울산 지역 당뇨 질환 노인의 항산화 영양소 섭취실태 및 혈액 항산화 영양상태 (Dietary Antioxidant Vitamins Intakes and Plasma Antioxidant Levels in Korean Elderly with Diabetes Living in Ulsan)

  • Diabetic mellitus in an older population is associated with increased basal oxidative stress and free radical accentuated by hyperglycemic challenge. Enhanced free radical in diabetic elderly can cause the oxidative damage and such damage can be protected by antioxidant defense system. It is believed that vitamin C, A and E are the most abundant and effective antioxidants in human plasma. The purpose of this study was to determine the antioxidant status in Korean diabetic elderly using the case-control study. The antioxidant status was examined by determining plasma levels of antioxidant vitamins (vitamin C, A, E, ${\beta}$-carotene), total antioxidant status (TAS) and thiobarbituric acid reactive substance (TBARS) and intakes of vitamin C, A, ${\beta}$-carotene and retiol. Fasting glucose and HbA1c levels and serum lipid profiles (triglyceride (TG), total cholesterol, HDL-cholesterol and LDL-cholesterol) were also determined. Diabetic subjects were 122 elderly persons over 60 years old, visiting public health center, and control subjects were 96 healthy elderly persons living in Ulsan, Korea and they were matched by age, gender, smoking and drinking status. The diabetic and control subjects were divided into sub-groups according to the status of using diet therapy and vitamin supplement. The subjects were interviewed to collect data on their general characteristics, disease history, vitamin supplement, diet therapy and health-related habits by questionnaires. Their dietary intakes were obtained by means of semi-quantitative food frequency questionnaires (SQFFQ). Fasting plasma glucose and HbA1c levels were significantly higher in diabetes than in control subjects, and plasma total cholesterol level of diabetes was not significantly different from that of control subjects. However serum HDL cholesterol level of diabetes was significantly lower and serum TG level of diabetes was significantly higher than those of control group. The average vitamin A and ${\beta}$-carotene intakes of diabetes were significantly higher than those of control subjects. There was no significant difference in plasma vitamin C, ${\beta}$-carotene, and TBARS levels between two groups, but plasma vitamin A, E and TAS levels were significantly higher in diabetes than those in control group. Plasma vitamin A and TAS levels of diabetic subjects using diet therapy were higher than those of control using diet therapy, and plasma vitamin E, ${\beta}$-carotene and TAS levels of diabetic subjects using vitamin supplements were significantly higher than those of controls using vitamin supplements. These results suggested that diabetic mellitus could enhance antioxidant defences against reactive oxygen species and interest in healthy eating such as consumption of more antioxidant nutrients.

산후 경과별 모유수유실태 (The present status of breast feeding practice according to the postpartum period)

  • The purpose of this descriptive study was to investigate the determinants of mother's choice of infant feeding method on immediate postpartum period & the breast feeding practice after 1, 2, 3, 4, 5months. The subjects were 170 mothers who were delivered at 2 hospital, 2 OBGY clinics, 1 midwifery in Jinju, the data were collected from Jan. 1 to Feb. 20 of 1991, form each subject at two periods of time ; postpartum 2 or 3 days via interview, 5months via telephone interview. The results were as fellows : 1. The propotion of mother planned exclusive breast-feeding was 66.5%, milk feeding 17.6%, mixed feeding 15.9%, End 38.9% of mothers planned to breast-feed their infants for 4-6moths, 25.7% for more than 12 months. The major reasons for selection of milk feeding method were the presence of mother's job(26.7%), the conditions of breast & milk production(23.7%). 2. There were significant relations between the choice of feeding method & the presence of s job(p<.01), & parity(p<.01). 3. The percentages of subjects that were received education about the advantages & method of breast feeding were 63.5%, 38.2%, respectively, & the informants were mass media, 89.8%, 81.5%. 4. The most of mothers had the knowledge about the advantages of breast feeding. 5. The percentage of mothers that practiced breast feeding on postpartum were 75.9%. 6. The rates of breast feeding practice by duration were more than 5months, 60.5%, 1 month, 27.1%, 2-4 months. 12.3%. 7. The reasons for the stop of breast feeding were the decrease of milk volume(22.9%), the presence of mother's job(17.4%). 8. The duration of breast feeding practice were differed significantly by the presence of mother's job, type of delivery, rooming-in, planned duration of breast feeding, the first feeding time.

