Purpose: This study aimed to determine a method to reduce stress in clinical practice by identifying how the empathy and self-regulation capacities of paramedic students influence their stress levels. Methods: Selected articles were statistically analyzed using SPSS 21.0. Data on 172 3rd-year and 4th-year paramedic students in Daejeon were used. The subject's general characteristics (sex, university education, the number of the medical training center, work type, satisfaction with college life, and satisfaction with their major) were analyzed according to empathy and self-regulation ability by t-test and ANOVA. Pearson's correlation coefficient was used for the correlation of empathy, self-regulation ability, and clinical practice stress. Hierarchical regression was used for factors influencing clinical practice. Results: Clinical practice stress levels were significantly different between general characteristics of sex (t=3.002, p=0.003), university education (t=2.815, p=0.006), the number of the medical training center (t=-2.998, p=0.003), work type (t=-2.998, p=0.003), satisfaction with college life (t=5.280, p=0.000), and satisfaction with major (t=5.132, p=0.000). Empathy (r=-.520, p<.001) and self-regulation ability (r=-.330, p<.001) significantly negatively correlated with major variables, and the major factor influencing clinical practice stress levels was empathy. Conclusion: Extension of education and various experiences for improving empathy and developing management programs should be performed to decrease clinical practice stress on paramedic students.
본 연구는 응급구조(학)과 대학생을 대상으로 전공선택동기와 직업가치관이 취업준비행동에 미치는 영향을 파악하고, 이의 결과를 토대로 취업준비행동 향상을 위한 기초자료를 제공하고자 시행하였다. D광역시, J도 소재 3·4년제 대학 응급구조(학)과 대학생을 대상으로 2019년 11월 4일부터 8일까지 연구 참여에 대한 동의를 받은 후 자기기입식 설문조사를 하였다. 수집된 자료 중 응답이 미비한 9부을 제외한 341부를 SPSS ver. 12.0(Window)으로 최종 분석하였다. 본 연구결과, 취업준비행동에 전공선택동기의 개인적 동기, 직업가치관의 내재적 직업가치관, 외재적 직업가치관 순으로 영향을 미쳤다. 따라서, 성별 및 학년의 특성과 전공선택동기 및 직업가치관의 특성을 고려하여, 개별 및 집단 상담과 정규 또는 비정규 교육과정에서 직업가치관 강좌의 개설이 필요하다고 사료된다.
Purpose: The purpose of this study is to examine the work experiences of nurses working as 119 paramedics and to provide basic information to develop administrative and institutional support system for the nurses. Method: The data were collected from December, 2007 to July, 2008 and analyzed using the phenomenological method provided by Munhall and Boyd. Participants of this study were 7 nurses currently working as 119 paramedics and who were willing to fully express their experiences. The grand mal question used in this study was "How is your work experience as an 119 paramedic?" Result: Narratives of the participants were analyzed and divided into 3 categories such as 'recognizing', 'withdrawal', and 'confrontation'. Conclusion: The results showed even if the participants think of their tasks fulfilling and be proud, they have some difficulties due to restrictions of tasks and work environment. Based on the findings, we suggest developing educational programs to improve their task abilities, establishing safety policy and compensatory reward system according to their accomplishments, and providing counseling opportunities for their traumatic experiences with difficult emotion.
Purpose: This research project addressed the need to designing more safe and efficient interior of the future ambulance in Korea. Methods: The study sample contained 760 paramedics in 4 districts. Data was collected by using a revised and complemented questionnaire based on literature review. Results: In relation to the efficacy and safety of work, answers related to storage closet showed to be the highest, and the most difficult part of paramedic work in an ambulance was lurching. CPR is the most frequently used emergency care inside an ambulance, but 66% of the paramedics responded that accurate CPR is not possible during vehicle transfer. Safety belts are not worn for 82.8% of the time, because of discomfort (51.3%). 13.8% of the paramedics responded that stretchers are unstable, 29.5% had an experience of having patients fall off the stretcher inside an ambulance. There were comments on installing equipments to prevent noise, and assist communication. Conclusion: The suggested practical layout contains five main modifications 1. Developing specially designed belt is needed for paramedic safety & efficient work. 2. The seats are molded to be ergonomically friendly. 3. Equipments to secure the body and safety devices for CPR are needed. 4. System improvement for communication between the driver seat and paramedics is needed. 5. The stretchers are molded to be maximize efficiency and minimize injury.
Purpose: The emergency medical service (EMS) system in Japan is similar to that in Korea. This study reviewed the development of paramedics who are major personnel in the respective EMS system of their country. Methods: We searched law for paramedics and publications in J-stage, MEDLINE and the webpage of Japan's Fire and Disaster Management Agency. Results: Paramedic training had started in Japan in 1991, and most paramedics were employed in fire departments. In Japan, ambulances belonging to fire department are staffed by a three-person unit, including at least one paramedic. Medical control advisory boards were established in all Japanese prefectures in 2003 with the purpose of upgrading the activities of ambulance personnel, including paramedics. The scope of paramedics' work has been widened owing to the continued efforts of medical control advisory boards. This allows them to perform invasive procedures including tracheal intubation, intravenous line placement, and epinephrine administration under medical control of a physician. Conclusion: As paramedics' responsibilities expand, further development of the paramedic role and an effective medical control advisory board system are imperative both Korea and Japan.
