An Analysis of Prognostic Factors Affecting the Outcome of Radiation Therapy for Nasopharyngeal Carcinoma (비인강암의 방사선치료 곁과 및 생존율에 관한 예후인자 분석)

  • Purpose: This retrospective study was conduced to analyze the treatment results and to evaluate the prognostic factors affecting the survival of nasopharyngeal carcinoma patients. Materials and Methods: From 1987 to 2002, we analyzed 43 patients who had nasopharyngeal carcinomas that were histologically confirmed and who had also completed the planned radiation therapy course at Keimyung University Dongsan Medical Center According to the 6th edition of American Joint Committee on Cancer staging system, 12 patients ($27.9\%$) were at Stage 11, 13 ($30.2\%$) were at Stage III and 18 ($41.9\%$) were at Stage IV Histopathologically, there were 15 ($34.9\%$) squamous cell carcinomas, 8 ($18.5\%$) nonkeratinizing carcinomas, 17 ($39.5\%$) undifferentiated carcinomas, and 3 ($7.0\%$) lymphoepitheliomas. Among the total 43 patients, 31 patients ($72.1\%$) were treated with only radiation therapy. Neoadjuvant chemotherapy was peformed on 7 patients ($16.3\%$) and concurrent chemoradiotherapy was performed on S patients ($11.6\%$). Cisplatin and 5-Fluorouracil were administered to 11 patients for 4 cycles, and Cisplatin and Taxotere were administered to 1 patient for 6 cycles. The range of the total radiation dose delivered to the primary tumor was from 61.2 to 84 Gy (median 70.4 Gy), The follow-up period ranged from 2 to 197 months with median follow-up of 84 months. Results: The local control rate at 6 months after radiation therapy was $90.7\%$. The five year overall survival and disease free survival rates were $50.7\%$ and $48.9\%$, respectively. On the multivariate analysis, the age, T-stage ($T_{1-3}\;vs\;T_4$), N-stage and AJCC stage were the statistically significant prognostic factors affecting survival (p<0.05). The patterns of failure were as follows: local failure only in 3 patients ($7.0\%$), local and systemic failure in 1 patient ($2.3\%$), and distant metastasis only in 11 patients ($25.6\%$). Conclusion: The prognostic factors affecting the outcome of nasopharyngeal carcinoma were age, T-stage (7$T_{1-3}\;vs\;T_4$), N-stage and stage. Because systemic metastasis was the main failure pattern noted for nasopharyngeal carcinoma, systemic chemotherapy is needed to decrease the rate of distant metastasis for nasopharyngeal carcinoma. In audition, research for more effective chemotherapeutical regimens and schedules is also needed.

Presence of Leukemia-maintaining Cells in Differentiation-resistant Fraction of K562 Chronic Myelogenous Leukemia (만성 골수성 백혈병 K562세포의 분화 내성 분획에서 백혈병 유지 세포의 동정)

  • The present study investigated whether leukemia-maintaining cells reside in a differentiation-resistant fraction using a megakaryocytic differentiation model of K562 cells. Treatment with phorbol-12-myristate-13-acetate (PMA) significantly inhibited the colony-forming efficiency of the K562 cells. At a PMA concentration of 1 nM or higher, colony was not formed, but approximately 40% of K562 cells still survived in soft agar. Approximately 70% of colony-forming cells that were isolated following the removal of PMA after exposure to the agent were differentiated after treatment with 10 nM PMA for 3 days. The differentiation rate of the colony-forming cells was gradually increased and reached about 90% 6 weeks after colony isolation, which was comparable to the level of a PMA-treated K562 control. Meanwhile, imatinib-resistant variants from the K562 cells, including K562/R1, K562/R2, and K562/R3 cells, did not show any colony-forming activity, and most imatinib-resistant variants were CD44 positive. After 4 months of culture in drug-free medium, the surface level of CD44 was decreased in comparison with primary imatinib-resistant variants, and a few colonies were formed from K562/R3 cells. In these cells, Bcr-Abl, which was lost in the imatinib-resistant variants, was re-expressed, and the original phenotypes of the K562 cells were partially recovered. These results suggest that leukemia-maintaining cells might reside in a differentiation-resistant population. Differentiation therapy to eliminate leukemia-maintaining cells could be a successful treatment for leukemia if the leukemia-maintaining cells were exposed to a differentiation inducer for a long time and at a high dose.