모유수유에 대한 여대생의 지식 및 태도 (The Knowledge and Attitude on Breast Feeding of Female University Students)

  • The purpose of this study is to provide the basic data in order to develop of some educational programs for increasing breast feeding by studying the female university student's knowledge and attitude on breast feeding, who will become a mother in future. The respondents of this research were selected at random for 462 female students at the university in Seoul and Kyongki area, and it was the period collected the data from Oct 28, 2000 to Nov 8, 2000. The method of study distributed the measuring tools of knowledge with 33 items and the tools of measurement with 20 items on the attitude of breast feeding to the respondents directly, and collected them. The data were analyzed to use SPSS program. Unpaired t-test, ANOVA, Pearson correlation coefficient and Multiple regression analysis were used for the calculation of difference between groups and the results were as follows ; 1. The breast feeding was 50.6% in the period of lactation for the respondents and the nuclear families were 81.7% in family constituent unit. In the future the wisher of breast feeding was 91.5%, the medical personnel was a major informer who enjoyed their best confidence, Besides the respond-ents responded that the proper period for education of the breast feeding was in a high school. 2. The level of Knowledge on breast feeding. The respondents's knowledge on breast feeding was average $16.40{\pm}4.59$ points on the basis of 33 points and On the merits and demerits ratio of breast feeding has shown highest but there was low in the field of such a concrete and practical plan as the estimate of breast feeding and the method and mindfulness for breast feeding. The higher grader, the college of the natural science showed significantly the higher points in the knowledge degree by respondents's characters and in such cases the persons of breast feeding or the informed of breast feeding by a medical personnel or the women of strong will for breast feeding action in the future. 3. The Attitude on breast feeding. There was relatively shown a positive attitude of the total average $60.50{\pm}7.59$ points and the average evaluation $3.04{\pm}.36$ points in the attitude on breast feeding. The attitude by each factors has the highest points in the practical action aspect but the lowest in the emotional aspect. The attitude on breast feeding by respondents's characters significantly showed a positive attitude in such cases the persons of breast feeding or the informed of breast feeding or the women of strong will for breast feeding action in the future. 4. Relation to knowledge and attitude on breast feeding. There was shown a correlation of definition in the relation to knowledge and attitude on breast feeding, 5.Factors which have an effect on knowledge and attitude on breast feeding. The factors which have an effect on knowledge of breast feeding were attitudes on breast feeding, graders, the college of natural science and the informed of breast feeding. Also the factors which have an effect on attitude on breast feeding were on will and knowledge on breast feeding, a large family, the informed of breast feeding. In conclusion, it will have to enforce a systematic education on the method of a practical breast feeding enlarged by a medical personnel and professional early enough as the information provision on breast feeding enables one to increase knowledge and attitude on it, besides it has relations with their practical will.

여성전문인의 능력발전에 관한 연구 - 전문간호사를 중심으로 - (A Study on the Development of Ability Women Specialist -Focused on the nursing specialist-)