Purpose: This study was performed targeted to paramedic and doctors, to examine the frequency, importance, and admissibility of the tasks of paramedics, and to establish as a basis for expanding their business scope. Methods: The subjects were 282 paramedics and 58 doctors, and the study was performed from October 20 to November 19, 2014. The statistical analysis was done by using SPSS/WIN 20.0. Results: The results showed the difference between the legal business scope of paramedics and the actually performed work. The frequently covered areas were trauma, cardiac arrest, and respiratory care; however, severity classifications were the most important. Concerning drug administration, epinephrine administration seemed to be the most necessary. Conclusions: The findings of this study can be used as basic data to broaden the capacity building and business scope of paramedics. Furthermore, to increase the resuscitation rate of emergency patients, the various tasks of paramedics must be incorporated, and these should be supported by laws and institutions.
본 연구는 119 구급대원들의 근로생활의 질이 조직유효성 및 고객지향성에 미치는 영향을 규명하고, 응급의료서비스의 질적 개선을 위한 기초자료를 마련하기 위하여 실시되었다. 대구 및 경북 소방안전센터에 근무하고 있는 119 구급대원 남녀 308명을 대상으로 2019년 2월 3일부터 5월 7일까지 설문조사를 실시하였다. 수집된 자료는 SPSS 20.0을 사용하여 빈도분석, 상관관계 및 다중회귀분석을 실시하였다. 연구결과는 첫째, 119구급대원들의 근로생활의 질이 조직유효성 및 고객지향성 간의 정적상관관계를 보이고 있는 것으로 나타났다. 둘째, 119 구급대원들의 개인차원, 관리차원의 근로생활의 질이 조직유효성의 조직몰입, 직무만족에 유의한 영향을 미치는 것으로 나타났다(p<.001). 셋째, 119 구급대원들의 개인차원, 조직차원, 관리차원의 근로생활의 질이 고객지향성에 유의한 영향을 미치는 것으로 나타났다(p<.001). 넷째, 119구급대원들의 조직유효성이 고객지향성에 미치는 요인으로 조직몰입, 직무만족도에 유의한 영향을 미치는 것으로 나타났다(p<.05). 이러한 결과를 감안할 때, 119 구급대원들의 복지제도를 개선함으로써 응급의료서비스의 질향상을 도모하기 위한 정책마련이 필요하다.

D 및 G광역시, J도에 소재한 전문대학으로 2019학년 1, 2학에 각각 1회 이상으로 총 2회 이상 일반상담, 진로상담, 출석상담, 학사경고상담을 받은 재학생을 대상으로 하였습니다. 2019년 12월 2일부터 6일(5일 동안)까지 본 연구자의 편의에 따라 무작위로 추출하여 연구 참여에 서면동의를 받은 후 자기기입식 설문조사를 시행하였다. 배부된 300부 중 불성실한 응답을 한 14부를 제외한 286부를 SPSS WIN 12.0 프로그램으로 최종 분석하였다. 본 연구결과 전공선택동기의 개인적 동기와 내재적 직업가치관 및 외재적 직업가치관은 취업준비행동에 정(+)의 영향을 미쳤고, 또한 멘토링의 조절효과가 있음을 검정하였다. 따라서 취업준비행동 향상을 위해 지속적인 개인 및 집단상담이 필요하고, 전공선택동기와 직업가치관을 바탕으로 한 멘토링 프로그램의 취업특강, 모의면접 등이 필요하다.
Multiple myeloma is characterized by the neoplastic proliferation of a single clone of plasma cells producing a monoclonal immunoglobulin and it is frequently associated with primary amyloidosis. I experienced a medical claims review case of plasma cell dyscrasia with primary amyloidosis. This medical consulting work to insurance claims will be helpful for another similar claims administration.
Purpose: The purpose of the study is to emphasize the importance of out-of-hospital cardiac arrest resuscitation. This resuscitation by paramedic is very effectively performed under the medical direction of the doctors. Methods: The cardiac arrest victim was 4 month old infant. Informed consent from the parents of the infant was received. CPR combined with 150J defibrillation was performed to the 4 month old infant. Results: We reported that the 4 month old infant survived the cardiac arrest. Out-of-hospital cardiac arrest infant survived after 150J automated external defibrillator and CPR performance. Conclusion: Specific operative protocol is important because the paramedic can apply the proper manual defibrillator effectively. It is important to extend the work scpoe of the EMT.