Genetic Polymorphisms of Blood Proteins and Enzymes in Korean Native Cattle (재래한우의 혈액단백질 및 효소의 유전적 다형현상)

  • This study was conducted to investigate the genetic constitution of blood proteins and enzymes in 238 Korean Native cattle reared at Korean Native Cattle Breeding Center, National Livestock Cooperative Federation. The genetic polymorphisms of post-transferrin-2(pTf-2), transferrin(Tf), post-albumin(pAlb), albumin(Alb), ceruloplamin(Cp), amylase-I(Am-I) and hemoglobin(Hb) were analyzed by using PAGE(polyacrylamide gel electrophoresis) and STAGE(starch gel electrophoresis). The genotypes and gene frequencies were estimated at these loci for each blood proteins and enzymes. The results obtained from this study were summarized as follows : 1. The pTf-2 locus were identified to be genetically controlled by codominant alleles designated pTf-2 F and S, and the distribution of genotypes were 46.22, 46.64 and 7.14% for pTf-2 FF, FS and SS types, and the gene frequencies of the pTf-2 F and S allele were 0,695 and 0.305, respectiveley. 2. The Tf locus were found to be controlled by Tf A, D1, D2 and E alleles, and the distributioin of genotypes were 0.84, 13.87, 13.03, 10.92, 22.27, 12.61, 2.94, 15.51, 6.72 and 1.68% for Tf AA, AD1, AD2, AE, D1D1, D1D2, D1E, D2E and EE types, and the gene frequencies of Tf A, D1, D2 and E were 0.197, 0.430, 0.191 and 0.081, respectively. 3. The pAlb locus were observed to be controlled by two alleles, pAlb F and S, and the distribution of genotypes were 42.86, 33.19 and 23.95% for pAlb FF, FS and SS types, and the gene frequencies were 0.595 and 0.405 for Tf F and S allele, respectively. Also the gene frequencies of Alb was 1.000 of Alb A allele. 4. The Cp locus were identified to be controlled by Cp F and S allele, and the distribution of genotypes were 23.11, 34.87 and 42.02% for Cp FF, FS and SS types, and the gene frequencies were 0.405 and 0.595 for Cp F and S allele, respectively. 5. The Am-I locus were observed to be genetically controlled by Am-I B and C allele, and the distribution of genotypes were 51.26, 16.81 and 31.92% for Am-I BB, BC and CC types, and the gene frequencies of Am-I B and C alleles were 0.597 and 0.403, respectively. 6. The Hb locus were found to be controlled by Hb A and B alleles, and the distribution of genotypes were 93.19, 16.39 and 0.42% for Hb AA, AB and BB types, and the gene frequencies of Hb A and B alleles were 0.914 and 0.086, respectively.

Seed Production of Pes-gallinaceua(Corydalis, Fumariaceae) Group (현호색속 Pes-gallinaceua절 집단의 종자생산)

