Nutritional Assessment of the Oldest-Old Population Aged 85 Over: Based on 2009 Korea National Health and Nutrition Examination Survey

  • As Korea is expected to become super high aged society in 2026, nutrition is important elements of health in the elderly and affects their life. In epidemiological surveys, anthropometric and biochemical measurements represent important components of nutritional assessment in elderly. In the present study, we investigated anthropometric index, biochemical markers, and nutrient intake for elderly aged over 85 by using data on 71 elderly (24 men, 47 women) over 85 years which was obtained from Korea National Health and Nutrition Examination Survey in 2009. According to BMI values, the prevalence of underweight and obesity was 9.7% and 34.5% in men, and 7.9% and 26.4% in women. The prevalence of sarcopenia was 70.7% in men and 25.0% in women. However, plasma biochemical data are normal range in the older population. In nutrients intake, daily Intake of energy, protein, dietary fiber, water, thiamin, riboflavin, niacin, calcium, and potassium was inadequate considering the Dietary Reference Intakes for Koreans(KDRIs). In conclusion, elderly need to be educated nutrition and their health dietary life to prevent malnutrition, and standard of nutrition intake should be rearranged in elderly aged 85 over considering their antrhopometric index.

고혈압 인지율의 평가와 연령별 특성 (Hypertension awareness and the related factors by age)

  • Objectives: This study examines the awareness of hypertension and its age-specific characteristics among Korean adults. Methods: Data were derived from the fourth Korean National Health Examination and Nutrition Survey (KNHANES IV, 2007-2009) of 14,637 subjects aged 30 and older. Self-reported prevalence of hypertension (HTN) was compared with measured arterial systolic/diastolic blood pressure, employing Kappa index, sensitivity, and specificity. Multiple logistic regression analysis was used to determine factors in hypertension awareness by age groups. Results: Kappa agreement between self-reported and measured data in hypertension was high (0.70, 95% CI 0.68-0.71). Awareness by sensitivity was relatively high (65.3%) and specificity was very high (98.8%) for all ages. After age stratification, the results revealed the very lowest level of hypertension awareness among the youngest age group: 29.1% of adults aged 30-49 years, 57.5% of adults aged 45-59 years, and 76.9% of those aged ${\geq}60years$. Predictors for awareness differed by age group. Among adults aged 30-49 years, men exhibited lower awareness to HTN than women. Among middle-aged adults, workers showed lower awareness than non-workers. Among those aged ${\geq}60years$, men and the less educated showed lower awareness to HTN than their counterparts. Conclusions: Age-specific prevention strategies are needed for hypertension management.

한국 성인 여성에서 연령에 따른 혈중 염증 표지자와 심혈관계 질환 위험 요인에 대한 연구 (Age-related Circulating Inflammatory Markers and Cardiovascular Disease Risk Factors in Korean Women)

  • The purpose of this study was to investigate the age-related changes of cardiovascular disease risk factors and inflammatory markers in non-obese Korean women. Subjects were 112 women over 20 years old with body mass index (BMI) less than $30 kg/m^2$ and were divided into 3 groups (< 40 years, $40{\sim}59$ years, ${\ge}60$ years). Mean weight and BMI in the oldest group were significantly higher than those in the other 2 younger groups (p < 0.05). Mean total cholesterol, triglyceride, LDL-cholesterol and apolipoprotein B/apolipoprotein A1 ratio (BAR) in the oldest group were significantly higher than those in the youngest group (p < 0.05), and mean HDL-cholesterol of the oldest group was significantly lower than that of the youngest group (p < 0.05). The older-aged group showed significantly higher mean values of atherogenic index (AI) and LDL/HDL ratio (p < 0.05) than the respective younger-aged group, and AI was significantly correlated with age, nitric oxide and thiobarbituric acid reactive substances (p < 0.01). In addition, mean vascular cell adhesion molecule-l (VCAM-1) tended to be higher in the older-aged group than the younger group. Tumor necrosis factor-${\alpha}$, a proinflammatory maker, was significantly positively correlated with serum homocysteine, a cardiovascular disease risk factor (p < 0.01). In addition, a significantly positive correlation was observed between C-reactive protein and BAR (p < 0.01). Overall results suggested that the aging might affect the increase of cardiovascular disease risk factors including the serum lipid profiles, weight and BMI, and age-related increases of weight and BMI might play a role in changes in certain biomarkers of inflammation. A Study on the Economic Preparation of Retired Women's Old Life

  • This study was conducted on 878 retired women in their 50s and 60s across the country using the 7th data from the National Pension Service's KReiS. We used SPSS WIN 18.0, and the analysis results are as follows. First, there were many highly educated people in their 50s or older and those in their 60s and undereducated. Second, families in their 50s had the largest number of members with more than three, while households in their 60s had the largest number of two. Third, both age groups chose themselves and their spouses as responsibilities for preparing for retirement. In addition, more people in their 60s chose the government as their responsibility for preparing for retirement than in their 50s. Both people in their 50s and 60s say they are "not prepared" to prepare for retirement expenses, raising concerns about elderly poverty. Fourth, economic strength, health, and medical care were important for retirement in both age groups as part of their preparations for retirement, and they chose economic strength, health, medical care and job as the things to do in society. Fifth, both people in their 50s and 60s have very low public and private pension subscription rates, requiring special attention from the government and society to their old age. Judging from the above results, both women in their 50s and 60s have retired, but preparations for retirement are very insufficient. Therefore, the government and society need to strengthen the public pension system and create jobs for the elderly.