  • In the present, since a five-year plan for economic development which was started in the early 60's has been successfully promoted for a quarter century, with consolidation in a department of social welfare in our country the participation. in economic society for women is more required than any other times. As a professional occupation for women is incereaing through a high-standard specialization, I think the upbringing for productive woman expert who has a strong motive of accomplishment as a developed person as well as a technical and skilful capacity which can be contributed to the growth of organization is very important. So in this study, I am evaluating the technical disposition of character of professional nurses working with hospital and also trying to supply the basic data being served to th extension of a skillful ability as a nurse, understanding the important factor related to it. The research method applied here is that we used 527 of formed questionnaires which were distributed to 7 University and General Hospitals, somewhat large in a scale, located in Seoul as an analytical material. It was performed between October 11, 1988 and October 18, 1988. An implement which was invented by Cho Moo-Sung is used after being amended and supplemented, which can measure the disposition of professional character. The formation of questionnaires of the disposition of character is 26 totally, 10 for Open-disposition, 11 for Active-disposition, 5 for wise-disposition, and it was measured. 'Ye', or 'No' through an one-half-standard and the environment of hospital organization is composed 12 questions from one point of 'Very Good' to 5 points of 'Very Bad.' Collected materials were analysed through an electronic calculation into the average value, the standard deflection, percentage, person correlative number, $X^2-test, m$ stepwise multiple regression. Summarizing the result from this research is as follows; 1. The average age of the subjective person of this investigation is 28.6 and the average career as a nurse is 6.0 years. 2. The Open-disposition that technical nurses showed is mostly half and half. 3. The Active-disposition of professional nurses was discovered affirmative largely and what they said in their questionnaires describes that they are very active answering $88.2\%$ for the disposition of self-control, $87.3\%$ for the people who think the training more seriously. 4. It was found out that the wise·disposition of technical nurse showed $90.7\%$ of 'Yes' about a new alternative of inquisitive question and we can see a progressive and profound aspect here. 5. As technical character of nurses, mutual relations between Active-disposition, Active-disposition, and wise­disposition were very profitably revealed as 0.42 in justice relations and also suggested that relations between Open-disposition, Active-disposition, and wise-disposition are 0.27 and 0.20 respectively. 6. What nurses recognize about the environment of hospital organization is reasonably acceptable while they feel very bad about rewards and punishments showing average 3.1 comparing to average 2.2 about time­control each other. Considering the prosperity of Active-disposition upon the result what I mentioned above, th possibility which is contributed to the productive improvement of hospital organization is very great and I think it can be more developed as a professional woman who has a strong motive of accomplishment, in advance.

미국 Texas 일부 지역에 거주하는 한국 유학생 부인들의 식생활습관에 관한 연구 (Research on the Food Habits of Housewives of Korean Students Residing in a Southern Region of Texas, U.S.A.)

  • The purpose of this study was to research on the preparing fermented foods and food habits of Korean women in Texas U. S. A. The preparing method of fermented foods and food habits of Korean housewives whose husbands are studying at the various universities in Houston, Texas are surveyed. The analysed results indicated were as follows ; 1. 64.4% of women were in the thirties at their age, those in twenties 29.7%. 75.2% of subject graduated college. Period of residence in America were 52.5% under 3yr, 25.7% over 5yr. The respondents were aged twenties(29.7%) to thirties(64.4%) who have graduated from college(75.2%) or graduate school(24.8%) and have had the residence period of less than three-(52.5%) to more than five years(25.7%). 2. The kind of cuisine chosen for dinner were Korean style food. The most considerable person when preparing menu were husband. 48.51% of those in age were 3∼39yr(p < 0.1). 60.40% of the response graduated college(p < 0.1). The Korean-style foods overwhelmed over western ones in their frequency for dinner(82.2% vs. 17.8%). The person most considered for choosing menu was their husbands as responded by the majority groups of aged thirties(48.5%), college graduates(60.4%) and residence period of less than three years(42.6%). They satisfied comparatively when they prepared the meal (p < 0.1). 55.4% of the response learned cooking method from neighbor, 39.6% of remainder studied from cuisine book, newspaper, magazine, TV. They satisfied comparatively for the preparation of the meal(56.4%) and have learned the new methods of cooking from neighborhoods(55.4%) or cuisine book, newspaper, magazine and TV(39.6%). 3. The frequency of eating-out mostly had once a month. 30.69% of response were 30∼39yr(p < 0.1). 33.66% of subjects lived under 3yr in America. Eating-out once a month occupied 30.7, 33.7, and 20.7% of the selected group of aged thirties, college graduates, and residence periods of less than three years, each respectively. The type of food selecting eating-out were western style. 38.31% of them were 30∼39yr(p < 0.1). 39.60% of them graduated college(p < 0.1). 29.70% of them lived under 3yr in America. For eating-out menu, the western-style foods were most favored by the groups of aged thirties(38.3%), college graduates(39.6%), and residence periods of less than three years(29.7%). 4. 50.5% of subject had beef once three days, 23.8% of the response had pork once two weeks, 27.7% of subject ate chicken once two weeks, 34.7% of the response enjoyed fish once a week. Eating animal foods once three days for beef(50.5%), once two weeks for pork(23.8%) and chicken(27.7%), and once a week for fish(34.7%) were most frequent. 5. Korean traditional fermented food were mostly enjoyed in order of kanjang(91.1 %), kochujang(87.2%), fermented fish(85.1%), deonjang(81.2%), jang-a-chi(76.2%). 81.2% of subject used prepared-food by themselves. 56.4% of response made kimch at home. Among Korean traditional fermented foods, kimchi(56.4%) and prepared side dishes(81.2%) were provided at home while kanjang(91.1%), kochujang(87.2%), fermented fish(85.1%), deonjang(81.2%), and jang-a-chi(76.2%) were mostly purchased from stores.

