  • Inpatients are mostly occupied in bed with restricted activity, nearly all patient populations are at risk for the occurrence of skeletal muscle atrophy due to decreased level of activity. Restriction of mobility is far greater in pediatric patients compared with adult patients since almost all the activities of daily living is performed by parents or caregivers. It could be assumed that pediatric patients are more vulnerable to skeletal muscle atrophy than adult patients, however, there have been no attempts to reduce the atrophy of developing muscle. Therefore it is important to determine the effect of exercise in developing muscle during decreased activity. The purpose of this study was to determine the effect of periodic weight support during hindlimb suspension on the mass and cross-sectional area of Type I and II fibers in developing soleus(Type I ) muscle. To examine the effectiveness of periodic weight support activity in maintaining mass and fiber size. the hindlimb of young female Wistar rats was suspended(HS) and half of these rats walked on a treadmill for 45min / day(15min every 4h) at 5m / min at a 15 grade(HS-WS). After 7days of hindlimb suspension, soleus wet weight was 28. 57% smaller and relative soleus weight was 28. 21% smaller in comparison with con-trol rats (p〈0.05) Soleus wet weight and relative soleus weight increased by 67.72% and 71.43% each with periodic weight support activity during hindlimb suspension (p〈0.01, p〈0.005), moreover soleus wet weight and relative soleus weight of the HS -WS rats were greater than those of the control group. No change was observed in fiber type percentage of the developing soleus muscle after 1 week of hindlimb suspension plus weight support activity. Type I and II fiber cross-sectional areas of the developing soleus muscle were 50.45% and 43.39% lower in the HS group than in the control group (p〈0.0001), type I and II fiber cross-sectional areas of the developing soleus were 24.49% and 29.93% greater in the HS - WS group than in the HS rats (p〈0.0001), whereas Type I and II fiber cross-sectional areas of HS - WS group were less than those of the control group, The results suggest that periodic weight support activity can ameliorate developing soleus muscle atrophy induced by hindlimb suspension, even in type II fibers that would not have been expected to be recruited by this type of neuromuscular demand. Clinical experimental study is needed to deter-mine the effect of periodic weight bearing exercise on developing atrophied leg muscle based on these results.

  • Introduction: In this study, we compared sleep structure, EEG characteristic of pediatric obstructive sleep apnea (OSA) and normal controls which were matched in sex and age. Methods: Fifteen children (male:female=4:11) who complained snoring and were suspected to have sleep apnea and their age and sex matched normal controls (male:female=5:10) have been done nocturnal polysomnography (NPSG). Sleep parameters, sleep apnea variables and relative spectral components of EEG from NPSG have been compared between both groups. Results: Pediatric OSA group were distinguished from normal controls in terms of apnea index, respiratory disturbance index and nadir of oxyhemoglobulin desaturation. Pediatric OSA group showed increased percent of sleep stage 1, decreased rapid eye movement sleep percent and increased delta power in O1 EEG channel. However other sleep parameters and spectral powers were not different between two groups. Conclusion: In pediatric OSA group, sleep structure parameter disruption may be not prominent as the previous studies for adult OSA group because of including mild OSA data in diagnostic criteria. In addition, EEG changes might not be distinct due to low arousal index compared to adult OSA patients. We can observe general characteristics and particularity of pediatric OSA through this study.

  • Prevotella intermedia has been implicated as a potent pathogen in many kinds of periodontal, pulpal and periapical diseases. However, it has been isolated from periodontally healthy adults and from edentulous children as well. The intraspecies heterogeneity of Prevotella intermedia has been demonstrated in early studies and finally Shah & Gharbia confirmed the existence of 2 DNA homology groups and proposed dividing Prevotella intermedia into 2 species, Prevotella intermedia and Prevotella nigrescens. This study was designed to examine the frequency of Prevotella intermedia and Prevotella nigrescens in diseased periodontal pockets and healthy gingival sulcus of Korean people by PCR based on 16s ribosomal DNA sequence. One hundred adults who had adult periodontitis but not taken any periodontal treatment or antibiotics during previous 6 months and 50 adults who had healthy periodontal tissue were selected for this study. The sulcular fluid was collected into VMGA by sterilized paper point and diluted to 1,000 times in anaerobic chamber. $100{\mu}{\ell}$ of sample was cultured in $37^{\circ}C$ for 10 days. Among the bacterial colonies, BPB were selected and cultured in BHI broth and then Prevotella intermedia was identified through Gram staining and biochemical test. Identified Prevotella intermedia was cultured again and centrifuged. DNA was extracted from the pellet using several reagents. PCR was performed by previously designed primer. The results were followed. 1. BPB were isolated from 39 of 100 samples of diseased periodontal pockets(39%). 2. Prevotella intermedia was identified from 24 of 39 BPB samples. 3. Among 24 Prevotella intermedia, 21 were confirmed as Prevotella inter - media(87.5) and 2 were confirmed as Prevotella nigrescens(8.33%). 4. BPB were isolated from 9 of 50 samples of periodontally healthy patients. Among them only two were identified as Prevotella intermedia, that is, one was confirmed as Prevotella intermedia and the other was Prevotella nigrescens.

