Measurements of simulated periodontal bone defects in inverted digital image and film-based radiograph: an in vitro study

  • Purpose: This study was performed to compare the inverted digital images and film-based images of dry pig mandibles to measure the periodontal bone defect depth. Materials and Methods: Forty 2-wall bone defects were made in the proximal region of the premolar in the dry pig mandibles. The digital and conventional radiographs were taken using a Schick sensor and Kodak F-speed intraoral film. Image manipulation (inversion) was performed using Adobe Photoshop 7.0 software. Four trained examiners made all of the radiographic measurements in millimeters a total of three times from the cementoenamel junction to the most apical extension of the bone loss with both types of images: inverted digital and film. The measurements were also made in dry mandibles using a periodontal probe and digital caliper. The Student's t-test was used to compare the depth measurements obtained from the two types of images and direct visual measurement in the dry mandibles. A significance level of 0.05 for a 95% confidence interval was used for each comparison. Results: There was a significant difference between depth measurements in the inverted digital images and direct visual measurements (p>|t|=0.0039), with means of 6.29 mm ($IC_{95%}$:6.04-6.54) and 6.79 mm ($IC_{95%}$:6.45-7.11), respectively. There was a non-significant difference between the film-based radiographs and direct visual measurements (p>|t|=0.4950), with means of 6.64mm($IC_{95%}$:6.40-6.89) and 6.79mm($IC_{95%}$:6.45-7.11), respectively. Conclusion: The periodontal bone defect measurements in the inverted digital images were inferior to film-based radiographs, underestimating the amount of bone loss.

  • Weeks who is an architect in England suggest that let us consider the hospital architecture as not determined architecture but an undetermined architecture. Because it has been changing unexpectedly. The changing process of hospital architecture is required that it has to involve the new function and be a role model because of variations of environmental factors. For silving the requirements above mentioned, hospital architecture takes the way of appropriateness inner origin form and finally it considers new formation. The 53% of hospital buildings in Korea were built in 1980s. For 30years, these buildings have been not only extended a buildings but also changed in parts of functions and sizes on buildings which are original. The purpose of this study is producing the basic references which suggest solution to face on the changing of hospital building during planning it in the future using by analysis of variation inside the hospital building and grasping of characters in each departments. Each department is analyzed as follows. There is no inner change but only expansion partly by enlargement of building. In the outpatient, there is not only expansion by enlargement of building and but also extension toward other parts and the rate of variation of inside is high. In the diagnostic treatment, there are differences in diagnostic treatment. Surgical suit and diagnostic imaging have been expanded by enlargement than the change of inside. But the others of the departments have been changed by change of inside and also there are sometimes changes of inner walls.

Comparative Daylighting Performance of an Interior with Lightshelves and Conventional Glazing; A Mock-Up Experiment (Mock-up을 이용한 일반창호 및 경사형 광선반 창호의 채광성능에 관한 비교 실험)

  • As sustainable design technologies, advanced daylighting systems with lightshelves have been developed and are currently under monitoring the daylighting performance. This study aims to evaluate the comparative daylighting performance of sloped lightshelf and conventional glazing window with mock-up model, reconstructed as a prototype of Korean office building, sized $12.0m{\times}7.3m{\times}3.7m$ ($w{\times}d{\times}h$) and $1.8m{\times}4.8m$($w{\times}h$) for the south facing side-window was installed on the rooftop of engineering building, Kyung Hee University in Korea. It has an identical configuration of reference room and the test room. For the test room, the sloped type lightshelf system was designed as 10mm transparency sheet glass, tilt angle degree $29^{\circ}$, and total sized 1.28m (interior length 0.88m, exterior length 0.49m). It consisted of daylighting collector, entrance glazing and reflector. To assess daylighting performance, the totally 37 measuring points for illuminance and 2 view points of luminance were monitored in every 30 minutes from 12:00 to 15:00. For the detailed analysis, photometric sensors of each room were installed at work-plane (8 points), wall (7 points), ceiling (3points), and exterior horizontal illuminance (1 point). Luminance of window, rare of the room was measured under clear sky. It is to be monitored by Agilent data logger, photometric sensor Li-cor and the Radiant Imaging ProMetric 1400. Comparisons with a light factor, increase-decrease ratio, uniformity, and luminance are discussed.