식사 전후의 사진 비교를 통한 스마트폰 앱의 영양소섭취량 타당도 평가 (Validation of nutrient intake of smartphone application through comparison of photographs before and after meals)

  • 본 연구는 만 19세 이상 60세 미만 성인남녀 98명을 대상으로 스마트폰 앱인 Gene-Health 이용하여 식사 기록을 통해 분석된 영양소섭취량과 동일한 날의 식사 섭취 전과 후의 사진비교를 통해 섭취량을 추정하여 분석된 영양소섭취량을 비교함으로 Gene-Health의 타당도를 조사하기 위해 수행되었다. 주요 결과는 다음과 같다. 첫째, Gene-Health의 영양소섭취량과 사진을 통해 추정한 영양소섭취량을 비교한 결과 에너지, 탄수화물, 지방, 지방으로부터의 에너지 섭취비율은 통계적으로 유의한 차이가 없었으나 단백질 섭취량과 단백질로부터의 에너지 섭취 비율은 Gene-Health가 높았고, 탄수화물로부터의 에너지 섭취비율은 사진추정군이 높았다. 둘째, Gene-Health와 사진을 통한 영양소섭취량의 상관성은 에너지, 탄수화물, 단백질, 지방섭취량과 탄수화물 비율, 단백질 비율, 지질 비율은 모두 상관계수 0.382-0.708로 유의적인 양의 상관관계를 보였다. 셋째, Gene-Health와 사진을 통한 에너지, 탄수화물, 단백질, 지방섭취량과 탄수화물 비율, 단백질 비율, 지질 비율의 가중 카파 계수는 0.588-0.662로 상당히 일치하는 경향을 보였다. 에너지와 다량영양소, 다량영양소 섭취 비율의 same agreement는 41.8%-48.0%이며 adjacent agreement는 75.5%-88.8%였다. 본 연구를 통하여 Gene-Health는 에너지와 다량영양소 섭취량을 추정하기 위한 타당한 도구라고 사료된다. 추후 연구에서는 다양한 연령과 여성 참가자를 확대하여 성별과 연령에 따른 Gene-Health의 타당도를 연구할 필요가 있다.

Cosmetic Outcomes and Quality of Life in Thai Women Post Breast Conserving Therapy for Breast Cancer

  • Purpose: To evaluate the correlation between cosmetic outcome (CO), body image, and quality of life in post breast-conserving therapy (BCT) women. Materials and Methods: This cross-sectional study concerned one-year post-completed BCT Thai women. The data included subjective and objective CO with a questionnaire covering demographic and clinical data, anti-hormonal treatment status, Eastern Cooperative Oncology Group (ECOG) performance status, Self-Reported Cosmetic Outcomes (SRCO), Self-Reported Breast Symmetry (SRBS), Body Image Scale (BIS), and the Functional Assessment of Cancer Therapy with Breast Cancer subscale (FACT-B). Participants had breast photographs taken for the evaluation of objective cosmetic outcome (OCO) after breast cancer conservation treatment. The relationship between CO and FACT-B was tested using Spearman's rank correlation Results: A total 127 participants volunteered for the study. The participant characteristics were age 52(${\pm}9$), Buddhist 87%, married 65%, body mass index 25.0(${\pm}4.6$), breast cup size A-C 91%, college educated 60%, employed 66%, ECOG 0-1 95%, tumor size less than or equal to 2 cm 55%, no lymph node metastasis 98%, and taking tamoxifen 57%. Two percent of the participants regretted their decision to undergo BCT. The SRCO was excellent in 2%, good in 68%, fair in 30%, and poor in 0%. For SRBS, rates were 17%, 58%, 24% and 1% for excellent, good, fair and poor cosmetic outcomes, respectively. The BCCT scores were excellent 24%, good 39%, fair 32%, and poor 6%. The median total QOL score of the participants was 130 (93-144). There was no significant correlation between CO and FACT-B scores. Conclusions: The significance of CO for FACT-B in Thai women with breast cancer could not be assessed in detail because of a very low level of correlation. The results may be due to the effects of cultural background.