  • Despite improvements in vaccine coverage, a resurgence of measles has been reported, especially in the infant and adult populations in recent years. We conducted a systematic review of seroprevalence studies conducted in the Western Pacific Region (WPR) to provide insights into seropositivity trends in different countries. This systematic review aimed to collect data from all available measles seroprevalence studies to characterize the differences in population immunity against measles in different countries. We searched the online databases PubMed and Embase to identify: 1) observational studies that investigated seroprevalence in all age groups, and 2) results reported as antibody levels. The following variables were extracted from different study arms: paper identification (title, first author, publication year), inclusion and exclusion criteria, study site, age of subjects, number of subjects, country/area, population, methods, and seropositivity (%). The search yielded a total of 69 studies included in the review. Among the 1-6-year-old group, seropositivity remained relatively high, at 81-100% in China, 86-94% in Korea, and 77-91% in Australia. In adolescents aged 7-18-years old, seropositivity was relatively constant in China and Australia over time; however, a decreasing trend was noted in Korea in 2011 (66%), 2014 (69%), and 2014 (50%) in this age group. A similar downward trend was observed among Korean adults aged 19-39 years in 2011 (74%), 2019 (71%), and 2019 (64%). Children are likely to be protected by universal vaccination programs in WPR countries and regions. However, susceptible individuals with waned immunity may be present among the adult population.

  • Working towards the compression of morbidity, we attempted to find the way to contribute to desirable aging and/or healthy old age. By looking into the factors affecting nutrient intake in middle-aged adults, we sought to find ways to improve their health status, Two hundred and fifty middle-aged adults from 40 to 60 years of age residing in Seoul and its vicinity were recruited for the survey. Subjects were interviewed for general characteristics, dietary habits lifestyle, status and food intake by a trained interviewer. Two -day food record was included in addition to the 1 -day 24 hr recall. Date were analyzed for the relationship among nutrient intake, health status, socio-economic status dietary habits and lifestyle using a statistical program (SAS) Although the mean energy intake of the subjects did not reach the recommenced dietary allowances(RDA), mean protein intake was well above the RAD for both sex groups(111.2% and 129.1% for men and women, respectively) The caloric contribution of fat was 15 to 20 % of total energy intake among different groups. Riboflavin was the most deficient nutrient and its mean intake reached only 71.0% of RDA for mean This seemed to be due to the lower intake of milk and dairy products among middle-aged Koreans who are more accustomed to traditional meals. Income and exercise habit were the most important factors influencing the nutrient intake of middle-age adults. Also, BMI, smoking habit, level of education regularity of meal, and hours of sleeping influenced the nutrient intake, In conclusion, middle-age adults appeared to have similar problems with the elderly in terms of nutrient intake and this age group should to included when considering nutritionally vulnerable groups in addition to the elderly and young children. The improvement in the nutrition status of middle-age adults is even more important because that could lead into the better health the elderly.

  • A variety of wireless devices are commercially available now. Most of studies, however, have been directed to the biological effects of mobile-phone EMF. In this paper, dosimetric analysis for wireless devices of head-mounted display type and a wristwatch type were made to investigate possible biological effects of these devices. SAR(Specific Absorption Rate) distributions were calculated using FDTD(Finite Difference Time Domain) method, for adult human models such as standard Korean human model and VHP(Visible Human Project) model, as well as scaled child models. Measurements were also performed for SAM phantom wearing a simplified prototype for a wireless device for validation of the simulation results. It has been found that children are more vulnerable to such exposure, and these devices could cause some biological effects even for relatively lower power compared to conventional mobile pones.

