한국형 환자분류체계의 개정연구 (Study for Revision of the Korean Patient Classification System)

  • Purpose: The purpose of this study was to revise the KPCS-1 and to standardize the three patient classification systems for general ward, ICU and NICU. The actual utilization of the KPCS-1 score and each nursing activity was evaluated and the relationships between KPCS-1 score and nursing related variables were reviewed. Methods: The 47,711 KPCS-1 scores of 6,931 patients who discharged from $1^{st}$ to $30^{th}$ April 2017 were analyzed and the statistical significance between KPCS-1 score and nursing related variables was reviewed by Generalized Estimating Equation. The revision of the KPCS-1 was carried out by Partial Least Square model. The 3 patient classification systems (KPCS-1,KPCSC and KPCSN) were standardized by professional reviews. Results: KPCS-1 was a valid instrument to express nursing condition adequately and was revised as a new version which has 34 nursing activity items. The names and terminologies of pre-existing 3 patient classification systems developed by KHNA were standardized as KPCS-GW, KPCS-ICU, KPCS-NICU. Conclusion: KPCS-1 was a valid instrument to represent diverse nursing conditions precisely and was revised as a 34-item KPCS-GW. The terminologies of the other patient classification systems by KHNA were standardized as KPCS-ICU and KPCS-NICU.

위탁급식업체 규모에 따른 급식관리자 직무만족에 영향을 미치는 개인, 급식소 및 조직특성 분석 (Identifying the Effect of Personal, Foodservice and Organizational Characteristics on Foodservice Managers' Job Satisfaction by the Contract Management Company Scale)

  • The purpose of the study was to investigate the influences of contract foodservice managers' personal characteristics, foodservice characteristics and organizational characteristics on job satisfaction, including interpersonal relationships, self-actualization and promotion opportunity categories. A survey was administered to four hundred contract foodservice managers of five large companies and five small/medium companies in the Seoul and Kyungin areas. The final response rate was 66%(N=265), and the data were analyzed using SPSS Windows(ver. 12.0). The respondents were 76.1% female, average age 28.8 years, and 73.0% were regular workers. Contract foodservices have profit and loss contracts(69.1%), single menu types(59.6%) and buffet serving styles(37.7%). There are significant differences of job satisfaction by some personal characteristic variables(gender, martial status, age, education, position, work hours, period of working for the present company, and payroll per year) and foodservice characteristic variables(type of contract and charge of food costs). In three job satisfaction categories, foodservice managers reported the highest interpersonal relationship satisfaction, following self-actualization satisfaction and promotion opportunity satisfaction in both large companies and small/medium companies. However, foodservice managers of large companies tended to be more satisfied regarding their promotion opportunities than foodservice managers of small/medium companies(p<0.05). Work hours, number of meals served/day, male, workload, communication with the clients, relationship with co-workers, obvious role and autonomy were significant factors to increase the job satisfaction in contract foodservices of large companies. On the other hand, relationships with co-workers and males were significant factors to increase the job satisfaction in contract foodservices of small/medium companies. This research suggests that contract foodservice companies need to understand the characteristics of their managers, foodservices and organizations to enhance the job satisfaction of foodservice managers and to develop specified human resource management strategies that can be applied to each company scale.

내원유형별 혈액투석 간호업무 표준개발 (Development of Nursing Practice Standards of Hemodialysis Care According to Admission Types)

  • Purpose: This study aimed to develop nursing practice standards of hemodialysis care according to admission types. Methods: The standards were developed in four phases. Phase 1: The preliminary standards of hemodialysis care were developed based on literature review and evaluation of professional experts. Phase 2: Content validity was evaluated by 34 professional experts and the standards were modified and revised. Phase 3: Clinical validity was evaluated by 212 hemodialysis nurses. Expert group validated and verified the final outcome. Phase 4: Final standards of hemodialysis care according to admission types were developed. Results: The standard of hemodialysis care for out-patients included 5 standards, 9 contents, 43 nursing activities. The standard for in-patients included 6 standards, 10 contents, 50 nursing activities. The standards for critical patients included 4 standards, 10 contents, 43 nursing activities. There were differences in nursing activities according to admission type. Time required to meet the standards were different according to admission types. Conclusion: The findings of this research demonstrated that the time required for hemodialysis care and appropriate number of nurse would be different according to admission types. Different nursing workload according to admission type of hemodialysis patients need to be considered in allocating nursing manpower.