임플란트 환자의 일반적 특성 및 구강관심도에 따른 기능별 만족도 연구 (Study for general characters of dental implant patients and satisfaction level of function by dental interest level)

  • For this study, self-completion survey for 149 implant patients of dental clinics and hospitals in Seoul and Kyunggi areahas been executed in order to provide basic data for increasing patient satisfaction level by effective consultation and various information for the patients who want dental implantation. The conclusion of the study is like below. 1. In terms of demographics of the study, sample size of female patients(n=85) was more than that of male patients(n=64) and patients in their 50s were major age group. When it comes to the educational level, university graduates were 85 and high school graduates were 44. And 132 were married and 6 were single. In the income level, monthly income between KRW 2~4 million was major group(n=59). 2. In the study, patients' most concern was complication or adverse events(n=41), and price(n=39) & annual dental check-up(n=49) were following it. And 80 patients responded that dental health is the most valuable(n=80) and whole body health was following(n=37). In terms of knowledge on dental implant, 100 patients responded 'know just a little', 31 and 13 answered 'generally know' and 'well know' each. 3. As a result of analyzing implant patients' satisfaction level by segment, general satisfaction rate was highest as 4.20, social satisfaction rate was 4.20, psychological function was 4.06 and masticatory function was 3.90. 4. Based on the result of comparing patient satisfaction level by general characters of dental implant patient, chewing function was the highest in the age group of 60s with score of 4.21 and in terms of social function, the group with over KRW 8 million of monthly income showed highest score with 4.54. When it comes to the psychological function, patients in their 70s showed highest satisfaction level with 4.52. In the end, patients in their 70s showed highest score with 4.81 in the overall satisfaction and it was also statistically significant. 5. In the overall satisfaction level by interest in dental health, appearance showed highest score with 4.57 and it was also significantly higher than that of whole body. In the variance by dental knowledge, 'well know' group showed highest score with 4.58, and that of 'just a little' group was 4.33. That means there are correlations between dental knowledge and satisfaction level. 6. As a result of analyzing correlations amongst variances of the study, there were strong positive correlations among them. And there were very significantly positive correlations especially between social and psychological function and between psychological and overall satisfaction level. In conclusion, higher psychological function has strong relations with both social function and overall satisfaction level(p<.001).

일부 중년여성의 식습관이 인지된 건강관련 체력에 미치는 영향 -창원지역을 중심으로- (The Impacts of Dietary Habits on Self-perceived Health-related Physical Fitness in Middle-aged Women -Focused on Changwon Province-)

