Effects of the exposure site on histological pulpal responses after direct capping with 2 calcium-silicate based cements in a rat model

  • Objectives: Direct pulp capping is a treatment for mechanically exposed pulp in which a biocompatible capping material is used to preserve pulpal vitality. Biocompatibility tests in animal studies have used a variety of experimental protocols, particularly with regard to the exposure site. In this study, pulp exposure on the occlusal and mesial surfaces of molar teeth was investigated in a rat model. Materials and Methods: A total of 58 maxillary first molars of Wistar rats were used. Forty molars were mechanically exposed and randomly assigned according to 3 factors: 1) the exposure site (occlusal or mesial), 2) the pulp-capping material (ProRoot White MTA or Bio-MA), and 3) 2 follow-up periods (1 day or 7 days) (n = 5 each). The pulp of 6 intact molars served as negative controls. The pulp of 12 molars was exposed without a capping material (n = 3 per exposure site for each period) and served as positive controls. Inflammatory cell infiltration and reparative dentin formation were histologically evaluated at 1 and 7 days using grading scores. Results: At 1 day, localized mild inflammation was detected in most teeth in all experimental groups. At 7 days, continuous/discontinuous calcified bridges were formed at exposure sites with no or few inflammatory cells. No significant differences in pulpal response according to the exposure site or calcium-silicate cement were observed. Conclusions: The location of the exposure site had no effect on rat pulpal healing. However, mesial exposures could be performed easily, with more consistent results. The pulpal responses were not significantly different between the 2 capping materials.

Development of dental zirconia (치과용 지르코니아의 발전)

  • Since the end of the 1990s, zirconia, which has been applied to dental prostheses, has been used in the form of coping or framework. The prostheses made in this way have been successfully used in the anterior teeth because they have no metal components as a structure. But in the posterior teeth, its use was limited due to the fracture or chipping of the veneering porcelain during the function. Later, as a solution to this shortcoming, a monolithic zirconia restoration was proposed in which a strong coping material was used in the form of a complete prosthesis. But, this resulted in some unesthetic results due to the characteristics of zirconia, which has excellent resistance to strong forces but is very white and opaque. However, now, due to technological advances in increasing the translucency of zirconia, it is possible to produce strong and esthetic zirconia restorations not only in the posterior region but also in the anterior region. In this article, the structural characteristics of various types of zirconia materials that have been developed so far, were discussed, and where the zirconia products actually in use belong to was explained.

Evaluation of the wear of the periodontal curet's cutting edge (치주 큐렛의 절단 연 마모도 평가)

  • The quality of periodontal instrument cutting edge is a basic element of effective root planing procedure. Using instruments, the sharp edge is changed into blunt or beveled edge. With the blunt instrument, the periodontal treatment can't be carried into accuracy and effective. The study on the wear of periodontal curet is insufficient, there are few publications about the change of sharpness of cutting egde after using instrument and a certen reports were published on the study of scanning electron microscope(SEM) examination. In this study, to declare the number of strokes for sharpening of instruments, the changes of cutting edge is measured by the clinical methods, tactile sensitivity examination and refraction light-white line test after scaling strokes and root planing strokes. SEM test was added for defined the changes of cutting edges. The 7/8 Gracey curets that have been never used was tested. Maxillary molars which were extracted from the School of Dental Medicine, Dankook University was used. Subjected teeth had attachment loss more than 6 mm in bucca-lingual surface and sufficient calculus of a band type in cervical area. The strokes of curet were executed 3, 5, 7, 9, 11, 13 times on scaling stroke and 10, 15, 20, 25, 30, 35 times on root planing stroke. A resident has periodontal experience over 3 years carried out the clinical examinations those tactile sensitivity examination and refraction light-white line test 5 times. The case there being tactile sensitivity certenly is 2, the case being felt tactile sensitivity is 1, and the case there not being tactile sensitivity is 0. The visual examination was recorded as following. The case that refracted white line is not recognised is 2, the case that uncerten is 1, and the case that acknowledged is 0. The results were obtained as follows. 1. After scaling strokes, the tactile sensitivity was reduced after 11 strokes and disappeared in 13 strokes. 2. In tactile sensitivity after root planing procedures, sensitivity was reduced after 25 strokes and disappeared in 35 strokes. 3. In case of visual examination, the detection of refracted white line was increased after 9 strokes of scaling procedures and the accuracy of wear wasn't showed after root planing procedures. 4. In SEM, metal projection was observed on new periodontal curet cutting edge and it was disappeared after scaling procedures. 5. In SEM, the cutting edge was showed changing linear into an aspect of the surface after 5 strokes of scaling procedures and 10 strokes of root planing procedures and showed beveled edge in 11 strokes of scaling procedures, 25 strokes of root planing procedures. The results of 3-type examination indicated that the sharpening of curet should be performed after 11 strokes of scaling procedures and 25 strokes of root planing procedures.