  • Background: The purpose of this study was to identify role ambiguity of comprehensive nursing care unit nurses. Methods: A concept analysis method by Walker and Avant was used to understand role ambiguity of comprehensive nursing care unit nurses. Results: The antecedents of role ambiguity of nurses at comprehensive nursing units were shortage of nurses, unclear admission criteria, and demands for customized nursing care according to severity. Attributes include ambiguity in role delegation, patient placement ambiguity, and professional ambiguity among nursing staff. The consequences were diminished job satisfaction due to excessive workload, difficulty in resolving role ambiguity due to the lack of work analysis studies, and poor outcome of nursing indicators. Conclusion: Improvement of nationwide awareness for comprehensive nursing care unit is required. Clear division at scope of practice for nursing staff in accordance of each medical institution's characteristics is essential. Nurses at comprehensive nursing care unit should understand nature of role ambiguity that occurs as they work in large groups. Nurses should promote communications between nursing staff and they must have volition to improve status quo. An additional research of comprehensive nursing care on the causes of role ambiguity in the practice of nursing care for ward nurses is needed, and management measures should be sought at the organizational level.

차세대 해상디지털통신기술 ASM2.0을 이용한 여객선 MSP서비스 시스템 설계 및 구현 (Design and realization of the passenger ship's MSP service system using ASM2.0 : focusing on captain's mandatory reporting items)

  • AIS-ASM은 AIS의 고유 기능인 선박의 위치 정보 외에 다양한 해양안전정보를 제공하기 위한 연구 중 하나로, 2004년부터 관련 규정을 정비하여 진행 중이다. 하지만 AIS 메시지 사용량 증가에 따라 기존 AIS 채널에서 ASM을 사용하는 것은 기존의 선박위치정보 전송 등의 안전메시지 처리 부분에 영향을 줄 것으로 보고된 바 있고, 기존 AIS의 낮은 통신속도로는 원활한 MSP 서비스가 어렵기 때문에 ITU-R은 VHF 대역을 이용하는 해상디지털 무선통신 권고서를 제정하고 개정작업을 진행하였다. 본 연구는 차세대 해상디지털 통신 기술인 ASM2.0을 활용하여 여객선의 출항에서 입항까지 안전항해를 위해 필요한 국내 여객선 선장의 의무보고 내용을 조사하고 업무 부하를 줄이기 위해 자동화 할 수 있는 부분을 자동화하는데 목적이 있으며, 국내 여객선 운항 관련 법규를 조사하여 상호 교환해야 하는 정보를 도출하고 국제적으로 사용하고 있는 AIS-ASM 메시지 분석을 통해 ASM2.0에서 활용하기 위한 신규 프로토콜을 설계하고 프로토타입을 구현하였다.

국내 선종별 선박승무원 피로도 분석에 관한 연구 (A Study on the Analysis of Crew Members Fatigue Survey for the Ship Types in Korea)

  • 본 연구에서는 승선 중인 해기사의 피로도 유발요인에 대한 현황과 개인별 피로도 자각증상 항목들에 대한 실태를 파악하기 위하여 설문조사를 실시하고 그 결과를 선종별로 분석하였다. 설문조사 분석결과 선종별로 많은 해기사들이 수면과 휴식시간의 부족을 겪고 있음을 확인할 수 있었고, 수면을 방해하는 요인은 다양하지만 특히 날씨, 선체의 운동 및 진동, 소음, 거주시설 등 환경적 요인이 많이 작용하는 것을 파악하였다. 그리고 일부 선종의 경우는 하루 평균 10시간 이상 당직근무를 수행하고 있었으며, 과다한 업무로 기인한 스트레스와 더불어 많은 피로감을 느끼는 것으로 조사되었고, 영역별(육체적, 정신적, 감정적) 피로도 증상 측정항목에 대해서 일부항목의 경우는 많은 해기사들이 상당한 피로도 증상을 경험하고 있는 것으로 분석되었다.

위 절제술 환자의 진료계획표 개발 및 전자 의무 기록화 (Development of a Flexible Critical Pathway with Electronic Medical Record for Gastrectomy Patients in a University Hospital)

  • Objectives : This study was conducted to evaluate the effect of fixed critical pathway with emr (electronic medical record) on the length of hospital stay, the cost and quality of care provided to gastrectomy patients in a university hospital and to develop flexible critical pathway with emr which can be used excluded or drop-out patients. Methods : Thirty-eight patients with gastrectomy were included as case group and Thirty-four patients included as control group. The comparison between control and case with using fixed critical pathway were done. To develop and to evaluate usefulness of flexible critical pathway with flexible data base, simulation was done for flexible critical pathway with drop-out patients. Result : The major results of this study were as follows: There were no significant differences in patient clinical conditions and no sign of deterioration of quality from critical pathway. The length of hospital stay was 11 days in control group, 8 days in path group(P<0.01). The total costs during the hospital stay were reduced in path group. However the cost per day was significantly increased from reduction of hospital stay(554,352 won in control, 645,669 won in path group). One hundred percentage of drop out patients(60) in the simulation of flexible critical pathway was successful. Conclusion : Computerized critical pathway reduced the length of hospital stay, total hospital costs and resource utilization without harming quality of patient care. The flexible critical pathway program can be used as one of the powerful management tools for reducing the practice variations and increasing the efficiency of care process and decreasing the workload of doctors and nurses in Korean hospital settings.