  • Properties of seed production in Corydalis, spring ephemeral, group were studied during two years at Namhansansung area. Corydalis´ flower was out in the early April and its seed dispersed in the early May. The period of total seed production was 30 days. The decrease rate of sex organs was the highest between deflowering time and early fruiting time. The numbers of flower and seed per plant were in the range of 1∼13 and 0∼76, respectively. And the number of seed per fruit was from zero to twenty. In a plant, seed production was the most (11.8 seeds) in the lowest fruit and conspicuously decreased along the upward fruit. In the same plant, seed production was various by each year. The plants of small tuber size produced more seeds and those of large tuber size produced fewer seeds in the next year than this year. In the early growth season, the flowering plants/total plants rate increased in proportion to plant size (tuber volume), the rate of the smallest size class (<100 ㎣) was in the range of 5.0%(1999)∼5.4%(2000), those over the 600 ㎣ size classes were 100%. The number of flower per plant at the same size class were higher in 2000 than in 1999. Especially, at the size class of 900 ㎣≤, the numbers of flower per plant were 13.2 in 2000 and 6.5 in 1999. In the late growth season, the flowering plants/total plants rates were 13.3%in the smallest size class (<100 ㎣)and 100% over the 500 ㎣ size classes. Therefore, the flowering plants/total plants rates along the size classes were higher in the late growth season than in the early one. The bumer of fruit or seed per plant increased in proportion to the volume and dry weight of tuber, there was not significant and varied along each plant in the same size class. The number of fruit or seed per plant conspicuously increased in proportion to the leaf area. Therefore, it was thought that seed production was related to photosynthetic ability during growth season.

  • The purpose of this study was to evaluate the amount and interrelationship of the soft and hard tissue change after mandibular setback surgery in skeletal Class III malocclusion. The sample consisted of 25 adult patient (12 male and 13 female) who had severe anteropostrior skeletal discepancy. These patient had received presurgical orthodontic treatment and surgical treatment which is bilateral sagittal split ramus osteotomy. The presurgical and postsurgical lateral cephalograms were evaluated. The computerized statistical analysis was carried out with SPSS/PC program. The result were as follows: 1. After mandibular bilateral sagittal split ramus osteotomy, lower facial soft. tissue horizontal posterior changes were high significance value. but vertical soft tissue changes were low significance value. 2. After mandibular bilateral sagittal split ramus osteotomy, relative upper lip protrusion increased(p<0.01) and relative lower lip protrusion decreased(p<0.01) and lower facial soft tissue thickness increased(p<0.01).

Diagnostic Analysis on Oral Health Education of Primary School's Health Teacher (초등학교 보건교사의 구강보건교육 진단)

  • Objectives : The systematic health education to form the basic healthy lifestyle should be realized from elementary school, so oral health education at elementary school can determine the whole lifetime oral health. The elementary school health teacher's recognition and behavior who in charge of health promotion of students is important. Therefore, the study was conducted to enhance oral health education. Methods : Total 114 people among of 131 from health teacher Gwangju elementary school. Survey system is configured by referring to PRECEDE model. Results : In behavioral diagnosis the proportion of oral Health Education is less than 10%(58.8%), mostly educated in activity time (86.0%), the health teachers educate when it is needed(53.5%). In predisposing diagnosis in the eight areas of health education, the oral health education is ranked as fourth, fifth. Even in the next year project, the oral health education ratio was 21.9 percent. In enabling diagnosing every year the Oral health education training experienced rate is 13.2%, satisfaction rate is 33.3%. In reinforcing diagnosing disability element in the regular education course are the lack of oral health-related information (46.7%), lack of materials needed for education (30.6%), lack of training opportunities (21.4%). Conclusion : In further research, oral health education textbooks, materials and methods should be developed. At the foundation of there developments, the elementary oral health education program should be more fully developed and conducted and also the evaluation of its effectiveness will need.

A FOUR TO SEVEN YEAR FOLLOW-UP STUDY OF SYMPTOMS OF AUTISTIC CHILDREN IN TAEGU AND KYUNGPOOK PROVINCE (대구 ${\cdot}$ 경북지역 일부 자폐장애 환자의 4 ${\sim}$ 7년후 추적 연구 - 자폐장애의 증상 중심으로 -)