Non-infected and Infected Bronchogenic Cyst: The Correlation of Image Findings with Cyst Content

  • We hereby report a case on bronchogenic cyst which is initially non-infected, then becomes infected after bronchoscopic ultrasound (US)-guided transesophageal fine-needle aspiration (FNA). The non-infected bronchogenic cyst appears to be filled with relatively echogenic materials on US, and the aspirate is a whitish jelly-like fluid. Upon contrast-enhanced MRI of the infected bronchogenic cyst, a T1-weighted image shows low signal intensity and a T2-weighted image shows high signal intensity, with no enhancements of the cyst contents, but enhancements of the thickened cystic wall. The patient then undergo video-assisted thoracic surgery 14 days after the FNA. The cystic mass is known to be completely removed, and the aspirate is yellowish and purulent. To understand the image findings that pertain to the gross appearance of the cyst contents will help to diagnose bronchogenic cysts in the future.

Resonance May Elucidate New Bone Formation Induced by Low amplitude and High frequency Mechanical Stimuli (고주파 미세자극에 의한 뼈의 생성에 관한 모델링)

  • Bone is a self-assembly material. It is known that the low amplitude and high frequency mechanical stimulus, which is much less amplitude but higher frequency than those induced by the normal activity, can induce new bone formation. The vibrating resonance is employed to elucidate why new bone is formed by this kind of mechanical stimulus. For example, as 30 Hz and $5{\mu}{\epsilon}$ mechanical stimulus is applied at the wall of canaliculus (the tiny tube type pathway of bone fluid flow and the diameter of canaliculus is less than 200nm), the osteocytic cell membrane experiences $1,000{\mu}{\epsilon}$ strain due to the vibrating resonance. Two experiments will follow after this pilot study; (1) observing the MAPK pathway of osteocytes by using in-vitro cell culture and (2) visualizing the actin filament network in the osteocytes by using the imaging technique, such as confocal laser scanning microscope.

Extrahepatic Biliary Obstruction Secondary to Traumatic Diaphragmatic Hernia in Dog (개에서의 횡격막 탈장에 의한 간외담도폐색)

  • A 3-year-old intact mixed hunting dog was referred to Gyeongsang Animal Medical Center with the clinical signs of vomiting, dyspnea, anorexia. The dogs had traumatic history that wounded at abdominal wall during the wild boar hunting 2 week ago. The dogs was diagnosed as extrahepatic biliary tract obstruction (EHBO) secondary to traumatic hernia. Diagnosis was based on radiographic features of a diaphragmatic hernia on survey radiography, dilated bile duct in abdominal ultrasonography and complete recovery due to the return of displaced organs into the peritoneal cavity and repair of the diaphragmatic rent. Diagnostic imaging and surgical intervention may result in a favorable outcome in dog with post hepatic obstruction secondary to diaphragmatic hernia. This case emphasizes the strangulation of bile duct by diaphragmatic rupture could be the cause of EHBO.

Improment of Diesel Combustion using multiple injection under Cold Start Condition (냉시동 조건에서 디젤 연소 특성 및 연소 개선에 대한 연구)

  • Startability and harmful emissions are the main issues in diesel engine development under cold conditions. The characteristics of combustion with multiple injection were investigated under cold start conditions. For quantitative analysis, the in-chamber pressure profile was measured and combustion visualization using direct imaging was accomplished. With multiple injection, the peak in-chamber pressure and heat release rate were increased compared to single injection. In addition, the period of flame luminosity detection was shortened using multiple injection. Combustion by main injection was improved with an increase in heat released by pilot combustion when the pilot injection quantity was increased. Finally, an increase in injection pressure also showed the possibility of combustion improvement. On the other hand, an increase of in the pilot injection quantity and injection pressure can cause an increase in harmful emissions, such as HC and CO due to wall wetting. Therefore, more sensitive calibration will be needed when applying a multiple injection strategy under cold start conditions.

