Development and Validation of the Measurement Tool of Public Benefits in Regional Cardiocerebrovascular Center (권역심뇌혈관질환센터의 공익성 측정도구 개발)

  • Background: Regional Cardiocerebrovascular Centers (RCC) were established for the prevention and treatment of cardiocerebrovascular disease and funded by the Ministry of Health and Welfare. The purpose of this study was to develop and validate the measurement tool of public benefits in RCC. Methods: Through the intensive literature review, experts surveys and their repetitive feedback, we selected the 46 items about the public benefits in RCC. Development of measurement tool involved content validity test using Content Validity Index (CVI), construct validity test through factor analysis and reliability test. Results: Thirty-five items were selected by content validity test, which CVI was 0.08 or higher. Through the construct validity test, 32 items in 7 factors were derived. And Cronbach's alpha was 0.951. Finally, public benefits measurement tool is composed of 32 items in 7 factors which are comprehensiveness of health care services, market complement, emergency care, cost, governance, quality improvement, and government control. Conclusion: Though we developed the measurement tool of public benefits in RCC, it would be utilized to measure the public benefits of various health agencies.

A Study on the Relationship of Benefits, Quality, and Continuity Intention in Community Services (지역사회 서비스에서 관계혜택과 관계품질, 관계지속의도와의 관계연구)

  • Purpose - This study aims to examine the correlations between relationship benefits, quality, and continuity intention of community services. It further investigates the mediation effect of relationship quality operating on the association between relationship benefits and relationship continuity intention. Thus, this study will aim to elucidate the correlations between relationship benefits, quality and community services intention. Research design, data, and methodology - To achieve these objectives, this study executed a questionnaire from the users of the child․adolescent psychological support service which represents part of community services. The research framework that was utilized in this study was of three main factors. They were relationship quality, relationship benefits, and intention of relationship continuity. In relationship quality, factors such as trust and commitment was included in this section. For relationship benefits, factors such as society benefits and commitment benefits were the essence. The control variable in this study was gender, age, income, hours of service used, and the number of times service was used in community services. Results - The key research findings are as follows. First, this study found out that the more the social benefits was enhanced in the community services, the higher the relationship continuity intention was. Second, this study has found out that the higher the trust of community services, the higher the relationship continuity intention. Third, this study has shown that the more the relationship benefits was enhanced in the community services, the higher the recognition by the users' relationship quality was. Fourth, the trust in the community services played a mediating role between the relationship benefits and relationship continuity intention. Conclusions - Many associations could be discovered on the correlations between relationship benefits, quality, and community service continuality intention. These results will have important implications for the future of child and adolescent psychological support services. In terms of relationship marketing, the three factors; benefits, quality, and continuity intention in community services were proved to be pivotal. In order for the continuity intention in community services, the two factors of benefits and quality were the influential factors. From a policy maker's standpoint, the relationship continuity was the most important factor.

Social investment in Europe: bold plans, slow progress and implications for Korea

  • ${\cdot}$ Recent social policy and labour markets debates in Europe, responding to the difficulties faced by the traditional neo-Keynesian welfare state settlement, stress the value of positive investment alongside de-regulation and greater flexibility as a way of achieving both economic and social goals. ${\cdot}$ Patterns of policy reform are complex and reflect differing national circumstances. A general move towards deregulation, constraints on entitlement to passive benefits, programmes to enhance employment, particularly among high-risk groups such as single parents and young people, targeted subsidies for low earners and casemanagement may be identified. ${\cdot}$ In relation to investment in education, research and development and combined training and benefit programmes to enhance mobility between jobs the picture is less clear. Education standards continue to rise, but research and development spending stagnates and few countries have developed substantial ‘flexi-curity’ programmes to support job mobility. ${\cdot}$ The labour market tradition in much of Europe has been one of conflict between labour and employers. As labour grows weaker, new approaches develop. These tend to stress productivity agreements and greater flexibility in work practices within firms and reforms to passive social security systems more broadly, but movement to support the more challenging investment and flexi-curity policies is slow. ${\cdot}$ In general, social and labour market policies in Europe stress deregulation and negative activation more strongly than social investment and ‘flexi-curity’. The countries with high growth and employment achieve that goal by different routes: Sweden has a closely integrated social democratic corporatism with high spending on benefits and training programmes and the UK a more liberal market-oriented system, with lower spending, highly targeted benefits and less mobility support. ${\cdot}$ Europe has something to learn from Korea in achieving high investment in human capital and R and D, while Korea may have something to learn from Europe in social investment, particularly flexi-curity and equal opportunity policies.