  • This study evaluated the improvement of autistic symptoms during the 4-7 year follow-up in autistic children. Fifty subjects(44 males and 6 females whose age ranged between one to 13 years) diagnosed with DSM-III-R autistic disorder were included in this study. Their autistic symptoms were assessed with DSM-III-R diagnostic criteria of autistic disorder(grouped as A, B and C which include 5, 6 and 5 items respectively). The mean of follow-up duration was 6.4${\pm}$1.1 years. Symptoms between baseline and follow-up were examined according to treatment types, age, sex, IQ and physical problems of them were identified. There were statistically significant decreases of symptom items at follow-up as a whole and A, B and C, respectively. The most symptom decrease occurred in communication and followed social interaction, and activities and interests at follow-up assessment. Symptom items that decreased more than 50% at follow-up evaluation include A1, A2, A3 and B1, B2, B3, B4, B5, and C1, C3. Groups with special education only and combined treatment of special education and seizure pharmacotherapy showed better improvement of symptoms than the group with psychiatric pharmacotherapy with special education. Age was a significant factors in the improvement of symptoms. The author concluded that some autistic children improved with aging, therefore they should be treated actively with special education, pharmacothrapy and other scientific methods.

A Status of Student Sickness and Medical Care in University Health Service, Ewha Womans University (이화여자대학교 학생들의 의료실태에 관한 조사 연구)

  • A study was carried out in order to obtain the status of student sickness and medical care in University Health Service, Ewha Womans University. This study was based on the clinical records of University Health Service and hospitals 'for student insurance pay claims during the year of 1981. And the findings from the study were as follows; 1. A total number of student patients cared at University Health Service in 1981 was 9,822 and the incidence rate of primary cared was 773 per 1,000 students. 2. A total number of student patients cared at hospitals was 393 and the incidence rate of secondary cared was 31 per 1,000 students and 5 student out of 31 per 1000 was cared under the haspitalization. 3. The evacuation rate of student patients from University Health Service to hospital was 393 out of 9,822 student primary cared or 4.0 percent. 4. The order of 5 major diseases of primary cared in University Health Service was respiratory system diseases (36.6%), Digestive system diseases (17.4%), Skin and subcutaneous tissue diseases (16.0%), Symptoms and undetermined diagnosis (13.7%) and Nerve and sensory organ diseases (12.0%) respectively. 5. The disease order of student patients(333) cared in hospitals as out-patients was Skin and subcutaneous tissue diseases (40.3%), Nervous and Sensory organ disease (19.2%), Digestive system diseases (10.8%) respectively. 6. The disease order of student patients (60) cared in hospitals as in-patients was Digestive system diseases (35.0%), Respiratory system diseases (13.3%), Nerve and sensory organ diseases (10.0%), Infectious and parasitic diseases (10.0%), and Symptom and Undetermined diagonsis (10.0%) respectively. 7. The evacuation rate of student patients in University Health Service to hospital was varied according to disease groups; the lowest rate was the diseases evacuated to Internal Medicine Department 1.5% or 75 out of 5,072 patient primary cared and the highest rate was Neuropsychiatry department 63.7% or 7 out of 11 patients. 8. The monthly distribution of student patients in University Health Service was the highest in September (17.9%) and April (15.5%) each semester. 9. The monthly number of student patients treated in hospitals was the range 20 to 40 in out patients and 2 to 9 in in-patients. 10. The hospital ill days per case were $4.3{\pm}5.0$ days in out-patients and $9.7{\pm}9.5$ days in in-patients.

A Bacteriological Study on the Indoor-Shoes, Desk and Bag of Student의s (실내화, 책상, 책가방의 위생학적 고찰)