연령증가에 따른 여성의 대퇴사두근과 슬괵근 근력의 변화 (Changes of Quadriceps and Hamstring Strength Ratio in Women of Different Ages)

  • The purpose of this study was to examine the isokinetic moment of quadriceps and hamstring strength ratio among women of different ages. The study population consisted of 1,184 women referred to the Health Promotion Center at the S district. All subjects were aged 20 to 69 years old and divided into 5 groups; 20s (n=248), 30s (n=255), 40s (n=248), 50s (n=228), and 60s (n=205). The strength of the knee extensor and flexor, quadriceps, and hamstring of all the participants were assessed at 60 degrees/second with an isokinetic machine. We calculated the peak torque, peak torque %BW (%Body Weight), deficit of peak torque and hamstring/quadriceps ratio of the knee. The data were analyzed by one way ANOVA to investigate statistical differences in strength variation between different age groups and were computed by ${\Delta}%$ difference from women in their 20's. The results were obtained as follows: 1. Peak torque of the knee extensor, quadriceps, were significantly reduced in women older than 30, but peak torque of the knee flexor, hamstring, were significantly reduced in women older than 50 compared to women in their 20's. (p<.05). 2. Peak torque %BW of the knee extensor, quadriceps, were significantly reduced in women older than 20, but peak torque %BW of knee flexor, hamstring, were significantly reduced in women older than 40 compared to women in their 30's (p<.05). 3. Compared to the women in their 20's, there was no significant difference among any of the age groups in the deficit of peak torque of the knee extensor and flexor, but the deficit of peak torque of knee extensor among women between 30 and 50showed significant difference within the normal range of deficit. 4. Compared to the women in their 20's, there was no significant difference among any of the age groups in the hamstring/quadriceps ratio These results showed that peak torque, peak torque %BW, deficit of peak torque, and hamstring/quadriceps ratio of the knee were reduced in each age group, but especially among the women over 50. Further longitudinal study may be needed to see if volume of muscle mass and intervention of exercise affect knee strength in spite of aging.

Prevalence of pre-obesity and above and its associated factors in adult women: an analysis of the 2020 Korea National Health and Nutrition Examination Survey

  • Purpose: This study was conducted to determine the prevalence of pre-obesity (overweight) and above in adult women and to identify associated factors. Methods: Data were obtained from the eighth Korea National Health and Nutrition Examination Survey (KNHANES VIII-2), conducted in 2020. The sample comprised 2,288 women aged 19-64 years who participated in the KNHANES VIII-2. Data were analyzed using complex sample design analysis with SPSS version 20.1. Results: The prevalence of pre-obesity and above among adult women was 46.5%, with 18.6% classified as having pre-obesity and 27.9% as having obesity. A higher prevalence of pre-obesity and above was observed in women aged 50-59 years (odds ratio [OR]=1.67, p=.019) or 60-64 years (OR=1.80, p=.029); women whose highest educational attainment was high school (OR=1.28, p=.018) or middle school or less (OR=1.60, p=.017); those in middle-income households (OR=1.55, p=.005); those engaging in muscle-strengthening activities less than 2 days per week (OR=1.37, p=.019); and those sleeping less than 6 hours per night during the week (OR=1.37, p=.025). Conclusion: As nearly half of all adult women have either pre-obesity or obesity, prevention and management strategies must target both groups. Interventions should be prioritized for women in their 50s and older, as well as those with low education or income levels. Additionally, receiving adequate sleep of 7 hours or more and engaging in muscle-strengthening activities at least 2 days per week are important components of obesity management.

재가 고령여성의 실내복 개발을 위한 디자인 선호에 관한 조사 및 디자인 제안 (A survey on design preference for the development of indoor wear for elderly women at home and suggestion of design items appropriate for them)

  • In this study, a survey focusing on the status of clothing interest, inconveniences resulting from clothing, preferred design items, etc. was conducted on 364 elderly women to suggest aesthetically and functionally appropriate indoor wear design for at home elderly women aged 60 years or older. The survey results showed that in general, the respondents' interest in clothing was high, and more respondents in their 70s or older had difficulty in the action of opening and closing. With respect to considerations when purchasing clothes, color was considered more important than design as respondent's age increased, and size was regarded as the most important factor especially among those in their 80s. The preferred top styles were T-shirts and blouses among those in their 60s and 70s, and T-shirts and shirts among those in their 80s. The preferred sleeve lengths were "below the elbow" and "above the wrist" in all age groups. The preferred sleeve hem type was "tightening" in all age groups. The most preferred bottom styles were "straight-leg pants" and "elastic waistband." This study suggests the design items of indoor wear, including top, bottom, and overgarment for warmth, appropriate for elderly women at home based on the survey results. The study results are expected to serve as basic data necessary for the revitalization of the clothing industry for elderly women.