  • Ross procedure is an ideal operative modality for diseased aortic valve especially in children, but homogrart for right ventricular outflow tract(RVOT) reconstruction is not easily available in Korea. We tried to perform REV procedure for RVOT reconstruction in 10 young piglets(15.3 $\pm$ 1.3 kg) In an attempt to e clude the use of homograft in Ross operation. 3 of them survived after operation and raised till adult pig(about 70 kg), then examined their pulmonary arteries and hearts. Without any stenotic residues in the great arteries, we only found the deformed monocusp patch with severe calcification, which deprived the adequate valve function, but kept the pig growing normally We are sure that this operative modality (REV + Ross procedure) could be extendedly applied to the diseased human aortic valve, but we need to develope the anti-calcification method for the heterograft patch.

  • Although the incidence of tuberculosis has been reduced recently because of the advance of anti-tuberculosis chemotherapy, improvements of public health, and early diagnosis, it is still high in developing countries. Intestinal tuberculosis can occur at any age, but young adults and female are more affected. Early diagnosis of childhood intestinal tuberculosis is difficult, largely because of vague symptoms and nonspecific signs. Recently, tuberculosis of the larynx combined with pulmonary tuberculosis has become quite a rare disease since anti-tuberculous chemotherapy and preventive activities are prevalent. Laryngeal tuberculosis, while relatively well recognized in the adult population, is a rare disease in chidren and adolescents. There are few reports of intestinal tuberculosis combined with endobronchial and laryngeal tuberculosis in children. We have experienced a case of intestinal tuberculosis which concurred with endobronchial and laryngeal tuberculosis in an 18-year-old girl who presented with abdominal pain, diarrhea, weight loss, a brassy cough, and anemia, which was diagnosed by a colonoscopy and bronchoscopy with biopsy.

  • The purpose of this study was to examine the household characteristics, the actual residential environment of the elderly living with their offsprings together in Gwangju. We also evaluated their satisfaction with the needs for remodeling of their houses and the different opinions about those things between the elderly and their off springs. We used comparative and descriptive T-test in statistical analysis. The results were as follows. There were more families living in the detached houses than those living in the apartments. The average size of the houses was 100.8 square meters. The mean residence period was 13.5 years. The sons were usually the legal householders. The residents who experienced remodeling before reassigned the furniture and installed the bathtub, which was designed to be sat on comfortably in a common. A few residents installed safety system such as alarms for incident fire or gas, convenient knobs for handling and faucets for running hot or cold water separately. After the installation of alarms, convenient knobs of windows or door, Customer satisfaction for those things were quite high. But that for the installation of safety bar were the lowest in contrast. The most things the old wanted to change were to rearrange bedrooms and bathrooms on the first floor and to decorate them in westernized styles. They didn't need to set up safety bars or shower instrument for modulating heights. The second generation wanted to install the blinds or curtains more than the third generations.

  • Background: Sevoflurane, a rotatively new inhalational anesthetic. has non-pungent odor and is less reluctant to pediatric patients. The purpose of this study is to examine the feasibility of sevoflurane in inhalational sedation instead of the nitrous oxide for short and simple dental treatments in pediatric patients. Patients and Methods: Fifteen healthy children, whose dental treatment was abandoned due to their little or no cooperation, were selected with their caregivers' written permission. Deep sedation was induced and maintained with oxygen and 1-5% sevoflufane via specially designed nasal mask. Blood pressure, heart rate, oxygen saturation, and electrocardiogram were monitored at 3-nin interval. A dental anesthesiologist, who was independent of dental treatments, was wholly responsible for the sedation procedure. Post-sedation complications and operator's and caregiver's acceptability of this type of inhalational sedation were also investigated. Results: The systolic and diastolic blood pressure, heart rate, and oxygen saturation was significantly depressed during the deep sedation using sevoflurane (P < 0.05). No severe post-sedation complications were found, however, bradycardia was reported in 3 patients. Almost all the operators and caregivers answered that they would adapt this sedation procedure again if possible. Conclusion: In this study, inhalational deep sedation using sevoflurane for dental treatments was found to be very useful. Furthermore, the application of sevoflurane to conscious sedation for pediatric and adult dental patients should be added.
