Neo). Conclusions: The indication and methods of blood culture for children varied according to institution, and few hospitals exert effort in improving the quality of blood culture. Institutions have to strive constantly toward improvement of blood culture quality and evidence-based recommendations for pediatric blood cultures should be standardized.

A Comparative Study on Urinary Ca Excretion in Young and Middle-Aged Korean Women (한국 젊은 성인 여성과 중년 여성의 소변중 Ca 배설과 이에 영향을 미치는 요인분석)

  • A study was conducted to compare urinary Ca excretion and factors influencing urinary Ca excretion in 30 young and 62 middle-aged Korean women. Mean daily intake levels of total protein and P were significantly higher in middle-aged women but Ca and animal protein intake levels of the two groups were similar. The average percentage of daily Ca intake from milk and milk products in young women was 45% while in middle-aged women it was about 24% The frequency of milk consumption was inversely correlated with blood pressures of the subjects. Mean 24-hour urinary calcium excretion in young and middle-aged women were 163.7mg and 174.9mg respectively. The difference was not significant. Menopause of the mid-dle-aged women did not affect urinary calcium levels. However the proportion of subjects with more than 250mg of Ca in 24-hour urine tended to be higher in middle-aged group Factors significnatly correlated with urinary Ca excretion of subjects were systolic and diastolic The study verifies the need for more systematic studies on Ca requirements and the interrelation-ship among Ca and na metabolism blood pressures and bone loss in the middle-aged Korean.

Sarcopenic Obesity Frequency and Associated Risk Factors in Young Korean Women: A Comprehensive Cross-Sectional Analysis

  • PURPOSE: Sarcopenic obesity (SO) is a clinical condition that combines sarcopenia and obesity. This study examined the frequency of SO in young Korean females between 20 and 29 years of age. METHODS: The study involved 1,000 participants. The height, weight, body mass index (BMI), waist circumference, skeletal muscle mass index, fasting glucose, triglyceride, total cholesterol levels, systolic and diastolic blood pressure, alcohol consumption, and smoking status were the research variables. The skeletal muscle mass index was calculated as appendicular skeletal muscle mass (ASM) divided by the BMI. The ASM was assessed using dual X-ray absorptiometry. Complex sampling analysis and multiple logistic regression were used for analysis. RESULTS: A .74(.30-1.80) frequency of SO was observed. The statistically significant risk factors in females were height, weight, BMI, waist circumference, skeletal muscle mass index, total cholesterol, systolic blood pressure, and diastolic blood pressure (p < .05). CONCLUSION: Young Korean adults with SO have a .74(.30-1.80) frequency of occurrence that is linked to specific risk factors. Hence, primary care clinicians and health care professionals should consider these factors when patients require a referral for early detection and treatment. Healthcare professionals and clinicians can identify potential SO patients by acknowledging these risk factors.

  • In order to increase the solubility and antibacterial activity of ampicillin, the lysinomethylene ampicillin trihydrate was synthesized. The blood concentrations of ampicillin trihydrate and synthetic lysinomethylene ampicillin trihydrate were determined in 10 healthy male volunteers by using large plate method with Sarcina lutea ATCC 9341. The mean blood concentration of synthetic lysinomethylene ampicillin trihydrate was higher ca. 1.3 times than that of ampicillin trihydrate in 10 test subjects after oral administration.