Effects of Commonality Strategy in Product Line Design on Social Welfare (제품군 디자인에서 공통속성의 활용이 사회적 효용에 미치는 영향)

  • Commonality strategy is a popular design practice in designing a product line as it enables the firm cost saving and simplification in design, manufacturing, and distribution processes. However the issue of commonality has been mostly analyzed from a profit maximizing firm's perspective and, to our knowledge, there is no literature that deals with the issue from a different perspective. in this paper, we consider the issue of commonality strategy from a social welfare maximization perspective, and argue that commonality strategy used in designing of public goods can bring certain benefits not just for a firm but also for consumers, i.e., for society as a whole. While we assume certain cost saving in production process due to economies of scale under commonality strategy, we conceptualize two different effects of commonality strategy, utility effect due to cost saving and weighted-averaging effect, and show how these two effects interplay to determine the design of common attribute and desirability of commonality strategy. We also discuss how the implementation of commonality strategy differs under different objectives of a product line designer : social welfare and firm's profit maximization.

Attitude toward the Increasing Role of Private Health Insurance (민간의료보험 활성화에 대한 인식과 그에 영향을 미치는 요인)

  • The purpose of this study was to examine the factors influencing the attitude toward the increasing role of private health insurance(PHI). In the Korea Welfare Panel Data 2007, a sample of 1,675 (adjusted by weight value: 1,607) respondents on an opinion on promoting PHI was used in the study. With independent variables including socio-demographic characteristics, health status, health-related behavior, and opinions on welfare service, ordered-probit model was used to analyze the attitude toward PHI. Negative opinion on the increasing role of PHI were responded by 54.6%(n=877) of the respondents, whereas 22.2%(n=373) were positive and 23.2%(n=357) were neutral. Old people, the better off, those with worse self-assessed health status, and those having an experience of health examination tend to have the positive attitude toward the increasing role of PHI. Women, those with chronic diseases or disorders and those who do not agree that comprehensive welfare benefits reduce work incentive showed negative attitude toward PHI. When comparing the needs for PHI before and after medical utilization, ex-ante need tends to strengthen the tendency to support private health insurance. This study will contribute to the discussion on the optimal mix of public and private health insurance in Korea by a better recognition of attitude toward PHI and health care system.

Parental Leave System and Women's Economic Activities in Sweden (스웨덴의 부모보험제도와 여성의 경제활동)

  • Parental leave in Sweden is a part of the health insurance in national social insurance system. It has two kinds of benefits. One is parental cash benefit paid for both husband and wife on the occasion of child birth, currently 450days for each child. The other is temporary parental cash benefit when a child under the age of twelve or a caretaker for him is illness, which is six months for a child a year. Parental insurance in Sweden permits parents to take care of their children just after birth at home with the amount of 80% of monthly income for 360 days and 60 Swedish krone each day for 90 days more. It also permits parents with children under the age of eight of part-time work and return to former job at full-time base when they want. It consequently encourages women's economic activity in her whole life and contributes to promotion of equality in sex roles between husband and wife. This insurance scheme is beneficient in that it enhances individual and family welfare and also secures labour force. This case study on Swedish parental insurance offers implication how to resolve the conflict between women's increased demand for economic activity and maternal role.