새꼬막의 선별과정 현황과 문제점을 고려한 자동화 선별 시스템 요소기술 (Elemental techniques for automated size sorting system considering problems and status of sorting process of ark shell (Scapharca subcrenata))

  • Seafood is attracting attention as a future food industry. In recent years, the demand for fishery equipment of mechanization, automation, and unmanned was increased due to the environment affected by seafood processing, stricter regulations on safety, decline and aging of fishery worker. Ark shell (Scapharca subcrenata) was being produced in many steps in the production process. The process has been made such as collection-landing-washing-first sort (goods/non-goods)-transports-second sort (size). It was undergone first and second steps by delivering to the consumer. Here, the first step is to sort goods to collection and the second step is to sort by size. The fishery workers need ten people in first step and six people in second step. The workload of one hour per kg is 4,247 kg/h in first step and 2,213 kg/h in second step. In addition, the goods ratio by work process was 79% in first step and 98% in the second step. In this process, a lot of fishery worker and working time is needed. Therefore, this study developed elemental techniques for an automated size sorting system considering the working process problem, time and situation for washing and sorting of ark shell.

복수의 프린터 관리효율을 증가시키기 위한 스마트한 인쇄작업 분배 프로그램 구현에 관한 연구 (A Study on the Smart Printing Work Distribution Program to Increase the Efficiency of Managing Multiple Printers)

  • 최근에는 인쇄량이 많아 지면서 동일 네트워크내에서 여러 대의 프린터들을 운영하고 있는 실정이며, 다수 개의 프린터를 운영하는 경우에 대한 관리가 필요한 상태이다. 이를 위해 2대 이상의 프린터와 유선 또는 무선 통신망으로 연결된 컴퓨터를 구동시키는 프로그램으로서 컴퓨터의 제어부가 지정파일에 대한 인쇄명령을 수령하게 되면, 제어부가 프린터로부터 상태 정보를 수신하고, 프린터를 선택하는 단계와 인쇄명령 실행을 전송하는 단계를 실행하게 된다. 연구의 방법으로는 선행기술연구와 문헌조사를 통해 본 연구와의 차별성을 선정할 수 있는 방법을 제시했다. 따라서 다수개의 프린터들의 실시간 상태 정보에 따라 스마트한 인쇄명령을 분배하여 프린터의 관리 효율을 증가시키고, 다수개의 프린터들의 누적 작업량과 가용 작업량에 따라 인쇄명령을 분배하여 다수개의 프린터들의 부품 교체를 일시에 할 수 있도록 하는 것이 연구의 목적이다.

교대근무를 하는 일반병동 간호사의 간호업무 수행실태, 수행주체와 직무만족에 대한 융복합적 조사연구 (A Convergence Investigation on Nursing Task Performance, Appropriate Performer, and Job Satisfaction of Nurses with Shift Work in General Ward)

  • 본 연구는 일반병동에 근무하는 교대근무 간호사의 간호업무 수행실태와 직무만족도를 확인하기 위한 2차 자료 분석 연구이다. 연구 결과, 전체 간호 업무 중 대상자가 수행한 직접간호는 45%, 간접간호는 55%를 차지하였다. 교대 근무별로 주간에 수행한 업무량은 전체 업무량의 37.2%였고 인계 후 수행한 업무는 '직접간호(34.5%)'와 '기록과 보고(25.8%)'였다. 대상자들은 본인이 수행한 업무 중 일부분에 대해 간호보조 인력에게 위임 가능하거나, 타 의료인과 경계가 모호한 업무가 있다고 응답하였다. 야간근무의 직무이행도와 직무만족도 간의 유의한 상관관계(rs=.43, p=.01)가있었다. 이러한 결과는 교대근무별 직무분석에 근거한 업무효율성 제고 및 EMR적용, 스마트 기기 등을 활용한 인계시간 감소 전략, 업무 수행주체를 명확히 하는 방안이 필요함을 시사한다.