  • 본 연구에서는 45~60세 중년여성을 대상으로 인지된 건강관련 체력 수준에 따라 대상자를 분류하여 그룹 간에 식습관 및 생활습관을 비교하고, 식습관이 인지된 건강관련 체력수준에 미치는 영향을 알아보고자 하였다. 조사 대상자의 평균 나이는 48.5세로 인지된 건강관련 체력 수준이 '높음' 군의 제지방량이 유의적으로 더 많았다(P<0.05). 혈중지질성분 중에서는 중성지방만이 체력 수준에 따른 유의적인 차이를 보였는데, 인지된 건강관련 체력 수준이 '낮음' 군에서 유의적으로 더 높았다(P<0.05). 생활습관 특성을 분석한 결과 인지된 건강관련 체력 수준이 '높음' 군에서 활력 수준(P<0.001), 주관적인 건강상태(P<0.001), 정기적인 건강검진 여부(P<0.05), 규칙적인 운동 실천(P<0.05) 항목 모두 유의적으로 높았다. 평상시 식습관에 있어서도 인지된 건강관련 체력 수준이 '높음' 군의 식습관 평균 점수가 유의적으로 높았으며(P<0.05), 개별 항목에서도 '일정한 식사 시간(P<0.05)', '일정한 식사량(P<0.05)', '식사 시 즐거움(P<0.01)', '단백질 섭취 빈도(P<0.05)', '채소 섭취 빈도(P<0.01)', '식욕', '적정한 에너지 섭취(P<0.05)' 항목의 점수가 인지된 건강관련 체력 수준이 '높음' 군에서 유의적으로 높았다. 또한 식습관은 교란 인자를 보정한 회귀분석 결과에서도 인지된 건강관련 체력과 유의적인 양의 상관성(P<0.001)을 보였다. 이상의 결과로 평상시 바람직한 식습관이 인지된 건강관련 체력에 긍정적인 영향을 미칠 수 있으며, 중년 여성의 체력 향상을 위해 건강한 식습관의 실천이 필요할 것으로 보인다. 또한 이를 위해 중년 여성의 식생활에 관심을 갖고 올바른 식생활을 실천할 수 있도록 관련 프로그램과 교육의 기회가 마련되어야 할 것으로 사료된다. 본 연구에서 식습관이 인지된 건강관련 체력과 유의적인 관계가 있다는 결과는 그 자체로 의미가 있다고 생각되나 18개 식습관 문항의 평균점수를 독립변수로 처리하여 회귀분석 하였으므로 개별 식습관이 체력수준에 미치는 영향은 보지 못한 연구의 한계점이 있어 추후 분석이 더 요구된다.

산모와 배우자의 태교인식과 실천에 관한 연구 (A Study on the Cognition and Practice of the Delivered Woman and Her Husband for the Fetal Education)

  • The purpose of this study was to perform the fetal education effectively for the delivered woman and her husband. This study was conducted by the questionnaire survey on 199 delivered women and their 171 husbands at several hospitals located in Seoul and Kyonggi Province from February 28 to March 26, 2002. The contents of questionnaire included the purposes, the cognition and the practice of fetal education. The SAS program was used for the statistical analysis of the data. The character of subject was analyzed by the percentage. The difference between the cognition and practice of fetal education was analyzed by t-test, ANOVA, Wilcoxon rank sum test. The factor analysis affected on the practice of fetal education was adopted by Multiple regression. The results were as follows; 1. In the purpose of fetal education, the well- balanced emotion showed the highest mark. The cognition of fetal education(woman:$4.39{\pm}0.52$, husband:$3.88{\pm}0.70$) and the practice (woman:$3.88{\pm}0.60$, husband:$3.83{\pm}0.70$) showed the relatively high mark, but the score of cognition showed higher than that of practice. 2. In the comparison of the cognition between the delivered woman and the husband for the fetal education the item of the mental or health state would affect unborn baby, which occupied the highest mark in both woman and the husband(woman:$4.81{\pm}0.44$, husband:$4.81{\pm}0.50$). But they were prohibited to eat the deformed food, which showed the lowest mark(woman:$3.19{\pm}1.12$, husband:$3.21{\pm}1.29$). 3. In the comparison of the practice for the fetal education between the delivered woman and the husband, the practice for the healthy baby showed the highest mark in woman ($4.51{\pm}0.71$), which had a statistically significant difference(P=0.025), compared with that of the husband($4.13{\pm}0.99$). 4. In the comparison of cognition and practice for the fetal education, the general character was associated with the duration of marriage, the satisfaction with marriage and the support of husband on pregnancy. The mark was associated with the age of woman, the level of education and the first birth. 5. The significant factors influencing on the practice for the fetal education were connected with the cognition of fetal education, age, satisfaction with marriage, the support of husband on pregnancy, the type of family, the experience of delivery and the state of health during the period of pregnancy. etc. In conclusion, it is indicated to make effort for transforming and developing the traditional fetal education in accordance with the modern fetal education. And it is suggested that the fetal education might be recognized by all members of family, and the importance of husband's role for the fetal education should be informed as well as that of woman's.