POSITIONAL RELATIONSHIP OF THE MANDIBULAR CANAL AND IMPACTED THIRD MOLARS BY USING DENTAL CONE BEAM COMPUTED TOMOGRAPHY (제3대구치와 하악관과의 위치 관계에서 치과용 Cone beam CT의 유용성)

  • We sought to evaluate the relationship between the mandibular canal and impacted mandibular third molars by using dental cone beam computed tomograph(CBCT) for third molar surgery. A total of 111 patients(177 teeth) offered the images through CBCT and panoramic radiography for the extraction of the mandibular third molars. In CBCT, the accurate relationship between the third molar and the mandibular canal were evaluated. In panoramic radiographies, we evaluated the impacted level and superimposition sign of the mandibular third molar with the mandibular canal, and also, the radiopacity of the white line in the canal. Data were statistically analyzed and estimated by $X^2$-test. In CBCT finding, high prevalence of contact between the mandibular canal and roots occured in the deep impacted third molars, narrowing mandibular canals, bending mandibular canals and cases where the radiopacity of white line of canals were "absence" on panoramic images. It showed statistical significance (P<0.05). When evaluating the mandibular canal and the roots through the panoramic radiography for third molar extraction, it could be difficult to diagnosis accurately. Thus, it is required to have an accurate diagnostic approach through CBCT that could evaluated the location between mandibular canal and root.

WHITENING EFFECT OF BLEACHING AGENTS ON TETRACYCLINE-STAINED RAT TEETH (테트라싸이크린에 의해 변색된 백서 치아에 대한 표백제의 표백 효과)

  • 본 연구는 테트라싸이클린에 의해 변색된 백서 치아에 대한 3 종 표백제의 표백효과를 비교하고 표백제의 침투 깊이 (법랑질 두께)에 따른 표백효과의 차이를 판별하기 위해 시행되었다. 30 마리의 웅성 백서에 매일 2주 동안 테트라싸이클린 용액을 복강내 주입하였다. 디스크 형태의 32개 백서 치아 표본을 16 시간 동안 자외선으로 조사하여 변색을 야기시켰다. 대조군은 생리 식염수에 보관하였으며 실험군은 3종 표백제(Opalescence, Rembrandt, NiteWhite)에 하루 5회씩 2주간 노출시켰다. 치아 색상을 표백전, 표백 1일, 3일, 5일, 7일, 9일, 11일, 및 14일에 각기 기록하여 치아내 변색부와 정상 색상간의 색상차 (${\Delta}$E)를 판독하였다; 대조군 - 9.78, 9.17, 9.36, 9.65, 9.40, 9.99, 10.57, 11.36; Opalescence - 10.08, 7.63, 6.72, 6.04, 5.10, 4.87, 4.89, 4.27; Rembrandt -9.83, 11.27, 9.55, 8.36, 7.75, 6.94, 7.11, 7.04; NiteWhite - 10.44, 9.92, 7.58, 6.80, 5.45, 5.05, 4.73, 4.01. 표백된 모든 치아들의 색상이 밝아졌다. (p<.01) 테트라싸이클린이 주입된 56개 백서 치아를 3 일간 자외선에 조사한 다음, 3가지 유형의 표백제 침투깊이를 설정하였다. 설측 상아질과 순측 법랑질을 통한 침투 (DN), 순측 법랑질만을 통한 침투 (RE), 1.0 mm 두께의 사람 법랑질로 피개된 순측 법랑질을 통한 침투 (HE). 표본들을 Opalescence로 하루 5회씩 1주일 및 4 주간 표백한 후 횡단하여 치아 색상을 측정하였다. 표백되지 않은 치아들을 대조군으로 이용하였다. 치아내 변색부와 정상 색상간의 색상차 (${\Delta}$E)는 다음과 같았다. 대조군 - 11.67; 1주 DN 군 - 13.55; 1주 RE군 - 12.80; 1 주 HE군 - 12.07; 4주 DN군 - 7.48; 4주 RE군 - 7.50; 4주 HE군 - 11.69. 4주 DN군과 4주 RE군의 색상 변화가 가장 크게 나타났다.(p<.01)