  • A bacteriological research of the school desks, bags and indoor-shoes was made from Oct. 2, 197 through May 28, 1976. The research included one girl's middle school, two boy's middle schools, two boy's high schools and 15 girl's high schools, alltogether 244 students from 28 schools, The research was analyzed and the results are as follows: 1) The research showed that 95% of the indoor-shoes materials were of cloth. Only 13% of the students carried the indoor-shoes in shoes sacks and 84.8% of them kept the shoes in schoolbags with books and lunchbox. 2) Only 5% of them cleaned their shoes every week and 79% cleaned every six. 3) 49.3% of the students kept the indoor-shoes in the school bag with other contents at home. 4) 88% of the bag was made of vinyl and 24.8% of the students carried their lunch in their bag without lunch box. 5) The research showed that 79.1% of the students never cleaned their bags. 6) In using the desk drawer, 58.2% of the students put their out-door shoes into the drawer, 18.9% of them kept their lunch in it, 11.9% used it keeping books alone and 11% keeping school bags with books. 7) 30.4% of the students had an experience of having cleaned their desks and 69.6% were unconcerned wirh desk-cleaning. 8) 34.4% of the indoor-shoes using school showed fecal coliform positive, 40.2% showed non-fecal coliform positive and 25.4% showed no reaction to IMViC. 9) The 34.4% of the fecal coliform positive showing schools, equivalant to 77 students, was composed of zero percents of girl's middle school, 18.2% of girl's high school, 36.4% of boy's middle school and 45.4% of boy's high school. 10) Among the 77 students, whose shoes showed fecal coliform positive, only 6.5% of them cleaned their shoes once in a week or a month and 62.3% of them cleaned them once in a six month. 11) The fccal coliform positive rate was higher in the cloth shoes materials than the vinyl materials. 12) The concrete-bottomed building higher positive rate (40.2%) than the wood-bottomed building (7.5%). 13) Even among the student's who frequently cleaned their desk drawer, 2.3% were in the positive, and among those who were unconcerned with drawer cleaning, 48.9% were involved in the positive. 14) In the fecal coliform positive rate of bags, 38.5% of the positive rate belonged to using indoor shoes while cleaning their class bottom with water, 53.8% belonged to using outdoor shoes while cleaning their class bottom with water, 7.7% belonged to using outdoor shoes while cleaning their class with oil.

Relation of Body Weight Control and Eating Disorder of University Students in the Gunsan Area (군산지역 대학생의 체중 조절 태도와 이상식이습관과의 상관관계 연구)

  • This study was carried out to investigate the body weight controls and eating disorders of university students in Gunsan. The subject of the present study included 229 male students (53.8%) and 197 female students (46.2%), totalling 426 students from Kunsan University. The subjects consisted of 56 underweight (13.1%), 265 with normal weight (62.2%), and 105 overweight (24.6%). Especially, in females, 20.8% of them were underweight, which is 6.6% higher than males, showing a significant difference (p<0.001). Regarding body image, 21.8% of the males thought they were fat, while 40.6% of the females thought they were fat. Regarding the perceived body type, shown in the difference between their own size and the ideal size, males recorded 0.03, while females recorded 1.75, showing a major significant difference (p<0.001). Regarding body satisfaction, 33.2% of the males were unsatisfied, while 52.8% of the females were unsatisfied (p<0.001). The social expectations of the male subjects' sizes and weights recorded 2.04, while that of females recorded -2.51, showing that people's opinions expected the female subjects' bodies to be thinner (p<0.001). Additionally, the social expectation of the subjects' weight control recorded an average of 15.22 (13.92 for males, 16.73 for females), showing that the female subjects' felt that people's opinions wished them to lose their weight (p<0.001). Regarding the recognition on weight control, interested males were 49.4%, while interested females were 86.8%, showing the females superior rate (p<0.001). The beliefs in relation to weight control recorded an average of 62.01, with the score of females of 67.89 being significantly higher than that of males which was 56.96. The subjects showed a relatively positive response towards weight control (p<0.001). EAT-26 score was employed to determine the eating attitudes in relation to gender. Here, the final score of males was 15.46, which was significantly lower than that of females, which was 22.21 (p<0.001). Furthermore, regarding the score of eating attitudes in relation to obesity index, the eating disorder scale scores on the questions related to bulimia and diets to lose weight as weight increases were high. However, the eating disorder scale score on oral control, which actually controls food, was low. To control the expansion of excessive weight control attempts and their side effects, the recognition of the students' body types needs to be corrected, and nutrition education programs on eating habits and eating disorders to foster proper weight loss are deemed necessary.
