오령산 효능에 관한 기초 실험 연구 문헌 분석 (Analysis of biological experiment on Oryeong-san (Wuling-san))

  • Objectives : To establish objective and scientific evidence of Korean medicine (KM), the papers on Oryeong-san (Wuling-san) frequently used in medical institutions of Korean Medicine were analyzed. Methods : The papers were classified by the registration of domestic or international journals, the year of publication, experimental fields and the kinds of studies on biological activities. The therapeutic mechanism was investigated in accordance with therapeutic effect of Oryeong-san (Wuling-san). Results : Out of 57 articles selected, 16 were published in domestic journals, 17 were in Chinese journals and 24 were in Japanese journals. Most papers reported as biological activities were on improvement of renal function. Oryeong-san (Wuling-san) increased urine factor such as urine excretion and electrolyte balance while decreasing proteinuria, serum factors including creatinine, cholesterol and triglyceride. In addition, injured renal tissue was recovered normally and gene expression controlling urine excretion was down-regulated. Conclusions : Improvement of renal function could be interpreted as objective and scientific evidence for Oryeong-san (Wuling-san).

오령산의 고혈압 치료 효과에 대한 체계적 고찰 : 임상 논문을 중심으로 (A Systematic Review on Antihypertensive Effects of Oryeong-san)

  • Objectives : The purpose of this study was to review the clinical studies on antihypertensive effects of Oryeong-san. Methods : We searched papers using KTKP, Oasis, CNKI, PubMed, Embase, Science Direct, CINAHL, J STAGE and CiNii, and the key words "Oryeong-san (韓)", "Wuling-san (中)", "Gorei-san (日)", and "Hypertension" were used. Search range included randomized controlled trials (RCTs), clinical reports, reviews and animal experiments. Papers not matched with inclusion criteria were excluded. Results : A total 479 studies were found, with 457 excluded during title and screening. After scanning 22 papers, a final 3 RCTs and 6 case reports were selected and analyzed. In the 3 RCTs, the number of patients varied between 65 and 156. Patients with hypertension were randomized into groups for treatment group and control group. They were all treated with antihypertensive drugs (Vamlodipine Besylate, Benazepril, Perindopril); the treatment group was also given modified Oryeong-san. After treatment, results were that blood pressure was significantly decreased in the treatment group, compared to the control. The other 6 case reports reported that modified Oryeong-san alone or the combination of Oryeong-san plus antihypertensive drugs had beneficial effects on blood pressure. Conclusions : Oryeong-san appears to be effective in improving blood pressure and hypertension-related symptoms. The results of this study could be applied to clinical treatment of hypertension. Further large-scale clinical researches should be conducted.

오령산의 고혈압 치료약물 가능성에 대한 개관 (An Overview of the Applicability of Oryung-san as an Antihypertensive Agent)

  • Objectives: This study assessed the application of Oryung-san (Wuling-san or Gorei-san), a common diuretic in traditional medicine, as an antihypertensive agent. Methods: Experimental studies of the pharmacological properties of Oryung-san, including diuretic action and lowering effects on blood pressure, and toxicology and clinical trials were reviewed. Results: In pharmacology, various, relatively safe diuretics are used to lower blood pressure and are the oldest and most studied antihypertensive agents. Despite many new drug approvals, antihypertensive diuretics are frequently used in Korea and Japan where high levels of sodium uptake are common. Oryung-san has been demonstrated to have diuretic, antihypertensive, and nephroprotective effects. Conclusions: Oryung-san might be effective for lowering blood pressure based on the results of this literature review. Further evaluations and large-scale clinical trials of Oryung-san to treat primary hypertension are warranted.

"상한론"에 나타난 계(悸)와 관련된 처방들의 현대 질환 범위 고찰 (Study on Diseases Scope of Prescriptions Related with the Palpitation in "Shanghanlun")

  • This article is a study on palpitation of which disease cause, disease mechanism and formulas were analyzed with reference to annotations on "Shanghanlun" and "Jinkuiyaolue". And the scope of modern diseases related with palpitation was drawn by research on clinical papers. The source books are "Zhujieshanghanlun" and "Jinkuiyaoluefanglun" and the clinical papers are searched in China Academic Journals(CAJ) of China National Knowledge Infrastructure(CNKI). 13 clauses in "Shanghanlun" and 9 clauses in "Jinkuiyaolue" and 12 formulas are related with palpitation. Disease mechanisms of palpitation were classified as yang deficiency, yin deficiency, qi deficiency, blood deficiency, retained fluid, cold, etc and these days, qi stagnation, phlegm turbidity, blood stasis and fire heat are also considered as disease mechanisms. Modern diseases related with palpitation are arrhythmia(extrasystole, atrial fibrillation, bradycardia, tachycardia, sick sinus syndrome, atrioventricular block), vascular diseases(arterial occlusion, phlebothrombosis, Buerger's disease, coronary artery disease, vasculitis), blood pressure disorder(hypertension, hypotension) and heart diseases such as heart failure, angina pectoris, myocardial infarction, myocarditis, cardiomyopathy, pericardial effusion. And diseases related with psychological change(cardiac neurosis, anxiety neurosis, neurosis, depression, hyperthyroidism, hypothyroidism), pyrexia, anemia, drug intoxication, etc are also related with palpitation. Zhen Wu Tang showing an efficacy in dilating blood vessels and strengthening cardiac function, Wuling Powder with diuretic effect and Fried Glycyrrhizae Decoction acting on the ${\beta}$ receptor are applied to heart failure in different ways. Fried Glycyrrhizae Decoction(308 cases), Zhen Wu Tang(154), Wuling Powder(54), Xiao Chaihu Tang(34), Sini San(20) are reported to have been clinically applied to cardiovascular diseases and Zhen Wu Tang and Wuling Powder mainly applied to heart failure, Fried Glycyrrhizae Decoction, Lizhong Wan, Sini San and Zhen Wu Tang chiefly applied to arrhythmia related diseases. This study focuses on the general research and consideration on clinical applications and is a preliminary study to understand relations between Korean Medicine's symptoms and categories of modern diseases.

오령산 열수추출물의 단회투여 급성독성 연구 (Acute Toxicity Study on Oryeong-san in Mice)

  • Objectives : Traditional medicine Oryeong-san (ORS) has been prescribed for a long time to treat light fever, thirst, dysuria, and accompanying edema. However, the acute toxicity and safety were not reported. In this study, we evaluated the potent acute toxicity and safety of ORS. Methods : ICR mice were used to evaluate acute toxicity and safety by oral administration of 0, 500, 1,000, 2,000mg/kg of ORS. Mortality, body weight, and clinical symptoms were observed, and organ weight and blood biochemical parameters were analyzed after necropsy. Results : We found no mortality and no toxic or abnormal clinical symptoms by administration of ORS. Comparing with control group, no significant alterations in organ weight and blood biochemical parameters were observed. Conclusions : ORS recognized as safe and non-toxic medicinal material, and median lethal dose considered to be over 2,000 mg/kg in both male and female ICR mice.