The effects of cement thickness and cement shade on the final color of lithium disilicate crowns

  • PURPOSE. The aim of this study is to evaluate the effect of resin cement color, cement thickness, and thermocycling on the final color of monolithic lithium disilicate crowns. MATERIALS AND METHODS. A total of ninety prepared central incisors of typodont teeth were restored with lithium disilicate crowns which have different cement thicknesses (40 ㎛, 80 ㎛, and 120 ㎛) and cement shades (clear, yellow, and white). Color parameters of restorations were measured with a spectrophotometer in three different steps 1) before cementing 2) after cementing and 3) after thermocycling with 10000 cycles. Color differences (ΔE00) were calculated with the CIEDE2000 formula and evaluated according to perceptibility (0.8) and acceptability (1.8) thresholds. The ΔE00 data were analyzed by using two-way ANOVA before and after thermocycling (α = .05). RESULTS. There was no interaction between the cement shade and the cement thickness factors. After cementation, the mean ΔE00 was under the perceptibility threshold in the group of 40 ㎛ cement thickness and clear cement while it was between the perceptibility and acceptability thresholds (0.8 < ΔE00 < 1.8) for all other groups. After thermocycling, the ΔE00 values were between the perceptibility and acceptability thresholds for all experimental groups. Although there were no significant differences among the groups, thermocycling increased the color difference values. CONCLUSION. The cementation of restorations with clear, yellow, and white resin cements resulted in color differences with uncemented restorations except for the group cemented with clear cement in 40 ㎛ cement thickness. All study groups revealed perceptible color change after thermocycling.

Laboratory model to evaluate efficacy of an experimental titanium oxide nanofibers bleaching agent

  • Objectives: This study aimed to use a laboratory model to evaluate the efficacy of an experimental bleaching agent. Materials and Methods: The model used human extracted molars that were treated and measured for bleaching efficacy. Teeth (n = 50) were distributed into 5 groups: Negative control (NC): immersion in water for 8 hours; Nanofibers (NFs): Experimental titanium dioxide nanofibers with stirring and light activation for 8 hours; Whitestrips (WS): Crest 3D White Glamorous White Whitestrips, 2 applications daily for 30 minutes, 14 days; 1% hydrogen peroxide (HP) standard: 1% hydrogen peroxide for 8 hours; and 30% HP standard: 30% hydrogen peroxide for 8 hours. Instrumental measurements were performed using a spectrophotometer. Results were recorded at baseline, 1-day post-bleaching, and 1-week post-bleaching. Kruskal-Wallis procedure was used to determine differences in color change. Pearson correlation was used to evaluate the relationship between visual and instrumental measurements. Tests of hypotheses were 2-sided with alpha = 0.05. Results: There was no significant difference in color parameters (L1, a1, b1, and shade guide units [SGU]) at baseline (p > 0.05). There was a significant difference among the groups for overall color change (ΔE*ab) and change in shade guide units (ΔSGU) at 1-day and 1-week post-bleaching (p < 0.05). The higher the HP concentration, the higher the color change as expressed in ΔSGU and ΔE*ab. The negative control exceeded the perceptibility threshold of ΔE* = 1.2 regardless of time point. NFs showed a decrease in chroma, but were not statistically different compared to the negative control. Conclusions: The laboratory model was successful in screening an experimental bleaching agent.