The Effects of Livelihood Benefits and Basic Pension Receipts on Depression in the Elderly: Multimediation Effects of Health Status and Community Satisfaction (생계급여와 기초연금 수급이 노인의 우울에 미치는 영향: 건강상태와 지역사회만족의 다중매개효과)

  • 본 연구에서는 노인의 생계급여와 기초연금 수급, 건강상태, 지역사회만족, 우울 간의 구조적 관계분석을 통해 노인의 우울 감소 방안을 제시하고자 한다. 분석자료는 2017년 노인실태조사 패널데이터 중 노인 본인이 직접 응답한 2,493명을 분석에 사용하였다. 분석결과, 첫째, 생계급여와 기초연금 수급은 노인의 건강상태에 정(+)적인 영향을 미쳤고, 지역사회만족에는 부(-)적인 영향을 미쳤고, 우울에는 정(+)적인 영향을 미쳤다. 둘째, 노인의 건강상태는 지역사회만족에 부(-)적인 영향을 미쳤고, 우울에 정(+)적인 영향을 미쳤다. 셋째, 노인의 지역사회만족은 우울에 부(-)적인 영향을 미쳤다. 넷째, 노인의 생계급여와 기초연금 수급과 우울사이에서 건강상태, 지역사회만족의 다중매개효과는 유의한 영향을 미치는 것으로 분석되었다.

Study on the Establishment of Fixed Night Shifts and Flexible Work Arrangements for Nurses (간호사의 야간전담 및 유연근무제도 정착을 위한 실태 조사)

  • Purpose: The purpose of this study was to investigate the welfare status of nurses and the status of fixed night shifts and flexible work arrangements in both general hospitals and advanced general hospitals in Korea. Methods: A total of 167 hospitals participated in this study, done from April to October 2018. Results: There were 103 hospitals utilizing the fixed night shift system, 22 in the two-shift system, 3 in the fixed holiday shift, 79 in the fixed shift system, and 39 in the short-time working system. The average welfare benefits for nurses increased to 33.02 million won on average. However, welfare services for the nurses were poor: 41.8% of the respondents had a nurses' lounge, 81.7% were given dormitory space at their place of employment, 41.8% had access to welfare facilities, and 56.4% were provided with welfare support countermeasures. Conclusion: The results show an increasing trend of hospitals to utilize fixed night shift and flexible work arrangements. An institutional strategy at the national policy level should be established to implement flexible work arrangements including the fixed night shift system, which has a poor level of welfare.

A Study of Incentive Problems of Welfare State (복지국가의 인센티브 문제에 관한 연구)

  • This paper is to critically review the economic reasoning of non-sustainability of welfare state due to its intrinsic incentive problems and to see how the nordic welfare state responds to them. The welfare state as a political design of state to pursue equality has social insurance as its main economic function. It survives market failure of private insurance to contribute to human capital investment and industrial restructuring. The universal tax-financed welfare state, however, has the problem of tragedy of commons such as reduced work incentive and work ethics. But, the existing nordic welfare state overcomes it through employment-focused policy arrangements, maintenance of work ethics and benefits moral, incentive mechanism of wage-compression, public educational investment and its complementation with social security. The Nordic model shows that problems of incentive and moral are not about those of theory and reasoning, but about their reality which policies and institutions could respond to.

A Comparative Study of Korea and Japan Relating to the Welfare Business Operation of Pension Accumulation (연금적립금의 복지사업 운용에 관한 한.일 비교연구)

  • The purpose of this study is to contribute the improvement of national welfare by presenting of pension welfare business of Korea and Japan. The reason why I take the management of Japan's pension accumulation as the object of comparative study is that the history of pension system of Japan is relatively longer than that of Korea. First of all, National Pension Fund comes into use for public sector, financial sector, and welfare sector, The scale of pension management for welfare sector is 50 small. Therefore, the study for welfare business investment reflecting the intentions of pension entries and pensioners is needs of times. This study defines the concept of welfare investment business and prospects welfare investment business of the future on the basis of Japanese experiences, and then suggests the direction of efficient propulsion of welfare investment business to the reasonable decision-makers. Especially this study redefine the concept of welfare investment business on the basis of pension entry's social benefits which are composed of pension entry's gains and pensioner's gain. Of course, welfare investment business has to be presupposed the stability of pension system and the continuous contribution to national economy. Thus, in order to efficiently perform welfare business, the policy-making for national welfare improvement has to be established after the good of business is set up like the results of this study.