The Fate of Partially Thrombosed Intracranial Aneurysms Treated with Endovascular Intervention

  • Objective : The fate of partially thrombosed intracranial aneurysms (PTIAs) is not well known after endovascular treatment. The authors aimed to analyze the treatment outcomes of PTIAs. Methods : We retrospectively reviewed the medical records of 27 PTIAs treated with endovascular intervention between January 1999 and March 2018. Twenty-one aneurysms were treated with intraluminal embolization (ILE), and six were treated with parent artery occlusion (PAO) with or without bypass surgery. Radiological results, clinical outcomes and risk factors for major recurrence were assessed. Results : The initial clinical status was similar in both groups; however, the last status was better in the ILE group than in the PAO group (p=0.049). Neurological deterioration resulted from mass effect in one case and rupture in one after ILE, and mass effect in two and perforator infarction in one after PAO. Twenty cases (94.2%) in the ILE group initially achieved complete occlusion or residual neck status. However, 13 cases (61.9%) showed major recurrence, the major causes of which included coil migration or compaction. Seven cases (33.3%) ultimately achieved residual sac status after repeat treatment. In the PAO group, all initially showed complete occlusion or a residual neck, and just one case ultimately had a residual sac. Two cases showed major recurrence, the cause of which was incomplete PAO. Aneurysm wall calcification was the only significantly protective factor against major recurrence (odds ratio, 36.12; 95% confidence interval, 1.85 to 705.18; p=0.018). Conclusion : Complete PAO of PTIAs is the best option if treatment-related complications can be minimized. Simple fluoroscopy is a useful imaging modality because of the recurrence pattern.

Responses of Autonomic Nervous System and Gastrointestinal Function to Acupuncture at Abdominal Anterior Cutaneous Nerve : A Pilot Study (복부전방피부신경 영역의 자침으로 유발한 자율신경 및 위장관기능 변화에 대한 연구)

  • Objectives: This study is designed to identify the responses of autonomic nervous system and gastrointestinal function which are induced by acupuncture at abdominal anterior cutaneous nerve. Methods: This study is one group before and after pilot study. Subjects were treated once, after having been fasting six hours. They had before tests, labeling points for acupuncture, acupuncture, and after tests in order. The points of acupuncture were motor points of rectus abdominis where the abdominal anterior cutaneous nerve came to the skin from abdominal wall. Before and after tests were consisted of three things: Digital Infrared Thermographic Imaging(D.I.T.I.), Heart Rate Variability(HRV), and Recording of bowel sounds. Results: There were significant differences on the skin temperature of upper body and the frequency of bowel sounds(p<0.001, p<0.001). The HRV parameters and volume of bowel sounds had no significant differences(p>0.05, p>0.05). Conclusion: Even though no significant differences in HRV parameters, the significant differences of skin temperature of upper body and frequency of bowel sounds could mean acupuncture at abdominal anterior cutaneous nerve could affect the autonomic nervous system and gastrointestinal function. However, this study had no group to compare with. Future randomized project should address this issue.

Segmentation of Intima/Adventitia of IVUS Image using Fuzzy Binarization (퍼지 이진화를 이용한 IVUS 영상의 내막/외막 분할)

  • IVUS is an intra-operative imaging modality that facilitates observing and appraising the vessel wall structure of the human coronary arteries. IVUS is regularly used to locate the atherosclerosis lesions in the coronary arteries. Auto-segmentation of the vessel structure is important to detect the disorder of coronary artery. In this paper, we propose a simple strategy to extract Intima/Adventitia area effectively using fuzzy binarization from intravascular images. The proposed method apply fuzzy binarization to find the adventitia but apply average binarization to locate the intima since they have different homogeneity of pixel intensity comparing with the environment. In this paper, we demonstrate an effective auto-segmentation method for detecting the interior/exterior of the vessel walls by differentiating the fuzzy binarization result and average binarization result from IVUS image. Important statistics such as Intima-Media Thickness (IMT) or volume of a target area can be easily computed from result.