Microleakage of endodontically treated teeth restored with three different esthetic post and cores (심미적 포스트 코어의 종류에 따른 미세누출에 관한 연구)

  • Statement of problem: At present, as the esthetic demands are on the increase, there are many ongoing studies for tooth-colored post and cores. Most of them are about fiber post and prefabricated zirconia post, but few about one-piece milled zirconia post and core using CAD/CAM (computer-aided design/computer-aided manufacturing) technique. Purpose: The objective of this study was to compare microleakage of endodontically treated teeth restored with three different tooth-colored post and cores. Material and methods: Extracted 27 human maxillary incisors were cut at the cementoenamel junction, and the teeth were endodontically treated. Teeth were divided into 3 groups (n=9); restored with fiber post and resin core, prefabricated zirconia post and heat-pressed ceramic core, and CAD/CAM milled zirconia post and core. After the preparation of post space, each post was cemented with dual-polymerized resin cement (Variolink II). Teeth were thermocycled for 1000 cycles between $5-55^{\circ}C$ and dyed in 2% methylene blue at $37^{\circ}C$ for 24 hours. Teeth were sectioned (bucco-lingual), kept the record of microleakage and then image-analyzed using a microscope and computer program. The data were analyzed by one-way ANOVA and Scheffe's multiple range test (${\alpha}=0.05$). Results: All groups showed microleakage and there were no significant differences among the groups (P>.05). Prefabricated zirconia post and heat-pressed ceramic core showed more leakage in dye penetration at the post-tooth margin, but there was little microleakage at the end of the post. Fiber post and resin core group and CAD/CAM milled zirconia post and core group indicated similar microleakage score in each stage. Conclusion: Prefabricated zirconia post and heat-pressed ceramic core group demonstrated better resistance to leakage, and fiber post and resin core group and CAD/CAM milled zirconia post and core group showed the similar patterns. The ANOVA test didn't indicate significant differences in microleakage among test groups. (P>.05)

Relationships between osteoporosis, alveolar bone density and periodontal disease in postmenopausal women (골다공증과 악골의 골밀도 및 치주 질환과의 상관 관계)

  • The purpose of this study is to determine if a relationship exists among osteoporosis, alveolar bone density and periodontal disease in postmenopausal osteoporotic women and postmenopausal healthy women. Twenty-two women were evaluated for this study. They were attending the postmenopausal clinic, Seoul National University Hospital and generally healthy except osteoporosis. They had experienced menopause not less than one year when we began to examine them. Bone densities of lumbar area(L2-L4) was determined by DEXA(LUNAR-expert Co,. U.S.A). We diagnosed osteoporosis when T-score was below -2.5 and healthy state when T-score was over -1. Osteoporotic(10 female), not hormone-treated group and healthy control group(12 female) were asked for their age, menopausal age, menopausal period and the number of remaining teeth and examined clinically for plaque index(PI), gingival index(GI), clinical attachment loss(CAL) on their 6 Ramfjord index teeth. Intraoral radiographs were taken in maxillary anterior zone. All films were equally exposed and developed. Each films was digitized and analysed using image processing software, Scion image. Alveolar bone regions of interest were selected and Intensity of each pixel was quantized in the array ranging from 0(white) to 255(black). The two groups were comparable with respect age, menopausal age, menopausal period and number of remaining teeth. The osteoporotic women had significantly lower alveolar bone density than controls in maxilla. But no significant difference was found with respect clinical attachment loss, plaque index and gingival index. Supported by the Ministry of Public Health and Welfare, Korea (HMP-00-CH-10-0009).

A STUDY ON THE EARLY DETECTION OF ENAMEL CARIES BY THE LUMINESCENCE EXCITED BY ARGON LASER (아르곤 레이저 광감각법의 법랑질 우식증 조기탐지 효과에 관한 연구)

  • The aim of the present study was to describe an safe and convenient method for the early detection of enamel caries using laser fluorescence. Fluorescence from natually carious lesion of human teeth illuminated by an argon laser(488nm) was observed and photographed using barrier filter. Intact enamel was found to fluorescence with a yellowish light. Whereas, incipient caries lesions in the enamel were dearly visible as dark areas in contrast to the fluorescence surroundings. For evaluation of accuracy of this method, lesion depth measured by the laser fluorescence in light microscope was compared with that polarizing microscope. The results from the present study can be summarized as follows : 1. Enamel caries of smooth surface was observed as pale white spot and undefined outline in ordinary light. Whereas, lesion was clearly visible as dark spot in laser fluorescence. 2. There was no difference between ordinary light view and laser fluorescence in occlusal surface and interproximal surface. 3. There was no significant difference between the lesion depth observed by laser fluorescence with light microscope and polarizing microscope. Apparent correlation exists between two groups.