한국인 60세 이상 노년층의 황반변성과 백내장의 위험요인 -국민건강보험공단 노인코호트 자료를 활용하여- (Risk Factors Associated with Cataract and Macular degeneration by in Korean aged 60 years and over)

  • 연구목적: 60세 이상 인구에서 나이 관련 안질환인 백내장과 황반변성의 위험요인을 알아보고자 한다. 연구방법: 건강보험공단 노인코호트(2002년-2013년)자료를 활용하여. 연구기간동안 노년층의 대표 안질환인 백내장(ICD-10; H25)과 나이관련 황반변성(ICD-10; H353)을 진단받은 341,588명을 대상(남: 44.18%, 여:61.887%)으로 두 질환의 유병률 결정 요인을 Cox 확률비례위험모형을 이용하여 분석하였다. 분석결과: 백내장과 나이관련 황반변성 유병율은 여성이, 나이가 많을수록, 고소득 집단일수록, 고혈압, 심장질환, 당뇨가 있으면 더 높게 나타났다(p<0.0001). 결론: 60세 이상 노인들은 고혈압, 심장질환 및 당뇨와 같은 만성질환이 많을수록 백내장과 황반변성 유병률이 더 높았다. 눈이 건강한 고령화 사회를 만들기 위해서는 노인들의 만성질환 관리가 필수적으로 요구된다.

한국 여성 노인의 대퇴경부 및 요추 골밀도 관련 요인 (Factors Associated with the Lumbar Spine and Femoral Neck Bone Mineral Density in Korean Elderly Women)

  • 본 연구는 국민건강영양조사 제 4기 2 3차년도, 제 5기 1차 년도에 참여한 60세 이상 여성노인을 대상으로 하였다. 연구의 목적으로는 신체계측, 생활습관, 식이섭취, 골절 과거력, 골다공증 가족력, 병력, 월경력 및 산과적 요인 등의 제 요인들과 대퇴경부 골밀도와 요추골밀도의 관련성을 알아보고자 시도하였다. 각 변수의 단위별 BMD의 변화를 표현하기 위해 비율차이 계산식 ${\beta}{\times}$(단위/BMD의 평균)를 이용한 다중선형회귀분석을 실시하였다. 모든 연속 변수의 단위는 1 표준편차를 사용하였다. 연구결과, 대퇴경부 및 요추 골밀도에 조사된 각 독립변수 중 여성노인의 골밀도에 가장 큰 영향을 미치는 것은 대퇴경부 부위에서는 제지방량(B: 0.257), 요추 부위에서는 체지방(B: 0.237)으로 확인되었다. 따라서 여성노인에서는 제지방량을 늘리고, 적절한 체중을 유지하는 것이 골다공증을 예방하는 효과적인 방안이 될 것이다.

Effects of Sarcopenic Obesity on Metabolic Syndrome in Korean Elders: Using Data from the Korea National Health and Nutrition Examination Survey (2008-2011)

  • Purpose: This study was conducted to examine effects of sarcopenic obesity on metabolic syndrome in Korean elders. Methods: This study is based on the analysis of the Korea National Health and Nutrition Examination Survey (KNHANES) with 1,155 subjects (524 men, and 631 women) aged 60 or older, from 2008 to 2011. Sarcopenia was defined as an appendicular skeletal muscle (ASM), divided by weight (%) of <1 SD (standard deviation) below the sex-specific mean for young adults. Obesity was defined as a total body fat percent (men${\geq}$25%, women${\geq}$35%). Results: The prevalence of SO (sarcopenic obesity) was 13.3% among men and 22.5% among women. Both sexes showed a higher total body fat percent, and the SMI (skeletal muscle index) was the lowest in the SO group. Metabolic syndrome was highly prevalent in the SO group (52.5% men, 60.4% women). The SO group showed a higher risk for metabolic syndrome (odds ratio men 6.57 [95% CI 5.19~7.27], women 3.89 [95% CI 2.41~6.29]) than the obese group (men 3.14 [95% CI 1.76~4.14], women 2.54 [95% CI 1.38~4.65]). Conclusion: SO is a major risk factor for metabolic syndrome in Korean elders. Therefore, a nursing program should be given to the Korean elderly SO group to prevent metabolic syndrome.
